Endothelial wound-healing of monkey cornea: fluorophotometric and specular microscopic studies.

The process of wound-healing in the corneal endothelium after transcorneal freezing was studied in the monkey, using the techniques of fluorophotometry, pachymetry and specular microscopy. After intramuscular injection of fluorescein, the dye exchange between the cornea and aqueous humor was studied by fluorophotometry and the results were analyzed on the basis of a two-compartment model: the transfer coefficient (kac) and the distribution ratio of the dye between the aqueous humor and the cornea (rac) were calculated, and the endothelial permeability coefficient (Kac) was computed using the thickness of the corneal stroma. The corneal thickness was measured at its center (CT) with a modified pachymeter. The endothelial cell morphometry was carried out at the central cornea using a wide-field contact-type specular microscope and a computerized image analyzer: the mean cell size (CS) was calculated. The values of Kac, kac, rac, CT and CS in 16 normal uninjured corneas were 3.35 +/- 0.91 X 10(-4) cm min-1, 0.56 +/- 0.15 hour-1, 0.60 +/- 0.17, 0.40 +/- 0.02 mm and 290 +/- 35 micron2, respectively (mean +/- SD). The values of Kac, CT and CS increased markedly after freezing and decreased gradually with passage of time. The Kac and CT recovered to normal level by the 6th and 4th week after freezing, respectively. The positive significant correlation was found between the values of Kac and CT (r = 0.84, P less than 0.001), and between those of kac and CT (r = 0.58, P less than 0.001). On the contrary, no significant correlation was found between Kac and CS. The CS did not recover completely and remained about 32% larger than the values of the normal eyes even 30 weeks after freezing. Transmission and scanning electron microscopy of the endothelium revealed that the junctional complexes were reformed soon after the endothelial cells had made contact with each other, and the complexes became almost normal about 4 weeks after freezing. Normalization of the junctional complexes appeared to coincide with recovery of the Kac and CT.

A case of localized Darier's disease.

A case of localized Darier's disease was reported. The patient was a 36-year-old Japanese woman, and hyperkeratotic papules in zosteriforme arrangement were persistently noted on the left forehead since the age of 20 years. Histological appearance was same as in classical Darier's disease. Skin atrophy due to long-term application of corticosteroid ointment was observed. Aromatic retinoid ointment was effective in the treatment of the lesions. The literature on localized Darier's disease is reviewed.

Biosynthetic Met-hGH: lack of immunoreactivity for other pituitary hormones.

The immunoreactivities of 5 hGH preparations were examined in RIA systems for hPRL, hACTH, hLH, hFSH and hTSH. All preparations derived from pituitary extracts showed distinct displacements in almost all RIA systems when added 10(3) to 10(4) times larger amounts than the proper hormones. On the other hand, biosynthetic methionyl-hGH did not manifest any immunoreactivity in any system with comparable amounts. These results indicate that the displacements caused by hGH preparations in RIA systems for other pituitary hormones are solely due to contamination, and not due to intrinsic cross reaction. In in vitro and in vivo studies using extractive preparations of hGH one should be cautious about contamination of other pituitary hormones.

Plasma LRH levels in chronic renal failure before and during haemodialysis.

Endogenous immunoreactive luteinizing hormone - releasing hormone (LRH) in plasma was determined in 6 male and 7 female patients with chronic renal failure before and during haemodialysis. Basal plasma LRH levels ranged from 5.8 to 23.0 pg/ml and, in 11 out of 13 patients, were above the levels seen in healthy subjects (less than 7 pg/ml for men and less than 8 pg/ml for women). This immunoreactivity was eluted iun identical fractions with synthetic LRH and plasma extracts from climacteric women on Sephadex G-25 chromatography, and the dilution gave a displacement curve parallel to the standard. Within 5 h of haemodialysis, these high LRH levels declined into the normal range. The concentrations of LH in plasma in these patients were also elevated, but those of testosterone in male patients were decreased. These results suggest 1) that elevated plasma LRH reflects decreased feedback inhibition by primary gonadal failure and might in turn be responsible at least in part for high concentrations of plasma LH in chronic renal failure, and 2) that plasma LRH is mainly not bound to plasma proteins.

[Surgical management of the gastroesophageal varices--experience with 118 cases].

Between June 1968 and May 1982, 118 patients with esophagogastric varices due to portal hypertension were treated and one hundred thirteen of these underwent surgical interventions. Ninety-eight patients had liver cirrhosis, fourteen were diagnosed idiopathic portal hypertension and one was diagnosed extrahepatic portal obstruction. More progressed impairment of hepatic function and severe manifestation of clinical symptoms were observed in the cirrhotics, compared with those in non-cirrhotics. We preferred the one-staged combined procedure of transthoracic esophageal transection, transdiaphragmatic splenectomy and paraesophagogastric devascularization, because of its therapeutic effectiveness and technical feasibility. Splenectomy with or without proximal gastrectomy was chosen for the remaining small number of the patients. While the overall operative mortality was 12.4%, 14 patients (13.5%) succumbed within a month after esophageal transection. These procedures were contraindicated for the patient with uncontrollable ascites, jaundice and encephalopathy and should be carried out electively because of high mortality of emergent operation.

Wound-healing of corneal endothelium in monkey: an autoradiographic study.

The mitotic activity of the corneal endothelium during wound-healing was studied in 8 eyes of 5 cynomolgus monkeys. By transcorneal freezing, the endothelium was damaged at the corneal center over an area with a diameter of 2.5 mm. At various intervals, 3H-thymidine was injected into the anterior chamber and its incorporation into the endothelial cells was studied by autoradiography. On flat preparations of the endothelium, the total number of cells labeled with 3H-thymidine was counted on all specimens. The labeling of the cells occurred exclusively within the zone of about 2 mm from the center of the wound. The maximum labeling occurred 2 days after freezing and it decreased abruptly after 4 days, when endothelial covering of the central denuded area had been completed. No labeled cells were encountered after 30 days. The percentage of labeled cells to all cells present in a given area was computed and its regional distribution studied. On the second day after freezing, the maximum percentage of 12-19% was found in the center of the wound in one cornea and about 2 mm from the center in another cornea. On the third day, the maximum percentage of only 3-7% was found in the center and it decreased toward the periphery. It was thought that the number of labeled cells was too small to cover the damaged area by cell multiplication. It is suggested that the endothelial defect in the monkey cornea is covered mainly by migration of the adjacent cells.

Relationship of changes in serum concentrations of prolactin and testosterone during dopaminergic modulation in males.

To evaluate the effect of PRL on the male pituitary-gonadal system, serum concentrations of PRL, testosterone, LH and FSH were determined in healthy young men daily before, during, and after 3-day oral administration of bromocriptine, metoclopramide or sulpiride. Bromocriptine (2.5 mg as a single dose) caused, concurrently with a marked suppression of serum PRL, a significant increase of serum testosterone and a transient decrease of serum LH. The changes of PRL and testosterone were negatively correlated. With metoclopramide (10 mg q.i.d.) serum PRL was increased and testosterone inversely decreased. There was no change in LH and FSH. Sulpiride (50 mg q.i.d.) evoked the elevation of serum PRL and LH, but no change in testosterone. A significant increase in serum concentration of testosterone was also observed in a patient with PRL-producing pituitary tumour and four out of seven patients with acromegaly during bromocriptine treatment. These results suggest an inhibitory effect of PRL on testosterone secretion at the gonadal level, or direct dopaminergic stimulatory control of testosterone secretion.

Wound-healing of the corneal endothelium in the monkey: a morphometric study.

Topographical aspects and the time course of the healing processes in the corneal endothelium were studied in 31 cynomolgus monkey eyes. The corneal endothelium was damaged at the center by transcorneal freezing, using a probe with a tip diameter of 2.5 mm. Flat preparations of the endothelium stained with silver nitrate were made at various time intervals after freezing. Panoramic photographs of the corneal endothelium along the entire length of one meridian were made, and the sizes of individual cells were calculated by a computerized image analyzer. The size and shape of the endothelial cells showed marked changes around the wound edge, and the cells migrated to the duodenal area, which was covered by the endothelial cells in 3 days. Subsequently, the cell size distribution recovered toward the normal pattern, but multinucleated giant cells persisted in the center of the wound after one year. A slow enlargement of the cells occurred in the periphery, where final average cell size was about 25% larger than the original level. It was thought that the cells of a very wide area were mobilized in the wound-healing processes and that cell proliferation probably played a minor role.
