Supracondylar fractures of the humerus associated with ipsilateral forearm fractures in children: a report of forty-seven cases.

Supracondylar fractures of the humerus associated with ipsilateral forearm fractures are uncommon and treatment recommendations are controversial. The purpose of this study was to determine whether pin fixation of both fracture components, humerus and forearm, would improve the outcome. In a two-center trial, 884 children sustaining supracondylar fractures of the humerus were retrospectively reviewed, and 47 (5.3%) showed associated ipsilateral forearm fractures. Of those, 29 underwent Kirschner-wire fixation of the forearm fracture, and 18 of the forearm fractures were treated with casting alone. Three of the 18 forearm fractures with casting alone reangulated. There were no reangulations in the patients who had pin fixation of their fractures. There were no complications due to pin fixation in the humerus or the forearm. In unstable supracondylar humerus and forearm fractures, stabilization with pin fixation to prevent reangulation should be considered.

Complex fractures of the femur: treatment with a long gamma nail: short- and medium-term experience.

The introduction of the gamma nail and of the long gamma nail, associating the principle of intramedullary nailing and of the sliding screw-plate, has led to good results in the treatment of complex fractures such as per-sub-trochanteric fractures and associated fractures of the femoral neck-diaphysis. In this study, our experience with the long gamma nail (Long Gamma Nail Howmedica) is presented. Of the 43 patients treated between 1993 and 1998, 21 (49%) with a mean follow-up of 2.5 years (1-5 years) were reviewed. Clinical and a radiographic evaluations were obtained for each of the patients. This study showed that there was a high incidence of poor consolidation (dysmetria in 10 patients, torsion deficit in 7, consolidation in varus of the femoral neck in 1 patient) particularly in associated fractures of the femoral neck and diaphysis, accompanied, however, by good functional recovery (only 3 patients with modification in the angle of progression of the foot) and by the satisfaction of the patients questioned. Thus, indications for the use of this device in proximal fractures of the femur, particularly in elderly patients and in high-energy traumas, are demonstrated.

[Absence of HTLV-I/II virus circulation in blood donors from the provinces of Santa Fe and Santiago del Estero].

In the present study, we evaluated the seroprevalence of HTLV-I/II infection among the blood donors in Santa Fe and Santiago del Estero provinces. A total of 1327 serum samples from blood donors from Rafaela blood bank of Santa Fe province and 3382 serum samples from blood donors from Dr Edgar Bouzon blood bank of Santiago del Estero province were studied. The antibody screening was done by particle agglutination assay (PA) (SERODIA, Fujirebio Inc., Tokyo, Japan) or by enzyme immuno assay (EIA) (Abbott HTLV-I/HTLV-II EIA, Abbott, Germany). The "in house" indirect immunofluorescence assay (IFA) and Western blot (Bioblot HTLV Biokit, Barcelona, Spain) were used as confirmatory assays. All the samples resulted negative for specific antibodies against HTLV-I/II. These results suggest that HTLV-I/II are not circulating in low risk populations in these provinces or that the prevalences of infection would be lower than that reported by blood banks in other provinces of Argentina.

Long-term peritoneal nutrition in dogs: metabolic and histopathologic results.

In order to test the use of the peritoneal cavity in prolonged nutrition, seven dogs were fed for 22 days via peritoneal catheter. The nutritive preparations were composed of 10% crystalline amino acids, 20% fat emulsion, 40% glucose, trace elements, Na+, K+, insulin and a multivitamin compound. The caloric rate was 45kcal/kg/24h plus 0.5g/kg/24h of amino acids. These animals were compared to another group of six dogs, which were catheterized and infused with a 3% saline solution in addition to an oral intake of 45kcal/kg/24h. Both groups had free use of water. There was a case of slight peritonitis in one dog on peritoneal nutrition but no deaths and no weight changes occurred throughout the experiment. Increases in serum levels of TG, FFA and cholesterol were observed in the animals treated with peritoneal nutrition as well as a decrease in albumin serum levels. Peritoneal hyperplasia and phagocytosis were noted in the animals on peritoneal nutrition but no damage was recorded in other organs.

Peritoneal nutrition in dogs after intestinal resection: comparative study.

A total of 18 adult dogs were kept on peritoneal nutrition (PeN) for 30 days via peritoneal catheter. Eleven dogs were put on peritoneal nutrition immediately, receiving a caloric rate of 45 kcal/kg per day, half of which was supplied by glucose and the other half by 20% intralipid, along with 0.5 g/kg per day of protein, as well as ions, trace elements, and vitamins. Two-thirds of the glucose was administered separately. Seven other animals underwent an 80% resection of the small intestine and were kept on special oral nutrition for 6 weeks. After this time, peritoneal nutrition was applied with the same caloric rate as in the previous group but with a protein dose of 1 g/kg per day and administration of the nutrients every 6 hours. We found two cases of peritonitis in the group without intestinal resection, whereas no infectious outbreaks occurred in the group subjected to intestinal resection. This group showed an exacerbated nutritional state, with a consequently greater loss of weight and albumin and a negative nitrogen balance. The peritoneal membrane showed signs of cellular hyperplasia and hypertrophy following peritoneal nutrition. These phenomena disappeared 1 month after PeN was terminated.

[Superoinferior ventricles].

We describe two patients with under-and-over ventricles. Both were diagnosed angiographically. One has atrial, situs solitus with discordant atrioventricular and ventriculoarterial connections and a ventricular septal defect. The second has atrial situs inversus with concordant atrioventricular connections and double-outlet right ventricle. We review the literature and analyze the probable embryogenesis of the malformation.

[Microscopic pulmonary arteriovenous fistulas].

A case of pulmonary arterio-venous microfistula in a fifteen year old male is reported. He referred dyspnea and cyanosis for 3 and 2 years respectively. Physical examination was normal except for finger clubbing. Chest radiograph and EKG were also normal. Lung function tests showed hypoxemia due to an increase in veno-arterial shunt (Qs/Qt). Its localization was not apparent by routine studies such as phonocardiogram, echocardiogram, right heart catheterization and pulmonary angiogram. However, a contrast echocardiogram and selective occlusion of the right and left pulmonary arteries performed with a Dotter-Lucas catheter were suggestive of an intrapulmonary localization of the shunt. A lung biopsy confirmed the diagnosis.

[Results of the cochlear transplant program of the University of Navarra].

From October 1989 to the end of 1991, 33 patients have received a Cochlear Implant Nucleus 22 Channel, that have been selected from over 150 patients that consulted the possibility of implantation. A cochlear implant program should count with a group of professionals of diverse areas, that work in coordination without losing contact with the rest of the member of the implant team. Cochlear Implant are considered an efficient technique in the treatment of profound neurosensorial deafness, in Pre and Postlinguistic patients. The results obtained have been better in the last group of patients, but results on the whole are considered satisfactory. In prelinguist there is a progressive positive tendency, therefore a longer amount of time is necessary to define the final results. The therapeutic capacity of this method of treatment is not limited only to the auditory area, but it has effect also in other fields related to communication and the integral development of the patients on whom it is applied.

[Cochlear implants in post-lingual persons: results during the first five years of the clinical course].

Abundant literature is available on the results of cochlear implants in postlingual persons. Nevertheless, there are few long-term studies with a follow-up of more than 1 year. We present the outcomes of 37 postlingual patients with a 5-year follow-up period. We examined the modifications that occurred with changes in coding strategy (MPEAK-SPEAK). After activation, the coding strategy was SPEAK in 10 patients. MPEAK in 20: 7 other SPEAK had changed from MPEAK. Significant improvement (p < 0.01) was observed in all the tests in the first 6 months. Subsequent assessments revealed a constant improvement in all tests. The results were statistically significant (p < 0.05) for vowels, consonants and bisyllables. Performance with the SPEAK strategy was better than with MPEAK. These results were statistically significant for consonants (p < 0.05), monosyllables (p < 0.05) and bisyllables (p < 0.01). We concluded that the results of postlingual patients after cochlear implantation showed clear benefits of these devices in profound bilateral deafness, better results with the SPEAK coding strategy than with MPEAK and constant improvement in results, even after the first year of surgery.
