[The aging person in Gurabo: a bio-psychosocial profile of the aging person in a community in Puerto Rico].

The Department of Family Medicine of the University of Puerto Rico Medicine School performed a census of the inhabitants 60 years and older in the community of Gurabo. A total of 1,890 elderly were interviewed 51% were female and 49% male. Sixty percent were married and twenty-five percent were widowed. Eighty two percent still lived within a family setting and only seventeen percent lived alone 86% owned their own home despite ninety percent reporting incomes below $400.00 per month. Ninety five percent have access to medical services but 63% receive services in multiples sites. Eighteen percent were functionally impaired, fourteen percent intellectually impaired and five percent had impairment in both areas. Preliminary data show that the latter group had an annual mortality rate of 15% compared to only 3.6% and 1.3% respectively in the other two group. We concluded that: 1) functional and mental impairment are indicators of increased mortality. 2) although access to health care is not a problem this health care is episodic and interrupted being provided in multiple different sites.

Breathing pattern--gas exchange relation and acute effect of almitrine in severe chronic airflow obstruction.

Using a double-blind cross-over design, a single oral dose of 100 mg almitrine bismethylate and placebo were administered to 7 patients with chronic airflow limitation. In all patients, arterial blood gases at rest, ventilation and breathing pattern at rest and on exercise were measured before and 3 h after administration. Ventilation increased and PaCO2 decreased after almitrine; the mean PaO2 increase was statistically significant after active drug but the value increased more when tidal volume increased. It is concluded that in man the well-documented improvement in the V/Q relationship after almitrine is in part related to a pure ventilatory effect though the possibility of increasing ventilation by mainly increasing tidal volume.

Growth temperatures of isolates of Sporothrix schenckii from disseminated and fixed cutaneous lesions of sporotrichosis.

In 1979 Kwon Chung described two varieties of Sporothrix schenckii based on the thermotolerance of isolates from fixed cutaneous (35 degrees C) and that of disseminated cutaneous forms (37 degrees C) of sporotrichosis. Since we had not noted such a difference previously in a study of 100 cases of this disease (55% localized and 45% disseminated) wherein all the isolates grew at 37 degrees C, we decided to repeat this work. Our results differ from those reported by Kwon Chung, since the isolates of both the fixed and disseminated forms of sporotrichosis grew at 37 and 38 degrees C, even when we used inocula of 30 conidia (20-50) which according to Kwon Chung were needed to observe this difference.

Fine needle aspiration in the treatment of ganglion cysts.

Twenty-eight ganglion cysts in 21 patients were diagnosed by fine needle aspiration (FNA) in the Cytopathology Service of The George Washington University Medical Center from January 1979 to December 1984. Twenty-four of these were aspirated by a pathologist. One patient had two ganglions. Seven of the 21 patients had recurrences and six were reaspirated. In five of those patients the cysts never attained their original size or interfered with function. Follow-up ranged from three to 71 months. Based upon our experience, we believe that FNA is a simple, fast, effective, nontraumatic, inexpensive method of treating ganglion cysts.

Increase of unidentified HLA antigens in pulmonary tuberculosis.

We studied the HLA-A, B,C and DR antigens in 50 patients with advanced pulmonary tuberculosis in an effort to detect some genetic factors which may predispose to the development of this disease. We found a decrease in HLA-A3, A9, B5, B8, B13, B17 and B27. The incidence of "double blank" antigens in the locus A was 18% (2.5% in controls, p less than 0.0005) in the locus B 16% (1.5% in controls, p less than 0.0005) and 28% in the DR locus (7% in controls, p less than 0.0005). Our results suggest that the cells responsible for the immune response in patients with tuberculosis appear to have a diminished number of HLA antigens serologically determined. That phenomenon could be explained by several hypothesis like poor expression of these proteins or masking for another substance. In spite of the mechanism, this abnormal membrane expression of HLA antigens could be important for the development of the disease since the cell-cell interactions critical for the mounting of an adequate immune response might not be appropriate.

A case of sporadic juvenile Parkinson's disease.

Juvenile Parkinson's disease is rare. Virtually all reported cases are either associated with a family history or additional neurologic findings. We report a sporadic case of classic Parkinson's disease, without other abnormal neurologic signs and age at onset of 15 years.

[The use of simultaneous angiography to determine the signification of vascular images in renal scintiscans with technetium (author's transl)].

Combined angiography and renal scintiscans were used by the authors in an attempt to determine the signification of the images and vascular tracings obtained after intravenous injection of technetium (99mTc). They conclude that the scintiscan images are essentially the reflection of cortical vascularization and that its duration, as determined by the scintiscan, is roughly equal to the mean transit time of the indicator employed (Tc). Arterial stenosis cannot be demonstrated by scintiscans using technetium.
