Evaluation of left ventricular diastolic function by radionuclide ventriculography in patients with coronary artery disease.

Radionuclide ventriculography (RNV) was used to assess the peak filling rate (PFR) in 20 patients with coronary artery disease (CAD). Dipyridamole RNV was used in 5 normal subjects and 10 patients with CAD with the view to evaluating PFR increase. Significant difference in PFR values was established between normal subjects and CAD patients. Decreased value of PFR was found in 78% CAD patients, and 72% of them had normal ejection fraction (EF). During dipyridamole RNV increase of PFR was not significantly different between normals and CAD patients. The peak filling rate is a very sensitive indicator of LV dysfunction in CAD patients, and is useful in early detection of abnormalities in diastolic function in CAD patients.

Treatment of rheumatoid arthritis with radionuclide combination.

Radiation synovectomy (RSIN) with radionuclide combination (N) was performed in 135 patients with rheumatoid arthritis or 417 joints: 155 knee joints (90Y), 230 MCF or PIF joints (169Er), 17 ankle, 7 wrist, 6 elbow and 2 shoulder joints (186Re). Therapeutic effect was followed up during 8 year period. After 3 years positive effect of the treatment was achieved in 70-80% of cases and complete treatment in 50-60% of cases, while 8 years later the same effects were maintained in 55-60%, i.e. 35-40% of the treated joints. The best effect was achieved in the treatment of the knee and the worst in the treatment of the wrist joint. At simultaneous application of one or several different RN, the total dose for one patient did not exceed 259 MBq while the maximum dose was 695 MBq during the long-term treatment.

Salivary gland scintigraphy in patients with primary Sjögren's syndrome.

Methodology, interpretation and results of salivary gland scintigraphy in 33 patients with primary Sjögren's syndrome (pSs) are presented. Patients were divided in two groups: 14 patients with isolated pSs and 19 patients with pSs and extraglandulary manifestations. There was no correlation between scintigraphic findings and duration of disease, and between scintigraphic findings and presence of recurrent swelling of parotid glands. The damage of salivary glands, detected by scintigraphy, was more marked in patients with pSs and extraglandulary manifestations than in patients with isolated pSs.

Evaluation of diagnostic reliability of radionuclide phlebography using 99mTc-MAA to detect deep venous thrombosis; its role in establishing indications for inferior vena cava filter implantation.

Radionuclide phlebography (RNP) of the lower extremities and pelvis was performed using 99mTc-MAA in 40 patients with pulmonary thromboembolism. Deep venous thrombosis (DVT) was found in all patients, more frequently in the right calf and in the left iliac veins in 20 patients (55%). RNP and contrast phlebography (CP) were performed in 18 patients and confirmed the presence of DVT in all cases. The greatest specificity of RNP was obtained in the left (92.4%) and in the right pelvis (80%). In 12 of 18 patients in whom a cava filter was implanted, specificity of RNP was 100% for the left thigh and 91.7% for other localizations. In establishing indications for cava filter implantation, RNP should be performed prior to CP and bilaterally, but in case of non-indicated CP, RNP findings should be sufficient.

Correlation of substance(s) immunologically cross-reactive with insulin, glucose and growth hormone in Hodgkin lymphoma patients.

The levels of substance(s) detectable by insulin specific radioimmunoassay (RIA), glucose and growth hormone (GH) were determined in the blood of patients suffering from Hodgkin lymphoma. In the relapse phase of the disease, the levels of substances immunologically cross-reactive with insulin (SICRI) were elevated and glucose concentrations were below normal. In these patients the basal and hypoglycemia-induced levels of GH in blood were strongly elevated. Contrary to this, both SICRI and glucose levels were normal in the blood of patients in remission, and GH levels were significantly reduced compared to those measured in patients in relapse.

[Use of 99mTc-pyrophosphate and coronarography in the diagnosis of simultaneous myocardial infarct of the right and left cardiac ventricles].

Myocardial scintigraphy was performed using 99m-Tc-pyrophosphate in 50 patients with suspected coexisting right ventricular and left inferior ventricular heart infarction and in 50 patients with left inferior ventricular infraction only. In all survived patients additional selective coronarography was performed and in all died patients autopsy was performed. The aim of the study was to evaluate reliability of myocardial scintigraphy using 99m-Tc-pyrophosphate in the diagnosis of coexisting infarction of both heart ventricles. In a group of patients with coexisting right and left ventricular infarction (45 proved by coronarography, and 5 by autopsy), scintigraphic finding was confirmed in 46 (93%) patients. In all 50 patients with left inferior ventricular infarction, scintigraphic finding was confirmed in 48 of them by coronarography and in 2 by autopsy (100%). Of 4 patients with false negative scintigraphic finding 2 were treated with fibrinolytic therapy immediately after admission and in 2 diffuse occlusive changes were found on all three blood vessels by coronarography. It has been concluded that myocardial scintigraphy using 99m-Tc-pyrophosphate is a very sensitive (92%), specific (100%), exact (96%), simple and safe diagnostical method in detecting coexisting acute infarction of both heart ventricles.

[Tissue polypeptide antigen and tumor trypsin inhibitor in pleural effusions of various etiologies].

In 47 patients with pleural effusions of different etiology concentrations of the tissue polypeptide antigen (TPA) and tumour trypsin inhibitor (TATI) were measured. Malignant disease was found in 24 patients and benign one in 23 patients. In a group of patients with malignant pleural effusion the average concentration of TPA was 3102 U/L and of TATI 76.8 ug/L. In a group of patients with nonmalignant effusion these concentrations of TPA were 2381.8 U/L and of TATI 28.3 ug/L. Higher concentration of TPA was found in pleural effusion of malignant nature with specificity of 96% but with a small sensitivity of 12.5% and positive predictive value of 43%. TATI concentrations were also higher in malignant pleural effusion with good sensitivity and specificity (78%, 50% respectively) and positive predictive value of 66%.

[Hürthle cell carcinoma of the thyroid gland].

The authors have analysed 9 cases of Hürthle cell thyroid carcinoma diagnosed on the bioptic material in the period 1975-1991. In 7 patients subtotal and in 2 total thyroidectomy was performed. In one patient only signs of metastases of the tumour in the region of the mediastinum, neck lymphatic nodes and brain developed and the patient died a year after surgery. For differential diagnosis of Hürthle cell thyroid carcinoma and Hürthle cell adenoma the authors recommend as necessary that beside the presence of the basic criteria for malignancy (infiltration of the capsule and adjacent tissue, invasion of the blood vessels, pleomorphism and pathologic mytoses) the electron microscopic examination and immunohistochemical methods should be also used which could be of a great help for establishment of a correct diagnosis on which depends further therapy as well.

[The role of radionuclide phlebography in the detection of deep venous thrombosis in the lower extremities and pelvis and in establishing indications for implantation of a caval filter].

In all 40 patients with pulmonary thromboembolism (PTE) radionuclide phlebography (RNP) detected deep venous thrombosis (DVT), most commonly in the left iliac veins, in 20 (50%). RNP and contrast phlebography (CP) were performed in 18 patients and RNP sensitivity was found in 100% in all localizations of DVT and the largest specificity was obtained in the left side of the pelvis, in 92%. In 12 out of 18 patients cava filter was implanted and BNP specificity in the left upper leg reached 100% and in the right one 89.3%. In establishing indications for cava filter implantation RNP should be performed first so that by detecting embologenous TDV localizations the risk of jatrogenous PTE at cava filter implantation would be avoided or CF, if necessary.

[Neuron-specific enolase in pleural effusions of various etiologies].

The concentration of neuron-specific enolase (NSE) was measured in the serum and pleural fluid of 42 patients with pleural effusions of different etiology. High levels (above 13.0 micrograms/l) of NSE in pleural fluid of 9 patients with malignant disease were observed (sensitivity 57%). However, pleural fluid NSE levels were also increased in 14 with non-malignant diseases (specificity 45%). Increased pleural fluid NSE activity is not specific for malignancy.
