A case of mucoepidermoid carcinoma of the bronchus with low grade histologic features exhibited an aggressive biological behavior. All gross, microscopic, and ultrastructural features were those of a morphologically low grade mucoepidermoid carcinoma. Grossly, the lesion was an exophytic endobronchial mass; microscopically, it contained sheets of monomorphic cells with well-formed mucus glands, no mitoses, and no necrosis; and ultrastructurally, it consisted of numerous goblet cells and mitochondria- and glycogen-rich cells with well-formed microvilli. However, the patient developed widespread metastases to skin, subcutaneous tissue, femur, vertebral column, and pericardium and died two months after the primary endobronchial tumor was excised. The metastases, like the primary tumor, had the appearance of "low grade" mucoepidermoid carcinoma. Explants of the original tumor grown in nude mice for three months showed persistence of the low-grade morphologic pattern. Although metastases are known to occur frequently from high-grade lesions, widespread metastases from histologically low-grade lesions have not previously been reported. Low-grade histologic features of mucoepidermoid carcinoma are not necessarily then indicative of low-grade biological behavior.
To assess the efficacy of the anabolic steroid stanozolol in the treatment of osteoporosis, a 29-month double-blind study was performed with 23 treated and 23 control postmenopausal osteoporotic women. Drug efficacy was assessed by serial determinations of total body calcium (TBC--total bone mass) by neutron activation analysis, regional bone mass (RBM) by single-photon absorptiometry, and by spinal roentgenograms. Total body calcium increased 4.4% from baseline values (P less than 0.01) in the treated group and remained unchanged in the control group; the difference in the change in TBC between the treated and control groups was significant (P less than 0.03). The effect of the drug on TBC persisted throughout the 29-month period. In contrast to TBC, measurements of RBM indicated no significant differences between the treated and placebo groups, suggesting a possible differential response to therapy at various skeletal sites. No new spinal compression fractures were noted in the treated group (compared with three new fractures in the control group). Assessment of serum and urine values indicated a decrease in the level of urinary calcium and an increase in the level of total urinary cyclic AMP in the treated group. These changes were observed even though the level of serum iPTH was significantly decreased during the study. An analysis of changes in bone biopsy specimens revealed no significant differences between the treated and control groups. Seventy-six percent of the treated subjects developed SGOT elevations or other side effects from the stanozolol therapy; at no time were these effects sufficiently severe to cause termination of medication. The data suggest that long-term use of stanozolol increases the net total bone mass above pretreatment levels.
Between 1975 and 1979, 271 patients with regional small cell undifferentiated (including oat cell) carcinoma of the lung were entered into a study involving treatment by radiation therapy (4500 cGy (rad) in five weeks) to the primary tumor, mediastinum and supraclavicular lymph nodes, and a randomization to receive or not receive prophylactic treatment of the brain (3000 cGy in two weeks) and a randomization to prophylactic or delayed chemotherapy (cyclophosphamide and CCNU). Analysis of the data indicates that the median survival for responders (53 weeks) was significantly longer than that of the non-responders and partial responders (37 and 34 weeks). Median survival by treatment arm was 48 weeks for thoracic irradiation (TI), brain irradiation (BI), and early chemotherapy (CT), 44 weeks for TI alone, 41 weeks for TI and CT, 38 weeks for TI and BI. Regional complete and partial tumor responses were 52 and 25% for prophylactic chemotherapy and 44 and 35% for delayed chemotherapy. The site of first failure was regional in 12%, regional and distant simultaneously in 21%, and distant only in 46%. Elective brain irradiation significantly reduced the incidence of brain metastases from 21 and 5%, but did not improve survival.

Indications, success-rate and disadvantages of endoscopic retrograde (n = 928) and percutaneous transhepatic cholangiography (n approximately 360) at the Surgical and Medical University Clinics of Freiburg/German are discussed. Direct cholegraphy is the method of choice in cases of cholestasis of unknown origin. Endoscopic retrograde procedures usually preceed the percutaneous transhepatic approach. PTC either complementary or decisive when ERCP has failed. Therapeutic possibilities of both methods are not yet totally utilized.

Six experiments were performed to investigate the effects of the rat's copulatory behavior on sperm transport in the female's reproductive tract. Data showed that (a) transcervical sperm transport in rats requires 6--10 min after ejaculation for completion; (b) transcervical transport of large numbers of sperm requires that the vaginal plug remain lodged tightly in the vaginal-cervical junction; (c) maintenance of an immobile posture by the male, and possibly by the female, at ejaculation facilitates the deposition of a tightly lodged vaginal plug and the transport of large numbers of sperm through the cervix; (d) a single intromission occurring within 2 min after ejaculation disrupts sperm transport, but more copulatory stimulation is required to disrupt sperm transport when delivered between 4 and 10 min after ejaculation; and (e) the average postejaculatory interval is long enough to prevent a male from disrupting the transport of most of his own sperm from the preceding ejaculation.
Examination of mule deer, Odocoileus hemionus, from Wyoming in Spring, 1974 (8) and in Winter of 1974-75 (8) revealed trypanosomes in all 16 deer. Spring samples showed dividing epimastigotes as well as normal bloodstream forms. Winter samples showed only normal blood stream forms. These differences are considered to be seasonally related.
