Our mini-facelift for Orientals.

Facelifts are classified into (1) the extra skin facelift in which redundant skin around the ear is excised without skin undermining, (2) the mini-facelift in which cheek skin is undermined up to the lateral side of the canthus, (3) the radical facelift in which subcutaneous undermining is done up to the nasolabial groove, and (4) the total facelift that includes forehead, eyelids, lips, chin, and so on. The authors checked the results of each facelift by measuring width of the preauricular that was excised. They report that the mini-facelift for Orientals yields the same results but less complications when compared with other facelift procedures except for the total facelift. The authors explain their mini-facelift technique and also the differences in the skin thickness, skeleton, and so on, between Caucasians and Orientals which influence the results of the facelift.

Simultaneous surgery for coronary artery disease and gastric cancer.

Of 26 patients who underwent both coronary artery bypass grafting and abdominal surgery at our institution between 1977 and 1992, nine had severe coronary artery disease associated with UICC stage I gastric cancer. They were treated by coronary artery bypass grafting followed by a curative operation for gastric cancer; the initial four patients underwent two-staged surgery (group A), and the most recent five patients underwent simultaneous surgery (group B). The cardiac surgery was performed first in all patients, and in group A the interval between the two procedures was 2 to 7 weeks. There were no significant differences between the two groups in terms of preoperative characteristics: sex, age, preoperative complications, NYHA class, prior myocardial infarction, ejection fraction, cardiac index, number of vessels diseased, or number of grafts. There were no significant differences between the two groups in terms of blood loss during the gastric operation (A: 649 +/- 194 ml; B: 842 +/- 326 ml) or the operating time (A: 371 +/- 106 minutes; B: 343 +/- 46 minutes). Two group A patients had postoperative complications (one had arrhythmia, and one died of sepsis caused by sutural insufficiency). On the other hand, four group B patients had complications (three cases of transient hyperbilirubinemia and one case of postoperative bleeding; none died). The postoperative hospital stay after gastrectomy was not prolonged in group B compared with group A (A: 41.7 +/- 22.7 days; B: 46.0 +/- 25.0 days). In conclusion, simultaneous procedure of coronary artery bypass grafting and gastric surgery can be performed safely, although careful management is indispensable.

Function of the sphincter of Oddi in patients with juxtapapillary duodenal diverticula: evaluation by intraoperative biliary manometry under a duodenal pressure load.

The purpose of this study was to elucidate the function of the sphincter of Oddi (SO) in patients with juxtapapillary duodenal diverticula (JDDs). The SO function was evaluated by intraoperative biliary manometry in three groups of patients. Group 1 consisted of nine patients with JDDs and a dilated common bile duct (CBD) (diameter > 10 mm). Group 2 consisted of six patients with JDDs and a normal-sized CBD (diameter < 10 mm). Group 3 consisted of 26 patients without JDDs and with normal-sized CBDs. In the absence of a duodenal pressure load, the patients in group 1 demonstrated a lower baseline SO pressure and lower resistance of the biliary outflow than patients in group 3. They also demonstrated a lower baseline SO pressure and shorter decay time (which represented terminal biliary ductal resistance) than patients in group 2. In the presence of a duodenal pressure load of 300 mm H2O, the patients in group 1 demonstrated a lower incidence of phasic SO contractions, a higher baseline SO pressure, and a higher resistance of the biliary outflow than group 2 and group 3 patients. The decay time in group 1 and group 2 patients was higher than that of group 3 patients. Based on these findings, we conclude that the SO function in patients with JDDs is impaired owing in part to long-standing compression of the terminal biliary ductal system by a distended JDD associated with a rise in intraduodenal pressure in daily life.

New method for measuring ICG Rmax with a clearance meter.

The maximal removal rate of indocyanine green (ICG Rmax) is considered to be an important parameter of hepatic function. However, the method of analysis has some flaws, and an abnormal value is obtained for about 15% of patients. We developed a new method of measuring the ICG Rmax with a clearance meter (RK-1000) that continuously measured the ICG concentration using a fingertip optical sensor. Twenty patients were examined. The histologic diagnosis was as follows: normal for 10, cirrhosis in 6, hepatitis in 4. The ICG concentration was measured in vivo continuously with the RK-1000. To obtain the Rmax by the Michaelis-Menten model, the ICG concentration in the VLDL compartment was subtracted from the values obtained by the RK-1000 because ICG binds to various serum proteins and its rate of removal in the VLDL compartment differs from that in other protein compartments. The removal velocity was calculated and a Michaelis plot obtained. Then Rmax was calculated from the reciprocal of the y-intercept of a Lineweaver-Burk plot. The Rmax in subjects with liver disease was significantly lower than in those with normal liver. It is concluded that our new method of measuring ICG Rmax with the RK-1000 reflects liver function appropriately.

Isolation and characterization of human liver cytochrome b5 cDNA.

Three cDNA clones encoding human cytochrome b5 have been isolated and sequenced from a lambda gt 11 cDNA library of a human liver. The largest clone (b5-C) contains 765 base pairs corresponding to a complete coding sequence of 134 amino acids and sequences for 5' non-coding (56 bases) and 3' non-coding regions (307 bases). In Northern blots, a band hybridizing to 32P-labelled b5-B cDNA clone was detected at around 12S in mRNA of human and rat livers. The deduced amino acid sequence was identical with the partial amino acid sequence (2-100th) previously reported, except that two amino acid replacements were observed between the 89th and 91st positions. The deduced amino acid sequence of human cytochrome b5 shares 88.0, 86.5, 86.5 and 74.4% similarity with those of rat, pig, horse and chicken, respectively. The sequences in the haem binding region were highly conserved among these species. In addition, high similarities in the hydrophobicity profiles were maintained throughout their entire regions, although some diversity was observed in the N-terminal sequences between human and chicken.

Cell reproduction cycle of mycoplasma.

The cell reproduction cycle of parasitic wall-free bacteria, mycoplasma, is reviewed. DNA replication of Mycoplasma capricolum starts at a fixed site neighboring the dnaA gene and proceeds to both directions after a short arrest in one direction. The initiation frequency fits to the slow speed of replication fork and DNA content is set constant. The replicated chromosomes migrate to one and three quarters of cell length before cell division to ensure delivery of the replicated DNA to daughter cells. The cell reproduction is based on binary fission but a branch is formed when DNA replication is inhibited. Mycoplasma pneumoniae has a terminal structure, designated as an attachment organelle, responsible for both host cell adhesion and gliding motility. Behavior of the organelle in a cell implies coupling of organelle formation to the cell reproduction cycle. Several proteins coded in three operons are delivered sequentially to a position neighboring the previous organelle and a nascent one is formed. One of the duplicated attachment organelles migrates to the opposite pole of the cell before cell division. It is becoming clear that mycoplasmas have specialized cell reproduction cycles adapted to the limited genome information and parasitic life.

Spontaneous nystagmus in normal subjects.

We investigated the incidence of spontaneous nystagmus in 30 normal volunteers using electronystagmography (ENG). In all subjects, ENG was performed with the eyes open in the primary position, with the eyes closed, and with the eyes open in total darkness. Spontaneous nystagmus was detected in 5 (16.7%) of 30 subjects. In 15 subjects who underwent 2 ENG examinations, 4 subjects (26.7%) exhibited spontaneous nystagmus. Caloric testing was performed in 26 of 30 subjects. Spontaneous nystagmus was detected in 3 subjects who had a right-left difference in caloric response of over 40%, and in 2 of 23 subjects who had a difference of less than 40%. Our results showed that spontaneous nystagmus was detected in more subjects when the ENG examinations were repeated. Thus, a single ENG examination cannot exclude the possibility of spontaneous nystagmus.

Segmental small bowel transplantation in the rat: comparison of lipid absorption between jejunal and ileal grafts.

BACKGROUND: From the immunological point of view, it is very important to evaluate the efficacy of segmental small bowel transplantation and to determine which part of the intestine, jejunum, or ileum should be used. In the present study, we investigated the absorptive function of the transplanted jejunum and ileum in the rat. METHODS: Syngeneic segmental small bowel transplantation (jejunal or ileal grafting) was performed. After surgery, body weight and fecal fat excretions were measured. In addition, bile acid concentration in bile juice was measured, and the response of both serum lipase and bile flow rate after oleic acid stimulation was evaluated. Recanalization of the lymphatic vessels was investigated by lymphangiography. RESULTS: There was no significant difference in body weight change between normal controls, jejunum-transplanted rats, and ileum-transplanted rats. In short gut rats, however, body weight was significantly impaired. Fecal fat excretion in short gut rats was the highest in the four groups, and it was significantly lower in ileal grafts than in jejunal grafts. Ileum-transplanted rats also showed a significantly higher bile acid concentration in bile juice than jejunum-transplanted rats. After oleic acid stimulation, serum lipase responded better in ileal transplants than in jejunal transplants, but the bile flow rate did not show significant change in either group. Recanalization of the lymphatic vessels was established on the 28th postoperative day. CONCLUSIONS: These results clearly show that ileal transplantation is more conducive to lipid absorption than jejunal transplantation.

Enzyme immunoassay for IgG rheumatoid factor combining with homologous IgG.

An enzyme immunoassay (EIA) was developed to detect, in human sera, IgG rheumatoid factor (RF) combining with human IgG. Wells of the EIA plate were coated with the Fc fragment of human IgG. Binding of RF was detected by goat antibodies to F(ab')2 of human IgG followed by rabbit anti-goat IgG conjugated with alkaline phosphatase and finally by the substrate. Using this procedure, RF was detected in 35%-85% of various pathological human sera, but only in 7% of normal human sera. An analogous procedure was also described for detection, in rabbit sera, of an RF combining with rabbit IgG.

The lactate dehydrogenase isoenzyme pattern in the overacting inferior oblique muscle.

The lactate dehydrogenase (LDH) isoenzyme pattern in normal eye muscles of fetuses, children, and adults in overacting inferior oblique muscles of children was studied and compared. No significant differences were found between LDH isoenzyme activity in the horizontal and vertical recti. Isoenzyme fractions, by percentage, were distributed in these muscles in the order of III greater than IV greater than II greater than V greater than I. A significant difference, however, was noted between the LDH isoenzyme patterns of the inferior oblique muscle in normal adults and children. The percentages were in the orders of III greater than II greater than IV greater than V greater than I, and III greater than IV greater than II greater than V greater than I, respectively. The isoenzyme pattern in the overacting inferior oblique muscles of children was in the order of III greather than II greater than IV greater than V greater than I, identical to that of the adult control group. In addition, the overacting inferior oblique muscle of the children contained more H than M subunit than did the rectus muscle or the normal inferior oblique muscle of children by comparison.

Budd-Chiari syndrome treated by Senning's procedure.

Two patients with Budd-Chiari syndrome who underwent a new surgical procedure developed by Senning are reported. A 33-year-old man was diagnosed as having Budd-Chiari syndrome with a membranous obstruction of the inferior vena cava (IVC) and right hepatic vein and short segmental obstruction of the left hepatic vein. Removal of the obstruction by dorsocranial resection of the liver and reconstruction of the veins by hepatoatrial anastomosis was carried out. In a 53-year-old female, the same procedure was carried out for a short segmental obstruction of the IVC and left hepatic vein. In both patients, postoperative examination revealed good patency of the IVC and the hepatic veins and increased portal venous flow as measured by Doppler-echography. This procedure is considered the method of choice for Budd-Chiari syndrome with membranous and/or short segmental obstruction of the IVC and hepatic veins.

Vertebral aneurysm of the neck.

We recently encountered a case of a huge vertebral aneurysm involving the entire right neck, extending from the clavicle to the mandibular area. The patient had undergone laminectomy of the cervical spine at age 45 for neuritis or spinal caries. This operative procedure apparently caused a small vertebral laceration and produced the giant aneurysm. The aneurysm was resected under hypothermia in order to protect the brain. One vertebral artery was ligated, but cerebral damage did not occur. Only a few cases of huge aneurysms extending from the clavicle to the mandible have been reported in the literature.

Reconstruction of portal vein with an autograft of splenic vein.

To re-establish the portal circulation following extensive resection of the portal vein, we interposed an autograft of the splenic vein between the portal and superior mesenteric veins during total pancreatectomy in three patients with cancer of the pancreas. The postoperative course in two patients was uneventful, and patency of the graft was demonstrated angiographically on the 41st and 78th postoperative days, respectively. In the remaining patient an episode of postoperative peritonitis occurred with leakage of the gastrointestinal anastomosis. The patient died on the 78th postoperative day from a massive metastasis to the liver. Autopsy revealed a narrowing of the graft due to technical failure and inflammatory changes, but there was no evidence of cancer invasion. Thus, the autograft of the splenic vein proved useful to bridge the portal vein system.

Lymphangioma of the mesentery and small intestine: a case report showing a solid tumor with a cystic component on US and CT.

We encountered a patient with multiple lymphangiomas of the mesentery and small intestine. Ultrasonography and computed tomography showed a mesenteric tumor composed of a solid component with a small cystic part. This unusual appearance made the differential diagnosis between lymphangioma and other intra-abdominal solid tumors difficult.
