Digital subtraction angiography for the evaluation of persistent uterine trophoblastic disease.

In three patients with persistent uterine trophoblastic disease, follow-up after primary chemotherapy was undertaken by digital subtraction angiography. It was found that the angiographic appearance of the intramural uterine lesions was comparable with that obtained by conventional pelvic angiography. Digital subtraction angiography may be applicable as an easy, safe, and valuable diagnostic method for gestational trophoblastic disease in place of conventional pelvic angiography because it is a relatively noninvasive procedure that can be performed on outpatients.

[Low energy tail spectra of characteristic X-rays detected by a Si(Li) X-ray detector].

Characteristic X-ray spectra are inherently accompanied by the tail spectra in the low energy side when X-rays are detected with a semiconductor X-ray detector. The tail spectra of Mg, Al, Si, P, S, Cl, K, Ca, Ti, Cr and Mn KX-rays have been observed using two different Si(Li) X-ray detectors. An annular source of 55Fe has been used to excite the Mg (1.25 keV) to Ti (4.5 keV) KX-rays, and 54Mn and 55Fe sources have been used for the detection of Cr (5.4 keV) and Mn (5.9 keV) KX-rays, respectively. Observed intensity ratios of the tail area Nt to the Gaussian X-ray peak area NP have exhibited to change remarkably at the Si-K adsorption edge energy 1.84 ke V. When X-ray spectra detected with different Si(Li) detectors are compared at some specific characteristic X-ray, different values of Nt/Np intensity ratios as well as different line shapes of tail spectra have been observed. Using a simple model, the thickness of Si layer which generates the tail spectrum has been estimated, i.e., the thicknesses are about 0.05 micron for one detector and 0.09 micron for the other detector. The generation of the tail spectrum is known to be partially due to the escape effect of photoelectrons or Auger electrons from the intrinsic region.(ABSTRACT TRUNCATED AT 250 WORDS)

[Plasma histamine level in anesthesia with enflurane].

Plasma histamine levels during enflurane anaesthesia were studied in 14 oral surgery patients without any history of allergy and general disease except for the oral region. Plasma histamine was measured using a fluorometric method combined with high speed liquid chromatography. The difference between the histamine levels measured at each time before and during oral surgery was not significant.

Quantitative analysis of smooth endoplasmic reticulum proliferation in hepatocytes of early postnatal and adult mice treated with phenobarbital.

The smooth endoplasmic reticulum in hepatocytes of early postnatal and adult ddY male mice injected with phenobarbital was analyzed stereologically. The animals received daily intraperitoneal injections of phenobarbital and were killed about 24 h after the last injection. In early postnatal animals (injected with 35 mg/kg of the drug at days 1 and 2 after birth, or injected with 35 mg/kg at day 2 and with 50 mg/kg at days 3 and 4 after birth), the smooth endoplasmic reticulum proliferated both in perivenular and periportal hepatocytes; there was no significant difference in amount of smooth endoplasmic reticulum between the cells of the two zones. In adult animals, however, proliferation occurred only in perivenular cells after the administration of 35 mg/kg of phenobarbital for 2 days, and predominantly in perivenular cells even after the administration of 150 mg/kg of the drug for 7 days. These results suggest that the conspicuous proliferation of the smooth endoplasmic reticulum in perivenular hepatocytes in response to phenobarbital administration, which is characteristic of adult liver, does not occur in early postnatal mice but becomes manifest during postnatal development.

Development of structural heterogeneity of mitochondria in mouse hepatocytes.

The volume and shape (the axial ratio:length/diameter) of mitochondria in periportal and perihepatic hepatocytes from newborn, 5 days old, 10 days old and adult male ddY mice were analysed by quantitative electron microscopy. In newborn animals, the volume and relative histogram of the axial ratio were not significantly different between periportal and perihepatic cells. Thereafter, the volume did not change significantly in periportal cells, whereas it decreased between 5 days of age and the adult in perihepatic cells. In adult animals, the volume in perihepatic cells was 26% of that in periportal cells. The relative histogram of the axial ratio for periportal cells deviated significantly to smaller values than in that for perihepatic cells in 5 days old, 10 days old and adult animals. Thus, the structural heterogeneity of hepatocyte mitochondria, evident in adult animals, is not present in newborn animals but arises during postnatal development.

Merkel cells and Merkel cell tumors. Ultrastructure, immunocytochemistry and review of the literature.

Certain monomorphic cellular tumors that occur in the dermis have been called trabecular carcinomas or Merkel cell tumors. Forty-six cases have been reported to date and the literature on these is reviewed here, with six additional cases reported. Cytologic features include sparse cytoplasm, dispersed chromatin with inconspicuous nucleoli in round nuclei and many mitoses. Trabeculae and pseudorosettes may be identified. Electron microscopy is required for definitive diagnosis. Like normal Merkel cells, tumor cells contain electron-dense granules (80-200 nm), 10 mm filaments and desmosomes. Filament-rich cytoplasmic spikes were found in four tumors. These resemble corresponding protrusions of normal Merkel cells and have not been described in other APUDomas.

Observation of cytochemical alkaline phosphatase activity on the plasma membrane of cultured rat hepatocytes by backscattered electron imaging.

Distribution of alkaline phosphatase (ALPase) activity on the plasma membrane of rat hepatocytes in primary monolayer culture was observed using the backscattered electron image (BEI) mode. Apparent reaction product was seen in hepatocytes cultured for 48 or 72 hours in a concentration of 8 X 10(5) cells per ml, and those cultured for 72 or 96 hours in a concentration of 6 or 4 X 10(5) cells per ml. The reaction product was observed as scattered fine dots, as spots, and as bands along the cell edge, with high contrast in flattened polyhedral hepatocytes forming cell trabeculae. The deposition of the reaction product was generally more abundant at the periphery of the cell trabeculae. X-ray microanalysis revealed that the reaction product contains both lead and phosphorus as real ALPase reaction product. In transmission electron microscopy, the reaction product was exclusively localized on the external surface of the plasma membrane. However, the plasma membrane adjoining the culture substratum was devoid of the reaction product. Further, the highest biochemical ALPase activity appeared in earlier culture stages when the density of cultured cells was larger. Thus, during a few days of culture, ALPase activity increases on the plasma membrane predominantly at the periphery of the hepatocyte trabeculae. This increase in the activity might be related to the mechanism of preserving the cell shape.

A bilateral total cleft of lip and hard and soft palate and transverse cleft face with an atypical paramedian defect of the lower lip and lower jaw on the right side. A case report of a new syndrome?

A three-month-old male baby who has a complete bilateral cleft of the lip and hard and soft palate, and a right unilateral transverse facial cleft, with an atypical paramedian defect of the lower lip and jaw is reported in detail. As these associated malformations are very rare, it may represent a new dysmorphic syndrome.

A comparison of 99mTc-MDP and 99mTc-pertechnetate by computerized quantitative joint scintigraphy.

Computerized, quantitative joint scintigraphies were performed using both 99mTc-MDP and 99mTc-pertechnetate. 17 joint pairs with asymmetric active arthritis were selected for the study. The joint activity ratio was calculated by dividing the curve peak value of the inflamed joint by the curve maximum of the contralateral symptomless joint. A very high joint activity ratio with 99mTc-MDP was observed in the case of septic arthritis with osteolytic lesions. In all the rheumatoid joints the activity ratios were higher with 99mTc-MDP than with 99mTc-pertechnetate. In reactive arthritis the mean joint activity ratio was equal with both tracers. It appears that 99mTc-MDP is more sensitive in detecting active rheumatoid arthritis than 99mTc-pertechnetate and that high 99mTc-MDP values might be prognostic of destructive changes.

[Immunochemotherapy of recurrent and advanced uterine and ovarian cancer using cisplatin].

A combination immunochemotherapy regimen containing cisplatin (20 mg, days 1-5), peplomycin (20 mg, days 2, 9, 16), +/- vinblastine (5 mg, days 1, 2), and picibanil (3-5 KE/week) was performed in twelve patients with advanced or recurrent uterine and ovarian cancers under intravenous hyperalimentation (IVH), except one patient receiving peplomycin by a continuous infusion method using IVH bag (10 mg/day for 5 days). This regimen was repeated every three weeks. Five (71.4%) of seven evaluable patients showed partial response. No patients yielded the complete disappearance of disease. No severe and lethal pulmonary or renal dysfunction occurred and all was well tolerated. In a regimen without vinblastine, myelosuppression, especially thrombocytopenia, occurred later compared to the regimen including vinblastine.

Septic sacroiliitis.

Five patients with septic sacroiliitis, 3 unilateral and 2 bilateral, are described. The illness was acute, with high fever, low back or buttock pain, severe sacroiliac tenderness and elevated erythrocyte sedimentation rate. Staphylococcus aureus was isolated from the blood in 3 cases and Salmonella typhi in 1. Diagnosis was confirmed by scintigraphy in 4 cases. Roentgenographic changes developed in 2 patients. Response to antimicrobial therapy was favourable. Awareness of this condition is mandatory for prompt diagnosis and correct therapy. Sacroiliac tenderness with increased radionuclide uptake, high fever and positive blood culture are the primary diagnostic criteria.
