A stress analysis of the proximal tibia after insertion of the endoprosthesis.

Detailed investigation for the local stress increment at the fixation post bottom of an artificial knee joint was performed by two-dimensional finite element method. A strain measurement was also conducted by wire strain gauges on a cancellous bone with a special method. It was made clear that a curvature with a certain extent of radius was necessary for the corner of a fixation post and application of acrylic cement with a certain extent of thickness (3 mm less than or equal to) was effective for stress relaxation, and that an optimum value (10(-1)-10 GPa) existed for elastic modulus of bone cement.

Depletion of hepatic coenzyme A derivatives is one of the markers of the toxicity of orally administered secondary autoxidation products of linoleic acid in rat.

When 400 mg/rat/day of secondary autoxidation products of linoleic acid was orally administered 3 times to rats, they died at 30-40 h after the third dose. To search the markers of the toxicity of secondary products in vivo, the rats were killed at 24h after the third dose, and conditions of their digestive tracts and liver were analyzed. In the stomach, macroscopically, inflation, retention of undigested food, and edema were seen. Slight congestions were detected in the small intestines. It was considered that these injuries led to reduction in food consumption and then depression of the growth, but did not lead to the death of the animals. The lipid peroxide levels in the liver and the activities of its detoxifying enzymes were increased as compared to those in the control groups. The hepatic lipid contents and unsaturated fatty acid compositions were also not changed. The endogenous lipid peroxidation, therefore, did not give the rats a severe stress. The activities of hepatic acetyl-CoA carboxylase and carnitine palmitoyltransferase were 20 and 35% lower than those of control, respectively. The levels of CoASH, acetyl-CoA, and long-chain acyl-CoA were 1/9, 1/2, and 1/4 of those in control, respectively. Thus, one of the markers of the toxicity of secondary products was the depletion of hepatic CoA derivatives. In rat, bio-energy was reduced by the decrease in the intestinal absorption of nutrients, and the depletion of hepatic CoA derivatives also failed to supply energy with beta-oxidation.

[Relationship between luteal function and prolactin in infertile women with endometriosis].

Luteal function in 44 infertile women with endometriosis were studied with reference to prolactin (PRL) and compared with 34 unexplained infertile women without endometriosis. To assess luteal function, serum progesterone (P4) levels were measured on the 3rd, 7th and 10th days of the luteal phase. On the 7th day, serum estradiol (E2) levels and PRL levels were also determined. The response of PRL secretion to TRH was examined at 30 and 60 after following TRH (500 micrograms, im.) administration. The incidence of hyperprolactinemia (basal PRL level greater than or equal to 25 ng/ml) and latent hyperprolactinemia (peak PRL level in TRH challenge test greater than or equal to 150 ng/ml) were 19% and 31%, respectively, in the endometriosis group and 14% and 33%, respectively in the control group. At the midluteal stage, serum P4 levels in endometriosis group were decreased significantly (p less than 0.05), whereas no difference was found between the serum E2 levels in the endometriosis group and the control. In the endometriosis group, there was no correlation between P4 and E2 levels and abnormal secretion of PRL such as hyperprolactinemia and latent hyperprolactinemia. These results indicate the close association of endometriosis with an inadequate luteal phase. However, it seems that the aberrant secretion of PRL has no relation to the impared luteal function in endometriosis.

[Regulation of aromatase activity by gonadotropin-releasing hormone and its agonists in cultured rat granulosa cells].

The effects of Gn-RH and its agonists on the activity of aromatase were examined by means of a rat granulosa cell culture system. A potent Gn-RH agonist, buserelin ([D-Ser (but)6 des-Gly-NH2(10)] Gn-RH ethylamide), alone stimulated aromatase activity in a dose-dependent manner at concentrations between 10(-13) and 10(-6) M. Both Gn-RH and another Gn-RH agonist, TAP144 ([D-Leu6, des-Gly-NH2(10)] Gn-RH ethylamide), also exhibited a stimulatory effect. Buserelin enhanced the activity of aromatase stimulated by FSH at concentrations of 1 and 10 ng/ml. However, it decreased the activity stimulated by a higher level of FSH (100 ng/ml). These results indicate that Gn-RH agonist increases the activity of aromatase in the absence of FSH. However, in the presence of FSH it exerts different effects on the aromatase activity depending on the concentrations of FSH.

[Roles of epidermal growth factor (EGF) in the growth and differentiation of human endometrium].

Endometrial tissues undergo drastic changes during menstrual cycle. After menstruation, they proliferate and differentiate into cells with secretory activity in the preparation for egg implantation. Although sex steroids play an important role in the development of endometrial tissues, sequential events occurring in the endometrium can not be fully explained by the direct actions of sex steroids. In this study, we offer evidences that EGF is released from endometrial cells and they possess the receptor for EGF. These findings prompted us to explore the biological roles of EGF in endometrial tissues. Here we clearly demonstrate that EGF is involved in the proliferation of endometrial cells. Moreover, EGF is found to enhance both glycogenesis and glycogenolysis, thus increasing the supply of glucose for blastocysts. We further set forth that EGF augments the capacity of progestin receptor and release of prostaglandins in endometrial cells. In summary, this study emphasizes that EGF may participate in the development of human endometrial tissues in concert with sex steroids, thus contributing to the acquisition of receptivity of eggs in the endometrium.

[Study on gypsum hardener].

The purpose of this study was to search for an appropriate condition for shortening the treatment time of gypsum hardener, Plaster Aid. In order to keep the increased strength of plaster with this new hardener, compressive strength and dimensional change in various conditions of heating temperature, heating time and drying time were examined. The following results were obtained. 1. High compressive strength was maintained in the condition of 70 degrees C of heating temperature, with more than 15 minutes of heating time and more than 15 minutes of drying time. In this condition up to 90 minutes could be saved in comparison with the conventional method. 2. Plaster models had a tendency to shrink in the condition of 70 degrees C of heating temperature.

[Surface accuracy of hydrophilic impressions. 1].

The purpose of this study was to examine detail reproducibility and surface roughness of stone models made from silicone, polysulfied rubber, alginate and agar impression materials using stainless steel test block (ISO). In addition, the influence of artificial saliva was studied. The following results were obtained: 1. Silicone impression materials showed the best detail reproducibility of all materials. 2. When artificial saliva was dropped on the stainless steel test block, hydrophilic silicone impression material showed the best detail reproducibility. 3. In the test using artificial saliva, alginate and agar impression materials showed better surface roughness, but poorer detail reproducibility than silicone impression ones. 4. Hydrophilic silicone material indicated the highest detail reproducibility, and the lowest surface roughness in the test using artificial saliva.

[The significance of serum specific IgA antibody to Chlamydia trachomatis in the diagnosis and treatment of chlamydial infection in female genital tract].

Chlamydia trachomatis IgA and IgG antibody titers were determined by indirect immunoperoxidase assay in sera of 88 female patients with C. trachomatis-positive cervicitis and 55 C. trachomatis negative healthy controls. Serial serum samples obtained until 66 weeks after treatment were also studied for IgA and IgG antibodies in 28 of the 88 patients with chlamydial cervicitis. In the 88 patients, the presence (94.3%) of antibody (titer greater than or equal to 16) was comparable to that (100%) of IgG antibody (titer greater than or equal to 32), but in healthy controls the presence (5.4%) of IgA antibody was lower than that (30.6%) of IgG antibody. Serum IgA antibody decreased gradually and was undetectable (titer less than or equal to 8) at 20 weeks after treatment in 78.6% of the 28 patients. Serum IgG antibody persisted at stable levels within the same period. These data suggest that the presence of serum IgA antibody to C. trachomatis is correlated with active C. trachomatis infection of the female genital tract.

[Studies of the treatment of recurrent abortion with husband's lymphocytes].

Thirty-two primary habitual aborters who had more than 3 abortions during the early stages of pregnancies without any known causes of habitual abortions were studied. The degree of HLA-DR compatibility of the patient couples was similar to that of 70 control couples. The patients were treated with intra-dermal injections of their husbands' lymphocytes 4 times every 2 weeks. The skin reactions at the sites of the injection were estimated 24 hours and 48 hours after the first and fourth injections. The mixed lymphocyte reaction (MLR) after the last injection decreased in the patients whose skin reaction was weaker following the last injection than the first (p = 0.007), and these patients had a good prognosis for subsequent pregnancies (p = 0.010). On the other hand, the MLR after the last injection did not weaken in the patients whose skin reaction did not become weaker (p = 0.007), and in these patients abortion resulted in subsequent pregnancies (p = 0.010). The HLA-DR compatibility of the patient couple and anti-HLA-DR antibody induced by the treatment did not affect the prognosis for subsequent pregnancies.

[Angiographic classification for the selection of treatment of spontaneous carotid-cavernous sinus fistula].

Sixteen cases of spontaneous carotid-cavernous sinus fistula treated in our clinic were angiographically classified into four types, based on the report by Barrow et al; Type A: direct shunts between the internal carotid artery (ICA) and the cavernous sinus (CS), Type B: dural shunts between meningeal branches of the ICA and the CS, Type C: dural shunts between meningeal branches of the external carotid artery (ECA) and the CS, type D: dural shunts between meningeal branches of both the ICA and ECA and the CS. Our fundamental modality of treatment for each type was described as follows; detachable balloon occlusion of fistula was performed for Type A immediately after the diagnosis was confirmed. On the other hand, conservative treatment was selected at first routinely for Type B, C and D with administration of hemostatic agents, control of the blood pressure and Matas test for a certain period. This selection was made because the latter three types sometimes showed a high rate of spontaneous regression of symptoms. Only when no improvement was obtained by conservative therapy with Type B, C and D, the following treatments were adopted respectively; Type B: irradiation----detachable balloon occlusion of fistula, Type C: embolization through ECA, Type D: embolization through ECA----irradiation.

[Ocular motor abnormalities in progressive supranuclear palsy].

Eleven patients, 7 males and 4 females, of progressive supranuclear palsy (PSP) were examined neuro-otologically for the purpose of elucidating the characteristics of ocular motor abnormalities. All cases were admitted to our hospital and age at onset was from 52 to 71 years old, duration of illness was 2 to 11 years. Range of voluntary eye movements and abnormal eye movements including nystagmus were examined on naked eyes and with electronystagmography (ENG). Smooth pursuit movements and saccadic eye movements were tested both horizontally and vertically by using visual tracking method with ENG recordings. Optokinetic nystagmus test and caloric test with visual suppression test were also performed. These neurotological examinations were made repetitively in 5 cases and their progressions were observed. Vertical gaze palsy and convergence palsy were observed in all cases as the initial symptom. In this study downward gaze was more severely disturbed than upward gaze. Using ENG, saccadic eye movements (saccades) were disturbed earlier than smooth pursuit movements. Hypometric saccades and decreased saccadic velocity were common abnormalities. In the later stage of the disease, horizontal eye movements were also disturbed. In four cases bilateral adduction palsy was added to vertical gaze paralysis so that the lesion of the MLF to oculomotor nucleus was suggested to exist. These voluntary eye movements were worsened gradually as the disease progressed. By using ENG we could find so called abnormal eye movements more frequently than the previous reports. Eight patients demonstrated horizontal gaze nystagmus, and rebound nystagmus were observed in four cases.(ABSTRACT TRUNCATED AT 250 WORDS)

[Retroperitoneal lymph node metastasis in patients with ovarian cancer].

From 1979 to 1987 retroperitoneal lymph node dissection was performed at the Tokyo University Hospital in 41 cases (pelvic lymph node biopsy was done in 4 cases, pelvic lymphadenectomy in 23 cases, pelvic and paraaortic lymphadenectomy up to the renal vessels in 14 cases) of Stage Ia to IV ovarian cancer following cytoreductive surgery. The incidence of retroperitoneal positive nodes was 11.1% (2/18) in Stage I, 50.0% (5/10) in Stage II, 50.0% (5/10) in Stage III and 0% (0/3) in Stage IV (FIGO criteria without considering the pathologic findings of retroperitoneal lymph nodes). The positive rate of lymph node involvement in Stage II and Stage III was significantly higher than that in Stage I. The tumors involving both ovaries were more likely to metastasize to retroperitoneal lymph nodes. Enlargement of tumors and increased ascites were not the risk factors of retroperitoneal lymph node metastasis. These data suggest that the occurrence of retroperitoneal lymphatic spread in ovarian cancer is comparable to that in uterine cancer and increased by involvement of both ovaries and extension to other pelvic tissues.

[Combined chemotherapy of CDDP and etoposide in intracranial germinomas].

Two cases with pineal germinomas were treated by the combined chemotherapy of CDDP and etoposide. The first case was a 15-year-old boy who had a pineal tumor of 2.2 x 1.8 cm in diameter. He was given intravenously 25 mg/body of CDDP and 30-60 mg/body of etoposide daily for five days. The tumor had already decreased at two days after starting chemotherapy and disappeared completely just after the first course of chemotherapy. The patient received another course of chemotherapy and focal radiation of 40 Gy to prevent tumor recurrence. The second case was 19-year-old man who had a pineal germinoma associated with CSF dissemination and hydrocephalus. He was also treated by 30 mg/body of CDDP and 80 mg/body of etoposide daily for five days. The tumor size decreased more than 50% just after the first course of chemotherapy, and complete remission was obtained by additional radiation of 6 Gy. From these results, this combined chemotherapy is useful for the treatment of intracranial germinoma, and this makes it possible to discontinue radiation therapy or at least reduce the radiation dose and radiation field.

[Intracranial fibrosarcoma fifteen years after radiotherapy in bilateral retinoblastomas: effect of combined chemotherapy with cisplatin and VP-16].

We report intracranial fibrosarcoma which developed after radiotherapy for bilateral retinoblastomas. A 16-year-old boy was admitted with swelling of his left eyelid as his chief complaint on September 10, 1987. He had a past history of bilateral retinoblastomas at one year of age, which was treated by bilateral ophthalmectomy and postoperative radiotherapy; 50 Gy to right eye and 60 Gy to left eye. Until fifteen years old, he had no trouble except blindness. On admission, a dumb-bell type huge tumor with the destruction of the orbital roof was demonstrated on CT scan and MRI. The tumor was composed of solid orbital tumor and cystic tumor located in the left frontal lobe. On September 18, partial removal of the orbital tumor and evacuation of the intracranial cyst following embolization of the left external carotid artery was performed. The histological diagnosis of the tumor was fibrosarcoma. For the residual tumor, combined radiochemotherapy was carried out. The patient was treated with intravenous administration of 20 mg/m2 of cisplatin and 60 mg/m2 of VP-16 daily for five days and then whole brain radiotherapy of 26 Gy. On March 27, 1988, he obtained a complete remission of the tumor and was discharged without neurological deficit except blindness. In this case, we have presented a radiation-induced secondary tumor after a treatment of bilateral retinoblastomas. Although the tumor was highly proliferative and invasive fibrosarcoma, we could control it well by multimodality treatment including surgery, radiation and chemotherapy. It is noteworthy that combined chemotherapy with cisplatin and VP-16 was effective against this type of tumor.

[A clinicopathological study of advanced ovarian cancers without an enlargement of the ovaries].

A clinicopathologic study of advanced ovarian carcinomas without an enlargement of the primary site has been performed. Cases studied were taken from 64 cases with a "common" epithelial tumor whose ovaries upon resection were directly measured for size. The ovaries of five cases (8%) were normal in size, and 3 (5%) of this number had been treated by chemotherapy prior to the resection of their ovaries. All three cases had been suspected to have an ovarian cancer at the start of treatment, one of these cases given a definite diagnosis of ovarian cancer, stage III, and the remaining two cases diagnosed as belonging to a special category. In these 3 cases, the ovaries had decreased in size and the primary cancerous site was difficult to determine, owing to the administered chemotherapy. A preoperative diagnosis of the remaining two of the 2 cases suspected an ovarian cancer in one, and a uterine corpus carcinoma, Ia in the other, associated with an ovarian cancer, stage III. These findings suggest that even though the enlargement of the ovaries was not to be remarkable, a laparotomy should be performed in cases strongly suspected of ovarian cancer, so as to ascertain the correct diagnosis by a careful inspection of the intraabdominal cavity, such as checking for the presence of ascites with malignant cells, or an elevated serum CA 125 without any other accountable primary foci.

Experience with the treatment of metastatic ovarian carcinoma.

Metastases from non-genital sites comprise about one in six of our malignant ovarian tumors. The mean age of our 24 patients with such metastases was 43.1 years and 81% had bilateral ovarian metastases. Out of the 24 patients we studied, 21 had a gastric primary. The overall survival rate was 41.7% in 6 months, 25.0% in 1 year and 12.5% in 2 years. Patients with no extraovarian metastases had a survival rate of 75.0% at 6 months, 62.5% at 1 year and 25.0% at 2 years after operation.
