Bilateral hydrothorax and hydromediastinum after a subclavian line insertion.

A 28-year-old male patient developed bilateral hydrothorax due to extravasation of fluid into the mediastinum from a subclavian line. The injection of radio-opaque dye through the central venous cannula confirmed spillage into the mediastinum. There was no direct communication between the central venous cannula and the pleural cavities. The hydrothorax appeared to develop as a result of a shift of fluid from the mediastinum into the pleural cavities due to pressure differences in the two compartments. Bilateral chest tubes were inserted, the subclavian cannula was removed and the patient made a good recovery.

Atracurium for short surgical procedures: a comparison with succinylcholine.

A comparison was made between atracurium and succinylcholine in 40 patients undergoing short gynaecological procedures of 30 minutes or less. Good intubating conditions were produced in 76.7 +/- 39.3 seconds (mean +/- S.D.) with succinylcholine 1 mg . kg-1 and 198 +/- 84 seconds with atracurium 400 micrograms . kg-1. Muscle relaxation was maintained with the initial dose of atracurium or with repeated boluses of succinylcholine. The mean time of surgery was 17.65 +/- 5.3 minutes in the atracurium group and 15.2 +/- 4.6 minutes in the succinylcholine group. Residual neuromuscular block with atracurium was reversed with neostigmine 0.036 mg . kg-1 and atropine 0.018 mg . kg-1. Recovery of neuromuscular function following reversal, assessed by return of all responses to train-of-four stimulation occurred in 5.05 +/- 4.6 minutes in the atracurium group but half the above doses of neostigmine and atropine were repeated in three patients. We conclude that a single dose of atracurium 400 micrograms . kg-1 is suitable for intubation and maintainance of muscle relaxation for short surgical procedures. However, the onset of action is slow, compared to succinylcholine. Residual neuromuscular block can be antagonised with standard doses of neostigmine, less than 20 minutes after the initial dose of relaxant. Atracurium appears to be a suitable alternative for short procedures where succinylcholine is unsuitable or contraindicated.

High dose epidural morphine for surgical analgesia.

A group of 30 patients undergoing various operative procedures were anesthetized using high dose epidural morphine 20 mg in 15 ml of normal saline, thiopentone induction, endotracheal intubation and muscle relaxation with pancuronium 0.1 mg/kg and controlled ventilation with oxygen and nitrous oxide. Operative conditions and patient acceptance of the technique were satisfactory. There was cardiovascular stability and prolonged postoperative analgesia. Pruritus was observed in three patients and urinary retention in one and were adequately treated. Early mild respiratory depression occurred in one patient and this was treated with naloxone. High dose epidural morphine is efficacious and safe in providing intraoperative and postoperative analgesia.

Caudal morphine for pain relief following anal surgery.

A prospective randomised trial was conducted to evaluate caudal morphine for pain relief following anal surgery. Thirty-three male patients were divided into two groups. Group I (17 patients) were given morphine 10 mg in saline 25 ml injected into the caudal epidural space at the end of surgery. Group II (16 patients) were given intramuscular morphine 10 mg on request in the postoperative period. Pain was assessed at 2, 6, 12 and 24 hours after surgery using the linear analogue of pain score. There was significant pain relief in Group I compared with Group II. There was no instance of respiratory depression in patients of either group. In Group I, five patients suffered urinary retention and three patients transient pruritus.

Cardio-pulmonary resuscitation--two years experience in a new teaching center.

Cardiac arrest calls at a new tertiary referral hospital are reviewed over a two year period. The formation of a committee responsible for conducting and supervising the resuscitation team is outlined. There were 146 calls for 120 patients, 49 patients (40.8%) were initially resuscitated and 8 (6.66%) of these were discharged home. Age did not appear to influence the prognosis. Coronary heart disease was the commonest pathology (40%) of ventricular fibrillation and tachycardia were the leading causes of cardiac arrest.

Anaesthesia with continuous thiopentone infusion, epidural morphine and relaxant.

Twenty-three patients undergoing surgery below T.6 dermatome were anaesthetized with epidural morphine 20 mg in 15 ml of saline, thiopentone drip 0.04-0.1 mg/kg/min. and pancuronium 0.1 mg/kg. The patients had satisfactory analgesia intra-operatively. Post-operative analgesia lasted for varying periods ranging from 13 hours to 24 hours and above. The cardiovascular system was stable. Post-epidural morphine complications of respiratory depression (three patients), urinary retention (one patient) and pruritus (one patient) were seen and they were managed successfully.

Prediction of outcome in senile dementia--a computed tomography study.

Forty patients suffering from senile dementia who had been subjected to detailed clinical and psychological assessment and computed tomography (CT) were followed up for a mean period of 28.78 months. All but one were traced. The deceased (27) were compared with the survivors (12). All males had died at follow-up. The mean age was not statistically significant, but the survivors differed significantly from the deceased in having performed better on a number of clinical and psychological tests, particularly those involving speech functions and constructional ability. Measures of ventricular size and cortical atrophy were not of predictive value, but a new technique of measuring radiological density showed that this was significantly lower in the right parietal region in the original CT scans of those who subsequently died. The study confirms that clinical involvement of the parietal lobes is an indication of poor prognosis and reports the first radiological support for this view. It also suggests that a more directly quantitative approach to computed tomography may yield results which are more useful than those obtained from visual reconstructions.

CT scanning in senile dementia. A follow-up of survivors.

Survivors from a group of 40 carefully evaluated subjects with senile dementia who had been scanned over two years previously were followed-up, re-scanned and given simple cognitive tests. Of the 12 survivors, 10 agreed to participate. These fell into two sub-groups: one who showed further deterioration in cognitive function and one who did not. The first sub-group showed an increase in ventricle-brain ratio while the second did not. Although the numbers were too small for clear statistical differences to emerge, there was a tendency for affective admixture to be a favourable prognostic indicator and low radiological attenuation density particularly in the right parietal region to be an unfavourable one.

Alternative carboxylation reactions in type II methylotrophs and the localization of carboxylase activities in the intra-cytoplasmic membranes.

The peripheral arrangements of the intra-cytoplasmic membranes (ICM) as well as the guanine + cytosine (G + C) content of 63% indicate that our obligate methanotrophic strain M 102 would belong to the type II Methylotrophs. There are no active transport mechanisms for some organic additives, some of which are known to stimulate CO2-fixation. The CO2-acceptor molecules, Pyr (pyruvate) and PEP (phospho-enolpyruvate), must be metabolically intracellularly furnished. More than one carboxylating system could be induced in cell fractions. The carboxylating activities were variably registered in cell-free extracts and fragments. Indications that the site of the carboxylating activities are the ICM are discussed. The fact that the CO2-fixation intensities decreased at about the middle of the active growing phase indicates that the formation of the intermediate C4-compound OAA (oxaloacetate) must be furnished through some reactions by-passing the PEP-carboxylation step. The common serine-pathway for C1-assimilation is rearranged so as to fit in the discussed findings.

Stoichiometry of methane oxidation in the methane-oxidizing strain M 102 under the influence of various CH4/O2 mixtures.

In laboratory-scale experiments with growing cells of the obligate methane-oxidizing strain M 102, an overall molar gas turnover ratio of the order given below could be postulated: 1 CH4+1--1.2 O2=0.3 CO2+water. Expectations that the optimal gas mixture of methane and oxygen should lie within the range of this stoichiometric consumption ratio have been verified in fermenter 5 1 batch culture experiments. The optimal range of methane-oxygen mixture, found under the experimental conditions described, is based on the estimated growth parameters as generation and doubling times, yield coefficients related to methane and oxygen, and the efficiency of methane metabolism as indicated in the absolute amounts of CH4, O2, and CO2 turned over. The mentioned stoichiometric relation of 1 CH4:1--1.202 did not change with varying the composition, i.e. the partial pressures of CH4 and O2 introduced as a mixture to the cells. The efficiency of methane oxidation was obviously influenced and decreased markedly when deviating from the broad optimal range of CH4/O2 mixtures. With non-growing cells, on the other hand, the stoichiometric relation showed a considerable shift (1:1.4--1.8 CH4:O2) with a clear tendency towards more O2 consumption. The oxidation potential of growing cells, seems then to have a linear interdependence to the substrate concentrations, i.e. partial pressures.
