[A rare complication in the suppression of a porto-systemic shunt in disabling encephalopathy].

The authors describe an uncommon complication in a case of side-to-side portacaval shunt which was suppressed due to disabling portal-systemic encephalopathy; the patient was treated by esophageal transection with esophagogastric devascularization and nonoperative secondary occlusion of the shunt by external elastic traction on a Silastic catheter surrounding the anastomosis: a stenosis of the caval vein under traction required another operation in order to close the shunt.

Lymph nodal metastases and the clinical stage of cervix carcinoma.

It has been proved that surgery is essential both for therapy and for diagnostic staging of gynaecological cancer. We have studied 131 patients who underwent pelvic and paraaortic lymphadenectomy. Out of the cases of the 1st clinical stage, the staging error was 16.4%. 10 cases were understage: 3 belonged to the 2nd surgical stage, 6 to the 3rd one and 1 to the 4th one. Out of 61 patients at the 2nd clinical stage, 44.2% were overstaged and 36.1% were understaged; in fact 16 belonged to the 3rd and 6 to the 4th surgical stage. 1 woman of the 6 belonging to the 3rd stage was at the 4th surgical stage. All 3 patients of the 4th stage had both clinical and surgical correct staging. The results show the determinant meaning of node metastases and their spreading for a correct surgical staging. According to the regular distribution of metastases, it has been shown that in 81.1% there is a regular rule while in 18.9% there is not a metastatic regular distribution. This study confirms the reliability of our surgical statement based on lymph nodal radical excision.

[Treatment of locally advanced breast neoplasms].

One particularly interesting problem in the treatment of breast carcinomas relates to the strategy to be adopted in locally advanced forms. In 10-25% of cases surgery in itself offers no guarantee of radicality and the prognosis is particularly poor. Once considered suitable only for radiation treatment, such cases now tend to be treated by mixed therapy adopting different modes and sequences. The present paper fully and carefully examines current concepts and treatment possibilities.

[Retrosternal thyroid cancers. Considerations on 25 cases].

Intrathoracic thyroid cancer is an unusual finding, and few reports are available on this item. Literature data have been reviewed, and some considerations on epidemiologic characteristics of the disease and on its treatment were gained from an analysis of reports based on the files of I Surgical Clinic of the University of Turin, Surgical service of the Oncological Department of the S. Giovanni Battista Hospital and II Pathological Anatomy Service of the University of Turin. No correlation was found between previous thyroid disease, sex, histological findings, previous radiotherapy and incidence of the disease. Older age at the onset and recurrent laryngeal nerve lesions are frequently associated with intrathoracic thyroid cancer; papillary carcinoma is rarely found. Intrathoracic situation is not likely, in Author's opinion, to affect the long term prognosis more than usual risk factors.

The role of diffusion-weighted imaging in patients with brain tumors.

BACKGROUND AND PURPOSE: Diffusion-weighted images (DWIs) have been used to study various diseases, particularly since echo-planar techniques shorten examination time. Our hypothesis was that DWIs and tumor apparent diffusion coefficients (ADCs) could provide additional useful information in the diagnosis of patients with brain tumors. METHODS: Using a 1.5-T MR unit, we examined 56 patients with histologically verified or clinically diagnosed brain tumors (17 gliomas, 21 metastatic tumors, and 18 meningiomas). We determined ADC values and signal intensities on DWIs both in the solid portion of the tumor and in the peritumoral, hyperintense areas on T2-weighted images. We also evaluated the correlation between ADC values and tumor cellularity in both gliomas and meningiomas. RESULTS: The ADCs of low-grade (grade II) astrocytomas were significantly higher (P =.0004) than those of other tumors. Among astrocytic tumors, ADCs were higher in grade II astrocytomas (1.14 +/- 0.18) than in glioblastomas (0.82 +/- 0.13). ADCs and DWIs were not useful in determining the presence of peritumoral neoplastic cell infiltration. The ADC values correlated with tumor cellularity for both astrocytic tumors (r = -.77) and meningiomas (r = -.67). CONCLUSION: The ADC may predict the degree of malignancy of astrocytic tumors, although there is some overlap between ADCs of grade II astrocytomas and glioblastomas.
