[Late presentation of a traumatic pulmonary arteriovenous malformation].

INTRODUCTION: Pulmonary arteriovenous malformations (PAVM) are rare, and most often of congenital origin. Among secondary causes of PAVM, trauma is exceptional. CASE REPORT: We describe here one such case, discovered very late over twenty years after a thoracic perforing wound. Recent vascular imaging techniques (angio-CT scanning and -RMI) have largely contributed to their diagnosis, but direct pulmonary arteriography remains the reference technique preoperatively when surgery is required. In all cases, treatment is as conservative as possible, and embolization of small sized PAVM, even multiple, by coils has become the standard procedure. However, surgery is required when the PAVM is large and/or proximal, particularly when its feeding vessel is complex. Itwas the case of our patient, who has been cured by a lobectomy. CONCLUSIONS: PAVM are rare but should be sytematically searche for in case of hypoxemia along with a pulmonary nodule.

Magnetic resonance imaging of the fetus: a study of 20 cases performed without curarization.

Twenty patients underwent magnetic resonance imaging (MRI) at a mean gestational age of 32 weeks. There were 12 patients with suspected fetal brain abnormality and four with intrauterine growth retardation (IUGR), while the remaining four cases were studied for other reasons. The MRI examinations were performed on a 0.5 Tesla machine, with surface coils. One minute acquisition time T1 sequences were used. All the studies were performed without fetal curarization, and only under maternal sedation using flunitrazepam given per os 1 h before MRI examination. Three examinations were incomplete because of fetal movement artefacts. In the remaining cases, MRI allowed the examination of fetal brain anatomy. In five cases, it helped to differentiate isolated hydrocephalus and corpus callosum agenesis. Sub-ependymal nodules were depicted in a case of fetal tuberous sclerosis. One suspected arachnoid cyst was proved to be an ultrasound artefact. Decreased fetal fat on MR images was correlated with low birth weight in cases of IUGR. Due to its better spatial resolution, ultrasonography was more accurate for the diagnosis of facial and lumbar anomalies. Fetal MRI may be performed without curarization. Surface coils allow the detailed analysis of brain parenchyma, and thus MRI is especially useful in the difficult prenatal diagnosis of fetal brain abnormalities.

Concentric MR patterns in multiple sclerosis. Report of two cases.

We report two cases of multiple sclerosis with concentric patterns of demyelinated foci on MR images. These were compared with the pathological data of Balò's disease. The T2-weighted MR images displayed alternating high and low signal areas which probably corresponded to the concentric type of demyelination. Our two cases were characterized by their acute onset and their regression following corticosteroid therapy. This suggests that Balò's disease may be a transient disease and not always associated with a fatal course.

Trigeminal neuromas: assessment of MRI and CT.

We report four cases of trigeminal neuroma. One of the patients had von Recklinghausen's neurofibromatosis with plexiform neurofibromas of the branches of the trigeminal nerve. MRI provided more information than CT as regards the spread of tumour: extension to the mandibular and maxillary division of the trigeminal nerve was well demonstrated on sagittal and coronal sections. This examination yielded an accurate census of the intraocular plexiform neurofibromas and allowed a correct preoperative diagnosis to be obtained. With Gd-DOTA, better definition of the outline of the tumours and of cystic components was obtained. However, CT was better for demonstration of bone erosions.

CT-guided vertebral biopsy. A report of 89 cases.

The authors report the results of 89 vertebral biopsies performed under CT guidance, classified according to the type of pathology and the site of the lesion. A correct diagnosis was obtained in 89 cases. No complication was observed. The value of CT guidance in vertebral biopsy is emphasized.

Aneurysmal bone cysts of the jaws: CT and MR findings.

Two cases of aneurysmal bone cyst of the jaws, one in the mandible and the other in the maxilla, are reported in young girls 7 and 15 years old. One was a primary lesion; the other was associated with fibrous dysplasia. In both cases CT and MR demonstrated characteristic fluid-fluid levels. The diagnosis was confirmed after biopsy and surgical resection of the lesions.

Hyperdense CT foci in treated AIDS toxoplasmosis encephalitis: MR and pathologic correlation.

Five AIDS patients with previously diagnosed and treated toxoplasmosis presented with hyperdense precontrast CT approximately 6 months after the CNS infection. An MR study was performed in all cases. The CT hyperdensities were characterized on MR by a high signal intensity on T1 images in four cases. In two of these, pathological correlation confirmed hemorrhage. In the fifth case, MR findings were consistent with calcification, which was verified by pathological examination.
