Forty-two shoulders in 40 patients operated on for anterior instability between 1986 and 1991 were reexamined. The mean follow-up was 47 months. The pathoanatomic findings at surgery were a Bankart lesion in 42 shoulders, a Hill-Sachs lesion in 31 shoulders, and a rounded or defective anterior glenoid rim in 29 shoulders. The surgical technique consisted of an open reinsertion of the anterior capsule-labral complex. Four shoulders had one or more recurrent anterior dislocations, and four shoulders had recurrent anterior subluxations. Three redislocations were caused by severe trauma. The presence and magnitude of a Hill-Sachs lesion did not influence the frequency of recurrence. An osseous defect of the glenoid rim > or = 3 mm was found in three patients, one of whom had redislocation after surgery. In patients with a large Bankart lesion the rate of recurrence was significantly higher.
PURPOSE: To investigate the relationship of demographic asthma, family, and child factors with self-concept in children with asthma. METHODS: Data were collected twice approximately 4 years apart from both the affected children and their mothers (N = 134) via interviews and self-report questionnaire. FINDINGS: Children who demonstrated more negative attitudes toward their illness, had less satisfaction with family relationships, and used more negative coping behaviors had the poorest self-concepts. Over time, the greatest improvement in self-concept occurred in children whose attitudes and satisfaction with family relationships improved and whose use of negative coping behaviors decreased. CONCLUSIONS: Results suggest that some children with asthma, especially girls with severe asthma, appear to be at risk for poor self-concept.
Merkel cells are neurosecretory cells of the skin with epithelial features such as desmosomes and expression of keratins 8, 18, 19, and 20. Merkel cells are scarcely distributed in adult human skin. Although they are present in hair follicles, their density is higher at hairless anatomic sites such as palms and soles. These cells are often innervated by sensory nerve fibers and are thought to be specialized mechanosensory skin receptor cells. However, their precise origin and function are not clearly established. The aim of this study was to localize Merkel cells in human hairless and hairy skin by immunohistochemistry with antibodies Ks18.174 and Ks19.1 directed against keratins 18 and 19, respectively. In glabrous skin of palm and sole, Merkel cells have been localized at the bottom of the rete ridges, in the epidermal basal layer. To study Merkel cell distribution at hairy anatomic sites, we have chosen breast skin, a tissue containing small hair follicles typical of those covering most of the body's surface. Merkel cells were present in the interfollicular epidermis. In hair follicles, they have been identified in the isthmus region.
The hypermetabolic state observed in thermally injured patients warrants aggressive nutritional management. Enteral support is the preferred route of nutrient delivery, however diarrhea is reported to be a persistent complication of continuous nasogastric or nasoduodenal hyperalimentation. Diarrhea adds to problems in patient care, disturbs fluid and electrolyte balance, and worsens nutritional status. There has been the impression that tube feeding hyperosmolality, antibiotics, and low serum albumin induce diarrhea. However, in view of the sparsity of published work, a prospective study was undertaken to determine the incidence of diarrhea and to define factors associated with its cause. Of the 50 patients studied, 16 (32%) developed diarrhea. Stool cultures were negative for pathogenic organisms. Although the risk of diarrhea was associated with antibiotics (p less than 0.005), several nutrients also had an impact. Results demonstrated a significant relationship between dietary lipid content (p less than 0.05) or vitamin A intake (p less than 0.001) and diarrhea. Implementation of tube feeding within 48 hrs postburn was also associated with a decreased incidence of diarrhea (p less than 0.001). This paper describes a modular tube feeding program in which diarrheal frequency is lessened (p less than 0.0001). Surprisingly, tube feeding osmolality, drugs used to prevent stress ulcers, or hypoalbuminemia did not have an adverse effect on intestinal absorption. The cause of diarrhea in burn patients is obviously multifactorial. It is concluded that a low fat (less than 20% of caloric intake), vitamin A enriched (greater than 10,000 IU/day), early enteral support program maximizes conditions which promote tube feeding tolerance while minimizing nutrient malabsorption during the nutritional rehabilitation of thermal injury.
BACKGROUND: Contrary to other countries, in which a unique entity is recognized, two sub-groups of light eruption, benign summer light eruption and polymorphous light eruption, are identified in France. Benign summer light eruption was individualized with a few criteria: age at onset between 25-35 years old, female predominance, onset within 12 hours after an intensive exposure to sun, presence of lesions on areas which have not been regularly exposed to sunlight such as the upper chest, absence of lesions on the face, improvement during the summer period and negativity of the polychromatic phototest. In fact patients usually presenting with benign summer light eruption do not have all the criteria and they gradually develop a polymorphous light eruption. The aim of this study was to quantify, among the patients presenting with a light eruption, the population with three main criteria of benign summer light eruption. PATIENTS AND METHOD: Eighty-seven patients presenting with a light eruption, a polymorphous light eruption or a benign summer light eruption, were selected by a dermatologist. For each patient the presence or absence of the 3 main criteria were noted: a) no lesion of the face, b) improvement of the eruption during the summer period, c) negative polychromatic phototest. RESULTS: Among the 87 patients, 9 of them (10 p. 100) had the three main criteria of benign summer light eruption. DISCUSSION: Benign summer light eruption is either rare or defined with wrong criteria. The polychromatic phototest was gradually replaced by the UVA phototest. In fact, provocation UVA phototests do not individualize benign summer light eruption from polymorphous light eruption. Individualization of the benign summer light eruption is not justified because there is a continuous spectrum of light eruptions, ranging from the benign eruption which improves during the summer period and the chronic eruption with a high photosensitivy and lesions of the face.
BACKGROUND: The phototest is used to confirm the diagnosis of polymorphous light eruption and to evaluate the different treatments. The different light sources in the different countries explains the lack of standardization. In France, we use a polychromatic source emitting a radiation close to the solar spectrum. The study compared the efficacy of a polychromatic source with a UVA source in the photo-induction of lesions. PATIENTS AND METHOD: Sixty-four patients presenting with a polymorphous light eruption were selected by a dermatologist. Polychromatic and UVA phototests were performed on each patient. At day 8, the phototest was considered positive if there were papules. RESULTS: Polychromatic and UVA phototests were positive in respectively 56 p. 100 and 23 p. 100 of the patients. Forty-one per cent of the patients had both polychromatic and UVA negative phototests. DISCUSSION: Polychromatic phototest seems to be more sensitive than UVA phototest. Induction of the lesions with a polychromatic source is so easy that it proves the significant role for UVB in the genesis of polymorphous light eruption. The percentage of negative phototests is identical to those published in the literature. Negative phototests are not necessarily secondary a bad methodology, they can individualize patients with those lower photosensitivity who present with a polymorphous light eruption that may resolve spontaneously.
