Electron microscopic characterization of filiform papillae in the normal human tongue.

The fine structure of filiform papillae on the normal human tongue was examined level by level, from the basal layer to the surface, in specimens taken from the dorsal side of the lingual body. Human lingual epithelia showed three distinct regions: epithelia on the anterior and on the posterior sides of filiform papillae and an interpapillary epithelium. While the basal and the squamous cell layers were similar throughout these three regions, differences were noted in the granular and the horny layers. The interpapillary epithelium actually lacked both the granular and the horny layers. The epithelium on the anterior side of filiform papillae was characterized by alternating layers of granular cells and of cornified cells. Granular cells possessed three types of keratohyaline-like granules within their cytoplasm: uniformly electron dense, relatively less electron dense, and a heterogeneous type. While the number of the keratohyaline-like granules was remarkably diminished in the epithelium on the posterior side of filiform papillae, a considerable amount of tonofibrils was present in the cytoplasm. In the uppermost portion of the anterior side of filiform papillae, coherence between adjacent epithelial cells depended mainly on the interlocking of cytoplasmic villi and poorly developed desmosomes on villi. On the other hand, epithelial cells on the posterior side of filiform papillae appeared to be more tightly adhesive compared with those on the anterior side. This was due to focal thickening of the plasma membrane and to desmosomes at the interface between the granular and cornified cells, and to the formation of a marginal band and increased intercellular cement presumably derived from lamellar bodies in the horny layer. These findings demonstrate distinct differences between the anterior and the posterior sides of filiform papillae in the human tongue with respect to keratinization patterns, structures associated with cell-to-cell adhesion and the strength of cellular cohesion in the uppermost portion, and the turnover of cornified cells. These differences may contribute to the formation of the unique external configurations of filiform papillae.

Folliculosebaceous cystic hamartoma with a neural component: an immunohistochemical study.

We present a case of follicular cystic hamartoma, a distinctive cutaneous malformation characterized by marked overgrowth of folliculosebaceous units accompanied by appreciable mesenchymal alterations, including fibroplasia, increased vascular components, and numerous adipocytes. A conspicuous feature of our case is an aggregation of thick trespassing nerve bundles in the deep portion of the neoplasm. An immunohistochemical study revealed the nerve bundles were immunoreactive for the general neuronal marker protein gene product 9.5. The nerves, however, stained negatively with antibodies against neuropeptides such as calcitonin gene-related peptide, substance P, vasoactive intestinal polypeptide, and neuropeptide Y, all of which are known to be contained in normal cutaneous nerves. The presence of these aberrant nerve bundles devoid of the normal expression of neuropeptides supports the concept that folliculosebaceous cystic hamartoma is a true, tumor-like malformation characterized by abnormal overgrowth of normal components of the skin.

Free eosinophil granules in urticaria: a correlation with the duration of wheals.

To determine the factors associated with the duration of wheals in urticaria, we carried out a quantitative study of mast cells and eosinophils in both peripheral bloods and skin lesions. The percentage of cytoplasmic vacuolation, indicating a result of eosinophil degranulation of all types of specific granules of circulating eosinophils was significantly increased in the long-duration group compared with the short-duration group. Many extracellular free eosinophil granules were observed in the vicinity of disrupted eosinophils in the long-duration wheals. On the other hand, the short-duration wheals had few free eosinophil granules. The number of free eosinophil granules and the percentage of free eosinophil granules to eosinophils were significantly different between the long-and the short-duration wheals, whereas there was no significant difference between the early stage and the late stage of the long-duration wheals. The numbers of all inflammatory cells, eosinophils, and activated mast cells were increased in the late stage of the long-duration wheals. These findings suggest that free eosinophil granules liberated from infiltrating eosinophils by some stimulus may provoke the persistent activation of mast cells, and the mast-cell-derived mediators may induce the persistent state of inflammation in the long-duration wheals.

Activity of eleven kampo formulations and eight kampo crude drugs against Propionibacterium acnes isolated from acne patients: retrospective evaluation in 1990 and 1995.

We reviewed the susceptibility of Propionibacterium acnes to eleven Kampo formulations and to eight Kampo crude drugs that had been studied by examining their minimum inhibitory concentrations (MIC) in 1990 and 1995. P. acnes strains were most sensitive to Oren-gedoku-to (OGT) among these Kampo formulations. Coptidis Rhizoma (CR) and Phellodendri Cortex (PC) inhibited the growth of P. acnes significantly among the eight Kampo crude drugs examined. The patterns of distribution of MIC of Kampo formulations and Kampo crude drugs to P. acnes in this study were almost the same as in our previous report in 1990. No significant increases in MIC of Kampo formulations and Kampo crude drugs to P. acnes were observed. We speculated that Kampo crude drugs such as CR and PC, were better than minocycline or erythromycin from the point of view of a progressive increase in MIC to P. acnes. CR and PC, which were each an ingredient of OGT, might contain some components with strong antibacterial activity to P. acnes.

Anti-lipase activity of Kampo formulations, coptidis rhizoma and its alkaloids against Propionibacterium acnes.

Anti-lipase activity of Kampo formulations, Coptidis Rhizoma (CR), and its alkaloids against Propionibacterium acnes were examined in vitro. The amounts of propionic and butyric acids in the medium were measured as growth and lipase activity of P. acnes, respectively. In tributyrin-PYG medium with each concentration of Kampo formulation, CR, or the alkaloids added, the production of propionic acid was suppressed remarkably more than that of butyric acid. The suppression of production of these acids by CR was higher than that of the alkaloids. Furthermore, no lipase-negative colonies were found on the medium to which Kampo formulations were added. From these observations, we concluded that not only Kampo formulations and CR, but also their alkaloids, showed suppression of growth of P. acnes, which reduced anti-lipase activity. Furthermore, it was suggested that Kampo formulations and Kampo crude drugs with anti-lipase activity to P. acnes should be synergistic when their ingredients are combined.

The correlation of Kampo formulations and their ingredients on anti-bacterial activities against Propionibacterium acnes.

The correlation between the susceptibilities of Propionibacterium acnes strains to two Kampo formulations and these to their ingredients (Kampo crude drugs) was examined. Two out of 19 Kampo crude drugs, Coptidis Rhizoma (CR) and Phellodendri Cortex (PC), inhibited the growth of P. acnes significantly. The patterns of distribution of the minimal inhibitory concentrations of Keigai-rengyo-to (KRT) and of Seijo-bofu-to (SBT) were almost the same as those of the Kampo crude drugs, except CR and PC. Furthermore, no correlations between MICs of Kampo formulations and those of their ingredients against P. acnes were found. This might be due to mutual inhibition or to differences in anti-bacterial mechanisms between the two Kampo formulations and their ingredients. The Kampo formulations, composed of CR, PC and a few other kinds of Kampo crude drugs, might be more susceptible against P. acnes, since higher concentrations of the effective ingredients can be achieved.

Effects of topically applied spironolactone on androgen stimulated sebaceous glands in the hamster pinna.

The effects of spironolactone (5% SYC-201G, a preparation developed for clinical use in acne vulgaris by Searle Yakuhin K.K.), which is known to have antiandrogenic effects by competitively inhibiting dihydrotestosterone at androgen receptor sites, was topically applied to the androgen stimulated sebaceous glands of adult female golden hamsters. Androgen stimulation, induced by intramuscular injection of testosterone propionate (TP) every other day over a two week period, resulted in a 2.5 to 2.7 time increase in the size of the sebaceous glands of the hamster pinna. Once-daily treatment with 5% SYC-201G or matching placebo was applied to androgen-stimulated hamsters on one pinna only during the same period as TP injection. Comparison between the treated and untreated sides revealed a significant suppression in the sebaceous gland size (p < 0.05) by 5% SYC-201G; no such effect was observed with placebo. The difference in the suppression rate of the sebaceous gland size between 5% SYC-201G (23%) and matching placebo (-4.7%) was significant (p < 0.01).

Distribution of transglutaminase in quail uropygial glands.

The holocrine secretion of the uropygial gland has been explained as autolysis due to intracellular lysosomal enzymes. In the present study, the distribution of transglutaminase, a marker of epidermal differentiation, in quail uropygial glands was observed by histochemical methods to confirm whether a mechanism similar to epidermal differentiation also exists in the uropygial gland. The transglutaminase activity was positive in the deep and shallow portions of the secretory tubules as well as the transitional and degenerating cell layers of the uropygial gland. This activity was stronger in the deep portions than the shallow portions. These findings suggest that the process of secretion in the quail uropygial gland involves not only the autolysis due to lysosomal enzymes but also a mechanism similar to that of epidermal differentiation, and that a phenomenon similar to epidermal differentiation is more evident in the deep portions than the shallow portions.

Expression of (Na, K) ATPase/Ca-ATPase chimeric proteins in Xenopus oocytes revealed that the COOH-terminal one-third of the (Na, K) ATPase alpha-subunit is involved in assembly with the beta-subunit.

Chimeric proteins consisting of parts from the alpha-subunit of Torpedo californica (Na, K) ATPase (N) and the rabbit sarcoplasmic reticulum Ca-ATPase (C) were expressed in Xenopus oocytes by injecting the respective chimeric cRNA in combination with cRNA for the beta-subunit of Torpedo (Na, K) ATPase. The chimeric protein (NCN) that consisted of the NH2-terminal and COOH-terminal one-thirds of the alpha-subunit of the (Na, K) ATPase and the central one-third of the Ca-ATPase was able to assemble with the beta-subunit in the same fashion as the wild-type alpha-subunit of the (Na, K) ATPase (NNN). On the other hand, chimeric proteins in which the COOH-terminal one-third was derived from the Ca-ATPase (NNC and NCC) were unable to form stable complexes with the beta-subunit. These results suggest that the COOH-terminal one-third of the (Na, K) ATPase alpha-subunit is required for the assembly with the beta-subunit.

Three-dimensional microvasculature of the hair follicle.

The three-dimensional microvasculature of the hair follicle of the adult Wistar rat was demonstrated by scanning electron microscopy of vascular corrosion casts. The anagen hair follicle was surrounded by the basket-like capillary network which was supplied by the branches of the subcutaneous artery and drained into the veins continuous with the subcutaneous vein. The capillary network surrounding the anagen hair follicle was most dense at its bottom, and became sparse at its upper part. The telogen hair follicle was surrounded by only a few capillaries. Transmission electron microscopy showed that the capillaries around the hair bulb possessed fenestrations. Our results indicate that the microvasculature of the anagen hair follicle is so organised as to supply the hair bulb with abundant blood, which is the most important area for hair growth.

Pathogenesis in pili torti: morphological study.

To clarify the mechanism of pili torti formation, hair materials obtained from a patient with the typical classical type of pili torti were examined. Light microscopy, scanning electron microscopy (SEM), transmission electron microscopy, (TEM) and 3-D analysis revealed that: (1) The hair shaft was partly flattened. The hairs twisted clockwise or counter-clockwise on their own axis and some hairs were knotted. (2) Short hairs showed disruptions at the flattened or unflattened regions, at the knot, or at the root of branched hairs caused by longitudinal ruptures. (3) Light microscopically, cell vacuolation and irregularity in thickness of the outer root sheath (ORS) at the suprabulbar level were seen. At the middle to upper levels of the follicles, some eosinophilic pyknotic ORS cells were observed scattered around. (4) At the upper levels, the ORS and inner root sheath (IRS) partly revealed irregularity of thickness and formed an irregular-shaped hair canal. (5) By DACM staining, cytoplasmic SH fluorescence showed an irregular arrangement of the ORS cells. Some abnormal SH- or SS-positive cells were observed scattered throughout the ORS. (6) Ultrastructurally, the irregularities in thickness and cell shape of the IRS and ORS became visible at the level, where the Henle's layer keratinized. In the hair cortex, there was no abnormality of nuclei and the keratin fibers were normally produced, although fibers were partially wavy. (7) By 3-D analysis, hair twist was already seen at the middle level of the hair follicles.(ABSTRACT TRUNCATED AT 250 WORDS)

Distribution of sulphydryl groups and disulphide linkages in tissue proteins of quail uropygial glands.

1. The distribution of sulphydryl (-SH) groups and disulphide (S-S) linkages in the tissue proteins of quail uropygial glands was histochemically observed by a staining method using N-[7-dimethylamino-4-methyl-3-coumarinyl]maleimide. 2. In the uropygial gland, the -SH groups of the tissue proteins of glandular cells displayed a progressive change to S-S linkages concomitant with the shift from the transitional cell layer to the degenerating cell layer, suggesting that cell destruction was occurring with the products being released as secretions. This phenomenon was apparently more pronounced in the deep compared with the shallow portion of secretory tubules. 3. The above results indicated that a mechanism similar to epidermal differentiation operates in the process of holocrine secretion in the uropygial gland.

Effects of azelastin on pruritus and plasma histamine levels in hemodialysis patients.

BACKGROUND: Persistent pruritus is the most common symptom in hemodialysis patients. Its causes are poorly understood and there is no effective treatment. We have studied the effect of azelastin HCL on the plasma histamine concentrations and pruritus in maintenance dialysis patients with or without erythropoietin therapy. PATIENTS: Twenty-eight hemodialysis patients were divided into four groups according to the presence or absence of pruritus and whether or not they received erythropoietin therapy. RESULTS: Histamine concentrations in the patient groups were significantly higher than in healthy volunteers, but a significant difference could not be found among the four groups. The patients with pruritus showed no change in their histamine concentration during treatment with azelastin HCL, but their pruritus scores decreased remarkably. CONCLUSIONS: The present data do not support the thesis that the increased plasma histamine concentration is causally related to pruritus in hemodialysis patients or that the antiallergic drug, azelastin HCL, alleviates the pruritus of dialysis patients by decreasing plasma histamine levels. The possible role of the increased tissue levels of histamine remains to be studied.

Chronic graft-versus-host disease with follicular involvement.

The clinical and histopathological findings in a 25-year-old Japanese male patient who suffered from chronic graft-versus-host-disease (GVHD) with follicular involvement are described. The patient had been diagnosed as aplastic anemia and underwent an allogeneic bone marrow transplant (BMT). In the eighth month thereafter, pruritic follicular red papules developed over his trunk and extremities. A biopsy specimen revealed histological exocytosis of lymphocytes into the hair follicles and basal-cell vacuolization of the follicular epithelium. Analysis of T-lymphocyte subpopulations in the dermis revealed a predominance of CD4 positive cells. To our knowledge, several cases of acute follicular GVHD have been reported; however, the occurrence of chronic GVHD with follicular involvement (chronic follicular GVHD) has not been clearly documented.

Effect of some alkaloids, flavonoids and triterpenoids, contents of Japanese-Chinese traditional herbal medicines, on the lipogenesis of sebaceous glands.

The effects of some alkaloids and flavonoids, which are major ingredients in some of the Japanese Kampoh drugs (Japanese-Chinese traditional herbal medicines) experientially known to be efficacious for the treatment of acne vulgaris, on the lipogenesis in the sebaceous glands of the hamster ear, an excellent animal model for the human sebaceous gland, were studied. Lipogenesis was assayed by determining 14C incorporation into sebaceous lipids extracted from the sebaceous glands which were preincubated with 14C-acetate. We found that the lipogenesis in the hamster sebaceous glands was suppressed 63 and 54% by 10(-4) M berberine (an alkaloid) and wogonin (a flavonoid), respectively. We suggest that the therapeutic effects of some Japanese Kampoh drugs experimentally used for acne vulgaris could be due to inhibition of lipogenesis by their active ingredients such as berberine and wogonin.

A case of erythromelanosis follicularis faciei with a unique distribution.

A 16-year-old Japanese boy with erythromelanosis follicularis faciei (EFF) with an unusual distribution is described. The clinical features of classic EFF consist of reddish-brown discoloration of the skin with fine follicular hyperkeratosis in the preauricular areas, and occasionally on the temples and the lateral aspects of the neck. The present case is unique in that similar lesions also occurred on the auricles and eyebrows. Both light and electron microscopic findings of the lesions were consistent with the diagnosis of EFF.
