Serum trace elements and Cu/Zn ratio in breast cancer patients.

Serum copper, zinc, and the Cu/Zn ratio were measured in 55 patients with breast disease (20 with benign breast diseases and 35 patients with breast cancer) and 30 controls. The mean serum copper levels were higher in breast cancer than in benign breast diseases (167.3 micrograms/dl vs. 117.6 micrograms/dl) (P less than 0.001) and controls (167.3 micrograms/dl vs. 98.8 micrograms/dl) (P less than 0.001). Patients with advanced breast cancer had higher serum copper levels than did patients with early breast cancer (177.9 micrograms/dl vs. 130.4 micrograms/dl) (P less than 0.001). The mean serum zinc levels were lowered only in patients with advanced breast cancer as compared with controls (88.6 micrograms/dl vs. 115.1 micrograms/dl) (P less than 0.001). Serum zinc levels were not decreased in patients with early breast cancer and benign breast diseases. The Cu/Zn ratio was increased in breast cancer patients (1.91 vs. 0.86) (P less than 0.001) but not in patients with benign breast diseases. The precise mechanisms responsible for the alterations in trace element levels in breast cancer patients are still unclear and require further evaluation. However, the serum copper levels and the Cu/Zn ratio may be used as biochemical markers in these patients.

Adult

Fulminant amebic colitis.

Nine patients with fulminating amebic colitis who were treated surgically from 1975 to 1982 are presented. Only those who had bowel resections with exteriorization of the cut ends survived. The pertinent literature is reviewed briefly.

Hydatid cyst of the spleen.

Though hydatid disease is quite common in Saudi Arabia, splenic echinococcal cysts are rare. Our experience with the diagnostic problems encountered in three patients seen during a three-year period is presented. The helpful role of ultrasonography and computerized tomography in the preoperative evaluation of this disease also is demonstrated.

Primary carcinoma of the gall bladder: a review of a 16-year period at the University Hospital.

A review of the most relevant clinical data of patients with primary carcinoma of the gall bladder referred to our hospital over a 16-year-period has indicated that this is the third most common and aggressive cancer of the gastrointestinal tract. The disease occurs predominantly in elderly females who often present with extremes of clinical symptoms indicating benign biliary disease on the one hand and incurable malignant disease on the other. Laboratory and roentgenographic data tend to confirm the clinical diagnosis of this almost entirely incurable disease. The overall 5-year survival of the patients with primary carcinoma of the gall bladder is less than 1%; a majority of the patients are dead in less than 6 months.

Adenocarcinoma

Primary hepatic pregnancy.

A case of a 28 weeks primary hepatic pregnancy in a 25 year old female is reported. A live male fetus weighing 1300 g was delivered after laparotomy and the placenta was left intact. The uterus was of 8 weeks size with patent tubes. The patient made an uneventful recovery.

Correlation of immune and nutritional status with wound complications in patients undergoing abdominal surgery.

The present study indicates the need for close scrutiny of the immune and nutritional status of patients undergoing abdominal surgery. Simple anthropometric measurements (weight and triceps skin fold thickness MAC) serologic testing (serum albumin), ALC, and skin testing are useful to identify patients with immunocompromise caused by nutritional deficiency. Early recognition and correction of immune nutritional deficits in patients undergoing abdominal surgery should lead to reduction in postoperative morbidity due to wound complication.

Abdomen

Host response and tumour biological behaviour in the two histological types of gastric carcinoma.

It appears that there is validity in categorizing gastric carcinoma into two histologic types, intestinal and diffuse. The local host tissue response in 92.5% of cases of the intestinal type of gastric carcinoma was of an exudative nature. Diffuse gastric carcinoma in 70% of cases incited a dense productive fibrosis. Pools of mucin and large number of 'signet-ring' cells were mostly encountered in the intestinal type of carcinoma. Applying Dukes' parameters the tumour was found to be more than three times more invasive in cases of diffuse carcinoma. The prognostic bearing of the two histologic types, different host tissue response, behaviour of the tumour in terms of mucous production and local extension are discussed and it is suggested that diffuse gastric carcinoma carries a worse prognosis than the intestinal type. Study of a larger series of cases and longer follow-up with controlled treatment is essential to confirm this assessment.

Carcinoma

Oesophageal carcinoma in India. Some epidemiologic and morphologic considerations.

Oesophageal carcinoma constituted 2.39% of all malignancies, and 17.11% of gastrointestinal tract malignancies, over a period of 12 years, in Sir Sunder Lal Hospital population in Varanasi. The highest frequency occurred in the sixth decade. The male:female ratio was 2.2:1, this ratio tended to diminish with age. Out of 202 cases the middle third of the oesophagus was involved in 104, the lower third in 72 and the upper third in 26 cases. Although squamous cell carcinoma was by far the most frequent histologic type (193 cases), genuine primary oesophageal adenocarcinoma did occur in 3, and the tumour was completely anaplastic in the remaining 6 cases. The prognostic bearing of certain morphologic features has been discussed. Oesophageal carcinoma is one of the most frequent malignancies of the gastrointestinal tract and a common tumour in elderly Indian men and women.
