Prevalence of benzodiazepine abuse and dependence in psychiatric in-patients with different nosology. An assessment of hospital-based drug surveillance data.

Frequencies of abuse and dependence assessed continuously within a drug surveillance system were analysed as a contribution to risk-benefit evaluations of benzodiazepines (BZDs). In 4.7% of 15,296 patients admitted to psychiatric hospitals between 1980 and 1985, BZDs had been involved in some kind of abuse or dependence. Primary BZD dependence, defined as physical dependence on BZDs in patients who had not been dependent before, was observed in about 1% of admitted patients. Linking these data with psychiatric diagnoses revealed a high risk of primary BZD dependence for in-patients (11.8%) with anxiety neurosis (ICD-9, 300.0), and a lower risk for neurotic (300.4) and for endogenous depressives (296.1) (risk 3.7% and 2.7% respectively). Older age was also related to primary BZD dependence. For depressive in-patients, the risk was twice as high in females as in males. Anecdotal observations advocate more systematic investigation of the emotional effects of long-term therapy with BZDs.

[Utilization of free and protein-bound lysine in the Japanese quail].

The utilization of dietary lysine for protein synthesis is affected by the digestibility of protein-bound lysine, by its intestinal resorption and by its oxidative catabolism. The approach chosen in this paper enables a comparison of the cumulative effect of these processes on the utilization of free and protein-bound lysine, respectively. The principle of the approach is based on a quantification of the expiration of 14C-labelled carbon dioxide after an oral administration of a diet, which contains L-(U-14C)-lysine either as a free amino acid or bound to yeast proteins. During an adaptation phase cockerels of the Japanese quail received a diet based mainly on ground wheat and wheat gluten. This diet was supplemented either with yeast proteins or with a mixture of L-amino acids which simulates the composition of the yeast proteins. In the main experiment the expiration of labelled carbon dioxide was measured during 240 minutes after the administration of the corresponding labelled diets. Just before treatment the animals were either in the postprandial phase or in a state of slight hunger. The maximum of expiration of labelled carbon dioxide occurred around the 60th minute after administration of the corresponding labelled diets. The cumulative expiration of labelled carbon dioxide, expressed in per cent of the radioactive dose used, amounts to 15.5% and 14.3% for free and protein-bound lysine, respectively. The utilization of both forms of lysine in the Japanese quail is lower than in broilers.

[Methoxydextran as an indicator of gastrointestinal transit in Japanese quail].

The recovery of polyethylene glycol, a substance frequently used as an indicator of the dynamics in the liquid phase of intestinal chyme, is considerably influenced by the presence of trichloroacetic acid in certain applications. The suitability of (methoxy-14C)methoxydextrane as an intestinal indicator in the Japanese quail was tested in the study. After the administration of this indicator, the bird expires less than 1% of the substance in form of 14CO2. The recovery of labelled methoxydextrane is 98 +/- 7%. The distribution of the indicator within the segments of the gastrointestinal tract does not differ significantly from the distribution of (14C)polyethylene glycol 4000, measured in the absence of trichloroacetic acid. The transit of the labelled methoxydextrane into the jejunum did not reduce the radio-chemical purity of the isotope, as compared with its original purity before administration. A slight decrease was only recorded during the measurement of the cumulative excretion of the indicator for 72 hours, but this decrease has no influence upon the suitability of methoxydextrane as an intestinal indicator. (Methoxy-14C)methoxydextrane does not interact with trichloroacetic acid, whereas when polyethylene glycol is used as an indicator, the substance precipitates from the liquid phase just at the concentration commonly used for the removal of protein from biological samples.

Quantification of lysine dynamics in the Japanese quail.

To cockerels of the Japanese quail four graduated flooding doses of 14C-labelled L-lysine (20-160 mg) in starch gel were administered into the crop. The animals were slaughtered between 40 and 210 min thereafter. The TCA soluble fraction of the gastrointestinal tract, of blood plasma, liver and muscle were analyzed for their lysine content and its specific radioactivity. Incorporation of radioactivity into the TCA insoluble fraction of the same organs was also followed. After quantification of lysine losses by catabolism and excretion a multicompartment model of lysine dynamics in the quail was set up. Model parameter values in general agree with independently obtained data. The necessity of model validation and extension as well as possibilities for model applications are pointed out.

Effect of antirheumatics and inhibitors of cyclooxygenase, lipoxygenases and thromboxane synthetase on cobra venom factor rat paw edema.

The cobra venom factor (CVF) rat paw edema was inhibited by cyclooxygenase (indomethacin, piroxicam, aspirin), lipoxygenase (BW 755c) and thromboxane synthetase (imidazole) inhibitors as well as by dexamethasone and, at high doses, by paracetamol and phenacetin. No definite results were obtained with slow acting antirheumatic drugs (D-penicillamine, levamisole, gold thiosulphate, chloroquine), since results varied considerably. The results are shortly discussed, with respect to the role of arachidonate lipoxygenase products as well as O-radicals.

Novel method of biotyping Haemophilus influenzae that uses API 20e.

One hundred Haemophilus influenzae isolates from various body sites were biotyped by conventional methods and by the API 20E system (Analytab Products, Plainview, N.Y.). By using a hemin- and a nicotinamide adenine dinucleotide-enriched saline solution as the inoculating fluid for the API 20E, a 100% correlation of results was obtained between the two methods. Ninety percent of the blood and cerebrospinal fluid isolates were biotype I. Biotype II was the predominant biotype encountered overall. No correlation was observed between beta-lactamase production and biotype. The API 20E is a reliable method and should prove useful for routine biotyping of H. influenzae in the clinical laboratory.

[Septo-optic dysplasia with congenital hypopituitarism (author's transl)].

The syndrome of septo-optic dysplasia with congenital hypopituitarism consists of optic nerve hypoplasia, midline malformations of the prosencephalon and hypothalamic hypopituitarism. There is great variability of these features and clinical manifestation is age-dependent: Newborns present with hypoglycemic seizures, apnea, cyanosis, hypotonia, prolonged jaundice (and micropenis in boys) because of growth hormone and/or ACTH-deficiencies. Wandering eye movements and more or less visual disturbance become evident during infancy and growth retardation even later in some cases. Early recognition is facilitated by the pathognomonic fundoscopic findings, together with normal electroretinogram, absent visually evoked potentials and computer tomography. Early hormone substitution is essential to prevent hypoglycemic damage.

[Prolonged jaundice by heterozygous alpha-1-antitrypsin-deficience? (author's transl)].

Of 28 newborns with prolonged jaundice and lowered or subnormal values of alpha-1-antitrypsin in the serum six patients were found to have a Pi-type MZ, three a Pi-type MS and one patient a Pi-type MP. One of the newborns had Pi-type SZ (double heterozygous), one infant was a homozygous carrier of the Pi-type S. It seems to the possible that one of the reasons for the not yet explained cases of prolonged jaundice of the newborns period can be found in the high frequency of homozygous or heterozygous genotype carriers of Pi-system.

[Fatal poison disease of a child following massive wasp stings (author's transl)[].

A child aged 4 years was stung by more than 250 wasps. There was no anaphylactic shock. There rather were increasingly heavy ictero-hemorrhagic symptoms with simultaneous insufficient kidney function. On the second day of the disease disorders of the CNS started, as well as circulation insufficiency and disorder of the heart function. According to the signs there was no intravasal coagulation or hemolysis. Heavy doses of corticoids were successless, just as a peritoneal dialysis starting on the 3rd day of the disease. The child died 5 and 1/2 day after the wasps' attack. The pathophysiology and the possible therapeutic consequences of the case as well as rapid protective actions in such a case of insect attack are being discussed.

Spectroscopic studies on effector-induced and substrate-induced conformation changes of phosphofructokinase.

The interaction of phosphofructokinase with NH4+, AMP, ATP, citrate, MgATP or fructose 6-phosphate, and in part with their mixtures forming either binary or ternary complexes has been studied by means of ultraviolet difference spectroscopy and circular dichroism spectroscopy in the wavelength range 265-300 nm with the aim of characterizing the conformational corollaries of the ligand effects on phosphofructokinase. The positive as well as the negative effectors change phosphofructokinase conformation in different ways, not easily interpretable in terms of one active and one inactive enzyme conformation. The spectroscopic equivalents of phosphofructokinase conformation changes resulting from catalytic activity are similar to those produced by the reaction products. The ligand concentration-dependent changes of absorption differences in the tryptophyl, tyrosyl and phenylalanyl region parallel each other, i.e. the interactions of the ligands with phosphofructokinase are not confined to specific aromatic side chains, but involve conformation changes of the large domains of the protein. ATP affinity to the enzyme shows temperature-dependent biphasic changes so that ATP binding appears to be either an entropy-driven or enthalpy-driven process. The dissociation constants of the ligands derived from spectroscopic titration of binary complex formation are comparable to those calculated from kinetic experiments. MgATP and fructose 6-phosphate each alone change phosphofructokinase conformation by binary complex formation in keeping with a random order of reaction sequence.

A mathematical model for the influence of anionic effectors on the phosphofructokinase from rat erythrocytes.

The influence of the positive effectors AMP, sulphate, glucose 1,6-bisphosphate and the negative effector 2,3-bisphosphoglycerate on rat erythrocyte phosphofructokinase has been investigated. The kinetic data have been fitted to the Monod-Wyman-Changeux model as well as to a model based on a closed association-dissociation equilibrium. The application of the fitting procedure yeilds for both models a good corresponding between theoretical and experimental data and equal results with respect to the action of the effectors on the enzyme. The corresponding dissociation constants for the binding of the positive effectors to the active state are: AMP 35 micronM, sulphate 0.43 mM and glucose 1,6-bisphosphate 15 micronM. 2,3-Bisphosphoglycerate as in inhibitor stabilizes the inactive state (dissociation constant: 1.4 mM). A preliminary discrimination between the Monod-Wyman-Changeux model and the association-dissociation model has been attempted.

An extended model of the glycolysis in erythrocytes.

An extension of a previous model [2] is proposed of the glycolysis of erythrocytes which includes realistic rat laws for the hexokinase-phosphofructokinase system and for the 2,3-P2G phosphatase. Whereas most conclusions previously drawn are reinforced, the mechanism of ATP regulation is different in the present model. The ATP concentration is mainly regulated by the inhibitory action of ATP and the activating effect of AMP on the phosphofructokinase. The role of the 2,3-P2G bypass as a buffer of changes in the ATP demand is of lesser significance than previously thought. Besides the feedback action of the adenine nucleotides on the hexokinase-phosphofructokinase system in the quasisteady state the role of 2,3-P2G as an energy source is important since it can yield ATP for a certain period of time. The present version of the model describes qualitatively the experimental data on the modulation of Na+-K+-ATPase.
