Substantiation of sexual abuse charges when parents dispute custody or visitation.

Recent news reports have implied that charges of sexual child abuse during parental separation or divorce are often deliberately falsified. Such a conclusion could be harmful if it biased practitioners faced with such allegations in clinical practice. To investigate this concern, sexual abuse cases in a hospital-based consecutive series and in one author's clinical practice were reviewed. Abuse allegations with and without a concomitant custody or visitation dispute were compared. A custody or visitation dispute occurred in 12 (39%) of 31 sexual abuse complaints lodged against a parent. Allegedly abused children whose parents contested custody or visitation were significantly younger than those for whom custody or visitation was not an issue (5.4 v 7.8 years, P = .02). Sexual abuse allegations were substantiated less frequently when there was concomitant parental conflict (67% v 95%, nonsignificant) but were nevertheless substantiated more than half of the time.

Family functioning and the adolescent mother: a systems approach.

This study examines unwanted adolescent pregnancy and early childbearing within the context of the family system. Fifty pregnant adolescents and their families were interviewed prenatally and again during the postpartum period. Utilizing the concepts of structural family theory and therapy as described by Minuchin, certain characteristics of family style and structure of organization were rated. These family variables were then related to aspects of the adolescent mother's adaptation postpartum. Boundaries, in terms of degree of intrusiveness and differentiation, were related to such variables as whether the adolescent is maintained in the household and to her continuing relationship with the baby's father. Similarly, the family's style of dealing with conflict was related to the relationship between the adolescent parents, among other outcome variables. Implications of the findings both for working with these families and for further research are discussed, and issues are raised about hypothesized relationships between independent and dependent variables which were not borne out.

Isoelectric focusing and immunofixation of cerebrospinal fluid oligoclonal IgG in neurological disorders.

Coded samples of cerebrospinal fluid (CSF) from 112 patients with a variety of neurologic disorders were investigated for oligoclonal IgG bands by isoelectric focusing followed by immunofixation. All 16 patients with probable multiple sclerosis had oligoclonal bands. All four patients with paraneoplastic syndromes having multifocal nervous system involvement also had bands, whereas five patients with lupus, three having multifocal nervous system involvement, did not show any oligoclonal bands in the CSF. This technique is highly sensitive in detecting bands in MS, whereas other disorders that produce demyelination showed various levels of sensitivity.
