Early intervention for very-low-birth-weight infants.

To assess the efficacy of early intervention (EI) for very-low-birth-weight (VLBW) infants, we evaluated 62 2 year old children who were enrolled in an EI program and 48 control subjects aged 2 years. We determined the subjects' developmental quotients (DQ) and obtained information about the parents' evaluation of the children from a questionnaire sent to the parents. There was no significant difference in the DQ between the EI group and the control group. However, based on the responses to the questionnaire, subjects in the EI group showed slight, but statistically marginally significant, improvements in behavioral problems, especially a decrease in hyperkinesia, in adjusting to a circadian sleep cycle, and an improvement in language development, as compared with the control group (P < 0.1). Thus, EI for VLBW infants is considered useful to enhance some areas of development.

Clinical and electroencephalographic findings in juvenile type DRPLA.

We present five different types of dentatorubral-pallidoluysian atrophy in one Japanese family. Two siblings and their paternal uncle manifested the juvenile type dentatorubral-pallidoluysian atrophy, the siblings' father had the late-adult type, and another paternal uncle had the early-adult type. Gene analysis confirmed the diagnosis for the proband and her sibling. By following the clinical courses and electroencephalographic changes, we found that the types of epileptic seizures and the electroencephalograms of the juvenile dentatorubral-pallidoluysian atrophy patients changed as the illness progressed. The siblings exhibited different levels of clinical severity despite the similar deoxyribonucleic acid expansion detected in their lymphocytes.

Evolutionary origin of two genes for chloroplast small heat shock protein of tobacco.

Two different cDNA clones for the chloroplast small heat shock protein (smHSP) were isolated from tobacco (Nicotiana tabacum cv. Petit Havana SR1). One of the cDNAs (type I) has a full-length open reading frame (ORF) of the smHSP of 26.6 kDa. By contrast, the other one (type II) contains an additional nucleotide that causes the frame shift inside a putative ORF for the smHSP. If this nucleotide is neglected, type II cDNA encodes the smHSP that is 89% identical to that encoded by type I cDNA. Southern blot and polymerase chain reaction (PCR) analyses with genomic DNA indicated that tobacco has two different smHSP genes while two ancestors of tobacco, N. sylvestris and N. tomentosiformis, have a single gene that each corresponds to one of the two genes of tobacco. It was also found that one of the tobacco genes has an ORF for the smHSP disrupted by nucleotide insertion in the same way as type II cDNA, while both ancestor genes have a functional ORF. These results suggest that the two smHSP genes of tobacco had been derived from the two ancestor species, and that one of the two genes had been disrupted by nucleotide insertion during the course of the evolution of tobacco. Northern blot and reverse transcription (RT)-PCR analyses demonstrated that both the tobacco genes are expressed upon heat stress, exhibiting different dependences on temperature.

[Renal tubular acidosis type II secondary to gamma-light chain excretion in an elderly patient with multiple myeloma].

A 73-year-old woman was admitted to the geriatric ward of the University of Tokyo Hospital with anemia, osteepeina, and renal dysfunction. Although symptoms typical of multiple myeloma such as punched-out lesions and hyperproteinemia were not found, protein electrophoresis revealed that lambda type Bence-Jones protein was excreted in urine. Multiple myeloma was diagnosed. Furthermore, renal dysfunction was accompanied renal tubular acidosis type II (proximal type). Renal dysfunction in patient with multiple myeloma in usually caused by so-called myeloma casts in the distal tubules, but renal tubular acidosis type II is rarely observed. It is possible that injury of the proximal renal tubular eithelium by Bence-Jones protein resulted in renal tubular acidosis type II in this patient.

[Tuberculosis statistics among the homeless population in Nagoya City from 1991 to 1995].

A survey was conducted to clarify the tuberculosis (TB) situation among the homeless during the period from 1991 to 1995 in Nagoya city, using 5,222 registration cards of TB cases registered at one of Nagoya City's 16 Health Centers. Out of 5,222 TB cases, there were 269 homeless cases (267 male and 2 female). Ninety-seven percent of them were pulmonary TB cases. The incidence and prevalence rates of TB per 100,000 among the homeless were estimated at around 1,500 and around 2,400, respectively, around 20 times higher than those of the non-homeless male over the 19 years of age. A decrease in the incidence rate of TB cases among the homeless was not seen, although the rate among the non-homeless decreased gradually. The percentage of infectious (bacillary and/or cavitary) tuberculous cases among the homeless was higher than in the non-homeless. In the infectious cases, the percentage of smear-positive bacillary cases or far advanced cavitary cases was 52.1% or 9.4% among the homeless compared to 48.1% or 2.6% among the non-homeless, respectively. The detection rate by chest X-ray examination of the homeless was 3.9%.

Cloning of a cDNA that encodes farnesyl diphosphate synthase and the blue-light-induced expression of the corresponding gene in the leaves of rice plants.

A cDNA encoding farnesyl diphosphate synthase (FPPS), a key enzyme in isoprenoid biosynthesis, was isolated from a cDNA library constructed from mRNA that had been prepared from etiolated rice (Oriza sativa L. variety Nipponbare) seedlings after three hours of illumination by a subtraction method. The putative polypeptide deduced from the 1289 bp nucleotide sequence consisted of 353 amino acids and had a molecular mass of 40 676 Da. The predicted amino acid sequence exhibited high homology to those of FPPS from Arabidopsis (73% to type 1, 72% to type 2) and white lupin (74%). Southern blot analysis showed that the rice genome might contain only one gene for FPPS. The highest level of expression of the gene was demonstrated in leaves by RNA blot analysis. Moreover, light, in particular blue light, effectively enhanced expression of the gene.

Cloning of cDNA encoding the rice 22 kDa protein of Photosystem II (PSII-S) and analysis of light-induced expression of the gene.

Cloning of rice cDNA encoding the chlorophyll-binding 22 kDa protein of Photosystem II (PSII-S) and the light-induced expression of the gene are reported. One of the light-responsive cDNA clones, isolated by screening with a light-specific subtracted cDNA probe, was shown to encode PSII-S of rice. Genomic Southern analysis suggested that the PSII-S gene, psbS, is a single-copy gene in rice. A brief exposure to red light induced a severalfold increase in the steady state level of PSII-S transcripts in etiolated seedlings. The red light effect was reversed by far-red light, suggesting involvement of phytochrome in the PSII-S gene regulation. Prolonged exposure (3 h) to blue light, however, revealed a much stronger effect than red light on the accumulation of PSII-S transcripts in the etiolated seedlings. In dark-adapted green plants, prolonged exposure to blue light induced re-accumulation of transcripts encoding PSII-S, whereas red light had little effect.

Longitudinal study on factors related to the course of vibration-induced white finger.

Follow-up surveys were conducted in 1982 and 1988 to investigate factors affecting the course of vibration-induced white finger (VWF). Subjects were 353 patients, aged 40 to 70 at the start of the 1982 survey, who were receiving treatment for hand-arm vibration syndrome. Between 1982 and 1988, the attacks of VWF decreased, while numbness and pain in the hand changed slightly. Finger skin temperature showed a tendency to increase, but recovery time in nail compression tests was unchanged. Vibration perception threshold and grasping power tended to become worse. Of the 177 patients with VWF in 1982, 55 (31%) had no VWF in 1988. The improvement in VWF depended on its severity assessed by the frequency of attacks, the extent of the affected finger phalanges and the Stockholm vascular (V) stage at the start of the 1982 survey. The improvement was observed in 46% of the 1V (mild) stage cases, against only 17% of the 3V (severe) stage cases. Patients in the 2V (moderate) and 3V stages had lower finger skin temperature than those without VWF (the 0V stage). Patients with milder VWF seemed more likely to improve. Continued use of vibratory tools was found to be an unfavourable factor for improvement of VWF. Age, smoking and drinking habits, and medical complications showed no significant effects on the course of VWF.

Blink induced centrotemporal spikes in benign childhood epilepsy with centrotemporal spikes.

A 10 year old girl with benign childhood epilepsy with centrotemporal spikes showed centrotemporal spikes induced by blinking even in a dark room. Spikes could not be induced by photic stimulation, eye closure, eye movement, eye deviation, or passive blinks. There have been no previous reports of spikes induced by blinks in benign childhood epilepsy with centrotemporal spikes.

Antibiotic sensitivity of recent clinical isolates from patients with ocular infections.

We examined the status of bacterial infection and drug sensitivity in patients who consulted the Ocular Infection Clinic of Niigata University between 1992 and 1995. Overall, 850 strains were detected. Gram-positive cocci were the most frequent (380 strains, 44.7%). We detected 63 strains of gram-negative bacilli (7.4%), 207 strains of anaerobic bacteria (24.4%) and 18 strains of fungi (2.1%). Among gram-positive cocci, coagulase-negative staphylococci (CNS) were the most prevalent (52.6%). Of gram-negative bacilli, glucose-nonfermenting gram-negative rods (GNF-GNR) were the most prevalent (27.0%). The detection rate of methicillin-resistant Staphylococcus aureus (MRSA) was 27.0% (20/74 strains). The detection rate of methicillin-resistant CNS was 6.8% (19/147 strains). Concerning drug sensitivity, most of the 54 strains of methicillin-sensitive S. aureus showed low sensitivity to penicillin G (PCG), ampicillin (ABPC) and erythromycin (EM). Most strains of MRSA showed low sensitivity to PCG, ABPC, cefazolin, flomoxef, imipenem (IPM), ofloxacin (OFLX), EM and clindamycin (CLDM), but high sensitivity to netilmicin, arbekacin (ABK), minocycline (MINO) and vancomycin. The drug sensitivity of CNS was similar to that of S. aureus. Of 27 strains of Streptococcus pneumoniae, 4 (14.8%) were resistant to PCG. Five strains of Pseudomonas aeruginosa showed high sensitivity to piperacillin, ceftazidime, IPM, tobramycin, ABK and OFLX. Most strains of other GNF-GNR showed high sensitivity to IPM, MINO and OFLX. Most strains of anaerobic bacteria showed high sensitivity to IPM, MINO and CLDM.

Ergonomic evaluation of working posture of VDT operation using personal computer with flat panel display.

The aim of this study was to evaluate working conditions using a personal computer with a flat panel display (FPD) in terms of visual and musculoskeletal comfort. Measurements of viewing distance, viewing angle, head angle, neck angle and electromyogram (EMG) activities of the neck, shoulder and back muscles of visual display terminal (VDT) operators were compared at workstations using an FPD and a desktop personal computer (DPC). A notebook personal computer (NPC) with a 10.4 inches FPD, was used in this experiment. Each of 10 healthy subjects performed word processing tasks using both NPC and DPC workstations. Significant differences in the work posture while using the NPC and DPC were seen with viewing distance, viewing angle and head angle. The characteristic features of the work posture using the NPC were a remarkably short viewing distance and a forward inclination of the head. The value of integrated EMG (IEMG) of the neck muscle was greater while using the NPC than when using the DPC. These phenomena were caused by the structure of NPCs; most NPCs have a display and keyboard which cannot be separated and are not adjustable without tilt and swivel mechanisms. VDT devices that cannot be adjusted may potentially make operators assume a poor posture while working, which could cause visual and musculoskeletal disorders. To prevent visual and musculoskeletal problems using NPCs, a more upright head and neck position was recommended. The addition of a mechanism for adjustment of the height of an NPC display would allow a comfortable downward gaze without the loss of correct posture.

[Lipoprotein(a) as a risk factor for cardiovascular disease in elderly patients with diabetes].

Lipoprotein(a) (Lp(a)) is an independent risk factor for cardiovascular diseases in non-diabetic people, but few studies have been done in diabetic patients. To investigate whether Lp(a) is a risk factor for cardiovascular disease in elderly people with diabetes, we examined the association of Lp(a) and serum lipid levels (total cholesterol: TC; triglycerides: TG; and high-density lipoprotein cholesterol: HDL-c) with the incidence of coronary artery disease and cerebrovascular disease. We studied 354 outpatients(131 men and 223 women, 60-97 years of age) with non-insulin-dependent diabetes mellitus. The mean concentration of Lp(a) was 21.1 +/- 19.6 mg/dl and the median was 14.0 mg/dl. The Lp(a) concentration did not correlate significantly with age or with sex, but it did correlate significantly with TC (r = 0.152, p < 0.05) and with the level of apoprotein B (r = 0.168, p < 0.05). The incidence of cerebrovascular disease was significantly higher in patients with high concentrations of Lp(a) (> or = 30 mg/ dl) than in those with low concentrations (< 30 mg/ dl). Multivariate logistic regression analysis revealed that male sex, hypertension, a high level of HbA1c, a low level of HDL, and a high level of Lp(a) were independent risk factors for cerebrovascular disease. The incidence of coronary artery disease tended to the higher in those with high concentrations of Lp(a) (> or = 30 mg/dl). However, multivariate logistic regression analysis revealed no significant correlation between Lp(a) concentration and the incidence of coronary artery disease. We conclude that a high concentration of Lp(a) is an independent risk factor for cerebrovascular disease in elderly patients with diabetes.

[Two cases of papillary adenocarcinoma of the thyroid gland associated with chylothorax].

Case 1. An 85-year-old woman had a papillary adenocarcinoma of the thyroid gland and a pleural effusion. The pleural effusion appeared to be a chylous exudate and it did not re-accumulate after thoracenthesis. Thoracic imaging indicated that the chylothorax was caused by direct invasion of the thoracic duct by the thyroid carcinoma. Case 2. A 53-year-old woman had a 20-year history of recurrent chylothorax. She died due to sepsis one year after the third admission for dyspnea and chylothorax. The autopsy findings included papillary adenocarcinoma of the thyroid gland with metastasis to the left supraclavicular lymph nodes. The thoracic duct was inflamed, fibrotic, and completely obstructed. Invasion by the carcinoma may have compressed and destroyed the thoracic duct, and caused chylothorax. Recurrent inflammatory granulation caused total obstruction of the thoracic duct. Reports of chylothorax associated with carcinoma of the thyroid gland are rare.

Digital sensory nerve conduction velocity and vibration perception threshold in peripheral neurological test for hand-arm vibration syndrome.

To evaluate peripheral neuropathy in patients with vibration syndrome, an examination was conducted of sensory nerve conduction velocity (SCV) in the digital segment of the median nerve in the middle finger and vibration perception threshold (VPT) at 125 Hz on the same middle fingertip. In addition, possible correlations were investigated between the two measurements. SCVs in the digital segment were measured by stimulating at the wrist electrically and recording from two pairs of electrodes in the finger. Fractionated SCVs were also measured in the palm-to-finger, wrist-to-palm, and elbow-to-wrist segments. The subjects were 52 patients with vibration syndrome and 40 healthy controls of similar age. SCVs in the digital segment and the wrist-to-palm segment were significantly slower in the patients than in the controls, and VPTs were higher in the patients. The strongest correlation of VPTs with SCVs among nerve segments measured was shown in the digital segment. With an increase in VPTs, SCVs in the digital segment tended to be slower, and slowed digital SCVs were encountered more frequently: 13% in VPTs below 5.0 dB and 56% in VPTs above 17.5 dB. Slowed digital SCVs were found in 43% of the patients and increased VPTs were encountered in 92%.
