Vertebral morphometric X-ray absorptiometry (MXA): relationship with bone mineral density in perimenopausal women.

OBJECTIVE: To evaluate the relationship between vertebral morphometry and vertebral bone mineral density (BMD) in a population of perimenopausal women. METHOD: We studied 120 healthy women (mean age (S.D.) 53.4 (8.3) years) referred for baseline bone densitometry around the menopause. Morphometric X-ray absorptiometry (MXA) was performed using a Hologic QDR 4500 A apparatus and assessed the anterior, mid and posterior heights of the third lumbar vertebra (L3). The anterior to posterior height ratio (AH/PH index) and the mid to posterior height ratio (MH/PH index) were calculated. Simultaneously, BMD of L3 was measured as antero-posterior dual energy X-ray BMD (AP-BMD DXA) and as lateral DXA BMD (L-BMD DXA). Subsequently, BMD of L3 was measured using single energy quantitative computed tomography, with a Siemens Somatom device (BMD QCT). RESULTS: The AH/PH index ranged from 0.840 to 1.150, with a mean of 0.995, the MH/PH index 0.880-0.980, with a mean of 0.937. The AH/PH index showed significant correlation with either L-BMD DXA (r = 0.54, P < 0.001) or BMD-QCT (r = 0.50, P < 0.001) but not with AP-BMD DXA (r = 0.05). Using an AH/PH index threshold of 0.97 we categorized the subjects into 42 women (group A) with an AH/PH index < 0.97 and 78 women (group B) with an AH/PH index greater than or equal to 0.97. BMD values were found to be lower in group A than in group B. The difference was not significant with AP-BMD DXA (1011.5 (155.7)) versus 988.5 (193.5) mg/cm2, P = 0.47) but reached high significance (P < 0.0001) with L-BMD DXA (797.2 (166.8) versus 462.9 (131.1) mg/cm2) and with BMD QCT (135.5 (24.8) versus 83.6 (18.3) mg/cm3). CONCLUSIONS: These results suggest that the AH/PH index of L3, assessed using MXA, shows some degree of correlation with BMD in perimenopausal women.

Deep benign fibrous histiocytoma of the knee: CT and MR features with pathologic correlation.

Magnetic resonance imaging and pathologic findings in a 28-year-old patient with a giant deep benign fibrous histiocytoma in the popliteal fossa of the right knee are described. The MR imaging findings include a well-delineated oval mass with low signal intensity on T1-, and high signal intensity on T2-weighted, images, and marked peripheral contrast enhancement. To the best of our knowledge, this is the first report on the MR findings in this entity.

Reproducibility of phalanx osteosonography and relation with forearm peripheral quantitative computed tomography: single finger versus average measurement on the last four fingers.

The aim of this study was to evaluate the in vivo short-term reproducibility of amplitude-dependent speed of sound (AD-SOS) measured at the proximal phalanx, using an Igea DBM Sonic 1200 device, as well as its relationship with bone mineral density of the ultradistal radius, assessed using peripheral quantitative computed tomography (pQCT). In particular, the authors wished to evaluate whether average measurements on the last four fingers, as recommended by the manufacturer, yielded or not more information than measurements on one single finger. For the precision study, the authors carried out phalanx osteosonography ten times once every other day in ten healthy young women. To compare with pQCT of the radius, 37 women referred for screening of osteoporosis (mean age 60 years) were studied. AD-SOS was reported for each finger separately, as well as an average; pQCT (XCT 960 Stratec) yielded trabecular (BMDt); cortical (BMDc); and total (BMDtot) bone mineral densities, as well as the cortical area (Ac). The reproducibility of AD-SOS was significantly higher for the middle finger (mean coefficient of variation (CV) 1.1%) and the index than for the ring and small (mean CV 2.7%) finger. The middle finger consistently showed the best correlations with pQCT variables; the relationships involving BMDc (r = 0.54), BMDtot (r = 0.62) and Ac (r = 0.61) were stronger than that with BMDt (r = 0.34). The authors conclude that phalanx AD-SOS is moderately correlated with BMD of the ultradistal radius--at least when it includes the cortical component--and that reporting AD-SOS for the middle finger--instead of an average value--improves the reproducibility and possibly the relevance of phalanx osteosonography.

MRI of extrapancreatic gastrinomas.

Preoperative diagnosis of gastrinoma--especially extrapancreatic gastrinoma--is very difficult. At the moment octreotide scintigraphy is considered the method of choice to detect gastrinomas. However, MRI may play a major role in the diagnostic imaging of gastrinoma.

Benign papilloma of the bladder in a young adult.

Ultrasonographic and pathologic findings in a 26-year-old patient with a benign papilloma of the bladder are described. Ultrasound features include an irregular polypoid mass extending from the bladder wall.

Ultrasound studies in ectopic pregnancies.

Early diagnosis of ectopic pregnancy reduces the mortality and morbidity associated with the disease, and the morbidity associated with the therapy. When diagnosis was limited to the cases presenting with tubal rupture, salpingectomy was the only possible treatment. If ectopic pregnancy can be diagnosed earlier, clinicians are able to use new therapies such as laparoscopic microsurgery or medical therapy with methrotrexate, which are less invasive and less tissue destructive. Transvaginal ultrasound has proven to be an essential tool in the early diagnosis of ectopic pregnancy. Colour Doppler capacities further enhance the diagnostic sensitivity of transvaginal ultrasound for the early recognition of abnormal and normal intrauterine pregnancy, and small extrauterine masses. The aim of this paper is to provide an overview of the major sonographic signs and pitfalls in the ultrasound diagnosis of ectopic pregnancy.

Calcium hydroxyapatite deposition disease of the neck: finding in three patients.

Calcium hydroxyapatite deposition disease may occasionally involve the superolateral portion of the longus colli muscle. Clinical symptoms include headache, neck pain, and dysphagia. Characteristic prevertebral calcifications with the epicenter at the C1-C2 level are depicted on lateral radiographs of the cervical spine.

Glenoid dysplasia: radiographic and CT arthrographic findings.

A 60-year-old man with dysplasia of the glenoid and a rotator cuff tear is reported on. Because of the tear, CT arthrography was performed, offering an opportunity to evaluate the various soft tissue abnormalities accompanying glenoid dysplasia. Marked thickening of the glenoid cartilage and labrum was apparent, as well as an abnormal posterolateral orientation of the glenoid cavity. A deep notch was observed along the central portion of the glenoid cartilage. Furthermore the middle glenohumeral ligament appeared thickened and cordlike.

Calcific tendinitis of the longus colli muscle.

BACKGROUND: To the best of our knowledge, only one patient with calcium hydroxyapatite deposition disease (CHADD) of the longus colli muscle has been reported in the otolaryngology literature. METHODS: Clinical findings and results of imaging studies in such a patient are reported. RESULTS: Two particular imaging findings of this disorder are discussed. First, disappearance of calcifications over the short time frame of a week, accompanied by the resolution of clinical symptoms. Second, the presence of an extensive region of high signal intensity extending from the skull base to the lower border of C5, on T2-weighted spin-echo magnetic resonance (MR) images. CONCLUSIONS: Knowledge of the characteristic clinical spectrum and imaging features of this disorder are crucial for a correct diagnosis of this uncommon cause of odynophagia and dysphagia.

A comparison between different imaging strategies for diffusion measurements with the centric phase-encoded turboFLASH sequence.

The study compared the results of three centrally reordered phase-encoded turboFLASH sequences for diffusion-weighted imaging (DWI). The sequences were conventional turboFLASH, turboFLASH with subtraction of T1-related effects, and turboFLASH with correction for T1-related effects during the imaging period only. The relative merits were studied with respect to image quality and accuracy by computer simulation and by experimental validation on phantoms and on in vivo rat brain. A T1-related underestimation of the diffusion coefficient ranging from -30% (T1 approximately 200 ms) to -5% (T1 approximately 1 s) was found to exist for the conventional sequence. Image artifacts, caused by longitudinal relaxation during the imaging period, are reflected in calculated diffusion maps. When the correction sequence is used, the artifacts and the systematic errors are reduced but longitudinal relaxation during the delay between preparation and imaging periods remains large enough to induce significant errors (-15% for T1 approximately 200 ms to -3% for T1 approximately 1 s). The subtraction sequence eliminates the influence of T1 effects on the calibrations, but leads to identical artifacts for all diffusion-weighted images.

Calcified, cystic brain metastases.

Calcifications are uncommon in metastases. Adenocarcinoma, osteopenic, osteosarcoma, lung and breast carcinoma can be the origin. A rare case of calcified cystic brain metastasis deriving from papillary cystadenocarcinoma is reported.

Imaging in sports-medicine--knee.

Since the last decade, the introduction of Magnetic Resonance Imaging has provided a powerful new tool for the clinician to diagnose sports-related knee injuries. The main objective of this paper is to familiarise the relevant specialists with the proven clinical indications and semiology of MRI of the knee. At the present time, the knee joint is the area in which the advantages of MRI are most spectacularly apparent. The MR appearances of meniscal tears in particular, are first discussed in detail, referring to recent literature and personal experience based on more than ten thousands MR examinations of traumatic knees. Also the mechanisms of injuries and the most commonly involved sports are described. The precise types of meniscal lesions, as visualised at arthroscopy, are abundantly illustrated on MR images: e.g., bucket-handle, radial and horizontal cleavage tears, meniscocapsular separations, discoid menisci and meniscal cysts. We discuss the numerous pitfalls due to neighbouring anatomical structures as well as non clinically or surgically significant intrameniscal hypersignals in athletes. Towards other imaging techniques, MRI is particularly suited for the diagnosis of ligamentous injuries, especially the cruciate ligaments. We develop the currently used direct and indirect signs of rupture, the diagnostic difficulties related to the age of trauma and the detection of associated lesions. Sport-induced tendinous and bursal pathology is another interesting indication of MRI, not only lesions of the more superficial extensor tendons but also the pes anserinus or hamstring tendons. We finally treat the recent developments of MRI in assessment of cartilage, subchondral and medullary bone disorders of traumatic or microtraumatic origin.

Effect of amino acid administration on uremic muscle metabolism: a 31P-spectroscopy study.

The effect of a six-month peritoneal amino acid administration on muscle metabolism at rest and during exercise was examined in 12 patients (4 control and 8 amino acid treated) on continuous ambulatory peritoneal dialysis. 31P-magnetic resonance spectroscopy was used to measure several high-energy phosphates (PCr, Pi) in resting muscle and during exercise and recovery. At rest, no significant changes were detected between the non-treated (control) and the amino acid treated (experimental) group. Before the administration of amino acids, the exercise-induced fall in [PCr], the increase in [ADP] and [Pi] and the pH were not significantly different in the control and experimental group. The initial rate of PCr recovery and the calculated maximal rate of ATP-synthesis (Qmax) for the control subjects was not significantly different at the onset and the end of the study. In the treated group, however, the fall in [PCr] and increase in [ADP] after exercise were significantly lower after than before treatment, while [Pi] and pH were identical. The initial rate of PCr recovery and Qmax were also significantly improved. These changes indicate an improved oxidative phosphorylation under the treatment and suggest that the impaired oxidative metabolism of the dialysis patients could be the result of their bad nutritional state.

Picture archiving and communication system (PACS): medical perspectives.

PACS represents the natural evolution from the digital new modalities (US, CT, MRI,...) to a global digital environment, where the film based activities are progressively replaced by their digital counterpart. The advantages of the technique, as well as the drawbacks of the first implementations, are described. The "second generation" PACS concept is presented (modular architecture, progressive implementation, multivendor environment, integration with the Hospital Information System, standardization,...). As "case study", the example of the A.Z.-V.U.B. Hospital Implementation is described.
