Measurement characteristics of the Women's Health Initiative food frequency questionnaire.

PURPOSE: The Women's Health Initiative (WHI) is the largest research program ever initiated in the United States with a focus on diet and health. Therefore, it is important to understand and document the measurement characteristics of the key dietary assessment instrument: the WHI food frequency questionnaire (FFQ). METHODS: Data are from 113 women screened for participation in the WHI in 1995. We assessed bias and precision of the FFQ by comparing the intake of 30 nutrients estimated from the FFQ with means from four 24-hour dietary recalls and a 4-day food record. RESULTS: For most nutrients, means estimated by the FFQ were within 10% of the records or recalls. Precision, defined as the correlation between the FFQ and the records and recalls, was similar to other FFQs. Energy adjusted correlation coefficients ranged from 0.2 (vitamin B12) to 0.7 (magnesium) with a mean of 0.5. The correlation for percentage energy from fat (a key measure in WHI) was 0.6. Vitamin supplement use was common. For example, almost half of total vitamin E intake was obtained from supplements. Including supplemental vitamins and minerals increased micronutrient correlation coefficients, which ranged from 0.2 (thiamin) to 0.8 (vitamin E) with a mean of 0.6. CONCLUSIONS: The WHI FFQ produced nutrient estimate, that were similar to those obtained from short-term dietary recall and recording methods. Comparison of WHI FFQ nutrient intake measures to independent and unbiased measures, such as doubly labeled water estimates of energy expenditure, are needed to help address the validity of the FFQ in this population.

Sources of error in bioimpedance spectroscopy.

Two different makes of bioimpedance spectrometer (UniQuest-SEAC SFB-3 and Xitron 4000B) were used for a series of measurements on volunteers and patients in intensive care. Although each machine was accurate over the frequency range 5 to 500 kHz when bench tested on model resistor-capacitor circuits, significant differences in their recorded impedance parameters appeared when used in vivo, especially on intensive care patients. A series of laboratory tests was performed on each machine simulating the situation in vivo to identify possible reasons for these differences. Whilst stray capacitance in the environment was identified as the major contributor to variability in high-frequency performance, interaction between electrode impedance and lead positioning was also a factor. The observed phase shift with frequency or time delay (Td) used in the Xitron modeling software appears to be the result of a time constant caused by stray capacitance and so is unlikely to have any biological meaning. Significant differences in the in vivo numerical values produced by bioimpedance spectrometers may be attributed to instrument design, data processing and, in particular, the clinical environment.

A battery-powered constant current stimulator for motor end-point detection.

The device described was developed as an external electrical stimulator to assist in the accurate location of motor end-points prior to the injection of nerve blocking agents in the treatment of spasticity. Monophasic pulses are produced with a duration of 10-100 microseconds and a current from 0 to 10 mA. The current is maintained over a wide range of electrode impedance and is independent of battery voltage.

Ethics of obesity treatment: implications for dietitians.

Dietitians face both a responsibility and an opportunity to address the ethical issues of obesity treatment and to promote weight management strategies that are beneficial and effective. The Code of Ethics for the Profession of Dietetics, which was adopted in 1989 to provide guidance to dietetics practitioners in their professional practice and conduct, can serve as a guide in an evaluation of current obesity treatments. Dietitians must help clients be realistic about their weight loss goals and address emotional conflicts that may be undermining weight management efforts. Referrals to therapists may be indicated, and dietitians must be prepared to recognize the need and make the recommendations. Dietitians should take the lead in developing more ethical and beneficial treatments by collaborating with government and industry to protect the consumer from ineffective or potentially harmful practices.

The Aberdeen Mark II single-photon-emission tomographic scanner: specification and some clinical applications.

The construction, operation and physical characteristics of a single-section multi-detector single-photon-emission scanner are described. The machine has 24 detectors arranged along the sides of a square. Movements and data collection are under the control of a series of distributed microprocessors. Both head and trunk tomograms can be produced. The spatial resolution at the collimator focus is 9 mm in the transverse plane, and the effective slice thickness is 14 mm. The volume sensitivity is 300 counts/s kBq ml with a 20 cm diameter cylindrical phantom filled with 99Tcm solution. The application of this machine to the examination of the brain, liver and heart has been found to be clinically useful.

A universal computer and interface system for the disabled (UNICAID).

A prototype text handling and computing system using microprocessor technology has been developed for the severely handicapped user. The input uses a serial 'suck-puff' code (TWC code version DR2). In addition to the usual video display and full editing and printout facilities of a word processor, the unit acts as a terminal for other computers and can also function as a stand-alone microcomputer using commercial software. The 'bus' structure allows simple expansion to incorporate additional functions, and a remote control link is used for the input. The prototype is currently being evaluated by a tetraplegic student.

Microprocessor detection of electrocardiogram R-waves.

ECG monitoring devices for coronary care units require reliable R-wave dectors as a prerequisite for subsequent analysis. A simple algorithm has been developed for the detection of R-waves in an ECG signal with a microprocessor (Motorola MC6800). Detection criteria used were gradient and duration of a single upslope or downslope. Rejection of muscle noise and other artifiacts was achieved by setting system constants to optimum values which were determined by a short preliminary trial. This constant threshold system has been shown to cope with sudden changes of QRS complex amplitude better than some self adjusting systems. Assessment runs using patient tapes from a coronary care unit show total errors due to false positives and negatives are typically about 1% of the actual number of R-wave occurrences.

A microvolt calibrator for EMG systems.

Non-evoked electromyographic signals range from several hundred microvolts for voluntary contractions to less than a microvolt far resting activity. Calibration of EMG recording equipment at such low levels may prove problematic. We have designed a useful battery operated calibrator for low level EMG recording in areas such as bio-feedback and stress management.

Technical note: a compact data logger for ambulatory skin temperature measurement.

A miniature, body-worn data logger has been developed for multi-channel recording of small changes in skin surface temperature. The unit has been designed for small size, long battery life and high accuracy together with full programmability and an infra-red (contactless) transfer of parameters and data. Temperature is measured using thermistors with a resolution of 0.005 degrees C and an absolute accuracy of better than 0.05 degrees C. A comprehensive software package running on a laptop PC is used for initial setting of the logger parameters and downloading and displaying the temperature data. The present system records eight channels of temperature at intervals from 30 seconds to 99 minutes. It contains 64 K bytes of non-volatile memory allowing 3274 sets of readings including subject identification, battey status, date and time. The logger size is 55 mm x 31 mm x 9.5 mm and weighs 20 g.
