The case reported concerns a 12 year old boy, admitted in 1970 with a clinical picture evoking a malignant hemopathy: thrombocytopenic purpura, leukopenia, multiple lymphadenopathies, enlarged spleen and, at histological examination, lympho-reticular hyperplasia of the spleen and lymphe nodes, simulating at the beginning, a nodular lymphoma. Immuno-hematological investigations revealed a complex auto-immune picture, which concerned essentially hematopoiesis. In fact, the pseudo-tumoral aspect of lymphenodes, was related to a dysfunction of the immune system, the pathogeny of which is presently better known.

Horton's disease is a generalized vascular disease characterized by giant cell arteritis. Medium and small-sized arteries are the most affected and involvement of the lingual arteries leading to lingual infarction and partial necrosis of the tongue is exceptional. We describe these complications in a 82-year-old female with histologically proved Horton's disease, and review the literature on the topic.
The present paper shows the results of a randomized, prospective trial carried out on 170 patients undergoing elective surgery for oropharyngeal cancer. These patients were distributed in two groups that received a prophylactic therapy with amoxicillin and clavulanic acid, for 24 hours and 5 days, respectively. Comparing results of short-term and long-term prophylaxis. Among patients undergoing only total or partial laryngectomies, no differences between both groups were found. Among patients who already had a tracheostomy, the rate of local infection was higher in the short-time group. Duration of prophylaxis in high-risk surgery, such as that requiring flap reconstruction, needs further investigation.
A case of chronic urticaria with macroglobulinemia (Schnitzler's syndrome) is described. To our knowledge, this is only the second case reported in the literature with osteolysis and the presence of cryoglobulins in sera. The characteristics of the previous 21 cases of this syndrome are reviewed. The pathogenesis of this peculiar entity is discussed.
The distribution of species and phenotypes of Yersinia isolated from environmental samples over an eight year period are compared to that of stool cultures obtained from patients of the same geographical location (Zaragoza, Spain). The number of samples and the percentage contamination were as follows: wastewater 362, 67.4%, freshwater 523, 13.4%, raw food 607, 24.5% and cooked food 1134, 7.9%. Yersinia enterocolitica was isolated significantly more frequently than other species in wastewater, while Yersinia intermedia was the most significant species found in freshwater. Significant differences between the percentage isolates of identified species in raw and cooked foods were not found. Fifteen different serogroups were identified from faeces, thirteen of which were also isolated from environmental samples. Three serogroups of Y. enterocolitica associated with human disease were isolated from the patients faeces as follows: O:3, 145 cases; O:8, 3 cases and O:5,27, 1 case. A low proportion were isolated from food: O:3, 3 strains; O:8, 2 strains and O:5,27, 5 strains. Only one isolate from serogroup O:3 was obtained from freshwater.
AIM: To study the significance of Gardnerella vaginalis in urine by comparing urine culture results, leucocytes count and clinical findings. METHODS: A total of 1,365 urine specimens submitted consecutively were inoculated on Bilayer media. G. vaginalis was recovered from 76 urine specimens with the following distribution: in 12 samples G. vaginalis was isolated as the only organism, 9 with counts > 10(5) CFU/ml and 3 between 10(5)-10(3); in the remaining 64 samples G. vaginalis was always found in counts > 10(5) CFU/ml but associated in 13 occasions with another organism and in 51 occasions with 2 or more organisms from the periurethral and vaginal flora. RESULTS: G. vaginalis in pure cultures corresponded to 8 women and 4 males whose ages ranged from 20 to 86 years. Six patients complained of symptoms of lower urinary tract infection (UTI), 3 had fever and in 3 the urine culture was practised as a control. Seven patients presented predisposing factors to UTI and 2 were pregnant. In only 2 cases pyuria was detected. Of 12 patients with pure cultures, only 3 were found to have G. vaginalis UTI. CONCLUSIONS: Our data indicate that G. vaginalis in urine not always implies an UTI and that this organism is found most often in patients with predisposing factors.

We analyze 402 drug-adverse events consisting of movement disorders or aggravation of parkinsonisms, submitted to Sistema Español de Farmacovigilancia until 1994. Our aim is to know patient characteristics and the drugs related with these submissions. Most of them (64) belong to calcium-entry blocker group (31%) and benzamides (27%). Case age intervals more frequent were 11-30 and 60-80 years-old and the events affect predominantly females. The percentage of serious adverse events were near 80%. We think that drug-related parkinsonisms have high prevalence rate and that the role of calcium-entry blockers in these events should be considered at the moment to prescribe groups.
A procedure has been developed for determination of total selenium in urine by hydride generation atomic absorption spectrometry. Mineralization was performed with a nitric acid-perchloric acid mixture on a thermostated digestion block. The method was validated by comparison with the method involving mineralization in a microwave acid digestion bomb containing nitric acid and small amounts of vanadium pentoxide. Se(VI) was reduced to Se(IV) by dissolution in 7N HCl. Sample recoveries, precision studies, and analyses of a certified reference material demonstrated the reliability and accuracy of this technique. Urine samples had selenium concentrations ranging from 4.6 to 50.3 micrograms/L. These values correspond to an average of 54.9 micrograms per person per day total ingested and bioavailable Se in the daily diet.
