The suppressive effect of mangiferin with exercise on blood lipids in type 2 diabetes.

The effect of mangiferin (MF) with exercise on bood lipids was studied in KK-Ay mice, an animal model of type 2 diabetes. MF (30 mg/kg) reduced the blood cholesterol (p<0.05) and triglyceride level (p<0.01) of KK-Ay mice with exercise 2 weeks after oral administration when compared with the control group. Diabetes also often has elevated lipid levels. Therefore, it may be that MF has beneficial effects on hyperlipidemia in type 2 diabetes.

Novel malonamide derivatives as alpha v beta 3 antagonists. Syntheses and evaluation of 3-(3-indolin-1-yl-3-oxopropanoyl)aminopropanoic acids on vitronectin interaction with alpha v beta 3.

In attempt to find novel integrin alphavbeta3 antagonists, we selected SC65811 and its guanidine analogue (1) as lead compounds. Modification of the glycine part of SC65811 led to a new series of malonamide derivatives that exhibited alphavbeta3 inhibitory activity. Among them, (R,S)-3-[3-[6-(3-benzylureido)indolin-1-yl]-3-oxopropanoylamino]-3- (pyridin-3-yl)propanoic acid (43a) showed not only potent activity with an IC50 value of 3.0 nM but also good selectivity for alphavbeta3 relative to alphaIIbbeta3, alpha5beta1, and alphavbeta5 with IC50 values of 19,000, 11,000, and 14 nM, respectively. Furthermore, optimization of 43a led to the most potent alphavbeta3 antagonist, (R,S)-3-(3-[6-[(4,5-dihydro-1H-imidazol-2-yl)amino]indolin-1-yl]-3-oxopropanoylamino)-3-(quinolin-3-yl)propanoic acid (431) with an IC50 value of 0.42 nM. The synthesis and the structure-activity relationships of these malonamide derivatives are presented.

The circadian blood pressure rhythm in non-diabetic hemodialysis patients.

This study investigates the circadian blood pressure variation of non-diabetic chronic hemodialysis (HD) patients on both HD and non-HD days as well as the factors affecting diurnal BP variation. Forty-nine HD patients aged 61.8 +/- 12.9 years who were on daytime HD for 97 +/- 68 months were studied. No significant difference was found in every daytime and nighttime BP between the first (HD) and the second (non-HD) day. However, the ratio nighttime/daytime BP was significantly higher on the second day. Each BP diurnal variability pattern was classified as either Dipper (D: the ratio nighttime/daytime mean BP 0.8-0.9), non-dipper (0.9 < ND < 1.0), or inverted dipper (ID > 1.0). More than 75% of the cases were classified as ND (26 cases) or ID (11 cases). The ultrafiltration rate in D was significantly less than that in ND and ID. The difference of plasma renin activity between pre- and post-HD (dRen) was significantly higher in ID than in D and ND. The amount of dialysis (Kt/V) was found to be significantly correlated with nighttime BP fall. Ultrafiltration, dRen and Kt/V were independent factors for the abnormal BP diurnal variability. In conclusion, the decreased nocturnal BP fall seen in non-diabetic HD patients is associated with increased extracellular fluid even in the patients without overt overhydration, whereas relatively insufficient amount of dialysis (low Kt/V) may be another possible cause. The increased dRen observed only in ID patients may reflect occult cardiovascular damage or functional disturbances in aortic and carotid baroreflexes caused by arterial structural changes.

Immunohistochemical characterization of hepatoblastomas in B6C3F1 mice treated with diethylnitrosamine and sodium phenobarbital.

Hepatoblastomas (HBs) were induced in B6C3F1 male mice by diethylnitrosamine (DEN) and sodium phenobarbital (PB). Six-week-old mice received a single intraperitoneal dose of DEN followed by a continuous treatment with PB in diet at a concentration of 0 (group 1) or 500 (group 2) ppm for 50 weeks. HBs were observed in 13 of 21 (62%) group 2 mice, with typical histologic features as reported previously, while no such tumors were observed in group 1. Seven of 13 (54%) HBs were found in and/or adjacent to hepatocellular adenomas (HCAs) or hepatocellular carcinomas (HCCs). Immunohistochemically, all HBs were positive for S-100 protein but negative for keratin, alpha-fetoprotein (AFP), albumin (ALB) and vimentin, while HCC cells occasionally reacted positively for AFP with a mosaic pattern. HCC and HCA cells were occasionally positive for ALB. Non-neoplastic hepatocytes and normal bile ducts were positively stained for ALB and keratin/S-100 protein, respectively. S-100 protein is known to be expressed in many mesenchymal tissues and neoplasms including neuroectodermal elements but negative in cells of the hepatic lineage. Thus, the present immunohistochemical results suggested that mesenchymal differentiation occurs in mouse HB cells as observed in human HBs, one of the most frequent infant liver tumors in humans. Although the susceptibility of mouse HBs to PB-promotion suggests a hepatocytic histogenesis, the present immunohistochemical results support the hypothesis that the mouse HB is derived from pluripotent endodermal stem-like cells.

Immunohistopathologic demonstration of pleuropneumonia associated with Morganella morganii in a piglet.

Serofibrinous pleuropneumonia in a piglet was examined microbiologically and immunohistopathologically. Large numbers of Morganella morganii were isolated from the pneumonic lesion, but no other pathogens were identified. A large amount of M. morganii antigen was demonstrated, and its distribution was closely associated with the histologic lesion. This finding suggests that pleuropneumonia in piglets might be caused by M. morganii.

The effect of cardiomyoplasty on left atrial function in experimental canine models.

BACKGROUND: Cardiomyoplasty utilizes the latissimus dorsi muscle to boost the failing ventricle. However, the mechanism for clinical improvement still remains controversial. We have previously shown that left ventricular contraction was improved in the long-term periods after cardiomyoplasty in the normal canine heart model and in the dilated failing heart model. On the other hand, right ventricular filling was impaired if a rapid volume loading test was employed in the long-term period after cardiomyoplasty. The purpose of the present study was to determine whether cardiomyoplasty impairs the left atrial function and affects ventricular filling. METHOD: Eleven mongrel dogs that had undergone cardiomyoplasty (study group, n = 5) or a sham operation (control group, n = 6) were studied for 12 weeks postoperatively. An IV 4.5% albumin solution (10 mL/kg) was infused into the right atrium, and hemodynamic changes in right cardiac catheterization and left atrial volume (LAV) were obtained by two-dimensional echocardiography. Atrial function was assessed by hemodynamic changes in Doppler echocardiography and hormonal changes. RESULTS: Stroke volume was significantly increased, from 17.0 +/- 4.4 to 21.1 +/- 7.0 mL, respectively, before and 15 min after albumin infusion in the control group (p < 0.05). Heart rate and cardiac output were significantly increased, from 131.2 +/- 18.1 to 152.0 +/- 9.8 beats/min and 2.72 +/- 1.29 to 4.03 +/- 1.67 L/min, respectively, before and 15 min after albumin infusion in the study group (p < 0.05). No changes were observed in mean right atrial pressure and pulmonary capillary wedge pressure. LAV and atrial natriuretic peptide (ANP) levels increased significantly, from 5.8 +/- 2.1 to 8.5 +/- 3.8 mL and 22.5 +/- 7.5 to 44.5 +/- 31.7 pg/mL, respectively, before and 15 min after albumin infusion in the control group (p < 0.05). In the study group, LAV and ANP levels were also increased, from 10.1 +/- 2.4 to 12.7 +/- 2.8 mL and 64.2 +/- 60.6 to 232.6 +/- 272.2 pg/mL, respectively, before and 15 min after albumin infusion (p < 0.05). The peak velocities and the time-velocity integrals in the pulmonary venous flow of the systolic and diastolic waves, as well as their ratios (systolic to diastolic peak velocity ratio and systolic to diastolic time-velocity integral ratio) showed no significant differences between the two groups. CONCLUSIONS: Cardiomyoplasty preserves left atrial filling and transport function; therefore, cardiomyoplasty may also activate ANP production by stimulating the atrium in the long-term period after cardiomyoplasty.

Application of molecular orbital calculations to interpret the chlorophyll spectral forms in pea photosystem II.

The energy and oscillator strength of electronic transitions of chlorophyll (Chl)-amino acid complexes were calculated by using molecular orbital methods. The energies varied widely with coordinated amino acids and the difference between the maximum and minimum energy was about 830 cm-1. This energy difference was comparable with the spreading of absorption bands for light-harvesting Chl-protein complexes of photosystem II (LHC II) of green plants. The feature of the Qy band for pea LHC II was interpreted with the aid of the calculated energies and oscillator strengths. Four spectral components of the band were assigned to individual Chl-amino acid complexes.

Clinical periodontal findings and microflora profiles in children with chronic neutropenia under supervised oral hygiene.

BACKGROUND: This is the first known case report that used a polymerase chain reaction (PCR)-based method to help identify the oral microflora in patients with chronic neutropenia. In this study, we report clinical periodontal findings and microflora profiles of 2 children, 1 with severe congenital neutropenia (SCN, Kostmann type) and 1 with cyclic neutropenia (CN). METHODS: The SCN patient had severe gingivitis, whereas the patient with CN had mild gingivitis in the gingival margins. Monthly oral cleaning instruction and review were performed without subsequent periodontal therapy. Oral hygiene conditions remained satisfactory and visible plaque was scarce, despite the persistence of mild gingivitis. Under supervised oral hygiene, we examined the presence of periodontal pathogens from patient plaque samples. RESULTS: By a PCR-based method, Prevotella nigrescens, Bacteroides forsythus, Campylobacter rectus, and Capnocytophaga gingivalis were detected in the SCN patient and P. intermedia, C. rectus, C. gingivalis, and C. sputigena in the CN patient, suggesting the existence of periodontal pathogens. Actinobacillus actinomycetemcomitans, Porphyromonas gingivalis, Treponema denticola, and C. ochracea were not found in either patient. CONCLUSIONS: Use of 1% povidone iodine solution and local antibiotic application under supervised oral hygiene were helpful to improve gingival conditions in patients with chronic neutropenia.

Immediate remission obtained by minocycline in a patient with histiocytic necrotizing lymphadenitis.

A 29-year-old man was admitted because of fever, arthralgia and swelling of the cervical lymph nodes. A diagnosis of histiocytic necrotizing lymphadenitis (HNL) was made based on the findings of a lymph node biopsy. At first, piperacillin sodium (PIPC) was started, but a spiking fever persisted. We therefore changed the PIPC treatment to minocycline. On the following day, his clinical and laboratory findings were dramatically improved. After administering minocycline for 10 days, the HNL symptoms completely disappeared. He has been in good health for 3 years since undergoing treatment. This case strongly indicates that minocycline-sensitive microorganism(s) may be related, at least in part, to the etiology of HNL.

Three patients with isolated adrenocorticotropin deficiency presenting with neuroleptic malignant syndrome-like symptoms.

We report 3 patients with isolated adrenocorticotropin (ACTH) deficiency presenting with neuroleptic malignant syndrome (NMS)-like symptoms. All patients were in their 60's or 70's and showed consciousness disturbance, a high-grade fever, extrapyramydal signs, and muscle enzyme elevations, which met the criteria for NMS. Also, they all showed hyponatremia induced by isolated ACTH deficiency. In addition to the standard therapy for NMS, corticosteroid supplement therapy was effective in all patients. There thus appear to be subjects with isolated ACTH deficiency among patients presenting with NMS-like symptoms, and adrenal and pituitary function should be checked in NMS patients with hyponatremia.

Rapid improvement of osteomalacia by treatment in a case with Sjögren's syndrome, rheumatoid arthritis and renal tubular acidosis type 1.

We present here a case of Sjögren's syndrome (SjS) with osteomalacia based on renal tubular acidosis type 1 (RTA-1). A 53-year-old woman, diagnosed as having rheumatoid arthritis (RA) at the age of 33, was admitted to our hospital because of sicca complex, fatigability and worsening general aching. The activity of RA had been low, but it was complicated by SjS, RTA-1 and remarkable osteomalacia. Acidosis was corrected by alkali supplement therapy. By treatment with a regimen consisting of alfacalcidol, calcium L-aspartate, elcatonin and ipriflavone, her bone mineral density (BMD) was remarkably improved within months and the generalized aching gradually diminished.

The effects of cytoplasmic transfer of mtDNA in relation to whole-body endurance performance.

The purpose of this study was to examine the relation between whole-body aerobic capacity and mitochondrial facilities. The mitochondrial enzyme system of oxidative phosphorylation (OXPHOS) is encoded both by mitochondrial DNA (mtDNA) and nuclear DNA. To identify the effect of mtDNA on whole-body aerobic capacity, we fused the platelets of the study subjects that contained mtDNA but that lacked nuclear DNA with rho(0) HeLa cells, which lacked mtDNA, and isolated repopulated cybrids. The mitochondrial respiratory functions of the cybrids, estimated from cell oxygen consumption and cytochrome-c oxidase (CCOX), were compared between endurance athletes and sedentary controls. The oxygen consumption was 18.5 +/- 3.9 and 18.2 +/- 4.1 nmol/min/ml/10(7) cells in athletes and controls, respectively. The CCOX activity was 98.8 +/- 17.5 and 116.7 +/- 9.8%, compared with fibroblasts in athletes and controls, respectively. No significant difference was noted between groups in either cell oxygen consumption or CCOX activity. These results show that the OXPHOS enzymes coded by mtDNA do not strongly influence whole-body aerobic fitness.

Relationship between mitochondrial DNA polymorphism and the individual differences in aerobic performance.

This study focused on the mitochondrial DNA (mtDNA) as the genetic factor most likely to bring about the individual difference in endurance capacity or its trainability. Platelets contain mtDNA but no nuclear DNA, whereas rho(0)-HeLa cells have nuclear DNA but no mtDNA. The oxidative capacity of mitochondria in the cultured cells, which were fused rho(0)-HeLa cell with platelets obtained from individual subjects (the so-called "cybrids"), reflects the individual mtDNA polymorphism in the gene-coding region. The purpose of this study was to investigate the relationship between the oxidative capacity of cybrids and the individual difference in endurance capacity, or its trainability. Forty-one sedentary young males took part in an 8-week endurance training program. They were determined by using their VO(2 max) as an index of endurance capacity on an ergocycle before and after the endurance training program. The relations between VO(2 max) before endurance training or the change of it by endurance training and the oxidative capacity of cybrids were investigated. There was no relation between them, and two groups were drawn from all subjects, based on one standard division of their initial VO(2 max): the higher pre-VO(2 max) group (n = 6) and the lower pre-VO(2 max) group (n = 5) (51.8 +/- 3.5 ml/min/kg vs. 33.3 +/- 3.8 ml/min/kg, p < 0.01). No significant difference was found between the O(2) consumption of the cybrids in the higher initial VO(2 max) group and that in the lower initial VO(2 max) group (16.3 +/- 4.9 vs. 15.9 +/- 2.0 nmol O(2)/min/10(7) cells, NS). Furthermore, neither the cytochrome c oxidase (COX) activity nor the complex I + III activity of cybrids showed a significant difference between the two groups. The oxidative capacity of cybrids between the high trainability group (n = 6) (Delta VO(2 max) 12.1 +/- 1.6 ml/min/kg) and the low trainability group (n = 9) (Delta VO(2 max) 2.3 +/- 0.5 ml/min/kg) was also similar. Thus the mtDNA polymorphism is very unlikely to relate to the individual difference in endurance capacity or its trainability in young sedentary healthy subjects.

Age-period-cohort analysis of lung cancer mortality in Japan, 1960-1995.

The mortality data on lung cancer in Japan from 1960 to 1995 was analysed based on an age-period-cohort (APC) model. Though the APC model has an 'identifiable problem' caused by the relationship of age, period and cohort parameters, non-linear components of them revealed their original (separated) effects. They were: (1) non-linear age effects had a peak in 55-59 and 60-64 years old in males and 50-54 in females, (2) non-linear period effects were very small in both genders, (3) non-linear age and period effects were small enough to neglect compared with their linear effects, and (4) there were five parts of trends in Japanese lung cancer mortality in both genders in the non-linear birth cohort effects. The 1961-65 birth cohort effect seemed to increase differently from previous birth years. This trend should be monitored carefully.

A necessary and sufficient condition of comparability for using standardized mortality ratio (SMR).

A necessary and sufficient condition of comparability for using SMR was studied mathematically by considering the equivalence between SMR and CMF, as CMF was a perfectly comparable index. This condition was expressed by either proportionality of mortality vectors or proportionality of projected person-years to the plane spanned by mortality vectors of reference and index groups. We could obtain another expression of the condition, in which affect of distortions were easily understood, which consist of three factors: distortion of proportionality of mortality, distortion of person-years and similarity of distortions. Our results were applied to study the mortality of biliary tract cancer in Ibaraki Prefecture. Places where absolute difference between CMF and SMR exceeds some criterion (say, 0.15) were Satomi, Ushiboiri, Nihari in males and Gozenyama, Suifu and Asahi in females. All three distortion indices exceeded their upper 95% percentiles in Satomi in males.

Oral health attitudes/behavior and gingival self-care level of Korean dental hygiene students.

The purpose of this study was to examine oral health attitudes/behavior and gingival self-care levels of Korean dental hygiene students (n = 271). A questionnaire known as HU-DBI (Korean version) was administered to each class. Higher scores of the HU-DBI indicate better oral health attitudes/behavior. The student's gingival self-care level was scored as excellent (+2), good (+1), questionable (0), poor (-1), or very poor (-2) according to the criteria of the Oral Rating Index (ORI) for youth. The mean score of the HU-DBI was 6.40, and that of the ORI was +0.12. Most samples scored 0 or +1. The mean HU-DBI scores of year-2 and year-3 students (7.06, and 7.61 respectively) were significantly greater than that of year-1 students (5.23) (P<0.001). Twenty-three percent of the students reported a belief that they may eventually require false teeth. Half stated that they put off going to the dentist until they had toothache. Year-1 students were more likely to have this attitude compared to year-3 students. HU-DBI scores were significantly associated with ORI scores (r = 0.315; P<0.001). The differences in the HU-DBI and ORI scores across the level of education were highly significant.

Development of muscle fatigue assessed by using superposition of evoked and volitional myoelectric potentials.

We studied muscle fatigue development using evoked myoelectrical potentials superimposed on volitional ones. The instantaneous frequency of superim posed M-waves and mean power frequencies of volitional electromyography (EMG) declined during sustained contraction, indicating that fatigue progressed. We divided fatigue into 3 phases, with 20 frames in each fatigue phase, corresponding to one-third of the total sample. The instantaneous frequency of superimposed M-waves and mean power frequencies of volitional EMG were correlated during early intensive isometric voluntary contractions and became increasingly uncorrelated as contraction proceeded. The coefficient between the mean power frequency and instantaneous frequency correlation was also greater at the first peak than at the second peak of the superimposed M-wave, indicating that the motor unit action potential was distorted. Distortion in the motor unit action potential shape depends on elongation of the depolarization zone of muscle fiber, because the superimposed M-wave is a peripheral indicator elicited by electrical stimulation. These results suggest that muscle fatigue develops based on the reduction of the conduction velocity of muscle fiber and on the elongation of the depolarization zone of muscle fiber.
