Astrocytoma adhesion to extracellular matrix: functional significance of integrin and focal adhesion kinase expression.

Evidence is accumulating implicating a role for integrins in the pathogenesis of cancer, a disease in which alterations in cellular growth, differentiation, and adhesive characteristics are defining features. In the present report we studied a panel of 8 human astrocytoma cell lines for their expression of integrin subunits by RT-PCR, and of integrin heterodimers by immunoprecipitation analyses. The functionality of integrin heterodimers was assessed using cell attachment assays to plastic or single matrix substrates. Downstream effects of integrin activation were studied by western blot analyses of FAK expression in human astrocytoma cell lines growing on plastic and on a fibronectin matrix, and in 13 primary human brain tumor specimens of varying histopathological grade. Furthermore, we studied tyrosine phosphorylation of FAK in astrocytoma cells growing on plastic versus fibronectin. Finally, we analyzed the effects of intermediate filament gene transfer on FAK phosphorylation in SF-126 astrocytoma cells. Our data show that astrocytoma cell lines express various integrin subunits by RT-PCR, and heterodimers by immunoprecipitation analyses. The beta1 and alphav integrin subunits were expressed by all astrocytoma cell lines. The alpha3 subunit was expressed by all cell lines except SF-188. By immunoprecipitation, the fibronectin receptor (alpha5beta1 integrin heterodimer) and the vitronectin receptor (alphavbeta3) were identified in several cell lines. Astrocytoma cell attachment studies to human matrix proteins suggested that these integrin heterodimers were functional. Using confocal laser microscopy, we showed that FAK was colocalized to actin stress fibers at sites of focal adhesion complexes. By western blot, FAK was variably but quite ubiquitously expressed in human astrocytoma cell lines, and in several primary human astrocytoma specimens. When U373 and U87 MG astrocytoma cells bind to a fibronectin matrix, FAK is phosphorylated. GFAP-transfected SF-126 human astrocytoma cells were shown to overexpress the phosphorylated form of FAK only when these cells were placed on a fibronectin matrix. This result is of interest because it suggests that manipulations of the astrocytoma cytoskeleton per se can bring about potential signaling changes that channel through integrins and focal adhesion sites leading to activation of key kinases such as FAK.

Nd-YAG laser vs bronchoscopic electrocautery for palliation of symptomatic airway obstruction: a cost-effectiveness study.

STUDY OBJECTIVE: To evaluate the cost effectiveness of the Nd-YAG laser and bronchoscopic electrocautery for palliation in patients with symptomatic tumor obstruction. DESIGN: A retrospective study. SETTING: Bronchoscopy unit of a university hospital. PATIENTS AND INTERVENTION: Thirty-one consecutive patients with inoperable non-small cell lung cancer and symptomatic intraluminal tumor underwent bronchoscopic treatment. Dyspnea relief was the primary goal of treatment. Fourteen patients were treated with the Nd-YAG laser and 17 patients with electrocautery. MEASUREMENTS AND RESULTS: Improvement of symptoms was achieved in 70% of patients treated by either Nd-YAG laser or electrocautery. Mean +/- SD survival was 8.0 +/- 2.5 months after Nd-YAG laser treatment and 11.5 +/- 3.5 months after electrocautery. The number of treatment sessions per patient was comparable: Nd-YAG laser, 1.1; electrocautery, 1.2. Duration of hospital stay was longer in patients treated with the Nd-YAG laser (8.4 vs 6.7 days). Average treatment costs, including admission charges, were $5,321 for the Nd-YAG laser and $4,290 for electrocautery. Higher costs in the group treated with the Nd-YAG laser were caused by a longer hospital stay before bronchoscopic treatment. Costs of equipment (electrocautery $6,701 and Nd-YAG laser $208,333), write-offs, maintenance, and repair were not included in this calculation. CONCLUSION: Bronchoscopic electrocautery is equally effective but is a less expensive and, in our hospital, a more accessible modality than the Nd-YAG laser for symptomatic palliation of patients with intraluminal airway obstruction.

A management model for professional wholeness.

The purpose of this article is to describe a management model for professional wholeness. A theory generating research design was followed. Based on this model the consequences of management could be a state, which varies dynamically between minimal and maximal professional wholeness. A list of descriptive words was developed to generate qualitative indicators of professional wholeness.

[Nurses' perceptions of inservice training in a private hospital].

A private hospital aims at quality health care delivery with a profit making motive. Inservice education to nursing staff therefore plays an important role--it should add value to the hospital and should also be cost-effective. This research hospital has a formal inservice education programme for the professional nurses, but the question arises: what are their perceptions of the inservice education in this hospital? The aim with this research is to explore and describe the perceptions of the professional nurses on inservice education and to formulate guidelines for improvement. A qualitative research strategy was utilised, with individual focus interviews as method of data collection, followed by a content analysis of the transcribed interviews. Two main categories were identified, being the positive or facilitative perceptions and the negative perceptions/obstacles. Concluding statements were formulated to serve as basis for the description of guidelines on inservice education within this hospital. Guidelines for quality inservice education were formulated, relating to the following: scientific principles to make provision for appropriate assessment, planning, implementation and evaluation of the inservice education programme; accessibility of the inservice education sessions; establishment of a positive learning climate by the learning accompanist; credibility of the inservice education programme in the private hospital. It is recommended that these guidelines be implemented and that selected statements be converted to hypotheses and exposed to testing.

[Standards for neonatal intensive care nursing: unit directed management standards].

The neonate has the right to quality nursing care and the Neonatal Intensive nursing care practitioner is professionally-ethically and personally liable for quality nursing care. The process of quality improvement is a structured, planned and purposeful action were standards are set and the nursing care is evaluated after which remedial steps are taken to improve quality nursing care. In this study the focus is on the first step in the quality assurance cycle:the setting of standards. The purpose of the study is to describe and formulate standards for Neonatal Intensive nursing care which can be utilised as an accreditation instrument for institutional selfevaluation to improve quality nursing care. Standards for Neonatal Intensive nursing care were developed and validated by utilising a three-phase research method. In phase one subjects for standards were identified by a panel of experts. The identification was done by means of a critical debate, after which a preliminary conceptual framework was formulated. During the second phase a literature control was done to refine the conceptual framework. It consisted of a conceptual framework pertaining to unitmanagement (article 1) and an conceptual framework pertaining to high incidency, high risk interactions in the Neonatal Intensive Care Unit. The conceptual framework pertaining to the high incidency, high risk interactions in the Neonatal Intensive Care Unit consisted of: assessment of the neonate preparedness for neonatal resuscitation mechanical ventilation humidification during ventilation physiotherapy and suctioning of the ventilated neonate weaning during mechanical ventilation ecstubation During the third phase, the standards were validated by means of a consencus debate. An accreditation instrument was developed for institutional selfevaluation to improve quality nursing care in the Neonatal Intensive Care Unit. The standards that were formulated consisted of standards for unitmanagement (article 1) and standards directed at high incidency, high risk interactions in the Neonatal Intensive Care Unit which is published in the second article in this series.

The derivation of knee joint types from the geometry of the cruciate ligament four-bar system.

The system of the anterior (a) and posterior (p) cruciate ligaments and their distances between attachments to femur (f) and tibia (t) as found in the knee joint of tetrapods is considered as a planar crossed four-bar linkage. The shape of the femoral articulating surfaces (condyles) can be calculated starting from a flat or curved tibial articulating surface and known bar-lengths (Menschik, 1974 Z. Orthop. 112, 481-495; Huson 1974 Orthopäde 3, 119-126). Regression analysis of the dimensions of the cruciate ligament four-bar system of 11 species of mammal and one species of bird revealed a general ratio of (a): (t): (p): (f) = (7.1): (7.9): (10.0): (6.1). These data differ from the results obtained by Badoux (1984 Acta Anat. 119, 60-64) who examined only dog and horse. Our data of the dog agree with those of Badoux, i.e. (a): (t): (p): (f) approximately equal to (10): (8): (10): (4). Based on these ratios between bar-lengths, two types of knee joint shapes were distinguished. The shape of the dog's joint ("type A") has a very large femoral condyle compared with the tibial articulating surface. Maximum knee angulation is 170-180 degrees. Sliding between the articulating surfaces of this joint is distributed approximately uniformly over the whole angulation range. The general shape obtained from the regression analysis ("type R") has a relatively small femoral condyle and an angulation range of about 174 degrees. Uniformly distributed sliding occurs with this range over an angle less than 90 degrees. Theoretically derived, limiting requirements concerning maximum angulation range (delta gamma max < or = 180 degrees), stabilization (e.g. avoidance of a perpendicular position of the cruciate ligaments to the articulating surfaces; delta gamma 78.5 max > or = 90 degrees) and uniformly distributed sliding (delta gamma s > or = 30 degrees) lead to at least two different possible knee joint shapes. These shapes correspond to the two real knee joint shapes found from the statistical analysis mentioned above. This was verified by studying quantitative characteristics obtained from the derivation of knee joint shapes from the bar lengths and vice versa. The bird (Ardea) possessed a knee joint shape, very different from the shapes described above (i.e. f > t, type D1).

Translation, validation, and norming of the Dutch language version of the SF-36 Health Survey in community and chronic disease populations.

The primary objectives of this research were to translate, validate, and generate normative data on the SF-36 Health Survey for use among Dutch-speaking residents of the Netherlands. Translation of the SF-36 into Dutch followed the stepwise, iterative procedures developed by the IQOLA Project. Following extensive pilot testing, the SF-36 was administered to: (1) a random sample of adult residents of Amsterdam (n = 4172); (2) a random, nationwide sample of adults (n = 1742); (3) a sample of migraine sufferers (n = 423); and (4) a sample of cancer patients undergoing active anti-neoplastic treatment (n = 485). Data quality across the four studies was consistently high. The rates of missing data ranged from 1% to 5% at the item level, and from 1.2% to 2.6% at the scale level. Multitrait scaling analysis confirmed the hypothesized scale structure of the SF-36 and associated scale scoring in all four samples. Cronbach's alpha coefficients surpassed the 0.70 criterion for group comparisons in all but one case (the Social Functioning scale in the cancer sample), with a mean alpha coefficient across all scales and samples of 0.84. Known-group comparisons yielded consistent support for the validity of the SF-36. In the two community samples, statistically significant differences in SF-36 mean scale scores were observed as a function of age, gender, and the prevalence of chronic health conditions. In the migraine and cancer samples, mean SF-36 scale scores varied significantly as a function of various indicators of disease severity. The SF-36 profiles for the two community samples were highly similar. The cancer sample yielded the lowest SF-36 scores, with the migraine sample holding an intermediate position. On-going studies will generate data on the responsiveness of the SF-36 to within-group changes in health over time. Efforts are underway to translate and validate the questionnaire for use among ethnic minority groups in the Netherlands.

Effect of seasonal acclimatization on the expression of the carp transcription factor Pit-1.

We isolated a clone comprising four exons of the carp Pit-1 gene. Using synthetic oligonucleotide probes derived from the carp Pit-1 sequence Pit-1 expression was assessed by in situ hybridization in pituitary sections from summer- and winter-acclimatized carp. Semiquantitative analyses of the hybridization signals revealed a significant higher Pit-1 expression in the proximal pars distalis (PPD) and pars intermedia (PI) of the pituitary glands from summer-acclimatized carp, compared to the winter-acclimatized fish. In both adaptive states, relative to the PPD and PI, only a basal Pit-1 expression was detected in the rostral pars distalis. Thus, during seasonal acclimatization of an eurythermal fish, Pit-1 seems to be involved in the mechanisms that underlie the compensatory response.

Functional analysis of ZNF85 KRAB zinc finger protein, a member of the highly homologous ZNF91 family.

We previously identified the ZNF85 (HPF4) KRAB zinc finger gene, a member of the human ZNF91 family. Here, we show that the ZNF85 gene is highly expressed in normal adult testis, in seminomas, and in the NT2/D1 teratocarcinoma cell line. Immunocytochemical localization of a panel of beta-Gal/ZNF85 fusion proteins revealed that ZNF85 contains at least one nuclear localization signal located in the spacer region connecting the KRAB domain with the zinc finger repeats. Bacterially expressed ZNF85 zinc finger domain bound strongly and exclusively to DNA in vitro in a zinc-dependent manner. The KRAB(A) domain of the ZNF85 protein and of several other members of the ZNF91 family exhibited repressing activity when tested in Gal4 fusion protein assays. The repression was significantly enhanced by the addition of the KRAB (B) domain, whereas further addition of other conserved regions had no effect. The ZNF85 KRAB(A) and (B) domains in vitro bound several nuclear proteins that might constitute critical cofactors for repression.

Pro239Ser: a novel recessive mutation of the Pit-1 gene in seven Middle Eastern children with growth hormone, prolactin, and thyrotropin deficiency.

Pit-1, a member of the POU-homeo domain protein family, is one of the transcription factors responsible for anterior pituitary development and pituitary-specific gene expression. Here, we describe seven children with GH, PRL, and TSH deficiency from three, reportedly unrelated, Middle Eastern families, harboring a newly recognized Pro- > Ser recessive mutation in codon 239 of the Pit-1 gene. The mutated residue is located at the beginning of the second alpha-helix of the POU-homeodomain and is strictly conserved among all POU proteins. The Pro239Ser mutant binds DNA normally but is unable to stimulate transcription.

Transcription factor AP1 is involved in basal and okadaic acid-stimulated activity of the human PRL promoter.

The tumor promoter, okadaic acid (OA), an inhibitor of protein phosphatases, stimulates the activity of the human PRL (hPRL) proximal promoter. We analyzed in detail the effects of OA on transcription factor binding to elements P1 and P2 of this promoter, sequences known to contain at least one Pit-1 binding site each. OA treatment induces binding of an AP1-related transcription factor to the P1 site. This effect is specific, as protein binding to the P2 site is not altered by the treatment. Specific antibodies were used to confirm that the OA-induced complex is related to AP1 and to show that it contains JunD and c-fos, but not Pit-1. The increase in AP1 binding to P1 and to a canonical AP1 site correlates to an increase in cellular JunD and c-fos content. Transient transfection experiments showed that both AP1 and Pit-1 are involved in the regulation of basal and OA-stimulated promoter activity. Our results demonstrate that a member of the AP1 family, containing JunD and c-fos, can bind to the proximal element P1 within the hPRL promoter. In addition, they show that AP1 is involved in both basal and OA-stimulated expression of the hPRL gene.

Leadership development in a nursing service. A South African ethnographic perspective.

Leadership development at operational level in a nursing service is important to cope with all the transformation required. The purpose with this study is to explore and describe the views of black professional nurses on leadership behaviour, within the context of an operating theatre department, and to determine the influence of their occupational history on these views. An exploratory and descriptive research design, with an ethnographic perspective, was employed by means of semi-structured interviews (views on leadership) and questionnaires (occupational history). The results compare positively with the theoretical description of leadership behaviour. Guidelines for leadership development of these nurses were formulated, and should be followed by the necessary capacity building regarding leadership practice by professional nurses.

[Grievances in South African hospitals: from a nursing service management perspective].

The nursing service manager is accountable for adequate and efficient personnel management in the nursing service and the management of grievances is an important aspect of the personnel management function. The question arises, however, how and when grievances in nursing services arose and developed? The purpose of this article is to give a historical description of the development and management of grievances in nursing services for the time frame 1652-1990. An historical analysis was undertaken by means of news paper analysis, as well as other written resources. The results show that the development of grievances are related to the development of hospitals in South Africa and that grievances were poorly managed. The following conclusions are made: grievances in nursing are related to the establishment of hospitals; the first official grievance was lodged in 1824 by a surgeon; grievance are mainly related to the working conditions, remuneration and management; complaints with salaries and food were lodged by nurses as early as 1869; it appears as if nursing service managers are not adequately skilled in the management of grievances experienced by nursing staff--the same mistakes are made leading to strike action by nurses/midwives; unhappiness with the inappropriate manner in which grievances are managed lead to industrial action by nursing staff since 1889. Continuous empowerment of nursing services managers in the management of grievances is important and therefore the development of a model for grievance management in nursing services is also recommended.

Experimental single controlled study of burned bones: contribution of scanning electron microscopy.

Burned bones were studied using Scanning Electron Microscopy. The samples were cut from a maxillary-mandibular block taken during an autopsy. These fragments were heated in a furnace under controlled temperature conditions for 60 minutes. The temperatures ranged from 150 to 1150 degrees Celsius. The results are as following: (i) there are significant alterations of the bone, more and more obvious as the temperature increases, (ii) it appears to be difficult to establish a precise correlation between the temperature and the scanning electron microscopy patterns.

The use of topoisomerase I inhibitors in multiple myeloma.

The standard treatment of multiple myeloma is systemic chemotherapy. Despite 30 years of drug development in myeloma, there are no new drug regimens significantly superior to melphalan and prednisone. In addition, phase II studies of new drugs in myeloma have been disappointing, with low response rates and no prolongation in survival. The topoisomerase I (topo I) inhibitors are a new class of anticancer agents with a wide spectrum of activity in human malignancies. Recent evaluation of the topo I inhibitor topotecan demonstrated activity in advanced myeloma, suggesting a possible role for these drugs in the treatment of this disease. Further evaluation of the mechanisms of resistance to topo I inhibitors, study of combination therapy with topotecan, and evaluation of other topo I poisons in multiple myeloma is proposed.

Macroscopic and microscopic aspects of incinerated teeth.

Fifty-eight premolars, extracted for orthodontic reasons, were incinerated for one hour in a furnace at temperatures varying from 150 degrees C to 1150 degrees C, increasing at 100 degrees C intervals. Between 150 degrees C and 700 degrees C the teeth changed from light yellow to bluish-white passing through brown. The scanning electron microscope showed that at 150 degrees C cracks appeared in the enamel, increasing in number as the temperature rose. At 450 degrees C the internal surface detached from the dentine and showed the same cracked surface as the exterior. Above 1100 degrees C the enamel fragments had a prismatic structure difficult to identify due to their melted structure. Dentine retained its tubular structure up to 1150 degrees C even when the tubules' diameter decreased at 700 degrees C. The cementum cracked as the temperature increased. Certain cracked areas actually detached at 600 degrees C giving a corroded aspect to the cementum and the residual zones became irregular when the temperature rose and eventually at 1150 degrees C the cementum was no longer identifiable.
