Successful treatment of a critically burnt neonate (case-report).

In a health institution in 2000, a premature neonate suffered critical burns. In the submitted case-report the authors describe the successful treatment of this quite exceptional case. Indirectly, they thus also draw attention to the very positive role played by interdisciplinary collaboration under such unusual circumstances.

Determination of adenosine phosphates in whole blood by capillary zone electrophoresis.

The pool of chemical energy in an organism represented by high-energy compounds can be assessed by means of adenosine triphosphate (ATP) determination in whole blood and tissues. The elegant manner for the determination of adenosine phosphates (ATP, ADP, AMP) in a single assay is offered by the technique of capillary zone electrophoresis. For this purpose, the BioFocus 3000 Capillary Electrophoresis System (BIO-RAD Laboratories, Inc., Hercules, CA, USA) was used. For the construction of calibration curves, pure preparations of ATP, ADP and AMP were analyzed. The method was used for adenosine phosphates determination in the umbilical blood samples from physiological and immature newborns. Capillary zone electrophoresis enables a specific and simultaneous determination of adenosine phosphates and, thus, monitoring of unusual metabolic situations.

Epidemiological analysis of Klebsiella infections at a neonate department.

At the Department of Obstetrics and Gynaecology of a Brno hospital, 133 Klebsiella strains were isolated from 23 mothers and 58 neonates during a period of increased incidence of Klebsiella and 73 strains were examined in the laboratory. The control group was comprised by 78 Klebsiella strains isolated at three other Brno hospitals. Four prevailing Klebsiella types were identified using bacteriocinotyping and phagotyping. Different bacteriocinotypes and phagotypes were found to prevail at certain intervals both at the examined clinic and in control patients from Brno hospitals. The findings suggest circulation of certain Klebsiella types and their introduction to hospitals. Most infants (37 out of 45, i.e. 82%) were colonized by Klebsiella, their mothers remaining without colonization. The factors influencing the onset and transmission of nosocomial infections in infants are discussed.

[Sonographic analysis of postextubation stridor in neonates and children with low birth weight after long-term artificial ventilation].

In a group of 23 neonates with mean birth weights of 1470 g (range 1200-3980 g) who had orotracheal intubation for an average period of 12.6 days (range 5-96 days) sonagraphic frequency analysis of 86 stridors 1-3 hours after intubation was performed. According to the results of the frequency analysis more than 97.6% of the stridors are supraglottic (84 of 86 analyzed stridors). The laryngeal and tracheal type of stridor was recorded only in two children. The finding is surprising and does not confirm the widely accepted view that neonates after extubation are threatened with oedema of the larynx or stricture at this or a distal level. As elective postextubation laryngoscopy is controversial, acoustic frequency analysis of stridor after extubation is a suitable alternate method in investigations of the clinical condition of the infant and serves the clinician as an indication on the level of stenosis of the airways.
