Bioavailability and pharmacokinetics of oral topotecan: a new topoisomerase I inhibitor.

The results of preclinical and clinical studies indicate enhanced antineoplastic activity of topotecan (SKF 104864-A) when administered as a chronic treatment. We determined the apparent bioavailability and pharmacokinetics of topotecan administered orally to 12 patients with solid tumours in a two-part crossover study. The oral dose of 1.5 mg m-2 was administered as a drinking solution of 200 ml on day 1. The i.v. dose of 1.5 mg m-2 was administered as a 30 min continuous infusion on day 2. The bioavailability was calculated as the ratio of the oral to i.v. area under the curve (AUC) calculated up to the last measured time point. The oral drinking solution was well tolerated. The bioavailability revealed moderate inter-patient variation and was 30% +/- 7.7% (range 21-45%). The time to maximum plasma concentration after oral administration (Tmax) was 0.78 h (median; range 0.33-2.5). Total i.v. plasma clearance of topotecan was 824 +/- 154 ml min-1 (range 535-1068 ml min(-1)). The AUC ratio of topotecan and the lactone ring-opened hydrolysis product (hydroxy acid) was of the same order after oral (0.34-1.13) and i.v. (0.47-0.98) administration. The bioavailability of topotecan after oral administration illustrates significant systemic exposure to the drug which may enable chronic oral treatment.

Increased gallbladder-related mortality among Hispanics: does education play a role?

OBJECTIVE: Hispanics, particularly Mexican Americans, are known to have a higher incidence of mortalities whose underlying cause is a gallbladder-related disorder. These analyses evaluate the role of educational attainment in the differential mortality experiences of these populations. METHODS: US mortality data for 1989-1991 were examined to determine ethnically-specific death rates using 'any mention' of the disease on the death certificate. RESULTS: Age-adjusted multiple cause mortality was found to be higher for all gallbladder-related disorders among Hispanics, particularly Mexican Americans. Mortality due to gallbladder cancer, gallstones and 'other gallbladder diseases' were found to be inversely proportional to educational attainment in all ethnic groups. When both age and education were used to adjust mortality, the gallstone and other gallbladder disease mortality among Hispanics was non-significantly higher than white, non-Hispanics. However, mortality due to gallbladder cancer remained significantly higher among Hispanics. CONCLUSION: Gallbladder cancer mortality is elevated in Hispanic populations, especially Mexican Americans, independent of educational attainment. However, increased mortality associated with gallstones or other gallbladder diseases among Hispanics may be partially due to differences in factors associated with educational attainment. Research and public health efforts to address these educational-related factors may improve this mortality pattern among Hispanics.

Educational reform in health administration.

The transformation of the health care marketplace now demands transformation of the academy. The stakeholders--students, industry leaders, business, government, consumers--are all demanding managed care education now for the students of health administration. The authors describe a graduate course on managed health organizations initially offered to students at a Southwestern university during 1994.

Dietary manipulation with lemonade to treat hypocitraturic calcium nephrolithiasis.

PURPOSE: Pharmacological treatment of hypocitraturic calcium nephrolithiasis requires as many as 12 tablets, or numerous crystal packages or liquid supplements taken throughout the day. In addition to added cost, this cumbersome regimen decreases patient compliance, which may increase stone recurrence rates. We evaluated the urinary biochemical effects of dietary citrate supplementation in hypocitraturic calcium stone formers in an attempt to decrease or eliminate the need for pharmacological therapy. MATERIALS AND METHODS: A total of 12 patients who were either noncompliant with or intolerant of pharmacological citrate therapy supplemented their routine diet with citrate in the form of lemonade, consisting of 4 ounces of reconstituted lemon juice (5.9 gm. citric acid) mixed with tap water to a total volume of 2 l. and consumed at uniform intervals throughout the day. Urine specimens (24-hour) were obtained for biochemical analysis after 6 days of lemonade therapy and compared to pre-lemonade baseline values. RESULTS: Of the 12 patients 11 had increased urinary citrate levels during lemonade therapy (average 204 mg. per day). Average levels increased from 142 mg. daily (range less than 10 to 293) at baseline to 346 mg. daily (range 89 to 814) after treatment (p < 0.001). Daily total urinary volumes were similar (2.7 versus 2.9 l.). Seven of 12 patients became normocitraturic while consuming lemonade. Urinary calcium excretion decreased an average of 39 mg. daily, while oxalate excretion was unchanged. The lemonade mixture was well tolerated. Two patients complained of mild indigestion that did not require cessation of therapy. CONCLUSIONS: Citrate supplementation with lemonade increased urinary citrate levels more than 2-fold without changing total urinary volume. Lemon juice, which contains nearly 5 times the concentration of citric acid compared to orange juice, is an inexpensive and well tolerated dietary source of citrate. Lemonade therapy may improve patient compliance, and may be useful as adjunctive treatment for patients with hypocitraturic calcium nephrolithiasis.

Quinidine pharmacodynamics in patients with arrhythmia: effects of left ventricular function.

OBJECTIVES: This study was undertaken to determine whether quinidine pharmacodynamics are altered in the presence of left ventricular dysfunction. BACKGROUND: Left ventricular function is an independent predictor of antiarrhythmic drug efficacy. However, the effects of left ventricular dysfunction on the pharmacodynamics of antiarrhythmic drugs have not been studied extensively. METHODS: Signal-averaged electrocardiograms were obtained and quinidine plasma concentrations measured during 24-h quinidine washout in 22 patients. RESULTS: Linear quinidine concentration-effect relations were observed for QRS and QT intervals corrected for heart rate. The slopes of the concentration-effect relation describing changes in the corrected QT (QTc) interval were significantly higher in the group with left ventricular ejection fraction > or = 0.35 ([mean +/- SD] 29.5 +/- 11.2 ms/micrograms per ml) than in the group with a low left ventricular ejection fraction (15.7 +/- 9.7 ms/micrograms per ml, p = 0.001). The QRS concentration-effect relations were not different in the two groups. A significant linear correlation was observed between the slopes of the concentration-effect relations describing changes in QTc intervals and left ventricular ejection fraction (r = 0.7, p < 0.001). Nineteen patients with inducible ventricular tachycardia underwent serial electrophysiologic studies for evaluation of quinidine efficacy. Ventricular tachycardia could not be induced during quinidine therapy in eight patients. The slopes of the quinidine concentration-effect relations for QTc intervals were significantly higher in quinidine responders than in nonresponders (p < 0.05). CONCLUSIONS: The effects of quinidine on ventricular repolarization are linearly related to left ventricular ejection fraction. Quinidine concentration-effect relations describing ventricular repolarization are associated with antiarrhythmic efficacy in patients with ventricular tachycardia.

Intracytoplasmic sperm injection: a major advance in the management of severe male subfertility.

OBJECTIVE: To determine the success of intracytoplasmic sperm injection for severe male infertility. DESIGN: A retrospective survey. SETTING: A tertiary infertility service. PATIENTS AND INTERVENTIONS: One hundred fourteen couples had 119 intracytoplasmic sperm injection treatments because of previous failure of standard IVF, poor results with subzonal insemination, sperm concentration < 2 x 10(6)/mL, other sperm defects, or male genital tract obstruction. MAIN OUTCOME MEASURES: Fertilization, implantation, and pregnancy rates. RESULTS: Of 1,185 oocytes treated by intracytoplasmic sperm injection, normal fertilization and cleavage occurred in 717 of 1,073 that survived (67% normal fertilization rate). Abnormal fertilization occurred in 113 oocytes (11% abnormal fertilization rate) and 112 oocytes did not survive the procedure (survival rate of 90%). In 117 couples, 251 embryos were transferred fresh, 409 embryos were cryopreserved, and 224 were transferred after thawing. The implantation rate was 7.4% (fetal heart per embryo transferred). To date 36 clinical pregnancies have been achieved (12% per fresh transfer, 20% per frozen transfer, and 30% overall), 24 are ongoing or delivered (6% per fresh transfer, 14% per frozen transfer, and 20% per intracytoplasmic sperm injection). The fertilization rates were the same (65%) with various sperm defects but higher with genital tract obstructions (75%). CONCLUSION: Intracytoplasmic sperm injection has improved the prognosis of severe male infertility.

Intracranial invasion from a primary adenocarcinoma arising in the middle and external ear.

Cerumenal adenocarcinoma is a rare neoplasm arising from the modified sweat glands of the ear. Before the diagnosis of primary tumour is made, a thorough search must be undertaken to exclude the possibility of metastatic disease, which occurs more commonly. Benign adenoma can also occur and this may be identical histologically to the malignant variety. The presence of local invasiveness may be the only differentiating feature. Aggressive local resection followed by postoperative radiotherapy form the cornerstone of management. Individuals treated with radiotherapy alone have a life expectancy of less than 12 months whereas cures have been reported with radical surgery.

Public health informatics: how information-age technology can strengthen public health.

The combination of the burgeoning interest in health, health care reform and the advent of the Information Age, represents a challenge and an opportunity for public health. If public health's effectiveness and profile are to grow, practitioners and researchers will need reliable, timely information with which to make information-driven decisions, better ways to communicate, and improved tools to analyze and present new knowledge. "Public Health Informatics" (PHI) is the science of applying Information-Age technology to serve the specialized needs of public health. In this paper we define Public Health Informatics, outline specific benefits that may accrue from its widespread application, and discuss why and how an academic discipline of public health informatics should be developed. Finally, we make specific recommendations for actions that government and academia can take to assure that public health professionals have the systems, tools, and training to use PHI to advance the mission of public health.

Removing communication barriers for the hearing-impaired elderly.

Hearing impairment is a significant disability affecting many older adults. A number of technologies and services can provide the means to improve communication with these individuals. Nurses play an important role in identifying patients experiencing hearing loss and providing information and assistance that can reduce the consequences of this serious problem.

Successful self-care home dialysis in the elderly: a single center's experience.

OBJECTIVE: This study was designed to prospectively review our experience with self-care home dialysis in the 60-years-and-older age group since the commencement of continuous ambulatory peritoneal dialysis (CAPD) in our center in 1989. DESIGN: Data on mortality, morbidity, dialysis technique and complications, quality of life, and rehabilitation were collected prospectively. SETTING: A regional dialysis unit providing full renal replacement services for a population of 292,000. PATIENTS: Twenty-five patients (16 male) with an average age of 64.4 years (range: 58.25 - 76.5 years) at commencement of dialysis. RESULTS: All patients were on self-care home dialysis. Patient survival rates were comparable with the national average at 12 months (90% vs 89%) and two years (84% vs 80%). Dialysis therapy was well tolerated and technique survival rates were comparable for both hemodialysis and CAPD. There was a very low peritonitis rate of 1 episode per 28.5 patient-months. CONCLUSIONS: Self-care home dialysis is a viable therapeutic option with a high degree of technique success and a good quality of life in the older population.

Unbalanced 5;16 translocation in a boy with papillary thyroid carcinoma.

This is the first reported case of an unbalanced chromosome rearrangement resulting in trisomy 5q35.5-->qter and monosomy 16p 13.3-->pter, in a boy with mental and growth retardation, minor anomalies, and a history of bilateral papillary thyroid carcinoma. This was the result of a familial balanced translocation. The clinical and cytogenetic manifestations of the case are presented and the possible role of the chromosomal rearrangement in the etiology of the thyroid carcinoma is discussed.

Synthetic scleral reinforcement materials: I. Development and in vivo tissue biocompatibility response.

The surgical treatment of pathological myopia involves the placement of a band cut from donor sclera around the back of the globe to provide support and to prevent elongation that leads to retinal detachment and blindness. In the search for a synthetic material that would overcome the disadvantages of the donor tissue band, we tested 11 types of bands made from 14 polymers in rabbit eyes. Bands were cut from porous and solid materials, as well as porous materials coated with solids and solid materials with porous patches affixed to the posterior surface. Criteria for suitability and biocompatibility included ease of placement and suturing, stable positioning, and absence of long-term tissue response on a cellular level. Testing included evaluation by the surgeons performing the implantations, measurement of the distance between the band and the globe, determination of fibrous capsule thickness and amount of collagen deposition, and enumeration of macrophages accumulated at the band/capsule interface. Based on the results of these studies, two of the porous materials (expanded polytetrafluoroethylene and porous polyurethane fabric) and two of the composite materials (porous polyurethane fabric coated with polyacrylonitrile and silicone with microvilli cut into the posterior surface) were selected for further study.

Nutrition education in Missouri schools.

This survey examined the status of nutrition education in Missouri schools, profiled teacher preparation in nutrition, and identified teacher preferences for additional nutrition education resources. Questionnaires were mailed to a systematic sample of 1,664 teachers representing different grade levels K-12 and subject areas including biological science, health, home economics, and physical education. Responses from 793 teachers indicated most integrated nutrition into other subjects such as health and science. Almost all taught the concept of food groups. Teachers expressed a need for materials on a range of concepts and preferred the videotape format. About one-third of elementary teachers reported completing a college-level nutrition course: 70% of secondary teachers had completed such a course. Results indicate the need for materials to integrate nutrition into various subjects and encourage teaching on concepts beyond the food groups.

Local and systemic antibody response to putative periodontopathogens in patients with chronic periodontitis: correlation with clinical indices.

Specific immunoglobulin G (IgG), IgA and IgM antibody titres to Porphyromonas gingivalis and Actinobacillus actinomycetemcomitans were measured by enzyme-linked immunosorbent assay in serum and gingival crevicular fluid at 5 sites in each of 20 chronic periodontitis patients. Specific serum antibody titres correlated with mean gingival crevicular fluid titres. The 3 immunoglobulin subclass responses (IgA, IgG and IgM) to P. gingivalis correlated. A comparison of sites with probing depth < 4 mm and > or = 4 mm showed that the latter group had significantly lower gingival crevicular fluid IgG titres to P. gingivalis. Sites with a gingival index of 3 had significantly lower gingival crevicular fluid IgG titres to this organism than those with a gingival index of less than 3. These findings support the concept that the humoral immune response is protective, as chronic periodontitis patients with greater pocket depths and more gingival inflammation had paradoxically lower antibody titres to suspected periodontopathogens.

Adrenergic regulation of renin secretion and renal hemodynamics during deliberate hypotension in humans.

To assess the relative contributions of neural and nonneural stimuli of renin secretion, the effects of an alpha 1-agonist, phenylephrine (Phe), or a beta-agonist, epinephrine (Epi), on plasma renin activity (PRA), renal blood flow (RBF), and glomerular filtration rate (GFR) were compared during sympathetic blockade with epidural hypotensive anesthesia [mean arterial blood pressure (MAP) = 60 and 50 mmHg]. Controls (NaCl) received saline alone to maintain MAP at 50 mmHg. Epi increased PRA (ng.ml-1.h-1) from 0.9 +/- 0.6 to 3.0 +/- 1.5 at 60 mmHg MAP and 4.7 +/- 1.8 at 50 mmHg MAP, with associated decreases in RBF (-33 and -60%, respectively) and GFR (-27 and -53%, respectively). During hypotension with Phe and NaCl, PRA and RBF were unchanged from baseline but GFR decreased. Urinary Na secretion decreased comparably in all three groups. In conclusion, during sympathetic blockade with epidural anesthesia, marked reductions in both renal perfusion pressure and distal nephron Na delivery were insufficient to increase renin secretion. beta-Adrenergic stimulation (e.g., Epi) was required to increase PRA. Epi decreased RBF suggesting an angiotensin II-mediated effect.

Effects of left ventricular dysfunction on the circadian variation of ventricular premature complexes in healed myocardial infarction.

Circadian variation in the onset of cardiovascular events including sudden cardiac death, myocardial infarction and ventricular arrhythmias has been described. The effect of left ventricular (LV) dysfunction on the circadian variation of ventricular premature complex (VPC) frequency was evaluated in 132 patients with frequent VPCs and reduced LV function after myocardial infarction. Patients were prospectively divided in 2 groups based on LV ejection fraction (EF) (those with LVEF less than or equal to 0.30, and those with LVEF between 0.30 and 0.45). Median hourly VPC frequencies and heart rates were compared between the 2 groups. Subgroup analyses based on treatment with beta-adrenoceptor blocking agents and on New York Heart Association functional class were also performed. In patients with LVEF greater than 0.30, a distinct circadian variation of VPCs, and the expected morning increase in VPC frequency were present. In contrast, a distinct circadian variation of VPCs was absent in patients with LVEF less than or equal to 0.30. A circadian variation of VPC frequency was also absent in patients with severe symptomatic congestive heart failure (New York Heart Association class III-IV). Treatment with beta-adrenoceptor blocking agents was associated with a loss of the circadian variation of VPC frequency. The circadian variation of heart rate was also blunted in the group treated with beta-adrenoceptor blocking agents. The proportion of subjects manifesting a positive correlation between heart rate and VPC frequency was lower in subjects with LVEF less than or equal to 0.30 (26%) than in those with LVEF greater than 0.30 (46%) (p less than 0.05). Thus, circadian variation of VPC frequency is absent in patients with severe LV dysfunction.
