A lower molecular weight gamma-chain variant in a congenital abnormal fibrinogen (Kyoto).

A gamma-chain variant with a lower molecular weight than the normal gamma chain was detected in a new congenital abnormal fibrinogen with impaired polymerization of the fibrin monomer and with normal release of fibrinopeptides A and B in a 45-year-old male. Purified fibrinogen analyzed on SDS-polyacrylamide gel electrophoresis under the reduced condition contained an abnormal protein band with an apparent molecular weight of 48,000 compared with the gamma chain with a molecular weight of 50,000. This abnormal protein band was found to be a gamma-chain variant from the molar ratio of A alpha chain:B beta chain:gamma chain:abnormal protein (about 2:2:1:1), with positive staining for carbohydrate and crosslinking ability. Crosslinked fibrin contained three types of gamma-gamma dimers with apparent molecular weights of 94,000 (the same as normal major gamma-gamma dimer), 92,000 and 90,000, and the plasmic digests of crosslinked fibrin in the presence of calcium retained three types of gamma-gamma dimer remnants. This suggests that the abnormal gamma-chain variant has a shorter polypeptide chain not in the NH2-terminal but in the COOH-terminal portion, probably at or near the polymerization site. This patient's two daughters had the same abnormal fibrinogen. This unique inherited abnormal fibrinogen was designated as fibrinogen Kyoto, and the gamma-chain variant as gamma Kyoto.

Blood Coagulation Disorders↗

[Subdural hematoma associated with dural carcinomatosis of gastric cancer and DIC].

We report a rare case of subdural hematoma associated with dural carcinomatosis of gastric cancer and DIC. Such subdural hemasma is supposed to be caused by tumor cell embolism in the dural vessels and DIC. Most cases show acute clinical symptoms such as consciousness disturbances, and their prognosis is extremely poor.

[Congenital abnormalities related to coagulation and the fibrinolytic system in patients with vein thrombosis of the lower extremities].

Plasma concentrations of antithrombin III (AT III), plasminogen (Plg), and protein C (PC) were assayed in 28 patients with venous thrombosis of lower extremities without distinct underlying disorders. Three abnormalities were found in 5 cases (17.8%) in relation to coagulation and fibrinolytic system (3 with congenital AT III deficiency, one each with dysplasminogenemia and congenital PC deficiency). In the patients with either one of these abnormalities, characteristic features of thrombosis are summarized as follows; 1. early onset of clinical symptoms. 2. high frequency of superficial vein thrombosis. 3. high recurrence rate. 4. high frequency of pulmonary embolism.

[Experience with microsurgical epididymovasostomy (specific tubule technic: end-to-end anastomosis by the Silber method)].

Here, we report 2 cases of epididymovasostomy operation in which good results were obtained. These operations were done for patients whose complaints were male sterility due to obstructive azospermia after bilateral epididymitis. The method of this operation was reported by Silber in 1978 as "specific tubule technique", and excellent results have been reported. Herein, we introduce this technique perhaps for the first time in Japan, and the characteristics of this method are considered. The important factors and characteristics of this method are that it is very reasonable with a high success rate, it is more difficult than microsurgical vasovasostomy, it is a time-consuming operation. Epididymovasography should not be done preoperatively but intraoperatively, if possible and chronic epididymitis should be ruled out by serological study and culture study.

Corneal endothelial cell damage associated with intraocular gas tamponade during pars plana vitrectomy.

A prospective study was conducted of corneal endothelial damage associated with intraocular gas tamponade during pars plana vitrectomy performed in 44 patients. In addition to vitrectomy or combined vitrectomy-lensectomy, 25 patients (the gas-treated group) underwent intraocular gas tamponade, which was not performed in 19 patients (the control group). The central corneal endothelium was examined with a specular microscope preoperatively and 3 months postoperatively. Endothelial changes were quantitated by computerized morphometric analysis of individual cells. A total of 20 eyes were phakic after vitrectomy, 13 eyes in the gas-treated group and 7 eyes in the control group. The mean endothelial cell loss for the gas-treated group (1.9%) did not differ significantly from that for the control group (1.8%). Twenty-four eyes were aphakic after vitrectomy, 12 eyes in the gas-treated group and 12 eyes in the control group. The gas-treated group had a significantly greater cell loss (28.3%) than those in the control group (16.3%).

A case of acute renal failure associated with type A acute hepatitis responds dramatically to plasmapheresis.

A case of acute renal failure (ARF) associated with type A acute hepatitis followed by a dramatic response to plasmapheresis is described. A female patient was transferred to our hospital with a diagnosis of ARF associated with acute hepatitis. Since radioimmunoassay for IgM antibody to hepatitis A virus showed a positive reaction, recent infection with hepatitis A virus was suspected. Hemodialysis immediately performed after admission was not effective. Plasmapheresis was started on the 5th hospital day and there was a dramatic improvement in ARF and hepatitis. It is concluded that plasmapheresis is effective in the treatment of ARF associated with liver disease if it is applied in the early stage.

[Recent progress and problems in bladder-sparing operations in bladder tumors].

Retrospective analysis of the bladder tumor patients was performed to reveal the clinical results of partial cystectomy or transurethral resection (TUR). The observed 3-, 5-, 10-year survival rates after 143 partial cystectomies indicated for the first tumors were 66.2%, 57.1% and 41.5% respectively. To obtain satisfactory results, however, the operation should be indicated for pT2 or G0-G1. Some of the G2 tumors could also be the candidates for this procedure, but the stage of the disease must be below T2. At present, all these tumors could be well controlled by TUR, and this statement was confirmed by the study of clinical results obtained by TUR. The analysis also revealed a poor outcome in 6% of the low grade or low stage tumors and it increased to 25% in high grade and pT2. The complete cure of the high stage or high grade tumor is still difficult, but to improve the survival rate, radical surgery should be employed more positively instead of partial cystectomy. The reasons for this conclusion are also discussed.

The effect of radial keratotomy on the corneal endothelium.

Morphometric analysis of the corneal endothelium was performed on 11 eyes of patients who underwent anterior radial keratotomy with the contralateral eye serving as the controls. We analyzed cell density, variation in cell size (polymegethism), and cell shape (pleomorphism) by computer analysis of central and midperipheral specular micrograms one year after surgery. The central endothelial density decreased from 2,503 to 2,419 cells/mm2 (3.3% decrease). The coefficient of variation in cell size (polymegethism) was 0.290 preoperatively and 0.309 postoperatively centrally. Central hexagonality was reduced centrally from 61.4% preoperatively to 56.8% in the operated on eye. These differences were not statistically significant (P greater than .05). In a separate subgroup of six patients, midperipheral specular microscopy under and between incisions disclosed a similar pattern of mild cell density decrease and morphometric remodeling one year after surgery. No morphometric characteristic was significantly different from the central control values (P greater than .05), suggesting that the corneal endothelial monolayer had stabilized one year after radial keratotomy.

Homozygous protein C deficiency combined with heterozygous dysplasminogenemia found in a 21-year-old thrombophilic male.

A congenital, probably homozygous deficiency of protein C combined with a heterozygous molecular abnormality of plasminogen was found in a 21-year-old male who had been suffering from recurrent venous thrombosis since the age of 14. Although the homozygous deficiency of protein C has been reported to be closely associated with fatal purpura fulminans or severe and massive thrombotic diseases during the neonatal period, the patient had survived the neonatal period without any clinical manifestations relevant to thrombosis. The patient apparently inherited the genetic abnormality of protein C deficiency from both of his parents and that of abnormal plasminogen from his father.

Angina pectoris before and during acute myocardial infarction: relation to degree of physical activity.

One hundred ninety-seven patients with a history of acute myocardial infarction (AMI) were interviewed to evaluate the character of angina pectoris relative to physical activity before AMI and at the onset of AMI. Ninety-two patients had no angina before AMI and 105 had angina. Among the 105 patients with angina, 58 had chronic stable angina that did not change before AMI, 22 noted worsening of symptoms within 2 weeks before AMI, and 25 had onset of angina within 2 weeks before AMI. In the 92 patients without angina before AMI, AMI occurred during heavy exertion in 10 (11%), mild exertion in 43 (47%), at rest in 28 (30%), and during sleep in 11 (12%). In the 58 patients with chronic stable angina, 47 had angina during exertion, 7 during rest and 4 during both. However, subsequent AMI occurred during heavy exertion in 9 (15%), during mild exertion in 16 (28%), at rest in 25 (43%), and during sleep in 8 (14%). In the patients without angina, or with chronic stable angina without worsening of symptoms, AMI occurred unpredictably or differently from the mode of physical activity precipitating angina before AMI.
