Delayed appearance of the catalytic activity by immunization of a rabbit compared with the hapten binding.

Polyclonal antibodies catalyzing the hydrolysis of carbonate ester were generated by immunizing a rabbit with hapten(4-nitrophenyl phosphate II) conjugated to keyhole limpet hemocyanin. The hydrolytic activity of IgG purified from antisera exhibited plateu one month later than the simple hapten-binding. The affinity of IgG with substrate increased even after the hapten-binding reached plateu. These suggest a strategy to generate good polyclonal catalytic antibodies and the day to fuse spleen cell with myeloma cell to get good monoclonal catalytic antibodies.

Antithrombotic effects of recombinant human soluble thrombomodulin in a rat model of vascular shunt thrombosis.

Thrombomodulin on endothelial cells is a cofactor for thrombin-catalyzed activation of protein C. We have investigated the anticoagulant function of recombinant human soluble thombomodulin (rsTM) in a rat model of arterio-venous (AV)-shunt thrombosis. A bolus injection of rsTM 30 s before the induction of AV-shunt thrombosis inhibited the thrombus formation in a dose-dependent manner. The dose of anticoagulant that inhibited thrombus formation by 50% was 0.4 mg/kg rsTM alpha, 0.15 mg/kg rsTM beta, and 13 U/kg heparin. Recently, we characterized three monoclonal antibodies (moAbs) against human TM whose epitopes are located in the TM epidermal growth factor-like domain (Nawa et al., 1994). moAb 2A2 inhibited thrombin binding to rsTM, and abolished both TM functions as a cofactor in thrombin-catalyzed activation of protein C and as an anticoagulant by modifying thrombin-induced fibrinogen clotting and platelet aggregation. moAb 1F2 preserved the latter activities as an anticoagulant, but inhibited cofactor activity. moAb 10A3 had no inhibitory effect on either activity. Analysis of the in vivo anticoagulant mechanism of rsTM was facilitated by the availability of these moAbs. After incubation at rsTM/moAb molar ratios of 1:1.25, the effect of the mixtures were examined in the AV-shunt thrombosis model. An injection of 0.8 mg/kg rsTM alpha or 0.4 mg/kg rsTM beta resulted in a significant reduction on thrombus formation, as expected. moAb 10A3 had no effect on rsTM activity. However, co-injection of rsTM with moAb 1F2 resulted in a significant decrease of the inhibitory activity on thrombus formation. moAb 2A2 essentially abolished the inhibitory effect of rsTM.(ABSTRACT TRUNCATED AT 250 WORDS)

[A case of left main coronary artery stenosis: additional insertion of saphenous vein graft to discontinue cardiopulmonary bypass].

A 62-year-old man with the lesions of the left main coronary artery and all the three vessels underwent a complete revascularization, in which the left internal mammary artery (IMA) was anastomosed to the large LAD. Although the operation proceeded uneventfully, the patient could not be weaned from the cardiopulmonary bypass (CPB). Another saphenous vein graft insertion into the LAD just distal to the IMA made it possible to discontinue the CPB. Postoperative angiogram revealed the patency of all grafts. But the IMA graft showed flow competition and string sign. We assume that failure to wean the patient from the CPB might be caused by hypoperfusion of the IMA graft, and in such a case, insertion of a saphenous vein graft is effective.

Stereotyped behavioral responses to an auditory stimulus in the course of repeated treatment with methamphetamine plus scopolamine and methamphetamine in rats.

The present experiment was designed to study the effects of an auditory stimulus with 300 Hz, 100 dB on gradual emergence of behavioral sensitization in the course of repeated administration of methamphetamine (MAP) in combination with scopolamine (SCOP) and MAP. Repeated intraperitoneal injection of MAP (4.0 mg/kg) plus SCOP (0.5 mg/kg) induced significantly progressive and enduring enhancement of stereotyped behavior, resulting in intense, focussed stereotypy intermingled with gnawing and licking compared to repeated MAP treatment. Vigorous stereotyped behavior induced by repeated MAP plus SCOP was progressively attenuated by the tone, resulting in attenuation of the development of behavioral sensitization. Conversely, the tone had no invariable effects on behavioral sensitization induced by repeated MAP treatment. The present findings indicate that repeated MAP plus SCOP treatment progressively induced attenuating effects of tone as low as 300 Hz on MAP plus SCOP-induced intense stereotypy and vigorous behavioral sensitization. These findings suggest that a reciprocal balance between dopaminergic and cholinergic inhibitory systems may be involved in producing behavioral sensitization, and in the attenuating effects of the tone on MAP plus SCOP-induced vigorous behavioral sensitization.

[Evaluation of local treatment for unresectable primary liver tumor].

To ascertain the effect of local treatment for unresectable primary liver tumor, 59 patients were investigated retrospectively. Patients were classified into four groups; transcatheter arterial embolization (TAE) group, intermittent intra-arterial infusion chemotherapy group, combined therapy group (TAE+intermittent intra-arterial infusion chemotherapy) and a group without adjuvant therapy. The results revealed that TAE and intermittent intra-arterial infusion chemotherapy both prolonged the survival period. We found that the survival rate depends largely on the therapeutic method. No correlations were confirmed with the liver function nor stage grouping of the tumor. Therefore, we concluded that intermittent intra-arterial infusion chemotherapy is a beneficial treatment, considering its minimal adverse effect, broad indication and the fact that it does not require hospitalization.

[A new index of acceleration plethysmogram and its clinical physiological evaluation].

The purposes of the present study were to examine a new index of acceleratingly plethysmogram (APG) and to discuss the possibility to apply the APG index. One hundred sixty-three subjects voluntarily participated in the present study. The mean values (n = 10) of individual coefficient of variance were 7.22% in wave height B, 8.26% in the total area of the lower part under the base line, and 7.28% in a new APG index. The coefficient of variance in the APG index calculated from the previous method (18.67%) was larger than that of the present study. We recognized the significantly polynomial relationship between the new APG index and types of APG. Further, the value of new APG index remarkably increased when the index was beyond 50. There were significant differences in age, blood pressure, rate of body fat, blood glucose, total cholesterol, HDL cholesterol, and index of arteriosclerosis between above 50 and below 50 in the new APG index. These results suggest that the present APG index is adequate about about the individual reproducibility, and it has the possibility to apply as an index of arteriosclerosis.

[Surgical intervention to tuberculous pericarditis: a case report].

A 32-year-old man was hospitalized because of cardiac tamponade with the thickened visceral and parietal pericardial layers. Marked high level of adenosine deaminase activity (ADA) in pericardial fluid strongly suggested the diagnosis of tuberculous pericarditis, which was later assured by positive smear and culture of the pericardial fluid for Mycobacterium tuberculosis. Before rigid adhesion of the pericardial space was established, pericardiectomy was easily performed with satisfactory improvement of the hemodynamic status and physical symptoms of the patient. The present case suggests the efficacy of early surgical intervention to tuberculous pericarditis when it shows a sign of rapid progression to constrictive pericarditis. Furthermore, high ADA in pericardial fluid might be an useful diagnostic modality for tuberculous pericarditis.

[Adverse effect of internal thoracic artery steal on perioperative hemodynamic condition--a report of three cases].

In three patients who had suffered from perioperative hemodynamic deterioration after myocardial revascularization with an internal thoracic artery to the left anterior descending coronary artery (LAD), blood steal from the ITA into large side branches was strongly suspected. In two of them an intractable ventricular fibrillation occurred before closure of the chest. Additional insertion of saphenous vein graft to the LAD enabled them to get out of jeopardy. Postoperative angiographic studies demonstrated large pericardial branches which had several fistulous communications with the pulmonary circulation. The other suffered from anteroseptal myocardial infarction immediately after an uneventful operation. Postoperative angiography revealed an enlarged lateral costal branch which gave off four intercostal arteries. Careful and meticulous harvesting of the ITA is mandatory to avoid tragic consequences caused by its large side branches.

[123I-MIBG scintigraphy in cardiotoxicity related to antineoplastic agents].

The purpose of this study is to determine the value of 123I-MIBG scintigraphy to evaluate cardiotoxic effects of antineoplastic agents. We performed 123I-MIBG scintigraphy in 19 patients with malignant neoplasms treated with multiagent chemotherapy including adriamycin. Based on the initial and delayed myocardial images (planar and SPECT), segmental myocardial uptake and wash-out ratio of MIBG were obtained. Low uptake in the myocardium and rapid wash-out from the myocardium especially in the lateral and inferior segments, were noted in patients with cardiotoxicity. Such cardiotoxic effect is dose-dependent. This abnormal areas corresponded to decreased glucose metabolism observed by 18F-FDG PET images. This was suggestive of mitochondrial dysfunction or myocardial damage. There was mismatch in the defect when compared to the result of 201Tl perfusion scintigraphy, and LVEF obtained failed to demonstrate abnormalities. Therefore, 123I-MIBG scintigraphy may be an useful measure in detecting cardiotoxicity related to antineoplastic agents in early stage.

[The usefulness of pre- and immediately postoperative chemotherapy in colorectal cancer].

In order to investigate the usefulness of pre- and immediately postoperative chemotherapy in colorectal cancer, 35 patients that underwent curative resection from April 1988 to March 1990 for the tumors having serosal (a2, s) or subserosal (a1, ss) invasion were divided into three groups (A, B and C) according to the treatment modality with anticancer drugs. In group A (9 cases), 400 mg of UFT was given orally for 7 days preoperatively; on the day of operation bolus 20 mg of MMC and the day after operation bolus 10 mg of MMC were given intravenously. After that, 800 mg of FT was infused intravenously for 2 weeks followed by daily oral administration of 400 mg of UFT. In group B (12 cases), bolus 20 mg of MMC on the day of surgery and bolus 10 mg the day after surgery were given by vein. Moreover, 400 mg of UFT was orally administered daily from 2 weeks after surgery. In group C (14 cases), 400 mg of UFT alone was given daily orally from 2 weeks after surgery. The three-year survival rates of group A, B and C were 100%, 91.7%, and 92.9%, respectively. The recurrence rates in the three groups were 0%, 16.7%, and 28.6%, respectively. Although statistical significance was not observed, the prognosis of group A seemed to be favorable. No recurrence was observed in group A, and the recurrences in group B and C were entirely hepatic metastasis. Accordingly, preoperative chemotherapy with immediately postoperative chemotherapy seems to be effective in the prevention of hematogenous spread in colorectal cancer.

[A case of gastric cancer completely responding to sequential methotrexate and 5-fluorouracil therapy].

A case of an 81-year-old male with Borrmann II type was reported. The lesion was diagnosed histologically as undifferentiated adenocarcinoma. Biochemically modulated chemotherapy preceded by administration of Methotrexate (MTX) and followed by 5-fluorouracil (5-FU) was given to him. The procedure was weekly intermediate-dose therapy consisting of MTX 100 mg (i.v.) followed 3 hours later by 5-FU 600 mg (i.v.) with leucovorin rescue in the following 2 days. After this treatment was repeated 6 times, advanced gastric cancer disappeared on endoscopic and histological findings, so the patient was regarded as showing a complete response. Ths, this biochemically modulated chemotherapy was thought to be in effective method against gastric cancer.

[Mitral valve replacement and closure of atrial septal defect in the case associated with Marfan syndrome].

This is a case report of 8-years-old boy, who was diagnosed mitral regurgitation, secundum type of atrial septal defect and annulo-aortic ectasia in association with Marfan syndrome. His diagnosis was confirmed at the age of 6 years when cardiac catheterization was carried out. Atrial septal defect was closed using PTFE patch (0.2 mm in thickness). His diseased mitral valve was replaced with CarboMedics prosthesis 27 mm in diameter. Because of intact aortic valvular function, we left the ectatic aorta alone at present time for future procedure. His postoperative course was uneventful. He is active and doing well at present and has been followed-up at the outpatient clinic routinely. He might be having to require AVR and graft implantation for ectatic ascending aorta.

Recombinant rat protein C: comparative studies of structure, function and synthesis with plasma protein C.

In order to elucidate the role of protein C (PC) in the rat, we expressed, purified, and characterized recombinant rat PC. The purified recombinant rat PC was 70-90% two-chain (41 kDa heavy chain; 22 and 23 kDa light chain) and 10-30% single-chain (61 kDa). Amino acid analysis confirmed the presence of 10 moles of gamma-carboxyglutamic acid residues per mol of protein. For comparison, plasma rat PC was purified from a barium citrate precipitate using similar method. Plasma rat PC was a two-chain form (41 kDa heavy chain; 22 kDa light chain) with no detectable single-chain nor 23 kDa light chain. For determination of the in vitro secreted species, primary cultured rat hepatocytes were incubated for 6 h with methionine-free MEM containing vitamin K1, aprotinin, and [35S]methionine. The supernatant was immunoprecipitated and analyzed by SDS-PAGE followed by autoradiography. Approximately 90% of the PC radioactivity migrated as a two-chain molecule. These results indicate that rat PC is secreted mainly as a two-chain molecule from the liver. PROTAC-activated forms of recombinant rat PC, plasma rat PC, and plasma human PC hydrolyzed the S-2366 chromogenic substrate at the same rate. Recombinant rat PC was also activated by the thrombin-thrombomodulin complex at a rate similar to plasma rat PC. The anticoagulant activities of the three activated PCs were examined in rat plasma. Both recombinant and plasma rat PC prolonged the activated partial thromboplastin time in a dose-dependent manner, but plasma human PC was less effective.(ABSTRACT TRUNCATED AT 250 WORDS)

[Hypothermic cardiopulmonary bypass with circulatory arrest of the lower half of the body and selective perfusion of the upper and lower half of the body for thoracic aortic aneurysm operation].

A 62-year-old man with thoracoabdominal aortic aneurysm of Crawford type IA and a 61-year-old woman with triple descending thoracic aortic aneurysms were operated on under total cardiopulmonary bypass with hypothermic circulatory arrest of the lower half of the body (rectal temperature of 17-19 C) and low flow selective perfusion of the upper and lower half of the body. Low porosity woven Dacron UBE grafts without preclotting were used for the replacement of the aorta. Postoperative courses were uneventful without paraplegia and angiographic examination revealed excellent hemodynamic results.
