[Traumatology of the midface--diagnostic and therapeutic guidelines for the practicing ENT physician from the viewpoint of the ophthalmologist].

Fractures of the midface combined with orbital injuries endanger vision, binocular vision, and the lacrimal system. The best results of primary surgical management are advised if the surgeon adheres to a strict time schedule. For example, a perforating injury of the eye must be diagnosed as early as possible, and be repaired immediately. Otherwise, all manipulations necessary for reconstruction of the bone and soft tissue of the face worsen the condition of the opened eyeball, including loss of its contents and function. On the other hand, competent repair of the lacrimal system may be done at the end of the operation, and management of disordered eye mobility as seen in the "blow-out" fracture may sometimes be postponed for days or even weeks. The optimal chronological order of diagnostic and surgical management as seen from the ophthalmologist's view is given in tables, and discussed in detail.

[Pindolol eye drops (Glauco-Visken) - half year's results in glaucoma therapy].

In results of studies published hitherto it has been found that Pindolol eye drops 1%, a nonselective beta-blocking agent with intrinsic sympathomimetic activity have little or no effect on the bronchopulmonary system. Therefore, a long-term study of the pressure-lowering and other effects of this drug seems in open-angle glaucoma appeared desirable. In a multicenter double-blind trial 41 patients were treated with Pindolol eye drops 1% and 40 with Timolol eye drops 0,5%. A comparison of the two groups showed that their effect in IOP was almost identical. The other parameters of the examination revealed no significant differences between the two groups. - Five patients in the Pindolol group had to discontinue the study due to allergic reactions of the lids and/or conjunctiva .

[Morphometry of the endothelium using automated television image analysis].

Report on the use of a new image processing system (IPS) for automated morphometry of the corneal endothelium. The method is demonstrated with endothelial photographs taken with the aid of a Keeler-Konan wide angle specular microscope. The original picture is enlarged ten times, then digitized, detected, and analyzed. Areas containing 100-250 endothelial cells can be evaluated automatically. Depending on the quality of the photographs, larger areas sometimes require interaction by the investigator. The evaluation of a 621-cell picture is demonstrated step by step. The time saving which the method offers and its objectivity are discussed, as well as its use in combination with different specular microscopes.

[Comparative studies of initial pressure reduction using 0.3% metipranolol and 0.25% timolol in eyes with wide-angle glaucoma].

In a short-term study the pressure-lowering effects of Metipranolol 0.3% and Timolol 0.25% were compared in eyes with open-angle glaucoma by means of one-drop curves, diurnal pressure curves and tonography. It appears that these two drugs may be regarded as equal in respect of extent and duration of action. Aqueous humor outflow was facilitated slightly by both drugs, although increase was only statistically significant with Metipranolol.

[On the clinical picture of Purtscher's disease (author's transl)].

Case reports on patients suffering from traumatic retinal angiopathy (Purtscher's disease) and fat embolism in acute pancreatitis, respectively. Differential diagnostic comparison of the most important clinical findings with the aid of ophthalmoscopy, fluorescein angiography, computer perimetry and simultaneous fundus photography.

[Electronic image analysis in ophthalmology].

Based on experimental investigations a broad concept for the use of television image analysis in basic and clinical ophthalmology is given. This summary outline contents a short description of the technique and some examples for the clinical use of image analysis, like the measurement of corneal width and erosions, infrared pupillography, morphometry of the iris and quantitative fluorescence angiography. "Static" and "dynamic" image analysis are defined, the role of pattern recognition is mentioned. The results of the study demonstrate that television image analysis can be of great importance for the future of quantitative ophthalmology.

[Ultrasonic echography. Current and future tasks in the finding of intraocular foreign bodies].

Unlike most publications ultrasonography is not seen as an alternative concept to x-ray localization of intraocular foreign bodies but as supporting aid. It is primarily used for the rather simple measurement of the form of the injured eye and not so much for direct search of the foreign body. The latter is rather difficult to perform intraoperatively. The combined method is based on the Comberg localization using a mobile x-ray television unit. The x-ray imagery shows quickly, exactly and clearly the localization of the foreign body in the orbita. The eye itself cannot be seen on the monitor, but ultrasonic data allow to add its contours, corrected individually. This work is done by a computer. The measurement of coordinates and angles usually performed by the investigator is done by a computer, too. The linkage of both x-ray and ultrasonic equipment to a computer relieves the surgeon of the graphical tasks and ultrasonic search for the foreign body; it also gives information about the localization of the foreign body in the individual case during the entire operation.

[Protein concentration in the aqueous humour of the rabbit after argon laser coagulation of the iris (author's transl)].

After coagulation of the iris of pigmented rabbits with argon laser, the protein concentration of the aqueous humour was measured at different times. An initial increase of the protein concentration was found both after low and high intensity coagulation. Already 5 hours later the protein concentration was again low and no further increase could be observed within 28 days. The contralateral untreated eye showed a lower but significant response. High intensity coagulation lead to a more severe reaction.

[Quantitative fluorometry at the ocular fundus (author's transl)].

Description of a videodensitometric method for the quantitative evaluation of fluorescence angiography photographs using the Quantimet 720 image analysis computer system. The clinical value of the new method is demonstrated in a case of central serous retinopathy. With the aid of these measurements the activity of leakage points can be assessed more exactly than by mere subjective inspection of the gained pictures, thus allowing a better control of the effects of a chosen therapy.

[Development of ophthalmological working procedures (author's transl)].

The work at a university clinic is characterized by teaching, research, and care of in- and out-patients. The combination of these basic tasks needs appropriate organization. To obtain optimal efficiency, the size and power of the single compartments, the teams working there are personal intentions of their members, and other special factors must be considered. Apart from local differences some basic outlines can be drawn characterizing the typical problems of an eye clinic taking into consideration its special matters. Moreover, the continuous clinical and scientific progress has to be taken in mind in order to create organizational forms feasable for change and adaptation. In this paper we present our considerations concerning the arrangement of our own clinic, and the experience gained in respect to the development of working methods. Some examples are given in order to demonstrate the realization of close conjunctions between the various parts of the clinic, and, likewise, the increase of efficiency of different methods and techniques gained by those means. This gives general ideas of the solution of some actual problems in a university eye clinic.
