An unusual case of adrenal failure due to isolated metastases of breast cancer.

We describe a patient affected by adrenal insufficiency due to metastases of breast cancer. Adrenal involvement became clinically evident 20 years after radical mastectomy and it was the only secondary localization of the tumor. There is still no evidence of other metastases after a two year follow-up. This case suggests that adrenal function evaluation should be included in periodical follow-ups of patients who underwent radical mastectomy for breast cancer.

Seroprevalence of Chlamydia pneumoniae antibodies in a young adult population sample living in Verona. European Community Respiratory Health Survey (ECRHS) Verona.

UNLABELLED: The aim of the study was to evaluate the prevalence of antibodies to Chlamydia pneumoniae in a random population sample of 369 young adults (aged 20-44 years), living in Verona, Italy. IgG and IgM titers were measured by micro-immunofluorescence. IgG antibodies, greater or equal to 16, were found in 104/177 (58.8%) men and 76/192 (39.6%) women (p < 0.001). No relationship was found between IgG seropositivity, age, social class, education and family size. Factors positively associated with IgG seropositivity included smoking (p < 0.001), occupational status (employed vs unemployed: p = 0.02; students vs unemployed: p < 0.01) and living area (suburban [65.0%] vs urban area [45.3%]: p = 0.03). The geometric mean of IgG titers was higher in students (GM: 26.05) than in both employed (GM: 11.02) and unemployed persons (GM: 4.80) (p < 0.01 and p < 0.001, respectively). IgG titres > or = 512 and/or IgM titers > or = 16 (suggestive of a recent C. pneumoniae infection) were found in 39 subjects (10.6%). Recent infection was more frequent in spring (14.9%), with no significant variation in the other seasons (mean prevalence 6.7%) (p < 0.01). Recent infection was also associated with cigarette smoking. On the other hand, no significant association was found between respiratory symptoms and serologic evidence of recent infection. IN CONCLUSION: 1) the prevalence of antibodies to C. pneumoniae in young adults from Verona is similar to that found in European countries, and therefore, in Europe, it seems not related to latitude or climate; 2) male sex, tobacco smoking, employment status and living in a suburban area are independent risk factors of infection; 3) the infection is subclinical in most cases.

Local nasal immunotherapy and bronchial hyperreactivity in seasonal allergic rhinitis: an observational pilot study.

The clinical efficacy of local nasal immunotherapy (LNIT) in patients with allergic rhinitis is amply documented. The aim of the study was to determine the effect on bronchial hyperresponsiveness (BHR) assessed at baseline and after 3 years of LNIT or pharmacological treatment alone. Forty-three randomized patients with allergic oculorhinitis were enrolled (26 positive to Graminaceae and 17 to Parietaria judaica pollens). All patients were asked whether they were willing to follow a 3-year treatment course involving preseasonal LNIT with a powder extract of Graminaceae or Parietaria pollens. Twenty-four patients (16 allergic to Graminaceae and eight to Parietaria) selected LNIT and the other 19 opted for symptomatic pharmacological treatment only. The latter was considered the control group. On the basis of positive or negative bronchial hyperresponsiveness and the LNIT, four subgroups were established and followed in open conditions, during which a record was kept of symptom scores, drug use, spirometry and methacholine test findings. After 3 years, patients treated with LNIT had a significant reduction of symptoms and drug intake. In the controls, symptoms worsened, thus requiring more drugs to control them. Bronchial hyperresponsiveness significantly improved in hyperreactive patients receiving LN7IT 10 of whom were no longer hyperreactive and one at a higher threshold. Among controls, bronchial hyperresponsiveness did not change, with the exception of three nonreactive patients who became hyperreactive, one of them with asthma. These findings confirm the efficacy of LNIT in allergic rhinitis suggesting that it might have systemic activity interfering with bronchial hyperresponsiveness and hence the onset of bronchial asthma.

[A century of legislation as regards Italy-Argentine immigration, 1860-1960].

This paper analyzes the changes in Argentine laws regarding migration, mainly in the hundred years since the Argentine Constitution was passed until the 1950s. The different subsequent laws and the debates they caused are related by the author to the political and ideological trends dominating Argentina in that period as to the characteristics and oscillations of the migratory afflux. Further the paper considers Italian laws and migration policies during the same period. The author concludes that after a liberal migration policy in the 19th century, growing legal restrictions and passionate discussion on the role of immigrants followed in this century, particularly between the 2 World Wars.

Influence of gastric pH changes on pharmacokinetic of a sustained-release formulation of theophylline.

The effect of magnesium-aluminum-hydroxide antacid (Maalox) on the oral absorption of theophylline anhydrous time-release capsules Somophyllin CRT was studied in eight healthy volunteers. The subjects were given 500 mg of theophylline alone or with 30 ml of antacid in a complete crossover design. Drug serum concentrations were determined by an immunoenzymatic method within 24 hours at frequent intervals. Antacid induced a significant reduction of theophylline serum levels and of the fraction absorbed at 1 and 1.5 hours of the monitoring period (less than 0.05), but no significant differences were found at later sampling time. Maalox did not influence the area under the curve from 0 to 24 hours and the peak concentrations; the time to peak was slightly but significantly longer with antacid (p less than 0.05). Our results indicate that physicochemical factors such as gastrointestinal fluid pH do not influence substantially absorption of the slow-release formulation Somophyllin.

[Gianotti-Crosti acrodermatitis. Description of a case of rare etiology].

Originally described in 1955, papular acrodermatitis is considered a rare disease whose manifestations are mainly cutaneous. The disease is characteristically associated to a non icteric hepatitis with B serologic tests positive. We recently observed a case whose etiology is unusual and, therefore, in our opinion worth of a brief description.

[Facio-auriculo-vertebral syndrome: description of a case].

The authors report the case of a fourteen months old girl who presented with emisomal, uropoietic and vertebral anomalies associated with neurologic handicap. These features can be considered characteristic of facio-auriculo-vertebral spectrum. This syndrome was not frequently described in Italian pediatric literature.

[Difficult diagnosis in a case of hematuria (Berger's disease)].

A seven years old boy presented with gross hematuria associated with clots and transitory renal insufficiency. The urinary sediment showed only monomorphic red blood cells. Subsequently urinary sediment completely modified: namely only dysmorphic red blood cells and hyaline granular casts were found and within a week renal function become normal. The clinical diagnosis of Berger's disease was possible only with renal biopsy.
