[A successful surgical treatment of mitral stenosis of double mitral valve combined with aortic regurgitation].

The purpose of this paper was to present a successful double mitral commissurotomy concomitant with aortic valve replacement for 45-year-old female who was found to have rheumatic fever in her history. Double mitral valve was incidentally diagnosed by means of preoperative echocardiography. Both orifice areas were almost same and measured as 0.70 and 0.71 cm2 in size. Double mitral commissurotomy was successful to obtain optimal areas of mitral valve at surgery and aortic valve replacement was carried out with #23 Björk Shiley prosthesis. Her postoperative course was uneventful and followed up one year after operation at present. This is the first case in the literature of successful surgical treatment of double mitral commissurotomy combined with aortic valve replacement.

[A successful treatment of Cabrol's procedure for annuloaortic ectasia with an acute aortic dissection (Stanford type A)].

A 67-year-old female who has an Annuloaortic ectasia with an acute aortic dissection was urgently admitted to our hospital. She was also suspected to have developed into an acute cardiac tamponade. An aortic dissection was arised from just above the right coronary artery orific and extended to the innominate artery and the right common carotid artery. In this case Cabrol's procedure with cerebral perfusion technique was considered to be a good indication, but standard Cabrol's procedure provided almost satisfactory results without utilizing a cerebral perfusion technique because disappearance of pressure difference between both hands and the right common carotid artery after reconstruction. Her postoperative course was uneventful and aortography confirmed competence of the prosthetic valve, good patency of the coronary arteries without false aneurysm formation. Cabrol's procedure is a reliable method for Annulo-aortic ectasia especially with an acute aortic dissection of the ascending aorta.

[Intraoperative ultrasound imaging and its application to cardiovascular operations].

Intraoperative ultrasound imaging was employed for qualitative and quantitative studies during cardiovascular operations in 36 patients. A 7.5M Hz high frequency ultrasound scanner was used to image. The ultrasound probe was sterilized by gas and placed directly on the heart by the hand of surgeon. On the clinical application, 10 cases of patients with coronary artery disease were scanned. Left main trunk and proximal portion of other three branches were visualized and pathological lesions were demonstrated clearly. Four cases of myocardial bridging were observed. Intramyocardial coronary arteries were easily detected and supraarterial myotomy was performed safely. Four cases of valvular disease were examined. We could evaluate valvular anatomy and physiologic aspects of valvular disease and 15 cases of aortic root disease were examined intraoperatively. In aortic root dissection, the size and site of the true and false lumens were clearly demonstrated in all patients. In conclusion intraoperative ultra-sound imaging is a versatile technique with quantitative and qualitative studies of cardiovascular surgery during operation and the perioperative period.

[A rare case of successful surgical treatment of dissecting aortic aneurysm (DeBakey IIIb) associated with total obstruction of the abdominal aorta and acute complicated renal failure].

A rare case of successful surgical management of dissecting aortic aneurysm (DeBakey IIIb) is presented. A fifty-year-old male was admitted to our hospital with a complaint of severe backache and both leg pain. A diagnosis of acute renal failure and assessed total obstruction of the abdominal aorta was made angiographically. Immediate hemodialysis was carried out. After 50 days, a right subclaviafemoral bypass was performed. However, on the 60th postoperative day, plasma creatinine and BUN levels were significantly elevated. A left renal revascularization was performed using saphenous vein graft from the right common iliac artery. The postoperative course was excellent and the creatinine and BUN levels were immediately reduced. And there was no need of hemodialysis postoperatively. Because of difficult management of this kind of complication, extra-anatomic bypass is preferable, instead of total replacement of the dissecting aortic aneurysm. And weaning from such a long term hemodialysis by revascularization of one-kidney is very rare.

[Surgical experience in arteriovenous fistula following disk surgery].

A 21 year-old woman was admitted to our hospital due to congestive heart failure with severe leg edema. A continuous murmur was heard around the lumbar spine close to a surgical scar after laminectomy of the L4-L5 and L5-S1 disc that the patient had undergone six months before. Aortography demonstrated an arteriovenous fistula between the right common iliac artery and the inferior vena cava. At operation, we found the moderate sized venous defect and it corresponded with the angiographic finding. It was repaired by direct suture from inner side of the right common iliac artery. Arterial reconstruction was made with a 8mm woven dacron graft. Postoperative course was uneventful. The cardiac silhouette diminished in size and cardiac output improved from 12.5l/min. to 8l/min. after surgery. This report is the fourth case of successful repair for the arteriovenous fistula after disc surgery in Japan.

[The clinical effects of a new antiviral 9-(2-hydroxyethoxymethyl) guanine (aciclovir) against herpes virus infections].

The clinical effects of a new anti-viral 9-(2-hydroxymethoxymethyl) guanine (Aciclovir) against Herpes virus infections have been investigated. The patients had malignant tumours or auto-immune disease complicated by shingles and chicken pox due to Vaicella zoster virus (VZV) (43 cases), Herpes simplex virus (HSV) (10 cases) and 9 cases which were clinically diagnosed as Herpes, though the virus was not confirmed as the causative agent. As a general principle the dosage of Aciclovir was 5 mg/kg, t. i. d. for 5 days by slow intravenous infusion. The clinically effective rate against VZV was 93%, being excellent in 42% and against HSV it was 80%, being excellent in 40% and when the results of the cases of unknown origin were included it was excellent in 40% and the cumulative effective rate was 88%. Concerning the efficacy in reduction of pain, swelling, disappearance of vesicles and new scab formation, the effect was most noticeable after the third day of treatment. Treatment given early in the disease is likely to provide better results. Concerning side effects, one of 62 patients had proteinuria and the other had a drug rash and an abnormal liver function test. It is likely that the combination of treatment and the primary disease had some influence, but the cause/effect relationship to Aciclovir treatment is not clear.
