Ultrastructural alterations of glomerular anionic sites in idiopathic membranous glomerulonephritis.

The relation of ultrastructural alterations of glomerular anionic sites to morphological changes of the glomerular basement membrane (GBM) was studied in biopsy specimens from patients with idiopathic membranous glomerulonephritis (MGN) using polyethyleneimine (PEI) as a cationic probe. The subepithelial deposits did not fix PEI, and only a very few anionic sites were found between subepithelial deposits and adjacent epithelial cells. Thus, anionic sites of the lamina rara externa (LRE) were markedly decreased in number and interrupted by subepithelial deposits in the earlier stages (stage I and II). As the deposits were incorporated into the GBM in the later stage (stage III), the anionic sites of the LRE were restored on the epithelial cell surface coat (ESC) covering the deposits, but foot process effacement persisted during the course of the disease. The anionic sites of the lamina rara interna (LRI) appeared much less altered in all stages of MGN. These findings suggest that subepithelial deposition of immune reactants is associated with focal loss and disorganization of anionic sites on the GBM, resulting in a defect of glomerular-barrier function.

[Clinical studies on cefixime in pediatrics].

A clinical study of cefixime (CFIX), a new oral cephalosporin, was carried out to evaluate its therapeutic effectiveness on bacterial infections in children. CFIX was orally administered to 13 patients including 6 with upper respiratory tract infection (RTI), 3 with pneumonia, and 1 each with bronchitis, otitis media, skin abscess, and urinary tract infection (UTI). The daily dosage per kg bodyweight ranged from 5.1 to 17.4 mg (average: 8.7 mg), and was given in 2 or 3 divided doses per day for 3 to 10 days (average: 5.8 days). The clinical response was excellent in 4 (30.8%), good in 7 (53.8%) and poor in 2 (15.4%), with an overall efficacy rate of 84.6%. Bacteriological efficacy was good, and 6 of the 8 identified causative organisms were eradicated. Side effects were observed in 3 children, i.e., loose stool in 1 and transient elevations of GOT and GPT in 2. The above results suggest that CFIX is a useful new oral cephalosporin for the treatment of bacterial infections in children.

[Combination chemotherapy with MOPP and intrathecal methotrexate in children with brain tumors].

Five children with brain tumors, who had had no prior chemotherapy, were treated with combination chemotherapy including nitrogen mustard, vincristine sulfate, procarbazine, prednisolone (MOPP) and intrathecal methotrexate. Four had progressive tumor or metastatic deposits, (one medulloblastoma, one malignant choroid plexus papilloma, and two were not explored) but responded to chemotherapy. One child with medulloblastoma underwent resection, which was considered to be macroscopically radical, and demonstrated objective improvement of neurological findings after chemotherapy. Toxicities were not severe enough to require termination of the chemotherapy.

[BAEPs of children with malignant tumor undergoing cisplatin treatment].

The effect of cisplatin on the auditory nervous system was examined in five children using brainstem auditory evoked potentials (BAEPs). The cases comprized of two neuroblastoma, one yolk sac tumor one neurofibrosarcoma and one Hodgkin's disease. All patients except for one infant aged eight months showed a normal audiogram in the pre-treatment examination. Cisplatin was administered at a dose of 75-105 mg/m2 (body surface area). BAEP tests were performed after cisplatin treatment from two weeks to 20 months. The patients were tested at rest in bed using stimulation with a 3 KHz, 80-dBHL click at 150-msec intervals 2,000 times. Four cases showed an abnormal BAEP pattern in the post-treatment examination. Two of them showed not only delayed conduction velocity of the first wave but also auditory disturbance, but these findings were improved after discontinuation of drug administration. We concluded that cisplatin frequently affects the auditory nervous system, and that this disturbance might be transient in the early stage.
