[Studies on the identification of sulfadimidine in pork by high performance liquid chromatography with photodiode array detector and gas chromatograph-mass spectrometry].

Sulfadimidine (SDD) was detected in pork imported from Mexico by HPLC with gradient elution system. UV spectrum of the peak was measured and confirmed by photodiode array detector, moreover identified by GC/MS after methylation with diazomethane. Content of SDD in the sample was 0.1 ppm and detection limit of SDD by the proposed method was 0.02 ppm. Application of this method to CLP, SMR, SMM, SDM and SQX was studied, and satisfactory results were obtained.

Aneurysm of the right gastroepiploic artery: a case report of laparoscopic resection.

A traumatic pseudoaneurysm of the right gastroepiploic artery in a 55-year-old woman was resected using laparoscopy. The durations of anaesthesia and operation were 260 and 220 min respectively. Blood loss was minimal. The perioperative period was uneventful; the patient needed no analgesic treatment and ate her first meal on the morning of the day after surgery. She was discharged 5 days later and has progressed well for 5 months since surgery without any abnormal findings.

Rheumatoid factor: a review of the mode of formation and role in transplantation.

The mode of formation and role of rheumatoid factors (RF) in transplantation were reviewed. RF might be produced through stimulation with altered transplantation antibody, which undergoes conformational changes resulting from the reaction with a graft. Some recipients might have such sources of infection as cytomegalovirus or human herpes virus 6 (HHV-6), because almost all patients were treated with immunosuppressive agents. Many bacteria and viruses have IgG Fc receptors on their surface. Herpes virus infection, such as herpes simplex virus, has been demonstrated to be capable of expressing Fc receptor on the infected cells. These Fc receptors could produce RF through the response of antibody production to anti-idiotype of Fc receptors. Also, IgG, which binds to Fc receptors, changes conformationally to produce RF. RF can also be produced as a result of graft versus host reaction. Although IgG RF reacting with homologous IgG is believed to have the most important role in pathogenic conditions, they have not been studied because of difficulties in their measurements. We developed an enzyme immunoassay for detection of IgG RF combining with homologous IgG. The wells of an assay plate were coated with Fc fragment of human IgG, and after incubation with tested serum, binding of IgG RF to the Fc was detected by means of goat anti-human IgG F (ab')2 antibodies followed by alkaline phosphatase conjugated rabbit anti-goat IgG and substrate. IgG RF were increased significantly more in transplantation sera of patients who rejected grafts than in those of patients who had satisfactory graft function. Some evidence for the role IgG RF in rejection, even if only circumstantial, has been provided.

Histopathological study of congenitally diabetic yellow KK mouse lens.

Lenses of Yellow KK mice with marked obesity and diabetic symptoms were examined by light and electron microscopy. At 2 months of age, the entire lens appeared normal. At 4 months, however, epithelial cells sometimes had become necrotic and showed intranuclear inclusions. At 6 months, the anterior cortical fibers were swollen. At 8 months, the number of intranuclear inclusions of the epithelial cells had increased, and the posterior cortex contained densely stained cells among the fibers, with swollen cytoplasm. At 10 months, these cells had become atrophic. At 12 months, the lens cells in the posterior sutural area showed marked swelling, and the suture was almost separated. This animal model showed a slow progressive cataract in the superficial cortical zone which is similar to the diabetic cataract in human eyes.

Aggravation of experimental allergic conjunctivitis by environmental chemical and physical factors.

The effects of trichlorfon (DEP, Dipterex, anticholinesterase pesticide) and paraquat dichloride (Gramoxon, inhibitor of superoxide dismutase) on passive anaphylactic reaction in guinea pig conjunctiva using Japanese cedar pollen were quantitatively studied. For estimation of allergic conjunctivitis, Evans blue after i.v. injection was extracted from conjunctiva and measured spectrophotometrically. Allergic conjunctivitis was apparently aggravated by extremely low dosages of organophosphorus pesticide (10(-5) mg/kg) and organochlorine herbicide (10(-4) mg/kg). The aggravation of allergic conjunctivitis was also observed after exposure to cathode ray tubes used in commercial television, possibly due to electromagnetic waves. IgE-mediated allergic reaction could be non-specifically potentiated by such environmental factors.

[E-cadherin expression and cancer invasion and metastasis].

E-cadherin (ECD) is a transmembrane glycoprotein that mediates the calcium dependent adhesion of cells and plays a major role in cell-cell adhesion of normal epithelium. In present study, we have established the positive ECD expressed clone (ECD(+)) and the negative ECD expressed clone (ECD(-)). Anti human ECD monoclonal antibody (HECD-1) disturbed the mutual adhesion of ECD(+) effectively but didn't disturb that of ECD(-). ECD(+) did not invade the collagen/fibroblast gel, but ECD(-) did. Furthermore, ECD(+) acquired invasiveness in the collagen gel when HECD-1 was used. Clinicopathologically, we have investigated the correlation between expression of ECD and metastasis in human esophageal and gastric cancers by immunohistochemical staining using HECD-1. ECD expression of these lymph node metastatic tumor in the primary sites was more reduced than that of non metastatic tumor significantly. These results indicated that reduction or dysfunction of ECD diminishes intercellular adhesion and acquires invasive and metastatic capacity in human cancer cells.

[Clinical and pathological study of lupus nephritis without clinical renal involvement].

To analyze the clinical and pathological changes in systemic lupus erythematosus (SLE) patients without clinical renal abnormalities, I performed histopathological examinations of renal biopsy specimens from 40 SLE patients, who were divided into two groups. Group A consisting of 16 patients had almost no clinical evidence of renal involvement. Group B consisting of 24 patients had clinical renal manifestations, proteinuria, hematuria or both due to lupus nephritis. Light microscopy showed minor to moderate proliferative changes in the patients of Group A, and severer changes in the patients of Group B. Immunofluorescent study revealed depositions of immunoglobulins and complement in varying degrees in all the patients of Groups A and B. By electron microscopic study, electron-dense deposits and mesangial deposits were found in all patients of Group A. However, subendothelial, subepithelial and intramembranous deposits were seen in less than one-half of the patients of Group A. The amounts of the deposits were small. By contrast, a large amount of electron-dense deposits existed in the patients of Group B. The prognosis was better for patients of Group A than those of Group B. I concluded that various abnormalities of the renal glomeruli exist in SLE patients even without urinary abnormalities.

[Participation of environmental chemicals in experimental allergic conjunctivitis].

The effects of organophosphorus pesticides (trichlorfon and fenitrothion) and an organochlorine herbicide (paraquat) on experimental allergic conjunctivitis were studied quantitatively. Guinea pigs were passively immunized with antiserum to Japan cedar pollen. The above chemicals were administered subcutaneously 2 days prior to challenge with cedar pollen in the conjunctival sac, resulting in allergic conjunctivitis. The intensity of the conjunctival allergic reaction was quantitatively estimated by measuring the extravasation of Evans blue in the conjunctiva which had previously been administered intravenously. The allergic conjunctivitis was apparently aggravated by extremely low dosage level of organophosphorus pesticide (10(-4) approximately 10(-5) mg/kg) and organochlorine herbicide (10(-3) approximately 10(-4) mg/kg). The most intensive aggravating reaction was obtained 10(-5) mg/kg of organophosphorus pesticide and at 10(-4) mg/kg of paraquat dichloride. The aggravating effects of organophosphorus pesticide and organochlorine herbicide persisted at least two weeks after the single administration.

[A case of Sjögren's syndrome accompanied by lymphadenopathy and IgG4 hypergammaglobulinemia].

A 78-year-old male patient suffered from slight dryness of his mouth and eyes, which was followed by swelling of the parotid and submandibular glands on both sides. Subsequently, he developed generalized lymphadenopathy accompanied by hypergammaglobulinemia IgG 7740 mg/dl, main subclass: IgG4, 5800 mg/dl. Histopathological examination of lymph nodes from his left neck showed follicular proliferation of plasma cells containing cytoplasmic kappa and lambda chains of IgG4, without destruction of the lymph node structure. His serum showed very low levels of complement: 0 U/ml of CH 50, 14 mg/ml of C3 and less than 11 mg/ml of C4. During treatment with prednisolone at 30 mg/day, the swelling of the salivary glands and lymph nodes reduced the IgG and IgG4 decreased and the complement increased. Finally, all lymph nodes, IgG and complement abnormalities were normalized. Anticomplementariness of IgG4 was demonstrated in this patient.

[Evaluation of lidocaine metabolite (monoethylglycinexylidide) as a liver function test].

In order to determine whether or not the lidocaine metabolism, monoethylglycinexylidide (MEGX) formation, could be used as a liver function test, we measured the serum levels of MEGX in 38 patients. There were significant correlations between values of MEGX (MEGX15, MEGX30, AUC15-30, AUC0-180) and conventional liver function tests (ICG R15, AT III, T. Bil). It appeared that value of MEGX 30 had maximum factor loading on conventional liver function tests by using principal component analysis. The advantage of adapting the MEGX formation as a liver function test of drug metabolism is simplicity of the method. MEGX formation could be useful index of the total liver function.

Reconstruction of the thoracic esophagus, with extended jejunum used as a substitute, with the aid of microvascular anastomosis.

BACKGROUND: After radical ablation of tumors, colon or jejunum is pulled up for reconstruction of the thoracic esophagus when stomach is unusable. However, esophagocolonic or esophagojejunal anastomotic leak is relatively frequent because of the potential for vascular insufficiency of conduit extended upward. METHODS: To overcome this disadvantage, 14 consecutive patients with esophageal or gastric carcinomas whose stomachs were unusable were subjected to thoracoesophageal reconstruction through upward extension of the jejunum with microvascular anastomosis. Jejunal vessels were used as graft vessels, and internal thoracic vessels were used as recipient vessels. If needed, arcade vessels were severed to ease extension. RESULTS: All extended jejunums survived, and there were no operative deaths. Two of four initial series of patients, whose arcade vessels were not severed despite undue tension at the anastomotic site, had esophagojejunal anastomotic leaks that healed spontaneously. CONCLUSIONS: Some clinical functional problems still exist, but we believe this a safe and useful method for reconstruction of the esophagus where stomach cannot be used.

Sulfite-containing Canadian pharmaceutical products available in 1991.

OBJECTIVE: To compile an inclusive list of Canadian pharmaceutical products available in 1991 that contained sulfites. DATA SOURCES: Written and oral responses from 94 pharmaceutical companies selected from the 1989 Compendium of Pharmaceuticals and Specialties. RESULTS: A list of sulfite-containing pharmaceutical products was compiled from data supplied by the 90 responding companies. Companies whose products contained no sulfites were separately identified. CONCLUSIONS: Sulfites are present in many pharmaceutical products and are one of many excipients and additives that have been reported to cause severe adverse reactions. The provided list should be a useful aid for health care practitioners when prescribing pharmaceutical products for sulfite-sensitive patients.

Primary malignant lymphoma of the spleen.

We treated two patients with primary splenic malignant lymphoma. One was a 63-year-old man with diffuse histiocytic non-Hodgkin's lymphoma accompanied by multiple liver metastases which were composed of necrotic tissue probably due to preoperative transarterial chemoembolization (TAE). He eventually died of liver failure two years and six months after splenectomy. The autopsy revealed that a large part of the cirrhotic liver had been occupied by a diffuse-type hepatocellular carcinoma, but no recurrence of the malignant lymphoma was found in the liver or other organs. The second patient was a 40-year-old woman with a massive invasion of the stomach, colon, pancreas, and diaphragm by a splenic tumor. The splenic tumor and the adjacent involved organs were resected. Pathologically, well-differentiated diffuse lymphocytic non-Hodgkin's malignant lymphoma was evident. No recurrence has been found for six years and two months. Based on an evaluation of the 71 patients with primary splenic malignant lymphoma reported to data in Japan, the patients treated by a curative resection in an early clinical stage have a more favorable prognosis.
