Randomized clinical trial of short-term antibiotic prophylaxis in 750 patients undergoing vaginal and abdominal hysterectomy.

We have carried out a controlled, prospective, randomized study with the aim of evaluating the efficacy of a short-term antibiotic prophylaxis on patients undergoing simple total hysterectomy. 750 patients entered the trial, starting in September 1977, 260 underwent vaginal hysterectomy, 490 abdominal hysterectomy. The patients were subdivided randomly into four groups, which were homogeneous in number, age, weight, associated disease, indication for surgery and hormonal status: control group, without treatment; prophylaxis with Cefazolin; prophylaxis with Thiamphenicol; prophylaxis with Cefazolin plus Thiamphenicol. Each antibiotic was administered parenterally, at the dose of 1 g, 1 hour before surgery, and 5 and 12 hours later. Postoperative infectious morbidity was evaluated according to febrile morbidity, the need for antibiotic therapy during the postoperative period and the length of postoperative hospitalization. The incidence and severity of febrile morbidity was significantly reduced in the three groups of patients who received prophylaxis. The use of antibiotics in the postoperative period resulted significantly reduced and the period of hospitalization was shorter for the groups with prophylaxis than for the control group.

[Hypokalemic rhabdomyolysis and thiazide diuretics. 3 clinical cases].

Rhabdomyolysis is an acute skeletal muscle disorder characterized by altered integrity of the cell membranes of muscle fiber cells and can be caused by a variety of factors whose nature is not necessarily traumatic. Hypokalemia is one of the most common metabolic causes, accounting for about 14-28% of rhabdomyolytic syndromes. We report and discuss three cases of hypokalemia caused by diuretic treatment with hydrochlorothiazide: the main clinical features were muscle symptoms following massive rhabdomyolysis.

[On the acute respiratory infections of anaerobes bacteria: clinical, bacteriological and therapeutic results (author's transl)].

In order to isolate and identify aerobe and obligate anaerobe (O.A.) bacteria investigations were carried out on 11 patients with acute respiratory tract infections (7 of them severely ill and hospitalized in an Intensive Care Unit). O.A. species were isolated in 7 subjects, in all cases in mixed culture (from 2 to 5 species per sample), including also potentially pathogenic aerobe bacteria (Pseudomonas aeruginosa, Klebsiella pneumoniae, etc). On the 24 strains of O.A. bacteria identified, M.I.C. was determined for penicillin, clindamycin, chloramphenicol, and thiamphenciol. The data obtained have been compared with the M.I.C. of 28 strains isolated from uro-gynecological infections. The strains isolated from the respiratory tract presented a sensitivity spectrum to antibiotic similar to those isolated from the urogenital tract. Chloramphenicol and thiamphenicol inhibited growth in 100% of the cases at concentrations equivalent or inferior to plasma levels obtained with normal therapeutical doses; clindamycin was active at lower concentrations but not on all the strains isolated (3 Bacteroides, 1 Fusobacterium sp., 1 Peptococcus sp. were resistent). Penicillin G was active on 21 out of 25 Gram-positive strains, but only on 7 out 26 Gram-negative bacilli. In view of these results the difficulty of obtaining a correct etiologic diagnosis and the consequent use of a correct antibiotic therapy are discussed. The usefulness of the association of different antibiotics is suggested.

Antioxidant enzymatic system and free radicals pathway in two different human cancer cell lines.

Resistance to oxidative damage is an important feature of cancer cells. Cellular anti-radical enzymes, lipid peroxidation, glutathione pathway, capability to produce ROS, and cells' susceptibility to H2O2 and menadione toxicity, were analyzed in DND-1A and HeLa cancer cell lines. SOD and GSHPx activities were higher in DND-1A than in HeLa cells. Lipid peroxidation was the same in both cell lines, while menadione stimulation of ROS production was tenfold higher in HeLa cells. Total and reduced, but not oxidized, glutathione levels, were tenfold smaller in HeLa cells. H2O2 proved fatal to HeLa cells after 12 hours' incubation, while it was ineffective on DND-1A; DND-1A cells were more sensitive to menadione toxicity than HeLa cells. The two lines behaved differently in response to the above treatments. These observations might be important in designing more specific cancer treatments.

Catheter seeding of hepatocellular carcinoma following placement of a total implantable access port system.

A 61-year-old cirrhotic patient underwent hepatic resection for hepatocellular carcinoma and placement of a total implantable access port system in the hepatic artery for chemotherapy infusion. A year later, he developed a parietal metastasis at the port site as a consequence of tumor seeding along the arterial catheter. The metastasis was excised but the patient died because of disseminated disease two years after the first operation. Tumor seeding along the catheter should be included in the group of potential complications after placement of total implantable access port systems for intrahepatic chemotherapy The possible causes of this rare but life-threatening complication are discussed.
