Recombinant Aspergillus fumigatus allergen I/a (rAsp f I/a) in the diagnosis of Aspergillus related diseases.

A. fumigatus allergen I/a (rAsp f I/a) was produced in Escherichia coli, affinity purified by Ni(2+)-chelate chromatography and refolded. rAsp f I/a binds IgE of A. fumigatus-allergic individuals as shown by ELISA, dot blots and Western blots. In addition, rAsp I/a elicited positive immediate type I skin reactions in individuals allergic to A. fumigatus but not in control individuals. Hence, rAsp f I/a can be used as a standardized antigen/allergen for an improved serological and clinical diagnosis of A. fumigatus-associated diseases.

The lungworm, Halocercus spp. (Nematoda: Pseudaliidae) in cetaceans from California.

Three cetaceans from Monterey Bay, California (USA), were infected with lung nematodes. A beaked whale calf, Mesoplodon sp. was infected with adult Halocercus sp. This represents a new host record for this nematode. The lungs of a northern right whale dolphin calf, Lissodelphis borealis, contained an unidentified nematode similar in size to Halocercus sp. The presence of Phyllobothrium delphini-type cestode larvae in its blubber is evidence that the calf was feeding on invertebrates. A pregnant harbor porpoise, Phocoena phocoena was infected with the lung nematode H. invaginatus, but the fetus was uninfected. We found no direct evidence of a transplacental mode of transmission for Halocercus sp.

Clinical experience with carvedilol.

Clinical experience indicates that carvedilol, a beta-blocker with vasodilating properties, is effective as once daily monotherapy in both young and elderly hypertensives. Titration is simple to an effective dosage, and response rates appear to be equivalent to those noted with other monotherapeutic agents presently recommended as initial therapy in hypertension. The drug is well tolerated by the majority of patients. No significant metabolic changes have been noted with carvedilol in short-term studies. Additional research is ongoing to determine the long-term effectiveness of this new antihypertensive agent.

[Microvib--a test battery for studies of physiological tremor].

4 simple tests to investigate physiological tremors in different load conditions on the musculo-skeletal system have been developed with regard to the Austromir space missions. Some new results on normal tremors from the space experiments as well as from ground studies using these tests are discussed. It is proposed to introduce these experimental methods in the area of clinics to evaluate precision of motor control.

Selenium status of elderly Austrians suffering from circulation disorders.

Whole blood selenium status of 299 persons, 144 women and 155 men, with a mean age of 62 +/- 10 years, has been determined. The individuals came from all Austrian provinces to Bad Hall to improve their heart and circulation disorders. The mean selenium content was 95 micrograms/l. We found no correlations between selenium levels and GSH-peroxidase, cholesterol, HDL-cholesterol and triglycerides.

Clinical significance of the metabolic effects of antihypertensive drugs.

Data from the Hypertension Treatment Trials do not indicate that the metabolic effects of the medications used, namely diuretics in all and beta-blockers in some, are of great clinical significance. Effects on lipids are mainly short-term (about one year) and the increased incidence of significant hyperglycaemia and/or diabetes in treated compared with control or placebo subjects is < 1% in most trials. The use of these drugs has improved the prognosis of subjects with both severe and less severe hypertension. Despite these findings, it is possible that medications such as the calcium blockers, ACE inhibitors, or multiple-action drugs, e.g. the alpha-beta-blockers which have no negative and possibly have some positive effects on lipids and glucose metabolism, will improve outcome still further. We must await additional long-term trials to determine this. It is possible that combinations of small doses of two different agents from different classes of drugs will prove to be the ideal approach to hypertension management in the future: efficacy is improved and any adverse metabolic effects are minimised.

Cloning and expression of recombinant Aspergillus fumigatus allergen I/a (rAsp f I/a) with IgE binding and type I skin test activity.

Aspergillus fumigatus secretes an 18-kDa nonglycosylated IgE-binding protein. This protein was previously shown to be a ribotoxin, like alpha-sarcin and mitogillin. A 686-bp long A. fumigatus cDNA encoding an 18-kDa ribotoxin was cloned and expressed in Escherichia coli as a fusion protein with six adjacent histidines (rAsp f I/a). rAsp f I/a was purified to homogeneity by Ni(2+)-chelate affinity chromatography and refolded. The recombinant protein was enzymatically active resulting in the cleavage of 28S rRNA within a universally conserved region. rAsp f I/a was cytotoxic for EBV immortalized or PHA stimulated human PBMC. Furthermore, rAsp f I/a was recognized by murine mAb made against an 18-kDa ribotoxin. IgE of individuals allergic to A. fumigatus bound to rAsp f I/a as shown by ELISA, dot blots, and Western blots. rAsp f I/a elicited positive immediate type I skin reactions in individuals allergic to A. fumigatus but not in healthy control individuals. The results show that rAsp f I/a has similar functional characteristics when compared to the native 18-kDa ribotoxin. rAsp f I/a expressed in E. coli can therefore be used as a standardized Ag/allergen for serologic and clinical diagnosis of A. fumigatus-associated diseases.

Incidence of dysplasia and carcinoma of the uterine cervix in an Appalachian population.

BACKGROUND: Cervical cancer mortality rates in the Appalachian population of southeastern Kentucky have been shown to be unusually high. To better understand the high cervical cancer death rate in this area, we developed a population-based cervical disease registry. PURPOSE: This study describes the incidence of cervical dysplasia, carcinoma in situ, and invasive cervical cancer in 1986 and 1987 among White women in a 36-county area of Appalachian Kentucky based on histologic diagnoses. METHODS: We compared average annual age-adjusted incidence rates for carcinoma in situ and invasive cervical cancer in the study area with those for women in the Surveillance, Epidemiology, and End Results (SEER) Program. RESULTS: The incidence rate of invasive cervical cancer for women in the study area (14.9 per 100,000) was nearly twice that for White women in the SEER population (7.8 per 100,000), but it was similar to that for Black women in the SEER population (15.3 per 100,000). The incidence of carcinoma in situ for women in the study population (38.2 per 100,000) was 21% higher than that for White women (31.5 per 100,000) or for Black women (31.2 per 100,000) in the SEER population. The average annual age-adjusted incidence rate for all grades of dysplasia among women in the study population was 194.6 per 100,000. No comparable population-based incidence rates for dysplasia could be identified. CONCLUSIONS: Cervical cancer incidence rates are higher in Appalachian Kentucky than in the SEER population. Poverty appears to be a factor associated with these rates. IMPLICATIONS: Low-density populations such as those in rural Appalachia deserve greater attention in cancer control research. The population-based cervical dysplasia rates reported here may be useful for comparisons in future investigations.
