[I.V. ketorolac in the therapy of postoperative pain].

Forty-one patients suffering from postoperative pain were admitted to an open, noncomparative trial. A dose of 10 mg of Kerotolac was administered i.v. to patients complaining severe pain. The analgesic efficacy of the drug was evaluated at 5, 15, 30 minutes and at 1, 1.5, 2, 3, 4, 6 hours after injection; the patients were asked to score their pain relief on the VRS comparing the pre-injection evaluation to each post-injection period. A significant decrease of pain intensity (less than p 0.01) was observed at 45 minutes after injection. No side effects were observed. These results suggest that Ketorolac can be successfully used in the treatment of postoperative pain.

Extraperitoneal aorticobifemoral grafts.

Out of 436 patients operated on using aorticobifemoral grafting, 60 cases (mean age 66) had an extra-peritoneal approach to the abdominal aorta. 28 patients underwent aorticobifemoral revascularization, three of them had, in addition visceral artery grafts. In the 60 cases, perioperative mortality was 1.7%. The extra-peritoneal approach to the abdominal aorta is safer than the intraperitoneal approach, because it reduces the risk of shock connected with the large laparotomy. It is emphasized that, by using an extraperitoneal approach, a greater number of elderly patients can be considered suitable for aorticobifemoral grafts.

Different kalemia in abdominal trauma.

Unlike previous reports, in 123 patients with abdominal injury or surgery significant and persistent hypokalemia was observed only after traumatic or surgical injuries of the liver. On the contrary, in all other extrahepatic abdominal trauma or operations kalemia was normal. This discrepancy may be related to the much greater abundance, in comparison with other abdominal organs, of adrenergic endings present in the liver. Trauma to or surgery of the liver stimulates such fibers, which locally release proportionally large amounts of norepinephrine. The consequent stimulation of beta-adrenergic receptors enhances the transfer of potassium from the blood into the cells, even if the affinity of norepinephrine for such receptors is less than that of epinephrine. The frequent finding of kaliuria at the upper normal levels may suggest an accessory role of aldosterone in causing and maintaining this hypokalemia.

Leukocyte adherence inhibition test in lung cancer immunodiagnostic detection.

The leukocyte adherence inhibition test (LAI) was used to study the cellular reactivity in 119 subjects: 48 were affected by lung cancer, 41 by non-malignant lung diseases, 10 by non-lung cancers and 20 were healthy volunteers. The LAI test is based on the observation that leukocytes do not adhere to glass when in the presence of an antigen against which a sensitization exists. Test selectivity was checked by the suppression technique: suppressed cells lose their competence to an antigen when incubated with it before testing. Peripheral blood leukocytes (PBLs) and bronchioloalveolar cells (BACs) were used. While controls showed no reactivity to lung cancer extracts, early cancer patients showed the highest reactivity and active cavitary tuberculosis patients gave false positive results. The blocking assay showed that patients with tuberculosis were sensitized to normal lung extracts. In lung cancer patients the blocking assay showed that a selective reactivity exists to cancer extracts derived from a histologically homologous cancer specimen.

Detection and measurement of a cellular immune-reactivity towards polyester and polytetrafluoroethylene grafts. Leukocyte adherence inhibition test.

Several studies were performed on polyester (Dacron) and polytetrafluoroethylene (PTFE) vascular substitute thrombogenicity. However, to date, the host-graft interactions have yet to be studied from an immunological point of view. For this reason, 4 classes of 10 patients each (Class 1: Dacron-+PTFE-grafted patients, Class 2: Dacron-, Class 3: PTFE-, and Class 4: controls) were submitted to a cellular immune-reactivity test: leukocyte adherence inhibition (LAI), in which leukocytes fail to adhere to glass on contact with a sensitizing antigen. The following blood cell populations were used: total leukocytes (PBL), mononuclear cells (MNC), T and B lymphocytes. This research demonstrated that a T cellular immune-reactivity towards Dacron and PTFE respectively occurs in Dacron- and PTFE-grafted patients, and that this reactivity is greater in the case of Dacron. More studies are required to determine the immuno-competent system role in fabric prosthesis patency.

A tool for transhepatic stenting.

A modified method for transhepatic tube stenting is presented. The use of a No. 25 gauge music wire (1.5 millimeters in diameter) allows for different surgical maneuvers to be performed without parenchymal splitting. This technique can also be used in a slightly dilated biliary tree.

Anaplastic adenocarcinoma of the thyroid gland: a study of ten cases.

Anaplastic carcinoma of thyroid gland, while rare, is the most common cause of death from thyroid cancer. Ten cases from a teaching department of surgery have been reviewed, with two-year follow up. Steps in pathological behaviour and management of this lesion are discussed. From the review, it seems that the only logical option in the treatment of anaplastic carcinoma of thyroid gland, is total thyroidectomy.

[Intra-operative manometry and hiatal hernia].

Personal experience of the intra-operative use of oesophageal manometry in cases of hiatus hernia is presented. Using this system the pressure in the air cushion surrounding the L.E.S. can be adjusted to avoid any complications due to overtight or overlax surgery. Various types of post-operative tests confirm the value of the technique.

[Jaundice as a symptom of traumatic diaphragmatic hernia].

The onset of jaundice as an atypical, early clinical symptom of traumatic hernia of the right diaphragm is described. Jaundice has never been reported in the literature as a symptom of herniated diaphragm. In this case, it was caused by a twisted main bile duct after herniation of the right lobe of the liver through a wide thoraco-diaphragmatic opening. The need for a careful investigation of all thoracoabdominal traumas to identify any concommitant diaphragmatic pathology is emphasised. This is especially true of injuries to the right diaphragm which are often easily identified by simple examination techniques.

[Correlation between hyperamylasemia and acute pancreatitis].

It is often difficult to differentiate acute pancreatitis (A.P.) from some other acute abdominal diseases, when there is an elevated serum amylase. In contrast, the renal clearance of amylase, expressed as a percentage of creatinine clearance, can separate patients with A.P. from patients with acute colecistitis, common duct stone without pancreatitis, hyperamylasemia after biliary surgery, acute peptic ulcer and acute salivary diseases.
