[A case of carcinoma corporis by intra-arterial hypertension chemotherapy].

We report a case in which bone metastasis from carcinoma corporis was treated by intra-arterial hypertension chemotherapy. The patient was a 53-year-old female whom we treated by intra-arterial hypertension chemotherapy of CDDP 80 mg and ADM 30 mg with Angiotensin II. Four weeks later she underwent abdominal total hysterectomy, bilateral salpingo-oophorectomy and right common iliac lymph nodes resection. The response of intra-arterial hypertension chemotherapy was charged microscopically. About half (Grade I b) the carcinoma tissue was necrosed.

[Immune parameters of peripheral blood in chemoembolization with microencapsulated anticancer drugs].

To investigate possible immunological consequences after arterial infusion of microencapsulated anticancer drug (chemoembolization), cellular immune parameters in peripheral blood were monitored for 4 weeks after chemoembolization in 20 patients with urological malignancies including 7 renal cell carcinomas, 2 renal pelvic carcinomas, 8 bladder carcinomas and 3 prostatic carcinomas. Natural killer (NK) cell activity and OKT4/8 ratio were augmented in 67% and 83% of the cases after chemoembolization respectively. Mature NK cell population (Leu7-OKT 16+) in these cases also showed a trend of continuous increase for more than 4 weeks. On the other hand, a small number of the patients showed an augmentation of OKT3+ cells, Ia-1+ cells or response to phytohemagglutinin. Histopathological examination of regional lymph nodes in 9 patients who underwent chemoembolization for renal cell carcinoma showed stimulating reactions in 8, while lymphocyte depletion in 1. These findings suggest that inhibition of micrometastasis by the enhanced natural killer cell activity could be anticipated in more than half of cases after chemoembolization.

Rearrangements of chromosome 3 in nonfamilial renal cell carcinomas from Japanese patients.

Cytogenetic studies were successfully carried out in 5 tumor tissues from Japanese patients with nonfamilial renal cell carcinoma, histologically diagnosed as clear cell subtype. Mitotic cells were obtained by a combined method of enzymatic disaggregation and short-term culture (6-12 days). The modal chromosome numbers were found to be diploid or near-diploid in all the cases examined. Every case showed characteristic structural and numerical abnormalities. Rearrangements in the short arm of chromosome 3 were observed as clonal abnormalities in all the cases, including a translocation t(3;6) resulting in a partial loss of 3p (3 cases), a terminal deletion of 3p (one case) and 2 different translocations involving 3p and 8p (one case). The other clonal abnormalities were a whole or partial trisomy of chromosome 7 and a loss of Y chromosome. The overall results in the present study were consistent with those of our previous data in American patients, and suggest that the rearrangements of chromosome 3 leading to a partial loss of its short arm may play primary and significant role(s) in the development of renal cell carcinoma.

In vitro evaluation of E1040, a new cephalosporin with potent antipseudomonal activity.

E1040 is a new parenteral cephalosporin with a broad antibacterial spectrum and potent antipseudomonal activity. The compound was four- to eightfold more active than ceftazidime and cefsulodin against Pseudomonas aeruginosa (MIC of E1040 for 90% of strains tested [MIC90], 3.13 micrograms/ml). E1040 also showed a potent activity against other glucose-nonfermentative rods, including Acinetobacter species. The activities of E1040 against most species of the family Enterobacteriaceae were roughly comparable to the activities of ceftazidime and cefmenoxime and exceeded that of cefotiam. Against Citrobacter freundii (MIC90, 0.78 micrograms/ml), Enterobacter cloacae (MIC90, 3.13 micrograms/ml), and Enterobacter aerogenes (MIC90, 0.2 micrograms/ml), E1040 was 16- to 256-fold more active than ceftazidime and cefmenoxime. The activities of E1040 against gram-positive cocci and anaerobes were comparable to those of ceftazidime, but the compound was less active than cefmenoxime. E1040 was at least as resistant as ceftazidime and cefmenoxime to hydrolysis by various beta-lactamases and showed high affinities for penicillin-binding protein 3 of both Escherichia coli and P. aeruginosa.

Existence of a new type of sulfite oxidase which utilizes ferric ions as an electron acceptor in Thiobacillus ferrooxidans.

A new type of sulfite oxidase which utilizes ferric ion (Fe3+) as an electron acceptor was found in iron-grown Thiobacillus ferrooxidans. It was localized in the plasma membrane of the bacterium and had a pH optimum at 6.0. Under aerobic conditions, 1 mol of sulfite was oxidized by the enzyme to produce 1 mol of sulfate. Under anaerobic conditions in the presence of Fe3+, sulfite was oxidized by the enzyme as rapidly as it was under aerobic conditions. In the presence of o-phenanthroline or a chelator for Fe2+, the production of Fe2+ was observed during sulfite oxidation by this enzyme under not only anaerobic conditions but also aerobic conditions. No Fe2+ production was observed in the absence of o-phenanthroline, suggesting that the Fe2+ produced was rapidly reoxidized by molecular oxygen. Neither cytochrome c nor ferricyanide, both of which are electron acceptors for other sulfite oxidases, served as an electron acceptor for the sulfite oxidase of T. ferrooxidans. The enzyme was strongly inhibited by chelating agents for Fe3+. The physiological role of sulfite oxidase in sulfur oxidation of T. ferrooxidans is discussed.

Klinefelter's syndrome with hypospadias and bilateral cryptorchidism.

A 3-year-old boy was found to have abnormality of the external genitalia at birth. Physical examination revealed hypospadias penis and bilateral cryptorchidism. Chromosomal analysis of peripheral blood showed the karyotype of 47,XXY, and the diagnosis of Klinefelter's syndrome associated with hypospadias and cryptorchidism was made. Klinefelter's syndrome is rare in infancy.

Neural systems activated by the aversive stimulation of dorsal central gray.

Neural effects of aversive stimulation of dorsal central gray (DCG) were studied by [14C] 2-deoxyglucose (2-DG) autoradiography in rats. After training the animals to escape DCG stimulation by pressing a lever, they were injected i.p. with [14C] 2-DG and then allowed to resume the escape lever pressing for DCG stimulation. Reliable effects of the brain stimulation on the autoradiogram were found in the dorsal fasciculus of Schütz, periventricular gray and superior colliculus. Moderate effects were found in the reticular formation near the periventricular gray and in the claustrum. These data indicate that the neural signal activated by DCG stimulation is transmitted through an ascending nerve pathway over the superior colliculus via the dorsal Schütz bundle and reaches the periventricular system in the diencephalon.

Twenty four-hour urinary excretion of sodium, potassium and urea in Japanese children.

The general profile of sodium and potassium intake during childhood was studied by the analysis of excretion in 24-hr urine samples collected from 119 healthy Japanese children at the ages of 1 to 18 years. In order to analyze sodium and potassium excretion under the changing body size and diet during growth, sodium and potassium were further expressed as values relative to creatinine and urea, as the reflection of body muscle mass and protein intake, respectively. Both of sodium and potassium (mEq/24-hr) increased significantly with age. Sodium/creatinine and potassium/creatinine, however, decreased significantly with age, suggesting that the increase in muscle mass exceed that in dietary intakes of sodium and potassium. When urinary sodium and potassium were expressed as ratios to urea-N, sodium/urea-N increased, but potassium/urea-N decreased with age. Sodium/potassium ratio increased with age. Thus, the results suggest that, in the given population of Japanese children, the amount of sodium per calorie or per protein in the diet increase, and that of potassium decrease with age.

Estimation of urinary 24-hr creatinine excretion by body size and dietary protein level: a field survey based on seasonally repeated measurements for residents living in Akita, Japan.

We aimed to investigate the modifying effect of dietary protein level on the estimation of 24-hr creatinine excretion using anthropometric measurements, in a group of 40 healthy Japanese adults, during usual daily life. To evaluate the variability of each measurement during non-restrictive life, we assayed 24-hr urinary excretions of creatinine and urea-N and measured heights and weights once in every three months in a total of 4 times during a year. Urinary urea-N was used as a marker of dietary protein level. Seasonal variability of anthropometrics and urinary excretions was observed on the individual basis. In comparison to the rather small individual variability of weight (mean of CV = 1.2 for males, 1.7 for females), variability of urinary creatinine (CV = 11.7, 8.4) and urea-N (CV = 18.9, 17.3) took larger values. The creatinine excretion was estimated by the following two sets of predictor variables; (I) age, height and weight, (II) age, height, weight and urea-N. Although, a statistically significant estimation was obtained by (I), estimation (II) resulted higher regression coefficient than (I), and contribution of urea-N to the estimation of creatinine was shown. Average level of seasonal variability of each predictor variable was further applied to the regression equation (II). Therefore, the possible difference of 24-hr creatinine excretion caused by the seasonal variability of dietary protein level was estimated to be 9.2% for males and 3.6% for females in the given population. Thus, when one predicting formula of 24-hr creatinine by the use of anthropometrics is applied to other populations, the difference of protein nutritional status should be carefully considered.

[Studies of intra-arterial hypertensive chemotherapy with cisplatin and peplomycin in advanced cancer of the uterine cervix--suitable dose of angiotensin II determined by intra-arterial digital subtraction angiography].

UNLABELLED: We have been treating advanced cancer of the cervix uteri with intra-arterial cisplatin (CDDP) and peplomycin (PEP) (injected into the bilateral internal iliac artery), combined with radiotherapy and hysterectomy. Concomitant angiotensin II (ATII) administered by intravenous drip infusion was found to double tumoral blood flow and thereby to enhance the efficiency of the intra-arterial chemotherapeutic regimen. In the present study, hypertensive intra-arterial chemotherapy utilizing ATII was administered to a small group of patients with advanced cancer of the cervix uteri while ascertaining the increase in intratumoral blood flow by intra-arterial digital subtraction angiography. The subjects were 10 patients with stage IIb or more advanced cancer of the cervix uteri. RESULTS: A 2-fold or greater increase in tumoral blood flow was attained in those patients who showed concurrently a 1.5 fold or greater increase in mean blood pressure and an increase in mean blood pressure to 150mmHg or higher. In 2 patients, ATII infusion failed to raise blood pressure to a therapeutically adequate level. At 1 week after the treatment in question histological evidence indicated unequivocal degenerative changes and necrotic changes in tumor cells. Many plasma cells, lymphocytes + and granulocytes appeared around necrosis changed carcinoma nests.
