[Surgical policy in reconstructive surgery on the colon].

Thirty-year experience with 1685 reconstructive operations on the colon is analyzed. Novel surgical policy has increased the rate of primary-reconstructive operations for the last 5 years from 78.8 to 87.9%, decreased the number of postoperative complications from 33 to 21.8%, intestinal anastomosis-dependent complications -- from 22.6 to 8.4%, postoperative lethality -- from 6.9 to 2.4%, raised the rate of good functional results from 64 to 88.2%.

[Nonspecific ulcerative colitis and Crohn's disease--the main problem of modern coloproctology].

There is adduced modern strategy of treatment of nonspeciphic ulcerative colitis and Crohn's disease, which owe medical and social significance due to their strictly progressive course, severe complications origin threat, invalidisation of patients. Conduction of conservative pathogenetically substantiated therapy of uncomplicated nonspeciphic ulcerative colitis and Crohn's disease in conditions of specialized gastroenterologic or proctologic stationary assists essential limitations of indications for surgical treatment of the patients. Conduction of an adequate operative intervention according to strict indications permits to improve results of the patients' treatment essentially.

[Abdomino-anal resection in the treatment of low-ampullar cancer of the rectum].

Surgical policy was developed for improvement of functional results and quality of life in patients operated on for low rectal cancer. This policy includes choice of the method of sphincter-saving operation depending on the stage of the tumor, grade of malignancy, distance from a low edge of the tumor to dentate line. Methods of prevention of ischemia and necrosis of brought down colonic transplant and anal incontinence were used. This increased the number of sphincter-saving operations in low location of rectal cancer by 35%, reduced the rate of necrosis of intestinal transplant from 7 to 4.4%, saved continent function and improved significantly quality of life in 89.8% operated patients.

[Application of preparations relief ultra and relief advance in practice of coloproctological department].

Efficacy of application of preparations Relief Ultra and Relief Advance as pathogenetically directed medicines was established, basing on analysis of the examination and treatment results in 133 patients with anal region diseases (acute and chronic hemorrhoids, nonspeciphic ulcerative colitis, the Crohn's disease, acute paraproctitis, rectal polyp). For local conservative treatment it is expedient to apply preparations in various pharmacological forms in complex. Application of preparation Relief Ultra endorectally in conjunction with preparation Relief Advance endorectally and locally on the wound surface is indicated after performance of elective and urgent operative interventions for rectal and anal zone diseases.

[Ultrasonic diagnosis of an acute appendicitis and its complications].

Retrospective analysis of the examination results in 292 patients with an acute appendicitis (AA) was conducted. There were 184 (63%) of patients operated on, for AA--168 (57.5%). Possibilities of ultrasonic investigation in the AA diagnosis were estimated. Due to introduction of this method it become possible to escape the operation performance in 58 (13.7%) of patients and to reduce the frequency of idle appendectomy for simple AA as twice. Broad application of ultrasonic investigation in urgent surgery would permit to establish the AA diagnosis correctly and substantially and to determine the tactic of patients treatment.

[The application experience of Relief and Relief Advance in practice of the coloproctological patients treatment].

The application efficacy of Relief and Relief Advance preparations, as pathogenetically directed remedies, was established, basing on the examination and treatment results of 129 patients with anorectal zone diseases (acute and chronic hemorrhoids, anal fissure, the perianal skin pruritus, nonspecific ulcerative colitis, Crohn's disease). For local conservative treatment it is expedient to apply the complex of various pharmacological forms of the preparation. Combined application of Relief and Relief Advance preparations endorectally and locally on the wound surface is indicated after performance of elective and urgent operative interventions for rectal and perianal region diseases.

[Application of apparatus AKA-2 in the reconstructive-restorative surgery of the left half of colon and of rectum].

Result of the AKA-2 apparatus application for the colorectal anastomosis formation in 129 patients after resection of left colonic half and rectum was estimated. After the operation all the patients survived. Insufficiency of the anastomotic edges conjunction have occurred in 6 (4.7%), cicatricial stricture--in 3 (2.3%) patients. Common postoperative complications were observed in 28 (21.8%) of the patients examined.

[The use of gravitational surgery of the blood. Plasmapheresis in the treatment of bronchial asthma patients].

It was noted while examination of 290 patients with chronic bronchial asthma (BA) that application of plasmapheresis (PA), contrary to hemosorption (HS), permits to achieve more rapid, but less durable remission. The conducted HS, contrary to PA, permits to normalize the common immunoglobulin E level in the blood serum. The advantages of the HS application in patients with atopic form of BA were established.

[The validation of the frequency of performing plasmapheresis in the treatment of bronchial asthma patients].

On the basis of the analysis of plasmapheresis (PPh) influence on clinical and functional and immunological indices in 105 patients with bronchial asthma the authors have come to the conclusion that PPh effect is the number-of-procedures-dependent. The most pronounced effect occurred in those groups having been given three to five PPh procedures. The above effect begins to reveal itself during the first postoperative week but the greatest positive changes take place between the 7th and the 14th days. As it won't make any difference whether three or five PPh sessions are conducted, the authors recommend plasmapheresis to be done on a three-procedure basis.

[The effect of chronic internal irradiation from incorporated radionuclides on liver function].

On chronic supply to the body of a mixture of nuclear division products lanthanum-140, barium-140, tellurium-132, neodymium-147, neptunium-239, zirconium-95, niobium-95, iodine-131, cerium-141, -144, cesium-134, -137, and ruthenium-103 are detectable in liver tissue within the first months. In the 2 to 4 years following the disaster, liver tissue primarily accumulates cerium-144, radium-226, thorium-228, -232, ruthenium-106, antimony-125, and europium-154. Within the first periods the liver radionuclide content was 19 to 31% higher than that in the blood, and in the following years it was 24 to 38% higher. The radionuclides indicated were actively excreted with the bile into the gastrointestinal lumen. Within the first months after the chronic internal radiation the functional liver disorders were detectable in 30 to 40% of the patients. Later on diverse acute and chronic diseases of the liver, gallbladder and pancreas were detectable in 20 to 30% of the patients. The radionuclide content in the body was found to be in parallel with functional liver disorders. Acceleration of radionuclide excretion from the body results in liver function improvement.

[Effect of hemosorption on the activity of natural antioxidant enzymes in acute pancreatitis].

All forms of acute pancreatitis are followed by the development of antioxidant insufficiency of the organism which aggravates the course of the disease, makes results of treatment worse. Hemosorption facilitates activation of natural antioxidant enzymes resulting in decreased levels of underoxidated products of lipid peroxidation: in 2/3 of the patients the use of repeated sessions of hemosorption with 24-48 h intervals is indicated in order to achieve normalization of natural antioxidant defense.

[Effect of hemosorption on kidney function in patients with destructive pancreatitis].

In patients with acute destructive pancreatitis, the kidney lesion is conditioned by pronounced volemic disorders, and damage of renal parenchyma resulting from the action of enzymes and toxic metabolites as well. Their effective elimination by means of hemosorption performed in supplementation of the circulating blood and plasma volumes leads to rapid normalization of functional state of the kidneys, thus facilitating reduction of lethality, shortening of the duration of treatment and rehabilitation of the patients. In destructive pancreatitis, the performance of repeated hemosorption after 24-48 h is indicated.

[Sorption methods and laser irradiation in the treatment of suppurative and septic diseases].

The application therapy of purulent wounds with sorbents during the first phase of the wound process reduces osmotic hyperhydration of tissues around the wound, promotes normalization of pH and phagocytosis of the wound exudate, makes the healing of the wounds 32.4% quicker. The combined use of the application therapy and external laser irradiation makes the time of healing almost 2 times shorter. In sepsis hemosorption results in the effective elimination of underoxidized products of free radical oxidation, in normalization of activity of the Kallikrein-Kinin system, improves results of the treatment.
