Reduced-dose chop therapy for elderly patients with non-Hodgkin's lymphoma.

While CHOP therapy is effective for malignant lymphoma, the optimum schedule for elderly patients remains controversial. The present study investigated the usefulness of reduced-dose CHOP therapy for elderly patients. Previously untreated patients aged 65 years or older with intermediate to high-grade non-Hodgkin's lymphoma were given up to 6 courses of reduced-dose CHOP therapy at 3-week intervals. Group A patients were given (5/6) of the standard dose and Group B received 7/12 of the standard dose. Filgrastim was administered when the white blood cell count fell below 2,000/microL. Fifty-seven patients were evaluable and the scheduled therapy was completed in 37. For patients aged from 65 to 79 years and for patients older than 80 years, the complete response rate was 79.5% and 46.2%, overall 3-year survival was 58.2% and 30.4%, and event-free 3-year survival was 49.3% and 44.4%, respectively. Major toxicities (> or = grade 3) included leukopenia in 42 patients and documented infection in 7 patients. Grade 3 cardiac plus renal failure, grade 3 peritonitis due to small bowel perforation, and grade 3 liver dysfunction occurred in 1 patient each. One patient died of toxicity (grade 4 hematological toxicity and pneumonia). In conclusion, it seems that in the elderly patients with non-Hodgkin's lymphoma, response to reduced-dose ((5/6) dose) CHOP therapy is comparable to that for standard CHOP in younger adults, mainly because of improved dose-intensity.

CAM-cytarabine, aclarubicin plus macrophage colony-stimulating factor in the treatment of acute myelogenous leukemia with trilineage dysplasia: usefulness of in vitro apoptosis in leukemic cells.

A 67-year-old woman was treated for acute myelogenous leukemia with trilineage dysplasia (AML-TLD) by combination chemotherapy with cytarabine, aclarubicin plus macrophage colony-stimulating factor (M-CSF) (referred to as CAM therapy). Complete remission was achieved after two courses of CAM therapy. After coculture of her bone marrow mononuclear cells with M-CSF in vitro, differentiation of leukemic cells into macrophages with apoptotis was observed. This case confirms an earlier report that an effect of M-CSF inducible by differentiation with apoptotic phenomena, against human leukemic cells was shown both in vitro and in vivo when achieving complete remission.

[Nitric oxide-dependent inhibition of platelet aggregation in elderly patients with hypercholesterolemia].

Hypercholesterolemia reduces production of nitric oxide (NO), a potent inhibitor of platelet aggregation, in endothelial cells. Recently platelet has been found to have NO synthase. Hypercholesterolemia may influence platelet NO production. We investigated NO-dependent inhibition of platelet aggregation in elderly hypercholesterolemic patients with total cholesterol (Tchol) of 240 mg/dl or more (n = 21). In elderly controls with Tchol less than 240 mg/dl (n = 61), L-arginine (5-50 mM) inhibited ADP-induced platelet aggregation in a dose-dependent manner (42.4% inhibition at 50 mM). However, L-arginine did not inhibit platelet aggregation in elderly hypercholesterolemic patients. L-arginine increased cyclic GMP production in elderly controls, but not in hypercholesterolemic patients (p < 0.02). Hypercholesterolemic patients showed increased platelet aggregation compared with elderly controls(p = 0.018). L-nitro-arginine methyl ester 12.5-50 uM increased platelet aggregation in both groups. Superoxide dismutase improved L-arginine inhibition of platelet aggregation in elderly hypercholesterolemic patients (p = 0.02). LDL cholesterol of 160 mg/dl or more was an independent predictor for loss of L-arginine inhibition of platelet aggregation (relative risk 3.9, p = 0.0098). This result suggests that hypercholesterolemia causes decreased NO-dependent inhibition of platelet aggregation due to reduced NO utilization. NO-dependent platelet aggregation may be a powerful tool for detection of vascular injury.

[A case of malignant lymphoma concomitant with multiple myeloma].

A 75-year-old woman was referred to us because of cough, high fever and skin erythema in April 1999. Malignant lymphoma (diffuse mixed cell type) was previously diagnosed in 1990 and she achieved complete remission after treatment with a series of CHOP regimen treatments. In 1998, multiple myeloma (IgG lambda type) was diagnosed and she was treated with a combination of melphalan and prednisolone. On physical examination, superficial lymphadenopathy and skin erythema were noted. Biclonal gammopathy (IgG kappa/lambda) was shown in serum, and Bence Jones protein in urine. Computed tomography showed pleural effusion and swelling of paraaortic lymph nodes. The bone marrow examination showed an increased number of abnormal plasma cells (19.2%) and no evidence of lymphoma. Left axillary lymph node biopsy revealed that she had non-Hodgkin's lymphoma (immunoblastic lymphadenopathy-like T cell lymphoma). She was treated with the CHOP regimen at reduced doses for both diseases. The lymphoadenopathy reduced after 6 courses of CHOP and 4 courses of CHOPE (CHOP + VP16), however, she had bone pain on November 1999 and received treatment with MCNU-VMP (MCNU + VDS + L-PAM + PSL). Her rib pain improved, but she died of systemic infection of herpes zoster virus. We report here a rare case of malignant lymphoma concomitant with multiple myeloma.

Ocular symptoms associated with a dural arteriovenous fistula involving the hypoglossal canal: selective transvenous coil embolization. Case report.

The hypoglossal canals are an unusual location for dural arteriovenous fistulas (AVFs) to appear. One previous report of dural AVFs involving the hypoglossal canal has been published. In the present paper, the authors describe a dural AVF within the hypoglossal canal, which presented with ocular symptoms and was successfully treated by selective transvenous embolization. Magnetic resonance imaging and contralateral carotid arteriography were useful for determination of the exact location of the fistulous pouch, which was later packed with coils. Selective transvenous coil embolization with careful assessment of the location and pattern of the venous drainage of the dural AVF is a safe and effective treatment.

Pyridoxal 5'-phosphate and pyridoxal inhibit angiogenesis in serum-free rat aortic ring assay.

Supraphysiological doses of vitamin B6 has been reported to suppress tumor growth and metastasis in rodents. To examine if its anticancer effect is due to suppression in angiogenesis, this study was conducted to investigate the antiangiogenic effect of pyridoxal 5'-phosphate (PLP), pyridoxine, pyridoxal and pyridoxamine in an ex vivo serum-free matrix culture model using rat aortic ring. Rat aortic rings were incubated with PLP or pyridoxine (25 micromol/l to 5 mmol/l). Higher concentrations of PLP (2.5 and 5 mmol/l) and pyridoxine (5 mmol/l) caused complete inhibition of microvessel outgrowth. However, the addition of pyridoxine at 2.5 mmol/l did not show complete inhibition of angiogenesis. PLP inhibited microvessel outgrowth almost completely at a concentration of 500 micromol/l and showed antiangiogenic effect in a dose-dependent manner within the range of 25-500 micromol/l. At 250 micromol/l, pyridoxal was as effective as PLP, but pyridoxamine was inactive, implying that the aldehyde group relates to the antiangiogenic effect. These results indicated the antiangiogenic effect of PLP and pyridoxal, and suggested that the antitumor effect of high levels of vitamin B6 might be mediated through suppression of angiogenesis.

Expression of Fas ligand in gastric carcinoma relates to lymph node metastasis.

Tumors may escape a host's immune response by means of various mechanisms. The Fas (CD95/APO-1)/Fas ligand (FasL) system is one of the major apoptotic pathways. Recently, it has been reported that tumor cells can express FasL, induce apoptosis in tumor infiltrating lymphocytes, and thus can escape host immune surveillance. In gastric carcinoma, tumor progression by way of the lymphatics is often seen, and lymph node metastasis is a critical factor influencing the recurrence of cancer and its prognosis. We, therefore, investigated the relationship between the expression of FasL and the lymphatic spread of gastric carcinoma. FasL-expression was examined by an immunohistochemical method using 100 surgically resected gastric carcinomas and 55 metastatic lymph nodes. Apoptotic cells among tumor infiltrating T lymphocytes were detected by T lymphocyte staining and the terminal deoxynucleotidyl transferase (TdT) mediated dUTP nick end labeling (TUNEL) method in a series of sections of metastatic lymph nodes. FasL-expression was detected in 86% of primary lesions and 71% of metastatic lymph nodes. In cases with high levels of FasL-expression, the observed expression of lymph node metastases was significant (p=0.047). Moreover, FasL-positive cases with both primary lesions and metastatic lymph nodes showed also distant lymph node metastasis beyond the regional lymph nodes (p=0.030). Apoptosis among tumor infiltrating T lymphocytes was more frequently seen in FasL-positive lesions (p=0.057). Furthermore, patients with FasL-negative primary lesions tended to exhibit longer survival times than patients with FasL-positive primary lesions. The results suggest tumor escape through the lymphatic pathway via FasL-expression in gastric carcinomas.

[Outcome of acute myelogenous leukemia in 41 patients treated with idarubicin: the prognosis of t(8;21) cases].

Idarubicin (IDR) has been used as the main drug in induction chemotherapy for acute myelogenous leukemia (AML) in the USA and Europe. Between May 1995 and October 1998, we treated 41 cases of fresh AML using IDR induction chemotherapy and analyzed the clinical course, remission rate, relapse rate and prognosis. The results obtained in these cases were similar to those in 26 cases treated with daunorubicin (DNR) in our hospital according to JALSG-AML92. The outcome in cases with abnormal chromosomes and cases showing relapse was very poor. In particular, all 5 t(8;21) cases in our series relapsed, suggesting that t(8;21) cannot be considered a favorable prognostic factor in cases treated with IDR-containing regimens. However, 3 of the 5 t(8;21) cases were positive for CD56, which itself is an unfavorable prognostic factor. Thus it is possible that CD56 was related to the poor outcome. Intensive post-remission induction chemotherapies will be required in order to obtain prolonged disease-free survival.

[Recent progress in epidemiologic research of uterine cancer].

Although the mortality and incidence of cervical cancer have been decreasing, those of uterine-body, or endometrial, cancer have been increasing. The proportion of endometrial cancer was reported to have become 33.6% of primary uterine cancers in 1995. Infection with certain types of human papilloma virus (HPV) is considered to be etiologically important for the occurrence of cervical cancer. Because HPV is sexually transmitted, some risk factors for cervical cancer are associated with certain kinds of sexual behavior such as a young age at first intercourse, multiple partners, and infrequent use of barrier-type contraceptives such as condoms. Frequent conceptions and deliveries and histories of sexually transmitted diseases like infection with herpes simplex virus type 2 or chlamydia also have been suggested to be associated with the risk of cervical cancer. Smoking habits and infrequent intake of vegetables and fruits may be related to the increased risk of cervical cancer by supporting persistent infection of HPV through impaired immunological function. Although host factors such as a variant of a tumor suppressor gene like p53 have been assessed in terms of the risk of cervical cancer, these are not yet clearly elucidated. Estrogen stimulation of the endometrium unopposed by progesterone stimulation, namely, unopposed estrogen stimulation, is thought to be involved in the etiology of endometrial cancer. Frequent intake of animal fat, obesity or being overweight, infertility, and histories of diabetes mellitus, hypertension, and polycystic ovary syndrome have been reported to be risk factors for endometrial cancer, and they are thought to increase unopposed estrogen stimulation. Estrogen replacement therapy for postmenopausal symptoms, tamoxifen therapy for breast cancer, and taking sequential-type oral contraceptives have been shown to be exogenous risk factors for endometrial cancer in that they increase unopposed estrogen stimulation to endometrium.

[Ovarian cancer].

Age-adjusted ovarian cancer deaths and mortality rates have increased annually in Japan from 1968 to 1995, with the absolute number of deaths increasing 4-fold during these years. Internationally, the mortality rates are high in North America or northern Europe, but their incidences have gradually decreased. However, the incidences of ovarian cancer have increased in France, Spain, and Japan. Risk factors for epithelial ovarian cancer include older age, being northern European or North American, family history of ovarian cancer, nulliparity, infertility, and obesity, and preventive factors include oral contraceptive use, gravidity, lactation, tubal ligation, and hysterectomy.

Lack of association between the platelet glycoprotein Ia C807T gene polymorphism and myocardial infarction in Japanese. An approach entailing melting curve analysis with specific fluorescent hybridization probes.

The platelet-collagen receptor, glycoprotein Ia/IIa (integrin alpha2beta1) plays a fundamental role in the adhesion of platelets to fibrillar collagen, an event leading to platelet activation and thrombus formation and contributing to the pathogenesis of thrombotic disease. Further, glycoprotein Ia/IIa receptor density and function may be associated with two linked and silent polymorphisms (807C/T and 873G/A) within the glycoprotein Ia gene. We tested the extent to which these polymorphisms serve as genetic markers of myocardial infarction in a Japanese population. A case-control study was carried out using 210 Japanese myocardial infarction patients and 420 age- and sex-matched controls. Genotyping was accomplished using PCR followed by melting curve analysis with specific fluorescent hybridization probes. The 807CC, CT, TT genotypes linked perfectly to the 873GG, GA, AA genotypes, respectively. Allele frequencies of the 807T (873A) variant were similar in the control and patient groups (0.373 vs. 0.352). The 807T and 873A variants of platelet glycoprotein Ia gene are common and in a perfect linkage in the Japanese population, but it appears unlikely that the 807T (873A) variant represents a useful marker of increased risk for myocardial infarction.

[Role of leptin and its receptor in the regulation of appetite and body fat].

The role of leptin and its receptor on the regulation of appetite and body fat was summarized. Leptin directly exerts its anorexigenic effects on arcuate nucleus via proopiomelanocortin and neuropeptide Y neurons. The anorexia and sympathetic nerve activation result in the reduction of body fat. But physiological concentrations of leptin could not reduce body fat in obese people, while genetic loss of central leptin effects induces obesity in children. Melanin concentrating hormone, orexin, and corticotropin-releasing hormone may be directly regulated by leptin. Serotonergic neurons may be separate from leptin effects. Phosphorylation of 985- and 1138-tyrosine of long-form leptin receptor activates SHP-2 and STAT3, respectively. Soluble leptin receptor concentrations in serum are negatively correlated with BMI. Clinical usefulness of leptin is now in progress.

[Helicobacter pylori test and medical reimbursement].

There are an estimated 60 million people with Helicobacter pylori(H. pylori) infection who occupied 50% of the population of Japan. In Japanese medical reimbursement H. pylori tests were introduced on November 1, 2000 and they are able to use only to patients with gastric and duodenal ulcer. H. pylori tests were composed of rapid urease test, urea breath test, antibody test, bacterial culture and pathologic test. Payment of each test is 700 Yen. Classification and cost of H. pylori tests are shown. Usage of laboratory tests for H. pylori infection is mentioned. Those particular tests are useful to decrease the number of gastric and duodenal ulcer in Japan.

Systematic immunolocalization of retinoid receptors in developing and adult mouse eyes.

PURPOSE: To determine the localization of retinoic acid receptors (RAR) alpha, beta, and gamma and retinoid X receptors (RXR) alpha, beta, and gamma in developing and adult mouse eyes at the level of single cells. METHODS: Immunohistochemistry was performed on paraformaldehyde-lysine-periodate-fixed cryosections of mouse eyes, from embryonic day 10.5 to adulthood, with polyclonal antibodies directed against each receptor isoform. Histologic sections from null mutant mice for each receptor served as negative controls. RESULTS: RARalpha was present ubiquitously in the prenatal eye and preferentially located in the posnatal retina and ciliary body. RARbeta was detected predominantly in the periocular mesenchyme and ciliary body. RARgamma was distributed in the periocular mesenchyme, choroid, sclera, cornea, conjunctiva, and lids. RXRalpha was found preferentially in the prenatal periocular mesenchyme and retina and in the postnatal ciliary body, cornea, and conjunctiva. RXRbeta was ubiquitous at all the stages. RXRgamma was detected mainly in subsets of prenatal retinal cells and in postnatal ganglion cells as well as a subset of photoreceptor cells that were characterized as cones in adults. CONCLUSIONS: RARalpha, beta, and gamma and RXRalpha and gamma exhibit specific and dynamic patterns of distribution in ocular tissues throughout the course of development. The abundance of RARbeta, RARgamma, and RXRalpha in the periocular mesenchyme suggests that this tissue represents an important site of retinoid actions during eye development and in adulthood.

[Virtual bronchoscope system].

In this article, we describe the features of Virtual Bronchoscope System(VBS) and its practical use. VBS is constructed based on 3-D chest CT images. The bronchus region is automatically extracted from 3-D chest CT images by a three-dimensional region growing method. The surface rendering is employed for construction of virtualized tracheo-bronchial tree. It gives us an environment where we can observe inside the bronchi from an arbitrary viewpoint and a view direction. By mouse operation, the user can control the viewpoint and the view direction to fly through inside the airway in real time. VBS is applicable for a variety of purposes such as diagnosis, surgical planning, informed consent, education and training. One of extension of this system is a teaching tool for medical students. In the module for educational use, we have developed four functions for using the system as a teaching tool as follows: (a) automated display of bronchial anatomical names, (b) presenting questions about the currently observed branch in the endoscopic view, (c) display of the path which the user should follow, and (d) display of a question about the location of the artificially created tumor in the bronchus. These functions use the processed results of automated anatomical labeling. The method proposed here combines the knowledge based processing technique 'automated labeling of bronchial branch' and the novel visualization technique 'virtual bronchoscope'. This is one of new teaching tools of medical images. We conclude that this virtual bronchoscope system might have an important role in the medial students' education.

[Laboratory features of acute myelogenous leukemia in elderly patients].

The hematologic features of 103 AML patients older than 60 years(elderly patients) were analysed. Prior to AML a myelodysplastic state was noted in 28% of those patients, 39.3% of patients had a white blood cell count below 3000/microliter, and in 25%, no blast cells were detected in the peripheral blood. Hypoplastic marrows were noted in 25.2%. In regard to the subtypes, M3 was noted less frequently in the elderly patients than in the younger patients. CD34 in blast cells was observed in 53% of patients and dysplastic changes in bone marrow cells other than blast cells were observed in 46% of elderly patients. Chromosomal abnormalities with good and poor prognosis were noted in 3.1% and 25.0% of patients respectively, and the most commonsites for chromosomal abnormalities were chromosomes 5 and 7. MDR1 abnormality was detected more frequently in the elderly patients(68%) than in younger AML patients(38%). We speculated that the laboratory features of the elderly AML patients may be related to the aging of bone marrow and the effects of environmental factors.

[The clinical significance of detecting small numbers of carcinoma].

The clinical significance of detecting small numbers of carcinoma cells has been emphasized. The techniques for detecting micrometastasis in the bone marrow, lymph node, and peripheral blood have progressed remarkably in recent years with the advance of molecular biology. We use molecular biological techniques, such as reverse transcribed-polymerase chain reaction (RT-PCR) targeting the carcinoembryonic antigen (CEA) gene, and magnetic-activated cell separation system (Miltenyi Biotec Inc., CA) plus modified MASA targeting for K-ras mutation in practical clinical diagnosis and treatment. We herein, we describe how to obtain samples, such as bone marrow, lymph node, and peripheral blood, and how to prepare these samples. We then discuss the pros and cons of tumor specificity and detectability in several methodologies. Thereafter, we describe our strategies with regard to RT-PCR for CEA mRNA and MACS plus MASA method for K-ras mutations. At present, however, we have no consensus for anyone method, and therefore, stress the need to establish an appropriate methodology through a cooperative study.

Synthesis and optical resolution property of (1 -->6)-alpha-D-glucopyranan tris(phenylcarbamate) as chiral stationary phase in HPLC.

The optical resolution property of 2,3,4-Tris-O-(3,5-dimethylphenylcarbamoyl)-(1-->6)-alpha-D-glucopyranan (5) toward racemic compounds has been studied as the chiral stationary phase (CSP) in high-performance liquid chromatography (HPLC). For the synthesis of (1-->6)-alpha-D-glucopyranan (4), the ring-opening polymerization of 1,6-anhydro-2,3,4-tri-O-allyl-beta-D-glucopyranose (1) has been proceeded through the stereoselective ring-opening mechanism to yield the 2,3,4-tri-O-allyl-(1-->6)-alpha-D-glucopyranan (2). For chromatographic enantioseparation of ten racemates, such as tröger's base and benzoin, using 5 as a CSP, it was found that the chiral discrimination ability of 5 with the high molecular weight polymer (CSP-5a) was higher than that with the low molecular weight polymer (CSP-5b). In addition, the elution tendency of racemates using CSP-5a was essentially identical with the results of enantioseparation using dextran tris(3,5-dimethylphenylcarbamate).
