Immunohistochemical analysis of human adenocarcinomas of the lung using the monoclonal antibody 44-3A6.

In this study, 39 primary, surgically resected, pulmonary adenocarcinomas of the following cell types were investigated: 19 Clara cell, 4 bronchial-gland, 15 goblet cell, and 1 type II alveolar epithelial adenocarcinoma. They were analyzed for the expression of the glandular differentiation-associated antigen recognized by the monoclonal antibody 44-3A6. Formalin-fixed, paraffin-embedded tissue sections were immunostained using the avidinbiotin complex/peroxidase method. All of the Clara cell tumors (19/19) expressed this antigen as well as 3/4 bronchial-gland tumors, as compared to only 2/15 goblet cell tumors. The single type II pneumocyte neoplasm studied was found to express this antigen. These findings parallel our earlier observations based on cytological features, and provide supportive evidence that there may be biological differences between human pulmonary adenocarcinomas and bronchioloalveolar carcinomas.

Influence of hormones on proliferation of ER-positive cells and ER-negative cells of human breast cancer (MCF-7).

The influence of endocrine therapy on the proliferation of estrogen receptor (ER)-positive cells and ER-negative cells of human breast cancer (MFC-7) serially transplanted into nude mice was analyzed by tumor growth, dextran-coated charcoal (DCC) method, ER-immunocytochemical assay (ER-ICA) and ER-immunocytochemically stained 3H-thymidine autoradiography. In the tamoxifen (TAM) group and the medroxyprogesterone acetate (MPA) group, tumor growth was inhibited, but it was promoted in the 17-beta-estradiol dipropionate (E2) group. The ER level by the DCC method was significantly decreased in the TMA, the MPA and the E2 groups. The ER-ICA showed that the percentage of ER-positive cells was decreased in the TAM and the MPA group, but it was increased in E2 group. However, the ER-immunocytochemically stained 3H-thymidine autoradiography showed that not only the labelling index of ER-positive cells but also that of ER-negative cells was significantly decreased in the TAM and the MPA groups, while the labelling index was significantly increased in the E2 groups. Therefore, it was concluded that endocrine therapy affected the proliferation of both ER-positive cells and ER-negative cells of ER-positive breast cancer.

Effects of high dietary fat on the total DNA and receptor contents in rats with 7,12-dimethylbenz[a]anthracene-induced mammary carcinoma.

The influence of high dietary fat on the malignant intensity and the hormone receptors of 7,12-dimethylbenz[a]anthracene (DMBA)-induced mammary carcinoma in female Sprague-Dawley rats were analyzed by the tumor incidence and growth, the DNA histogram type, the DNA index, the S-phase fraction, and the estrogen receptor (ER) and progesterone receptor (PgR) assays. The rats were fed either a low-fat (0.5% corn oil) diet or a high-fat (20% corn oil) diet after the DMBA administration. Tumor incidences in the low-fat and the high-fat diet groups were 46 and 86%, respectively (p less than 0.01). Tumors in the high-fat diet group were also significantly larger than those in the low-fat group. Average tumor latent period was significantly shorter in the high-fat diet group, comparing with that in the low-fat diet group (p less than 0.01). Sixty-nine percent of the tumors in the high-fat diet group had aneuploid type, while only 8% of those in the low-fat diet group had aneuploid type. The DNA index and S-phase fraction also were significantly higher in the high-fat diet group (p less than 0.01). But the ER and PgR contents were not different between both groups. Therefore, these results suggest that a high dietary fat could increase the malignant intensity of the tumor but does not influence the hormonal responsiveness of these tumors.

Effect of enflurane on contractile reactivity in isolated canine mesenteric arteries and veins.

The effects of enflurane on responses of isolated canine mesenteric arteries and veins to transmural nerve stimulation and to exogenously administered norepinephrine (a mixed alpha 1- and alpha 2-adrenoceptor agonist), phenylephrine (a selective alpha 1-adrenoceptor agonist), and tyramine were studied. The contractile responses of the arteries and the veins to transmural nerve stimulation and to norepinephrine were attenuated by exposure to enflurane; the responses to phenylephrine were decreased more than those to norepinephrine. When compared with the effect of enflurane on transmural nerve stimulation-induced responses, exposure to enflurane resulted in slight attenuation of the contractile responses caused by tyramine, suggesting that enflurane may inhibit the responses to tyramine by interfering with an interaction between released norepinephrine and postjunctional alpha 1-adrenoceptors rather than with tyramine-induced norepinephrine release. The data are also consistent with the view that enflurane acts on sympathetic nerve endings to inhibit release of norepinephrine associated with electrical stimulation-induced nerve membrane depolarization.

[Quality evaluation of oriental medicines. I. Quantitative analysis of marker substances in the ethical extract preparation of Hangekouboku-to].

For the purpose of evaluating a Kampo preparation, Hangekouboku-to, the contents of 6-gingerol (Gi), guanosine (Gu) and rosmarinic acid (R) in twelve kinds of ethical extract preparations were compared with the self-made decoctions. Little difference was found in the availability of the three compounds among three decoction methods. A great difference was found in the R content in the ethical extract preparations while no significant difference was found in the Gi and Gu contents, showing that they were not lost in the manufacturing process. These results suggested that Perilla Herb of quite different quality was used as raw materials.

[Studies on concentration of glycyrrhizin in plasma and its absorption after oral administration of licorice extract and glycyrrhizin].

The present study was carried out to elucidate the difference in the concentration of glycyrrhizin (G) in the rat plasma and in its absorption after oral administration of licorice extract and G. The concentration of G in the plasma was determined by high performance liquid chromatography. It was found that the time required for a maximum concentration (Tmax) of G was 8 h after administration of licorice extract. On the other hand, G reached a maximum plasma concentration at less than 6 h after administration of G. The plasma level of G fell slowly within 24 h after their oral administration, and it was still detected in the plasma even after 24 h. The maximum concentration (Cmax) of G in the plasma after oral administration of licorice extract was somewhat lower than that of G. On the other hand, the area under the plasma-time curve (AUC24h) of G after oral administration of licorice extract was almost the same as that of G. The effects of licorice extract and G on excretive urine volume was also investigated in rats. Licorice extract and G, at the dose used in this experiment, did not show any effect as compared with a control group.

[Studies on the preparation and evaluation of Kijitsu, the immature citrus fruits. III. Relation between diameter of Kijitsu and synephrine content].

For the purpose of evaluating Kijitsu, the content of synephrine in dried unripe citrus fruits was analyzed by high performance liquid chromatography after cleaning up the MeOH extract with an ion exchange cellulose column. Among them, Citrus unshiu had the highest amount of synephrine. No difference was found between C. hassaku and C. aurantium. The synephrine content decreased corresponding with an increase in diameter of Kijitsu.

Transferrin receptor expression in adenocarcinoma of the lung as a histopathologic indicator of prognosis.

Immunohistochemical reactivity with OKT9 (antitransferrin receptor) monoclonal antibody was studied in 58 surgically resected adenocarcinomas of the lung. The cell surface and cytoplasm of the tumor cells were stained in 44 cases (76 percent). The expression of the transferrin receptor (TrfR) in tumor cells was compared with histopathologic prognostic factors, such as TNM factors, degree of histologic differentiation, degree of nuclear atypia, and frequency of mitotic figures. The expression of TrfR showed significant correlation with the degree of histopathologic differentiation (p less than 0.025), degree of nuclear atypia (p less than 0.025), and frequency of mitotic figures (p less than 0.001). However, there was no definite correlation with TNM factors. These results indicate that the expression of the TrfR in pulmonary adenocarcinoma corresponds to the elevated proliferative activity of the tumor cells, and that immunohistochemical reactivity with OKT9 can be used as one of the histopathologic indicators of prognosis of pulmonary adenocarcinoma.

Ultrastructure of an anaplastic giant cell tumor of the thyroid.

A case of anaplastic, multinucleated giant cell tumor of the thyroid was studied by light and electron microscopy. The coexistence of anaplastic sarcomatous tumor and well differentiated follicular carcinoma, and the presence of desmosomes among the mononuclear cells suggested that this tumor originates in thyroid follicular cells. The multinucleated giant cells, which characterize this thyroid tumor, appeared to be formed by fusion of follicular carcinoma cells and mononuclear epithelial cells, and not by nuclear division without cytoplasmic division.

Immunohistochemical localization of prostaglandin F2 alpha in reperfused gerbil brain.

Immunohistochemical localization of prostaglandin (PG) F2 alpha synthesized during recirculation following experimental forebrain ischemia was studied in Mongolian gerbils. The bilateral carotid arteries were clamped for 5 minutes, and the brains were frozen in situ after 5 minutes of recirculation. Sham-operated gerbils not subjected to arterial occlusion served as controls. Cryostat sections containing dorsal hippocampus 10 microns in thickness were incubated in rabbit anti-PGF2 alpha serum and stained with avidin-biotin-peroxidase complex after fixation in carbodi-imide and Zamboni's solution. In specimens from control animals, blood vessels were faintly stained with anti-PGF2 alpha serum, while neurons and glial cells were less intensely stained or unstained. Specimens from reperfused gerbils demonstrated strongly positive staining for PGF2 alpha in blood vessels, neurons (especially hippocampal pyramidal neurons), and glial cells. Staining for PGF2 alpha in the reperfused animals was substantially diminished by pretreatment with indomethacin (10 mg/kg, i.p.). The results indicate that recirculation after forebrain ischemia results in an increase in PGF2 alpha in neurons, glial cells, and blood vessels.

[Evaluation of thrombus scintigraphy using 67Ga-deferoxamine-dialdehydestarch-fibrinogen in patients with internal A-V shunt].

We evaluated the diagnosis of thrombus and the effect of antithrombic therapy by using 67Ga-DFO-DAS-Fibrinogen (67Ga-Fbg.) scintigraphy in vivo. The 11 patients with chronic renal failure had the operation of the internal A-V shunt. The two groups were divided the group I (the patients were injected 67Ga-Fbg. within 5 days after the operation of A-V shunt) from the group II (the patients were injected 67Ga-Fbg. from 7 months to 6 years after A-V shunt). We examined 72 h after the injection of 67Ga-Fbg. in all patients. In group I, the scintigraphy showed the deposition of 67Ga-Fbg. in all 5 patients, but 3 patients were given dipyridamole (75-300 mg). The scintigraphy examined 3 weeks after A-V shunt were negative in 2 patients in group I. In group II, the scintigraphy showed the deposition of 67Ga-Fbg. in the two of 6 patients. Within 5 day after A-V shunt, we observed there was the tendency of the thrombus formation. The scintigraphy of 67Ga-Fbg. was useful for the detection of thrombus in vivo.

Early diagnosis of hepatocellular carcinoma.

We describe the results of our study on the early detection of the development of hepatocellular carcinoma among patients with chronic liver disease. Over a period of 18 years, 33 patients were diagnosed as having hepatocellular carcinoma. From 1970 to 1978, we used serum alpha-fetoprotein determination, liver palpation, and radionuclide liver scans. In addition to alpha-fetoprotein determination, computed tomography and ultrasonography were introduced in 1979. However, we did not have any general guidelines for the use of these imaging modalities. From 1984 onwards, ultrasonography and serum alpha-fetoprotein determination have been performed every three months and computed tomography every year in patients whose right hepatic lobe had atrophied due to liver cirrhosis. On the basis of this screening program, 50% of the detected hepatocellular carcinoma (9/18) were found to be smaller than 1.9 cm in diameter, and tumor resection was performed in 11 out of 18 patients (61%).

[A case of lateral thoracic meningocele without neurofibromatosis].

A case of lateral thoracic meningocele without neurofibromatosis is reported. CT-myelography revealed the mass has the communication with CSF space. So we could differentiate it from a posterior mediastinal tumor. It is important to think of the presence of lateral thoracic meningocele when any posterior mediastinal mass is detected.

[A case report of pleural involvement in primary macroglobulinemia].

A case of primary macroglobulinemia with pleural and gastric involvement was presented. A 48-year-old female was admitted with productive cough. On physical examination neither lymphoadenopathy nor hepatosplenomegaly were found. In addition, no bleeding tendency nor disturbance of the visual acuity were detected. Her chest roentgenogram showed a moderate amount of pleural effusion in the left pleural cavity without infiltration in the lung fields and no evidence of swollen hilar or mediastinal lymphnodes. A monoclonal M-band of to IgM-kappa type was observed in her serum and the pleural effusion. The diffuse ulcerative lesion in the gastric mucosa was detected by gastrofiberscopy. The lymphoid cells taken from the pleural effusion and the gastric mucosa stained positively with fluorescein-conjugated antiserum to u or the kappa chain. Pleural effusion and gastric infiltration of lymphoid cells improved remarkably following ACOP therapy.

[Effect of high dietary fat on the total DNA and receptor contents in rats with 7,12-dimethylbenz[A]anthracene-induced mammary carcinoma].

The influences of high dietary fat on the malignant intensity and the hormone receptors of DMBA-induced mammary breast carcinoma were analyzed in female Sprague-Dawley rats. The rats were fed either a low-fat diet or high-fat diet after the DMBA administration. As the results, incidence, size, average latent period, DNA histogram, DNA index and % of S-phase fraction of tumor were significantly different between both groups. But the ER and PgR contents were not different between both groups. The results suggest that a high dietary fat could increase the malignant intensity of the tumor but does not influence the receptor contents of the tumors.

Large cell carcinoma of the lung secreting human chorionic gonadotropin which responded to combination chemotherapy: case report.

A 68-year-old man was admitted to the National Cancer Center Hospital on November 10, 1988 to undergo chemotherapy for recurrent lung cancer, which had been resected on June 22, 1988. The tumor was diagnosed histologically as large cell carcinoma with trophoblastic differentiation, and human chorionic gonadotropin (HCG) was immunohistochemically found in tumor cells. Chemotherapy was initiated on November 14, 1988. The patient received three courses of methotrexate, actinomycin D and cyclophosphamide (MAC); and two courses of cisplatin and etoposide (PVP). Following this therapy, partial response was achieved, however, the tumor soon progressed and the serum HCG level increased to 7,571 mIU/ml. The patient was consequently given four courses of cisplatin, adriamycin, cyclophosphamide and etoposide (PACE) up to September 13, 1989, and received 52 Gy irradiation to the chest from July 20 to August 24. The tumor regressed markedly and the serum HCG decreased to within normal limits. The changes in serum HCG levels in serial samples correlated well with the clinical tumor burden. The patient is now in partial remission having survived for more than 15 months after the initiation of chemotherapy.

[Lumbar bone density and serum osteocalcin in pre- and postmenopausal women].

The bone mineral density of the lumbar spine (L3) and serum osteocalcin (OC) were measured in 31 pre- and 25 postmenopausal women. Bone density was measured by single energy quantitative computed tomography (QCT value). A significant inverse correlation between the QCT value and age was demonstrated in postmenopausal women but no correlation was found in premenopausal women. Mean QCT value was significantly lower in postmenopausal women than in premenopausal women. Serum OC increases with advancing age and mean serum OC was significantly higher in postmenopausal women than in premenopausal women. A significant inverse correlation between the QCT value and serum OC was demonstrated. These data suggest that there is an association between menopause and accelerated loss of lumbar bone density and that the serum OC level may be useful in evaluating the bone mass.
