Massive splenomegaly responsive to proguanil and with features of hairy cell leukaemia.

A recent Ethiopian immigrant to Israel presented with pneumococcal sepsis, massive splenomegaly and lymph-adenopathy. Investigations revealed many features of both hairy cell leukaemia (HCL) and hyperreactive malarious splenomegaly (HMS). Proguanil therapy for HMS was followed by rapid, marked decrease in spleen size, disappearance of the tartrate-resistant acid phosphatase-positive cells characteristic of HCL, and increasing eosinophilia, but unchanged lymphadenopathy.

Hyperreactive malarial splenomegaly and AIDS: proguanil prevents recurrent pneumonia.

An Ethiopian immigrated to Israel and gave a positive test result for human immunodeficiency virus. Soon after, he was thrice admitted to hospital in 3 months for severe pneumonia. Acquired immunodeficiency syndrome was diagnosed, as was hyperreactive malarial splenomegaly (HMS). Thereafter, during maintained proguanil treatment for HMS, the patient remained well for 16 months. Then, after another pneumonic illness, pneumococcal vaccine was administered. Proguanil was maintained for another 10 months and, despite declining CD4 cell counts, good health continued for a further 18 months.

Histiocytic medullary reticulosis (malignant histiocytosis) in Zambia.

Fifteen cases of histiocytic medulary reticulosis are reported from Zambia, Africa. The importance of liver biopsy in the diagnosis is stressed. Forty cases have now been described from East and Central Africa suggesting a much higher incidence than in other parts of the world. It is thought that this may be due to prolonged stimulation of the reticuloendothelisl system.

Splenomegaly in Zambia.

In this paper is reported the results of spleen surveys carried out as the opportunity arose in various groups in Zambia during the period January 1965 to October 1968. The lowest spleen rates were found in urban workers and in long stay adults in leprosy and psychiatric hospitals. The highest rates were in rural school children and children in a rural hospital. Intermediate between these groups were adults in an urban and in a rural general hospital.

Hepatic bruit in Hodgkin's disease.

Two cases of stage IV B Hodgkin's disease of the mixed cellularity type are described who had loud hepatic bruits. This physical sign is previously described only in cases of hepatocellular carcinoma and acute alcoholic hepatitis. The aetiology of the bruit in one of the cases is probably arteriolar compression by tumour tissue.
