[Influence of the angle between the nasal cavity axis and nosepiece in acoustic rhinometry].

We evaluated the influence of the angle between the nasal cavity axis and the nosepiece (made with acrylics) in acoustic rhinometry (AR). This study was composed of three experiments. In the first experiment, the influence of angulation on nasal volume was evaluated using a silicone nasal model based on a cast of the nasal cavity of a cadaver. The angle between the nosepiece and the horizontal plane was changed by 10 degrees, 40 degrees or 70 degrees, vertically; the angle between the nosepiece and the sagittal plane was similarly changed horizontally. The nasal volume, obtained as the AR area-distance curve from 6.9-14.1cm, was measured under each condition. While the horizontal change in angle did not significantly influence the result obtained by AR, in the vertical change, the measured value rose as the angle was decreased from 70 degrees to 10 degrees. In the second experiment, an evaluation in healthy subjects (5 males, 4 females, mean age 28.6 years) revealed the same tendency in changing the horizontal angle, whereas vertically a change of 10 degrees produced a significant decrease over that seen with angles of 40 degrees and 70 degrees. This result contradicted that obtained in the model study mentioned above. In studying the effect of changing the vertical angle, the volume of the posterior 5 cm and that of the anterior 2 cm of the nasal cavity was evaluated in the same subjects. The volume of the posterior 5 cm did not differ significantly among angles, while that of the anterior 2 cm showed the same trend as observed with the whole nasal cavity.(ABSTRACT TRUNCATED AT 250 WORDS)

[Historical and herbalogical studies on coloring crude drug (Part 3) "Shu wei cao"].

The usage of Shu wei cao as a coloring matter was described for the first time in Er Ya as the name of Jing shu wei. But, in modern Chinese and Japanese literature, few papers reported the use of Shu wei cao as a dye. As a herbal medicine, it seems that it was first described in Ming Yi Bie Lu. In this study, we found that in Chinese and Japanese literature, there is much confusion about the origin of Shu wei cao. It is likely that the original plant of Shu wei cao is to be Salvia japonica Thunb. In Japan, it is feasible that Shu wei cao is to be recognized as Aki-no-tamura-so, while some suggested that misohagi (Lythrum anceps Makino) was identical to Shu wei cao.

Different gene expression of mouse transforming growth factor alpha between pregnant mammary glands and mammary tumors in C3H/He mice.

Mouse transforming growth factor alpha (TGF alpha) gene expression was determined by reverse transcriptase-polymerase chain reaction (RT-PCR) method in pregnant mammary glands and mammary tumors in C3H/He mice. Both normal pregnant and tumorous mammary tissues expressed mRNA for mouse TGF alpha. When primers from rat TGF alpha cDNA sequence as well as those in the previous report [12] were used, the PCR amplified products were the same sizes in both normal pregnant mammary gland and tumor. However, there were obviously different PCR products between pregnant mammary gland and tumor in case of using primers according to the mouse TGF alpha cDNA sequence. In tumors, various sizes of PCR products were detected in addition of a predicted size. This is unlikely the experimental artifact, but the polymorphic TGF alpha gene expression is induced in mammary tumors.

A case of renal sarcoidosis showing central necrosis and abnormal expression of angiotensin converting enzyme in the granuloma.

We describe a 66-year-old man who developed renal failure related to granulomatous renal sarcoidosis without systemic manifestations. Renal failure was severe enough to require hemodialysis transiently. Renal biopsy of this patient revealed the central necrosis of the granuloma which is usually absent in sarcoid granuloma. Serum level of angiotensin converting enzyme (ACE) was not helpful for diagnosis in this patient because serum ACE level is often elevated in the condition of chronic renal failure. Immunohistochemical detection of ACE was of diagnostic value in this patient. Subsequent course in which glucocorticoid was used for therapy was consistent with the diagnosis. This is the first report of identification of ACE in renal sarcoid granuloma.

Radioimmunolocalization of human brain tumor: fundamental studies with indium-111 labeled monoclonal antibody G-22.

Monoclonal antibody (MCA) G-22 reacts selectively with human glioblastoma. Whole immunoglobulin G (IgG) and the F(ab')2 fragment of G-22 were labeled with indium-111, and injected into athymic nude mice bearing xenografts of a human glioblastoma cell line (U-251-MG). Radiolabeled G-22 retained its antigen-binding activity allowing clear visualization of transplanted tumors. Although the 111In-labeled F(ab')2 fragment had a higher tumor-to-tissue ratio than whole IgG, tumor concentration of 111In-labeled G-22 MCA was higher in the latter and it provided much better images for a longer time when visualized by gamma-scintigraphy. These results suggest that 111In-labeled G-22 may be a useful agent for brain tumor imaging.

Interferon-beta endogenously produced by intratumoral injection of cationic liposome-encapsulated gene: cytocidal effect on glioma transplanted into nude mouse brain.

The cytocidal effect of endogenously produced human interferon-beta was tested on glioma transplanted into the brain of nude mice by intracranial injection of a cell suspension of human glioma cell line U251-SP. When plasmid pSV2IFN-beta, bearing the human interferon-beta gene, was encapsulated into cationic multilamellar liposomes composed of N-(alpha-trimethylammonioacetyl)-didodecyl-D-glutamate chloride, dilauroyl phosphatidylcholine, and dioleoyl phosphatidylethanolamine in a molar ratio of 1:2:2 and injected intratumorally, human interferon-beta was produced in the tumor. The tumor completely disappeared if the injection was done shortly after the transplantation. Even when the tumor did not disappear, survival of the tumor-bearing nude mice was markedly prolonged. In control experiments made with normal brain having no glioma, interferon-beta was not detected.

[Encephalomyelitis with cavitary necrosis of the white matter. A clinico-pathologic study].

The authors report the clinico-pathological study of an encephalomyelitis with necrotic cavitation of the white matter occurring in a 61-year-old female. The disease of sudden onset progressed slowly to the death, 20 months after the onset of the first symptoms. They stress the rarity of the entity and the difficulties to establish the diagnosis clinically. The nosological situation of the condition is discussed and compared with the previously reported cases.

A soluble secretory protein is first concentrated in the endoplasmic reticulum before transfer to the Golgi apparatus.

A soluble secretory protein is usually present at a much higher concentration in the Golgi apparatus than in the endoplasmic reticulum (ER) inside eukaryotic secretory cells in the steady state. We show by immunoelectron microscopic experiments with the soluble secretory protein serum albumin, inside Hep-G2 human hepatoma cells in culture, that the secretory protein is first concentrated at isolated sites within the ER before it is transferred to the cis face of the Golgi apparatus. This is contrary to expectations of the bulk-flow hypothesis of ER-to-Golgi transfer, and it suggests the involvement of concentration and transfer mechanisms within the ER that have not previously been recognized.

A protein kinase similar to MAP kinase activator acts downstream of the raf kinase in Drosophila.

D-raf, a Drosophila homolog of Raf-1, plays key roles in multiple signal transduction pathways. Dsor1, a putative factor downstream of D-raf, was genetically identified by screening of dominant suppressors of D-raf. Dsor1Su1 mapped on X chromosome significantly suppressed the D-raf mutant phenotypes, and the loss-of-function mutations of Dsor1 showed phenotypes similar to those of the D-raf null mutations. Dsor1Su1 also significantly suppressed the mutations of other terminal class genes acting further upstream of D-raf. Molecular cloning of Dsor1 revealed its product with striking similarity to the microtubule-associated protein (MAP) kinase activator and yeast PBS2, STE7, and byr1. Our genetic results demonstrate the connection between raf and the highly conserved protein kinase cascade involving MAP kinase in vivo.

BMPs induce direct bone formation in ectopic sites independent of the endochondral ossification in vivo.

Bone formation in vivo occurs via two major processes, one of which depends on pre-existing cartilage, and the other does not. Bone morphogenetic proteins (BMPs) have been suggested to induce cartilage formation from non-skeletogenic mesenchymal cell population, which results in osteogenesis through the endochondral sequence. In the present study we examined if BMPs could cause direct bone formation independent of pre-existing cartilage using bovine fibrous collagen membrane (FCM) as a carrier for BMPs. Bovine metatarsal bone was extracted in 4 M guanidine HC1 and BMPs were partially purified through the hydroxyapatite chromatography and the Heparin-Sepharose CL6B chromatography. The carrier was loaded with BMPs and then implanted in Wistar rats subcutaneously. The implants were fixed together with surrounding tissue every week after implantation and processed for von Kossa stain, immunohistochemistry, and electron microscopy. The phenotypes of bone and cartilage were identified histologically and immunohistochemically using antibodies against type I and type II collagen. Cartilage and bone were independently induced by 2 weeks. The bone formed directly on the collagen substrate of FCM without pre-existing cartilage. Calcification occurred in the carrier as well as the cartilage and bone matrix. The present study suggests that the BMPs induce osteogenesis in vivo independent of the endochondral sequence.

Sexual behavior, contraception and the risk of contracting HIV.

Recently, concern has been expressed on a possible association between the use of oral contraceptive pills (OCs) and an increased risk of contracting the HIV infection. As a consequence it has been postulated that a low prevalence of condom use due to widespread utilization of oral contraceptive pills would result in an acceleration of the spread of AIDS. The International Committee for Research in Reproduction (ICRR) a group of eleven experts in the field of gynecology, endocrinology and contraception, wishes to comment on the concerns raised about OC use and AIDS.

Neuro-otological findings in patients with spinocerebellar degeneration: with special regard to the duration of illness.

Neuro-otological findings in 150 patients with spinocerebellar degeneration were analyzed based on the duration of illness since onset. The results of each kind of neuro-otological examination varied in close relation to the duration of illness. Also, the results of each kind of examination correlated closely. In particular, the optokinetic nystagmus test and the visual suppression test correlated most closely. Neuro-otological examinations were confirmed to be of great value in gauging progression in spinocerebellar degeneration.

A carcinoid tumor of the rectum removed by strip biopsy.

In a 29-year-old male complaining of constipation, total colonoscopy revealed a 5 mm yellowish submucosal tumor at a distance of 6 cm from the anal ring. The histologic diagnosis of specimens obtained by biopsy was rectal carcinoid tumor and transanal ultrasonography revealed the tumor localization within the submucosal layer in the rectum. A strip biopsy was performed. Histopathological examination confirmed the diagnosis and ascertained complete excision. Rectal carcinoid tumors should be removed completely and strip biopsy may be a suitable procedure for removal of rectal submucosal tumors less than 10 mm in diameter.

Distribution of asialoglycoprotein receptor in human hepatocellular carcinoma.

Altered expression of asialoglycoprotein (ASGP) receptors on hepatocytes has been reported during hepatic neoplasia mostly in animal models. In this study, we examined immunohistochemically the distribution of the ASGP receptor in humans with various liver diseases, including ten cases of hepatocellular carcinoma (HCC). In livers of acute hepatitis, chronic hepatitis, cirrhosis and the non-cancerous tissues (mostly cirrhosis) adjacent to HCC, the receptor was present in its normal distribution, i.e. mostly along the sinusoidal margin and partly on the lateral surface of hepatocytes. In four of six well-differentiated HCCs, the receptor was also normally distributed on the plasma membrane; by immunoelectron microscopy, it was seen in the endoplasmic reticulum and in pits in the plasma membrane but not on bile canaliculus-like structures, suggesting that it was synthesized, transported, and integrated into the plasma membrane in a polar manner. In contrast, there was no surface expression of the ASGP receptor in the remaining six HCCs (two well-differentiated and four poorly differentiated). In two of the poorly differentiated HCCs, the receptor, although absent from the cell surface, was prominent in the endoplasmic reticulum, suggesting disturbed transport of the ASGP receptor to the cell surface. When we examined proliferative activity of HCCs by immunohistochemical labeling of DNA polymerase alpha, HCCs with high percentages (above 30%) of DNA polymerase alpha-positive cells had lost the cell-surface expression of the receptor. Thus, the expression of the ASGP receptor in human HCC appears to be closely related to differentiation and proliferative activity of the tumor cells.

[An epidemiological study on chronicity and incidence of hepatocellular carcinoma in the sequence of epidemic hepatitis].

From November 1951 to 1954, 416 patients in Kumayama Town, Okayama Prefecture, suffered from epidemic hepatitis, so-called "Kumayama Hepatitis". The mortality rate was 13.98% in the first one year, and the rate of progress to chronic hepatitis after 10 years was 24.6%. In the present study surveyed in 1991, 720 residents in this area had 13.6% positive anti-HCV (C100-3), significantly higher than in control areas. The rate of positive anti-HBs and abnormal liver functions in the subjects with positive anti-HCV was 42.9% and 40.8%, respectively. In 29 patients with clinically typical "Kumayama Hepatitis", the positivity rate of anti-HA IgG, anti-HBs, and anti-HCV, and the prevalence of abnormal liver function was 82.8%, 41.4%, 34.5%, and 10.3%, respectively. In conclusion, it is suggested that the hepatitis epidemic in Kumayama Town was caused by HAV and superinfected by HCV and/or HBV, then its clinical manifestations became complicated.

Subarachnoid hemorrhage presenting an "abnormal movement"--case report.

A 61-year-old male presented with subarachnoid hemorrhage manifesting an abnormal movement as the initial symptom. The movement was rhythmic with phases: tongue protrusion with eyes wide open, and tongue retraction with eyes closed, lasting for about 10 minutes. Neuroradiological methods identified a small aneurysm as the origin of the hemorrhage. The movement never after recurred clipping the aneurysm and clot drainage. Transient increase in the intracranial pressure was thought to be the cause.
