[Instillation chemotherapy with peplomycin in superficial bladder cancer: clinical study on the effects of the direct antitumor activity and of the prophylaxis of recurrence].

Two kinds of clinical studies for intravesical instillation chemotherapy of peplomycin (PEP) were carried out, one to evaluate the direct antitumor activity in superficial bladder cancer (study I), and the other to evaluate the prophylactic effect on recurrence of bladder cancer after transurethral resection (study II). In study I, 22 patients with low stage bladder cancer were entered, and 16 of them were eligible for evaluation of efficacy. Twelve patients were treated twice a week with intravesical instillation of 20 mg of PEP/20 ml of saline for 4 weeks. Four patients were treated in the same way with 40 mg of PEP/20 ml of saline. Among the patients treated with 20 mg of PEP, two showed complete remission and one showed partial remission. Of the patients treated with 40 mg of PEP, one showed partial remission. Thus, the overall response rate was 25%. The tumors of the two complete responders were so tiny that they might have been damaged mechanically by the biopsy forceps. Therefore, we considered that the instillation chemotherapy of PEP at a dose of 20 or 40 mg/20 ml of saline had little effect on superficial bladder cancer. In study II, we performed a randomized trial designed to compare the recurrence rate after transurethral resection (TUR) alone with that after TUR followed by prophylactic instillation of PEP. Ninety-four patients newly diagnosed as having low stage bladder cancer were entered in the study. They were divided into three groups after TUR. Group A consisted of 38 patients who were treated with weekly instillation of 20 mg of PEP/20 ml of saline for 8 weeks.(ABSTRACT TRUNCATED AT 250 WORDS)

[Analysis of human mitochondrial DNA in hepatocellular carcinomas].

Based on the following 3 points: 1) tumor proliferation is energy-dependent, 2) mitochondrial energy-production system is dominant for cell growth, and 3) liver mitochondria (mt) possess their own DNA and RNA synthesizing some of their own proteins including respiratory enzymes such as cytochrome oxidases, a possible relationship between mutations of mt-DNA and clinical status of cell proliferation was examined in 10 HCC patients who underwent liver resection. Mt-DNA at the cancerous and the noncancerous portions of 1g resected liver specimens were separated from the nuclear DNA, and then digested with Hinf I endonuclease. DNA filters were made of the digested mt-DNA fragments on the agarose and polyacrylamide gel. The filters were hybridized with a nick-translated 32P-labeled DNA fragments. In two cases, abnormal mt-DNA were detected. In the first case, the tumor was the massive type and grew rapidly invading the bile duct. One restriction fragment of 3.0 Kb of the cancerous and non cancerous portion became larger by 60 bp. In the second case, regarded as metachronous multicentric HCC, the second largest band of the 3.4 Kb fragment of the cancerous portion showed a wider range but not of the noncancerous portion. The former change may indicate polymorphism but the latter indicates an occurrence of the mutation of mt-DNA. Further studies are required, including examinations on the rest of mitochondrial fragments.

[Endocrine therapy of prostatic carcinoma with slow release (depot) formulation of the LH-RH analog ICI 118630 (Zoladex)].

To investigate the clinical efficacy, safety and endocrinology of ICI 118630 (Zoladex) depot formulation at 3 different dose levels (0.9, 1.8 and 3.6 mg), 90 patients were randomized to receive either one of the 3 doses from April, 1985 to March, 1986 in 28 centers. The depot preparation was injected subcutaneously every 4 weeks 3 times (for up to 12 weeks). Clinical efficacy was evaluated in terms of tumor response and overall subjective response. In 70 patients eligible for tumor response evaluation, 14 out of 22 (63.6%) in the 0.9 mg group, 11 out of 23 (47.8%) in the 1.8 mg group, and 17 out of 25 (68.0%) in the 3.6 mg group showed clinical improvement, that is, either complete response or partial response. In 72 eligible patients for overall subjective response evaluation, clinical subjective improvement was observed in 75.0, 81.8 and 88.0% of the patients in the 3 groups, respectively. There was no significant difference between the groups. As for endocrinology, there were 75 eligible patients. Endocrinological effect was observed in 23 out of 25 (92.0%) in the 0.9 mg group, 100% in both 1.8 mg and 3.6 mg groups. There was no significant difference between the groups. Castration was achieved by week 3.5 +/- 1.7 of therapy on average and by week 2 in the earliest case. There was no significant difference in incidence of side effects between the 3 groups: 5 out of 26 (19.2%) in the 0.9 mg group, 8 out of 29 (27.6%) in the 1.8 mg group, and 2 out of 30 (6.7%) in the 3.6 mg group. Flares presented as an increase in bone pain in 2 and as ureteric obstruction in 2 all in the 1.8 mg group but none in the other 2 dose groups. These flares disappeared on further treatment with Zoladex. These patients showed a clinical-response. The blood level of Zoladex was dose dependent, reaching its peak at week 2 of therapy in all 3 dose groups. There was no evidence of accumulation. Since these results demonstrate that 3.6 mg produces medical castration earlier, it may well be considered as an optimal dose in men.

[Clinical efficacy of long-term treatment with LH-RH analogue, ICI 118630 (Zoladex), in prostatic cancer patients. The Zoladex Multicenter Study Group].

The Zoladex (ICI 118,630) multicenter trial included 149 patients with stage B +o D prostatic cancer recruited from 1984 to date. Of them 53 clinical responders on the treatment for 40 weeks or longer were subjected to assessment of antitumour response, overall subjective response, endocrinological response, safety and usefulness. The responders consistently showed a clinical response as evidenced by antitumour response in 32 of 50 patients (64.0%) at week 12 and in 35 of 51 (68.6%) at week 40, and overall subjective response in 42 out of 47 patients (89.4%) at both week 12 and week 40. Endocrinologically, all of the 47 eligible patients maintained a 40-week or longer response. Adverse reactions were observed in 19 out of 73 patients (26.0%), subdivided by the time of occurrence as 15/73 (20.5%) up to week 12, 3/71 (4.6%) between week 12 and week 40, and 5/53 (9.4%) in and after week 40. No patient required the discontinuance of treatment. Usefulness of the drug was observed in 51 out of 52 patients (98.1%). The results indicated that Zoladex in a once monthly regimen may be of great advantage to elderly patients with prostatic cancer, and allow an improved patient compliance: Zoladex may not only produce clinical remission but also improve the quality of life.

[Endocrine therapy for prostatic carcinoma--the clinical trial to compare the efficacy of LH-RH analogue, ICI 118630 (Zoladex) with castration or estrogen].

A multicenter randomized clinical trial was carried out between May, 1986 and May, 1987 involving 82 patients with stage B-D prostatic carcinoma from 29 centers. The clinical efficacy, endocrine effect, safety and usefulness of the luteinizing hormone-releasing hormone (LH-RH) analogue and other endocrine manipulations in the treatment of prostatic carcinoma. Zoladex depot containing 3.6 mg of ICI 118,630, an LH-RH analogue, was administered every four weeks 3 times in total. Patients in the control group received either 300 mg of diethylstilbestrol diphosphate orally daily for 12 weeks or orchidectomy. An antitumor effect (CR + PR) was observed in 21 of the 33 patients (63.6%) in the Zoladex group and in 22 of the 33 (66.7%) in the control group, showing no significant difference between the two groups. There was no significant difference in overall subjective response either; 21 of the 24 (87.5%) in the Zoladex group and 24 of the 30 (80.0%) in the control group. In both groups, 100% endocrine effect was obtained as shown by achievement of castration in all patients. Adverse reactions were observed in 14 of the 39 (35.9%) patients treated with Zoladex as compared with 19 of the 34 (55.9%) control patients, resulting in no significant difference in the incidence between the two groups. These adverse reactions were not so severe as to require withdrawal from the study. In both groups, the treatment was assessed as slightly or more useful in 29 of the 33 (87.9%) patients. From these results, it is concluded that Zoladex, 3.6 mg depot, is a useful drug for treatment of prostatic cancer, having clinical efficacy and endocrine effects comparable to those of the conventional endocrine manipulations, being safe, and causing less physiological and psychological pain.

[Association of cerebral aneurysm and dural arteriovenous malformation in the anterior cranial fossa: a case report].

A case of dural arteriovenous malformation (AVM) in the base of the anterior cranial fossa associated with a ruptured anterior communicating aneurysm was reported in a 67-year-old man. The dural AVM was fed by the left anterior ethmoidal artery and drained by the left frontal ascending vein with a varicose dilatation. The aneurysm was clipped and the dural AVM was removed in two stages. Dural AVM in the base of the anterior cranial fossa is rare, and we could find only 17 previous reports. Two of them presented with exophthalmos, whereas the remaining 15 cases presented with an intracranial hemorrhage from rupture of the dural AVM. An association of an aneurysm with the dural AVM in this location has not been reported previously.

[Congenital vesical diverticulum in a child].

A case of congenital vesical diverticulum in childhood is reported and 28 other cases in children from the Japanese literature are reviewed. A 9-year-old boy presented with hematuria and two-phase micturition. Excretory urogram and voiding cystourethrogram showed a diverticulum of the bladder on the left side and left vesico-ureteral reflex. Diverticulectomy and left ureteroneocystostomy (by Paquin's method) were performed. Post-surgical recovery was very good. All 28 cases we reviewed were in males whose average age was 6 years and 2 months. Their chief complaints consisted of dysuria, urinary infection and hematuria. The two-phase micturition appeared to be rather rare.

[Endocrinological profile of human spermatogenic impairment--evaluation of function of Leydig and Sertoli cells in vivo or in vitro studies].

In vivo and in vitro studies were performed to determine the function of Leydig and Sertoli cells of the human testis with various degrees of spermatogenic impairment. The increases in basal and peak serum levels of luteinizing hormone (LH) and follicle stimulating hormone (FSH) after LH-RH administration correlated with the degree of impairment of spermatogenesis, and the basal peripheral blood levels of testosterone and that after human chorionic gonadotropin (hCG) administration for patients with moderate or severe impairment were significantly lower than the values for those with mild impairment. The concentration of testosterone in the internal spermatic vein of varicocele patients with or without hCG treatment did not differ between in mild and moderate impairment. In studies on cultured Sertoli cells, the production rate of plasminogen activator in patients with severe impairment was significantly lower than that in patients with moderate or mild impairment. The decrease in testicular high-affinity binding site for FSH correlated with the degree of hypospermatogenesis found in idiopathic male infertility, but, on the contrary, the hCG (LH) receptors showed no correlation with the degree of impairment of spermatogenesis. In the investigation of the relationship between testicular FSH receptors and the effectiveness of human menopausal gonadotropin (hMG)-hCG treatment on idiopathic male infertility, the presence or absence of testicular FSH receptors predicted the responsiveness to the treatment.

[Report of a case of Kearns-Sayre syndrome associating with Isaacs-Mertens syndrome].

The patient is a woman who was born in 1936. Her father died of a valvular heart disease and her mother died of apoplexy. She was healthy until 1976 when she noticed stiffness of extremities and so she came to our hospital. At this time, her symptoms disappeared without any specific therapy. In 1979, she entered this hospital because of generalized edema. She was diagnosed as heart failure and treated effectively with frusemide. In 1982, stiffness and gait disturbance aggravated and entered the hospital. At this hospitalization, she was diagnosed as Kearns-Sayre syndrome. After the discharge, she was followed periodically. In May 1983, it was found that she could not abduct and spinate her thumbs. This abnormal hand posture was seen constantly thereafter although the severity of it varied. In April 1985, she was admitted to this hospital because of vomiting and the aggravation of the stiffness. EMG study disclosed spontaneous continuous motor unit discharges. F wave was exaggerated in both frequency and amplitude. The spontaneous activity of the thenar muscle was reduced by blocking the median nerve at the wrist. The patient was administered 250 mg of carbamazepine. Abnormalities of hand posture and EMG were markedly ameliorated by the regimen. We considered that the patient was a rare case of Kearns-Sayre syndrome which was complicated by a syndrome of continuous muscle fiber activity (Isaacs-Mertens syndrome). We speculated that disorders of energy metabolism of motor neurons and inter neurons in the spinal cord might causally relate to spontaneous neuronal discharges.(ABSTRACT TRUNCATED AT 250 WORDS)

Detection of early myocardial infarction in formalin-fixed, paraffin-embedded tissue.

Whether the early infarct area in formalin-fixed, paraffin-embedded tissue could be delineated by the immunohistochemical method using myoglobin-antibody was studied in 23 pig hearts without collateral circulation. Five hearts were examined at 20 minutes, 2 hours, 4 hours, and 6 hours after occlusion of the distal one third of the left anterior descending coronary artery, respectively. Three pigs were killed 24 hours after occlusion. Heart rate and aortic pressure before and after occlusion did not change in any groups. The subepicardial and subendocardial regional blood flows were reduced to almost zero in all hearts after occlusion (0.88 +/- 0.10 to 0.02 +/- 0.02 mL/g/min). Slight myoglobin defects in the ischemic tissue were noted in the five pigs examined 2 hours after occlusion and definite myoglobin defects were detected in all pigs examined at 4, 6, and 24 hours after occlusion. Nitrotetrazorium blue stain of myocardial tissue before formalin fixation showed slight demarcation of the ischemic area at 4 hours after occlusion and definite demarcation at 6 and 24 hours after occlusion. Slight demarcation was noted at 2 hours after occlusion in Masson trichrome stain and 4 hours after occlusion in the hematoxylin-eosin stain. However, definite demarcation of the ischemic area was seen in Masson trichrome stain only at 24 hours after occlusion and was not noted in hematoxylin-eosin stain even at 24 hours after occlusion. Our previous electron microscopic study revealed that, in the pig heart, irreversible cellular damage was transmurally seen at two hours after occlusion of the coronary artery. Therefore, a definite myoglobin defect reflects irreversible cellular damage such as infarction.(ABSTRACT TRUNCATED AT 250 WORDS)

[Two-dimensional and Doppler echocardiographic findings of a sudden death case due to multiple pulmonary thromboses].

This is a case report presenting interesting findings on two-dimensional and Doppler echocardiograms due to multiple pulmonary thromboses. A 67-year-old woman had a history of surgery for colon polyps in August, 1986. After surgery, she suffered from tachycardia and dyspnea, and was admitted to our hospital because of a loss of consciousness. Sinus tachycardia, complete right bundle branch block and T-wave inversion in leads III, aVF and V1-4 were observed on her electrocardiogram. Cardiomegaly was noted on her chest radiograph. On the third hospital day, echocardiography was performed. On two-dimensional echocardiography, marked right ventricular dilatation with hypokinesis of the wall, and a flattened interventricular septum were observed on the short-axis view, and M-mode echocardiograms showed disappearance of the E wave and delay of the opening of the tricuspid valve, resulting in a monophasic triangular pattern. Doppler echocardiography showed the tricuspid valve flow to be delayed, mainly observed during atrial systole. These findings indicate decreased right ventricular compliance. The patient died on the 6th hospital day, and multiple new and old thrombi were found in the pulmonary artery at autopsy.

[The clinical value of urinary polyamine analysis in urological disease].

To study the clinical usefulness of the determination of urinary polyamine levels, voluntary urine of several urological diseases including 56 bladder tumor patients was analyzed by high performance liquid chromatography. The obtained values were adjusted by the concentration of urinary creatinine and expressed as the unit of mumol/g creatinine (mumol/g Cr) From the measurement of 8 normal adults, the normal upper limit of each polyamine was decided by mean + 2SD, and the limit for total polyamine was 59.1 mumol/g Cr, putrescine 38.1 mumol/g Cr, spermidine 16.6 mumol/g Cr and spermine 9.2 mumol/g Cr, respectively. In the patients with non-neoplastic benign urological disease, the polyamine levels were statistically not different from those of the normal adults. In the case of bladder tumor, the urinary levels of total polyamine, putrescine and spermine were significantly elevated compared with the control group. The true positive rate of this determination in bladder tumor patients was 26/56 (46%) by total polyamine level, 21/56 (38%), by putrescine level, 11/56 (56%) by spermidine level and 16/56 (29%) by spermine level. Grade or stage of the bladder tumor did not have any significant correlation with the urinary polyamine level. This determination would not be included in routine clinical examinations due to the difficulty of measurement, difference of urine sampling and lack of high sensitivity and specificity.

Sudden death in hypertrophic cardiomyopathy, a guideline to prevention in daily life.

To establish guidelines for individuals with hypertrophic cardiomyopathy (HCM) to follow in their daily lives and minimize the chance of sudden death (SD), circumstances of SD in 24 autopsied definite HCM cases were compared with those with 905 autopsied cases of other heart diseases. Circumstances of symptoms experienced in daily activities of 35 age-matched surviving HCM cases were also studied. In the autopsied HCM cases, SD occurred during exertion more frequently (79.2%) than in the other autopsied cardiac patients (22.7%). Sudden changes in pace or movement accompanied by mental excitement, such as hurrying to board a train, were more frequent causes of SD in HCM (47.4%) than in other cardiac patients (8.3%). In the surviving HCM cases, symptoms of distress occurred frequently during very hard work or with sudden rapid movement. Thus, one probable daily guideline for HCM sufferers would appear to be not only to eliminate very hard work but also to lead mentally and physically quiet and relaxed life.

An apparently higher molecular weight gamma-chain variant in a new congenital abnormal fibrinogen Tochigi characterized by the replacement of gamma arginine-275 by cysteine.

A gamma-chain variant with an apparently higher molecular weight than the normal gamma-chain was detected in a new congenital abnormal fibrinogen with impaired polymerization of the fibrin monomer and with normal release of fibrinopeptides A and B in a 51-year-old male. Purified fibrinogen analyzed on sodium dodecyl sulfate-polyacrylamide gel electrophoresis (SDS-PAGE) under the reduced condition in the system of Laemmli contained two protein bands in the gamma-chain region (molecular weight, 50,500 as compared with 50,000 for the normal), both with normal crosslinking ability. The presence of two types of gamma-chains was more clearly detected when reduced and carboxymethylated fibrinogen was analyzed by SDS-PAGE or when reduced fragment D2 was analyzed on SDS-PAGE followed by Western blotting, and identified by positive staining for anti gamma-chain monoclonal antibody. Cyanogen bromide- or lysylendopeptidase-cleavage of purified gamma-chains analyzed on reverse-phase high performance liquid chromatography showed the decrease of one peptide compared with the normal and the appearance of an abnormal peptide peak. Amino acid sequence analysis demonstrated that the gamma arginine-275 of gamma-chain variant was replaced by a cysteine. These data suggest that some regions or conformations containing gamma 275 will affect the polymerization of fibrin monomers. The propositus' two daughters had the same abnormal fibrinogen. This unique inherited abnormal fibrinogen was designated as fibrinogen Tochigi, and the gamma-chain variant as gamma Tochigi.
