Eight respiratory parameters which might affect the amount of carbon dioxide rebreathing were assessed in seven patients who were breathing spontaneously from large-bore T tube system during the recovery phase from acute respiratory failure. With multivariate regression analysis, the absolute amount of rebreathed CO2 at the connector of endotracheal tube (VINSPCO2) were approximately estimated by using relatively small number of parameters, including minute volume (VEXP), fresh gas inflow to T piece system (VFGI) and preferably by additional parameters concerning CO2 output of the patients.CO2 rebreathing ratio, VINSPCO2 divided by gross outward flux of CO2 at the connector (VEXPCO2), was predicted with simple regression equation by using (VEXP/VFGI) as follows,(VINSPCO2)/(VEXPCO2) = 0.405 + 0.33 x ln (VEXP/VFGI)The maximum (VEXP/VFGI) ratio to prevent rebreathing of CO2 at the connector was 0.30, whereas the ratio to prevent CO2 accumulation due to rebreathing was 0.45.
The concentration of urinary fibrin(ogen) degradation products (FDP) was determined by using enzyme immunoassay and radio immunoassay, and their urinary levels were compared with histologically classified types of glomerulonephritis. Urinary FDP levels were higher in the severe type of proliferative glomerulonephritis and at the active phase of systemic lupus erythematosus (SLE). They were also high in the membranous glomerulonephritis. Molecular weight of urinary FDP of a patient with severe proliferative glomerulonephritis was determined to be 150,000 and 68,000, respectively after gel filtration. Urinary FDP levels were higher in patients with glomerular fibrin deposit than in patients without fibrin deposit or normal volunteers. The amounts of excreted protein in urine was not related to the amounts of FDP. Decrease in the reciprocal of serum creatinine levels resulted in high urinary FDP levels, indicating that high urinary FDP levels may represent deterioration of renal function.
We report a case of multiple, simple bone cysts in long tubular, short tubular, and flat bones. The internal pressure of the cysts and the oxygen tension of cyst fluid were measured simultaneously. These values were similar to those of typical solitary cysts in other patients. Curettage and bone graft onto the calcaneal cyst were effective, although steroid injection therapy into the metacarpal cyst had no effect. Piercing the ulnar and radial cysts a number of times was uniformly effective. These results may suggest that venous obstruction of the lesions play an important role in the etiology of multiple simple cysts, as shown in typical bone cysts. However, we could not exclude the possibility that simple bone cysts, solitary or multiple, represent some kind of secondary forms of other disorders, or that a simple bone cyst is a heterogeneous entity.
Disappearance of proteinuria was observed in a patient with hepatitis B-associated chronic glomerulonephritis after treatment with leukocyte interferon. The decrease of proteinuria was preceded by the disappearance of both deoxyribonucleic acid polymerase activity and hepatitis B e antigen from the patient's sera.
The adsorption of ferricytochrome c and ferrocytochrome c on biomaterials was studied by measuring the spectrum of samples assuming the molar extinction coefficients of the ferricytochrome c and ferrocytochrome c at 480 nm and 550 nm. Silica, alumina ceramic, zirconium ceramic, tricalcium phosphate ceramic, hydroxyapatite ceramic, unidirectional calcium phosphate glass-ceramic, bulk crystallized calcium phosphate glass-ceramic and bulk crystallized calcium phosphate glass-ceramic containing large amount of titanium were used as adsorbents. The adsorption of ferricytochrome c or ferrocytochrome c on silica essentially followed the Langumuir adsorption isotherm. Quantitative analysis showed that ferricytochrome c was adsorbed more and dissolved less than ferrocytochrome c on batch adsorption on silica. Total amounts of adsorbed and non-adsorbed ferricytochrome c increased significantly in batch adsorption of ferrocytochrome c on silica. From these experiments it appears that materials such as silica induce a transfer of electrons of cytochrome c.
By repeated biopsies, time-sequential changes of the injected colloidal carbon distribution were investigated in glomeruli of NZB/W F1 mice. The intraglomerular shift of the carbon, evaluated by counting the particles at several time intervals after carbon injection, demonstrated the movement of the carbon from the glomerular capillaries to the extraglomerular areas. Electron-microscopic examination disclosed that most of the increased cells of the glomeruli were mononuclear phagocytes rich in ingested carbon particles and that extracellular carbon was scarcely present. This strongly suggested that the carbon particles observed at the light-microscopic level reflected those ingested by the phagocytes. The mesangial cells per se scarcely ingested carbon particles. Instead, mononuclear cells, extending their cytoplasmic protrusions toward the dense (immune complex) deposits, were frequently noticed. It is concluded that the mononuclear phagocyte is a principal component of the hypercellular glomeruli, presumably contributing to the scavenging of the mesangium, and also that there is a pathway in the mesangium for these cells to shift from the capillary to the extraglomerular area by way of the vascular pole and lacis area.

The effects of intralesional versus ip administration of cisplatin (total, 24 mg/kg) given on different schedules were studied using transplantable squamous cell carcinoma of mice. The size of the tumor in the control group increased approximately 60 times at the end of the experiments, whereas it decreased to 0.04 of the initial volume following a single intralesional injection of cisplatin. The ratio of treated/control mice was 0%. The comparable values in the groups with intralesional cisplatin at 3- and 1-week intervals were 7% and 20%, respectively. The responses of the tumor to cisplatin in the ip groups were lower than those of the intralesional groups and were not dependent on the schedule of administration. Toxic deaths were less frequently encountered in the intralesional groups of animals. Platinum concentration in tumors of the intralesional group was two to five times higher than that of the ip group 15 minutes to 96 hours after injection of 12 mg/kg of cisplatin, whereas the concentration in sera of the intralesional group remained lower than that of the ip group until 48 hours after the injection. The results indicate that the intralesional route produces a good response of tumor to cisplatin by maintaining high levels of the non-protein-bound form of the agent in the tumor tissue that cannot be provided by other routes of administration.
A sensitive enzyme immunoassay was developed for the determination of urokinase in the plasma and urine. Normal human urine contained 2.68 +/- 0.36 u/ml of UK. Gel filtration of the mixture of normal urine resulted in 2 peaks in the profile, one with molecular weight of 32,000 and the other with molecular weight of 22,000. Patients with glomerulonephritis had higher UK levels. Kidneys with minimal changes and mild type or with the inactive phase of systemic lupus erythematosus (SLE) secreted relatively high levels of UK in the urine.
Glomerular responses to angiotensin II (AII), arginine vasopressin (AVP), and norepinephrine (NE) were estimated in rabbits recovering from uranium-mediated nephropathy or ischemic acute renal failure (ARF) to examine roles of intraglomerular events in resistance to ARF. Uranyl acetate (UA, 0.8 mg/kg) produced ARF in some animals but did not in others. Rabbits recovering from UA-induced ARF were highly resistant to a rechallenge with a larger dose of the agent (2 mg/kg). Their glomeruli did not respond to AII, AVP and NE in vitro. In animals having not experienced ARF following the initial insult, however, resistance to the rechallenge was lower than in animals recovering from ARF, and the glomerular response to contractile stimuli was well sustained. A two hour clamping of the renal artery induced ARF in uninephrectomized rabbits. These animals were not resistant to an additional ischemia in the recovery phase, despite inhibited glomerular contractile responses to AII. These data indicate a nonspecific inhibition of glomerular responses to contractile stimuli in the recovery phase of ARF. It is unlikely, however, that resistance to ARF can be attributed to the loss of the glomerular contractile response.

The cases of nineteen patients with sternocostoclavicular hyperostosis were reviewed retrospectively. Of particular interest were the responses to antibiotics and prostaglandin inhibitors. The prostaglandin inhibitors relieved the pain within three to four weeks in sixteen of eighteen patients so treated. However, the inhibitors gradually became less effective in most patients. Oral antibiotics were more effective than the inhibitors in relieving the pain of eight of the eleven patients who were given antibiotics. Pustulosis palmaris and plantaris, commonly associated with sternocostoclavicular hyperostosis, diminished after antibiotic therapy, as did the chest pain in most patients. The similarities between the age and sex distributions and the responses to antibiotics of the patients with sternocostoclavicular hyperostosis and those with pustulosis suggest that these disorders have a common etiology, and that the pustulosis may be a so-called bacterid reaction and the hyperostosis, a manifestation of a systemic reaction to a focal infection.

The plasma corticosterone levels and incorporation of 14C-l-acetate into corticosterone and cortisol were measured in dehydroepiandrosterone (DHA)-treated persistent estrous rats and control estrous cyclic rats exposed to cold (-5 degrees C) for 12 hrs. Cold exposure significantly facilitated incorporation of 14C-l-acetate into corticosterone and cortisol in both DHA-treated and control rats. However, the rates of increase of plasma corticosterone level and 14C-incorporation into corticosterone are 61% and 77% in controls, and 19% and 44% in DHA-treated rats, respectively. The activity of stress-induced pituitary-adrenal function showed masculine patterns in DHA-treated rats. The diurnal patterns of plasma corticosterone in DHA-treated rats are similar in controls. Plasma corticosterone levels in DHA-treated rats are evidently greater than those in controls. It is suggested that neonatal administration of DHA caused the changes not only in pituitary-gonadal system but also in pituitary-adrenal system.
5 cases with the compatible serological criteria of mixed connective tissue disease described earlier are presented. In 1 of them with a moderate degree of proteinuria, the renal biopsy disclosed membranous nephritis. However, despite the absence of overt clinical renal disease in the other 4 cases, biopsies disclosed membranous nephritis in 1 and mild mesangial proliferative glomerulonephritis in the remaining 3 cases. In the follow-up of these 4 cases, 2 subsequently developed abnormal urinalysis. Electron microscopic examinations demonstrated electron-dense deposits in glomeruli, and 4 of these patients also had microtubular structures in the endothelial cytoplasm. Contrarily to the original concept, our findings suggest that mixed connective tissue disease also induces immune complex disease.
Six skeletal congeners of polychlorinated quaterphenyls (PCQs), namely polychlorinated o-quaterphenyl (2,2'-PCQ), 2,3'-diphenylbiphenyl (2,3'-PCQ), 2,4'-diphenylbiphenyl (2,4'-PCQ), m-quaterphenyl (3,3'-PCQ), 3,4'-diphenylbiphenyl (3,4'-PCQ) and p-quaterphenyl (4,4'-PCQ), were orally administered to Wistar rats at a dose of 10 mg/rat. On the 5th day after administration of PCQs, the rats were examined for accumulation of PCQ congeners, hepatic enzyme activities and organ weight changes. Accumulation of 3,3'-PCQ, 3,4'-PCQ and 4,4'-PCQ were 1.5-3.2% of dose in the liver, while those of 2,2'-PCQ, 2,3'-PCQ and 2,4'-PCQ were only 0.1 to 0.2%. The amount of 4,4'-PCQ accumulated in the mesenteric adipose tissue, 20 micrograms/rat, was much higher than those of other PCQ congeners. Large amounts of the PCQ congeners administered were excreted in the feces on the first day, accounting for 96 to 98% of dose for 2,2'-PCQ and 4,4'-PCQ, and 55 to 75% for the other PCQ congeners, and the daily excretions of PCQs after the second day were very small, less than 10% of the dose. Benzo [a] pyrene 3-hydroxylase activity was significantly depressed by the treatment with 3,3'-PCQ, 3,4'-PCQ and 4,4'-PCQ, contrasting to the toxic congeners of polychlorinated biphenyls and dibenzofurans which enhanced markedly this enzyme activity. DT-Diaphorase activity was also depressed by the treatment with 2,3'-PCQ, 2,4'-PCQ and 3,4'-PCQ. Significant atrophy of the thymus was observed by the treatment with 4,4'-PCQ.
