[Lymph node and cutaneous tuberculosis revealed by a pyloroduodenal stenosis: a case report].

Lymphadenitis is the most frequent form of extrapulmonary tuberculosis. In immunocompetent patients, its clinical presentation is usually superficial, less frequently intra-abdominal located, and exceptionnally symptomatic. We report a case of scrofuloderma characterized by the lymph nodes extent of spread which led to a pyloro-duodenal stenosis. An 18 years old immunocompetend man is admitted in the internal medicine department, presenting a pyloroduodenal stenosis syndrome, fever, weight loss, and ulcerated axillary and cervical lymph nodes. The upper gastrointestinal endoscopy and ultrasound examination reveal the extrinsic duodenal compression by large numerous lymph node. A biopsy confirms the tuberculosis disease for which no other localisation was detected. Despite of the diagnostic and therapeutic delay, which explains the expanse and depth of the lesions, the antituberculosis therapy was effective on the digestive symptom and cleared all the cutaneous manifestations.

[Hepatic tuberculosis of the pseudotumoral form].

The authors report a case of a 30 years old immunocompetent woman with liver tuberculosis with an unusual pseudotumoral presentation and secondary occurrence of abcedation with cutaneous fistulization. The diagnosis was based on the bacteriological positivity for acido-alcoolo resistant bacillus in pus of the abscess obtained by ponction guided by ultrasonographic examination. The authors emphasize in the differential diagnosis with the other causes of liver abscess (amibiasis and pyogenic microorganisms) and liver carcinoma. They also note the importance of the function guided by ultrasonographic examination permitting histologic and bacte+ériologic study in the diagnosis of this unusual presentation of liver tuberculosis. The patient was treated by antibacillary antibiotics and evacuation function. The prognostic was good with 6 months of follow-up.

[Biermer's disease without anemia: apropos of 2 cases manifested as glossitis with macrocytosis].

Although it is recognized that glossitis is a classical sign of pernicious anemia, occurring in the evolution of this disease, it is unfrequent for this sign to reveal this affection. We report two cases where diagnosis was evacuated on the presence of glossitis and macrocytosis despite absence of anemia. Confirmation was done by low serum cobalamin level, gastritis atrophy and presence of intrinsic factor antibody. We emphasize that increased clinical suspicion may lead to early diagnosis even if anemia is lacking.

[Epidemiological and clinical aspects of pancreatic cancer in Senegal].

The objective was to verify if the introduction of ultrasonography in our practice has entailed modifications of the epidemiology and the clinic of pancreatic cancer in Senegal. We have performed a retrospective study of cancer of the pancreas diagnosed on the basis of clinical signs, ultrasonography, surgical findings and necropsy from March 92 to October 97. The chi-square test according to Mantel-Haenszel has been used to compare qualitative variables. The routine practice of ultrasonography has allowed to establish the diagnosis of 107 cases of pancreatic cancer within 5 years and half. The sex ratio was 1.05 and the average age was 60 years +/- 13 without significant difference according to the sex. The mean period of diagnosis was 5 months. However the classic form associating thinness and cholestatic jaundice or palpable abdominal mass dominated the clinical symptoms (74.5% of cases). A very deep alteration of the general status has been noticed in 25.5% of cases. At surgery, according to the TNM classification, 22% of our patients were at stage II and 78% from stage III to IV. In Senegal, the prevalence of pancreatic cancer is increasing since the introduction of ultrasonography. This technique has shown that the male predominance is less than reported in previous studies. Nevertheless the diagnosis remains late. As far as we cannot identify risk groups, recent appearance of digestive disturbance even atypical after 50 years old, is enough to suspect the diagnosis.

[Evolutive aspects of systemic lupus erythematosus in Dakar. Apropos of 30 cases].

To determine the characteristics of evolution of systemic lupus erythematosus (SLE), the authors studied 30 cases retrospectively. All were black women aged from 16 to 73 years (with a mean of 30 years) at the time of diagnosis. Dermatological manifestations consisting in discoid lupus or alopecia inaugurated the disease in 12 cases, joint symptoms in 10 cases. Polyarthritis was the most common inaugural manifestation, followed by discoid lupus. Corticosteroids therapy alone or associated to chloroquine or immunosuppressor led to good results; 88% of patients who received treatment had good outcomes in the first 5 months after diagnosis. After this time lapse, 6 cases of complications related to the corticosteroids therapy occurred. After one year, 5 patients presented one or more flare-ups and had to be re-hospitalized; 8 others were lost to the follow-up. The overall mortality rate was 27% (8 cases out of 30). Causes of death were first renal failure (3/8) followed by infectious complications (2/8). The management of SLE could be improved by a close follow-up and providing the patients and their family with adequate information.

[Tuberculosis among hemodialysis patients in Dakar, apropos of 2 cases].

Hemodialysis permits a long term survival to patients with End Stage Renal Disease (E.S.R.D.). However the patients ongoing hemodialysis presented a immunodeficiency and a important modification of drugs biodisponibility. Tuberculosis is an endemic disease in our countries. Extrapulmonary tuberculosis is reported from these two cases among 13 patients treated in C.H.U.A. Le Dantec hemodialysis center. Clinical symptoms are not specific bacteria is not found. Diagnosis is obtained by a bundle of arguments. Antituberculosis treatment need to be adjusted in this field. Even if Rifampicine can be administrated at normal dose. The others drugs must be adapted on their clearance and the underlying disease. The two patients presented psychiatrical symptoms motivating a reduction of isoniazide dose witch threshold toxicity is lowered by renal failure state. These observations must increase watchfulness on nephrologist of undeveloping countries, confronted with tuberculosis renewed out break.

[Frequency of Helicobacter pylori infection in symptomatic patients in Senegal].

This prospective study was carried out in Dakar, Senegal, to assess the prevalence of Helicobacter pylori infection in symptomatic patients undergoing endoscopy and to evaluate the factors of risk for infection by this type of bacteria in the population. From October to December 1995, 134 patients were included in this study and replied to a standardized questionnaire designed to determine socioeconomic level and living conditions. Diagnosis of Helicobacter pylori infection was based on the combined results of the urea breath test (Clo-test) and histological findings. Helicobacter pylori infection was detected in 82.8% of patients with no significant difference according to sex, age, ethnic group, or living environment (urban or rural). The incidence of infection was also the same in all socioeconomic groups. It was already high in the age group between 11 and 20 years (90.9%). Helicobacter pylori was identified in 76.2% of patients with normal endoscopic findings and in 100% presenting ulcers, erosions, or gastritis. This study shows that the incidence of Helicobacter pylori infection is extremely high regardless of socioeconomic level and that infection begins at a young age. These findings are consistent with the poor hygiene of most people in Senegal.

[Current aspects of gastric cancer in Senegal. Epidemiological and clinical study of 220 cases (1984-1991)].

220 cases of gastric cancer were selected from Dakar Hospitals during seven years (1984-1991). This retrospective study confirms that gastric cancer is the most common cancer of the Gastro-intestinal tract (48.4%). The sex ratio was 2.7 (M/F) with mean age of 50 years. However, a steady progression of gastric cancer was noticed beginning at 40 years. Despite the introduction of upper gastro-intestinal endoscopy the classical evolved form was the most commonly found (83.2%). The associated lesions known to be premalignant were found in 22.7% of cases. The patients were seen with a long delay after the first symptoms. This is why we have long delays of diagnosis in our centers. 61.7% of them are seen 3 to 12 months after their first symptoms. In 70% of cases, lesions were found on antropyloric region. The most common histological type remain adenocarcinoma (90% of cases). Even though endoscopy has contributed to increase the incidence of gastric cancer, it did not allow to discover early gastric carcinoma.

[Field trial of an attenuated vaccine against heartwater disease].

The results are given of a field trial using a Senegalese stock of Cowdria ruminantium which had been attenuated by passage in cell culture. Thirty vaccinated and thirty control sheep were exposed in the Niayes region of Senegal and were monitored daily. In the control group, 22 animals died of heartwater, associated in one case with anaplasmosis. In the vaccinated group, 13 animals died; Cowdria was found only in two sheep which had previously suffered from ehrlichiosis or anaplasmosis; three other cases of ehrlichiosis and two of anaplasmosis were also observed among these 13 animals. The resistance of the two vaccinated animals which showed Cowdria in their cerebral cortex was apparently lowered by the intercurrent infections. The other animals of the vaccinated group showed no evidence of Cowdria infection.

[Ticks and hemoparasitoses of livestock in Senegal. VI. The Soudano-Sahelian zone].

The authors report the results of a study on ticks and hemoparasitoses among cattle, sheep and goats in the Sudano-Sahelian zone. During a period of 15 months, ticks were systematically removed from 40 cattle, 40 sheep and 40 goats in order to assess the population dynamics and determine more accurately the preferential sites of settlement of the different species. The following species were collected in ruminants: Hyalomma truncatum, H. marginatum rufipes, Rhipicephalus evertsi evertsi, Rh. guilhoni, Amblyomma variegatum, Boophilus decoloratus. Concurrently, studies were made on hemoparasitoses using blood smears and splenectomy. The following species were found in cattle: Anaplasma marginale, Ehrlichia bovis, Theileria mutans. Infections detected in sheep and goats were caused by Anaplasma ovis, Ehrlichia ovina and Theileria ovis. The PCV values of apparently healthy nature animals as well as the seasonal variations in this hematological parameter were investigated.

The authors report the results of a study on ticks and hemoparasitoses among cattle, sheep and goats in the North-Guinean zone. During a period of 15 months, ticks were systematically removed from 40 cattle, 40 sheep and 40 goats in order to assess the population dynamics and to determine more accurately the preferential sites of settlement of the following different species collected from these animals: Amblyomma variegatum, Boophilus geigyi, Hyalomma truncatum, H. marginatum rufipes, Rhipicephalus sulcatus, Rh. senegalensis, Rh. lunulatus. Studies on hemoparasitoses were conducted simultaneously using blood smears and splenectomy. The following species were found in cattle: Anaplasma marginale, Ehrlichia bovis, Trypanosoma vivax, Theileria mutans, Theileria velifera. Infections detected in the small ruminants were Anaplasma ovis, Ehrlichia ovina, Trypanosoma congolense, Theileria ovis. The PCV values of apparently healthy animals as well as the seasonal variations in this haematological parameter were investigated.

Cowdriosis in Senegal: some epidemiological aspects.

The results of a study on the abundance of Amblyomma variegatum in different ecological zones and of Cowdria ruminantium infection rates in nymphal and adult ticks of the North Guinean zones are given. Joint research is also conducted on the evaluation of seroprevalence. In this study, it appears that the vector is most important in the North Guinean zone, followed by the South Sudan and the coastal Niayes zones. Elsewhere, the tick populations are not significant or absent. The infection rate in the North Guinean zone is high: 1.1% at least for adult ticks and 7.8% for nymphs. The seroprevalence in the North Sudan and the Sudano-Sahelian zones is very low in the hinterland whereas the values are higher near the coast.

[Epidemiology of Cowdria infection in Senegal. I. Study of the transmission and infection level in Amblyomma variegatum (Fabricius, 1794) in the Niayes region].

The authors report the results of research on infection rates of Amblyomma variegatum nympha and imago populations by Cowdria ruminantium in the Niayes region as well as on the transmission rates of A. variegatum by the various stases. The experiments were carried out on sheep from the Sahelian zone where the vector tick is not endemic. The infection rate, estimated by inoculating sheep with ground ticks, gave the following prevalences: t = 13.3% for nymphae and t = 1.2% for adults. The transmission rates calculated on the basis of a daily count of nymphae and adults (which were marked), gave the following minimum values: 11.1% for nymphae and 9.5% for imagos. In-station studies, using nymphae and imagos gorged at their larval or nymphal stase on infected sheep, showed infection rates of 100%. The transmission by nymphae and imagos originating from the same groups amounted to 80%. Intrastadial transmission assays by males and females previously gorged on sheep in hyperthermia, were unsuccessful.

[Prevention of cowdriosis and cases of sheep diseases in the Niayes region of Senegal].

Sheep originating from an heartwater-free area were immunized by the infection and treatment method, using long-acting oxytetracycline. The animals were then exposed, together with control sheep to natural infection in an enzootic area. Tick counts were made to monitor the infestation and establish a correlation between infestation by certain tick species, in particular Amblyomma variegatum, and the occurrence of cowdriosis. In some cases of death associated with ehrlichiosis and anaplasmosis in the group immunized against cowdriosis, groups of Cowdria ruminantium were found in smears of the cerebral cortex, which leaves question marks as to the real nature of immunity to cowdriosis. Furthermore, these losses show the necessity of an integrated approach to the control of diseases of livestock in a given area. The presence of Rhipicephalus e. evertsi, formerly absent from this area, was significant in this respect.

[Ticks and hemoparasitic diseases in cattle in Senegal. IV. The southern Sudan area].

The authors report on the results of an investigation on ticks and hemoparasitoses of cattle, sheep and goats in the South Sudanian area of Senegal. Systematic routine dipping against ticks of cattle, 40 sheep and 40 goats was set during 15 months, with a view to determine the population dynamics together with an acurate localization of the different species concerned. The following parasites were collected from these ruminants: Amblyomma variegatum, Boophilus geigyi, Hyalomma truncatum, H. m. rufipes, Rhipicephalus lunulatus, Rh. sulcatus, Rh. e. evertsi, Rh. senegalensis. At the same time joint research was conducted on hemoparasitoses by mean of blood smears and of splenectomy. In cattle were found Theileria velifera, Th. mutans, Anaplasma marginale, Babesia bigemina, Ehrlichia bovis, and microfilaria of Setaria labiatopapillosa. Anaplasma ovis, Theileria ovis, Ehrlichia ovina, Trypanosoma vivax, T. congolense are involved in infections detected in goats and sheep. Among grown up and found apparently healthy animals, the hematocrite values have been studied as well as the seasonal variations of the haematological parameter.

[Ticks and hemoparasitoses of livestock in Senegal. III. The Northern Sudan area].

The authors describe the results of a study on ticks and hemoparasitoses of cattle and small ruminants in the Senegalese north-sudanian area. For 15 months, 40 bovine, 40 sheep and 40 goats received a routine dipping treatment, aimed at the determination of the tick population dynamics together with an accurate localization of the preferential sites for the different species. The following parasites were collected from the animals: Hyalomma marginatum rufipes, H. truncatum, Rhipicephalus lunulatus, Rh. e. evertsi, Rh. sulcatus, Rh. senegalensis, Boophilus decoloratus. Joint studies were conducted on the hemoparasites using blood smear and splenectomy. Among bovine, Anaplasma marginale, Ehrlichia bovis, Theileria mutans, Th. velifera, Trypanosoma congolense, T. brucei and microfilariae from Setaria labiatopapillosa were observed. Babesia bigemina was observed after a splenectomy. In small ruminants, the detected infections are brought about by A. ovis, Th. ovis and T. vivax. Hematocrite value of apparently healthy animals are studied as well as the seasonal variation of this hematological factor.
