A 55-year-old man consulted us on December 3, 1986 with the chief complaints of left leg pain, disturbance in gait and dysuria. On digital rectal examination his prostate was found enlarged to a hen's egg size and increased in consistency. He was admitted to the hospital under the suspicion of prostatic cancer on December 8, 1986. Scout kidney-ureter-bladder X-ray revealed extensive osteolytic lesions in the left iliac and pubic bones as well as in the sacrum. Needle biopsy of the prostate demonstrated moderately differentiated adenocarcinoma, leading to a diagnosis of prostatic cancer with osteolytic bone metastases. Bone scintigraphy showed increased radioactivity uptake by the left iliac and pubic bones. Pelvic CT disclosed large tumor masses in the left ilium and sacrum, which on bone biopsy were identified as poorly differentiated adenocarcinoma. Endocrine therapy with estramustinphosphate and castration was performed along with transurethral resection of prostate. At week 16 after initiating the therapeutic regimen the patient was entirely free from disturbance in gait and the tumor mass of the left iliac bone had disappeared almost completely on the computed tomogram at week 18, although osteolytic lesions still persisted on the x-ray. This case deserves special note because endocrine therapy markedly reduced the size of the bone tumors, in spite of osteolytic bone metastases of prostatic cancer with computed tomographic evidence of large metastatic tumor masses.
To detect myocardial lesions in patients with subendocardial (nontransmural) infarction, myocardial perfusion images of thallium-201 (T1-201), using single photon emission computed tomography, were obtained in 17 patients during and three hours after coronary vasodilatation induced by the administration of dipyridamole. Controls consisted of 28 patients with transmural infarction and 11 with angina pectoris. Dipyridamole was infused intravenously at a rate of 0.142 mg/kg per min for four min, and the washout rate of T1-201 was calculated by the segmental ROI method. The results were as follows: 1. Many patients with nontransmural infarction exhibited various T1-201 myocardial perfusion images. 2. The prevalence of redistribution among patients with nontransmural infarction was 88.2% (15 of 17), and its reverse redistribution was 64.7% (11 of 17). Every subject exhibited a number of abnormal T1-201 myocardial perfusion images. 3. In patients with nontransmural infarction the occurrence of reverse redistribution images was significantly higher (64.7%) compared to those with transmural infarction (10.7%), and the occurrence of redistribution images was significantly higher (88.2%) compared to those with angina pectoris (45.5%). 4. The abnormal perfusion images most frequently present in the areas corresponding to the sites of ST-T changes on ECG were reverse redistribution (26.7%), redistribution (17.3%), and fixed defect (6.7%) in order. 5. The phenomenon of reverse distribution images was found irrespective of stenotic coronary artery lesions on selective coronary angiography. Most areas corresponding to the sites of reverse redistribution showed ST-T changes on ECG and high washout rates of T1-201. In conclusion, these findings suggest reflections of uneven coronary blood flow and the mixed normal and scarred myocardium in patients with nontransmural infarction.

To study the contribution of alpha 1 antitrypsin (alpha 1AT) and antithrombin III (AT III) to the inactivation of F.XIa in plasma, we developed the assay system of F.XIa-AT III complex according to our method of F.XIa-alpha 1AT complex, and measured the levels of both complexes in the patients with disseminated intravascular coagulation (DIC) derived from various triggers. F.XIa-alpha 1AT complex level was always higher than F.XIa-AT III complex level in each patient, independently on the trigger of DIC, the administration of heparin and the levels of F.XI, alpha 1AT and AT III. These results indicate that alpha 1AT is the main inhibitor of F.XIa in plasma.
A method was established for measuring membrane fluidity of polymorphonuclear leukocytes (PMN) by the excimer-forming lipid technique with pyrenedecanoic acid in flow cytometry. When cells were labeled, the use of 2-25 microM of pyrenedecanoic acid provided similar results. Neither the removal of the unincorporated pyrenedecanoic acid nor adjustment of PMN counts exhibited any effect. By the gate analysis method, membrane fluidity of PMN could be measured with 100 microliters of heparinized whole blood in a short time and results with PMN in whole blood was similar to those with purified PMN. Therefore, purification and count adjustment of PMN could be omitted. By this method, membrane fluidity of PMN, which were treated with membrane fluidizer, was measured successfully. This method could be applied to the study of PMN function in various diseases.

Esophageal ultrasonography (EUS) is a new diagnostic method which allows improved preoperative diagnosis of esophageal cancer. Careful examination of EUS findings and comparison of these findings with operative and pathological findings yielded criteria useful for the determination of tumor depth and lymphnode metastasis. Concerning tumor depth, EUS findings of 30 patients were reviewed and compared with operative and pathological findings. Based on this comparison some diagnostic criteria were then chosen; it was found that the accuracy was around 87% and that the differentiation between intramural and extraluminar invasion could be easily made using these criteria. Regarding the lymphnodes, at the time of operation, the site and size of each lymphnode (a total of 267 nodes) were recorded. These findings were grouped according to the site and size. We found the range of sizes of metastatic lymphnodes to be great and not so different from that of normal nodes. We then compared these anatomical findings with the EUS findings in an attempt to determine appropriate diagnostic criteria, which would include size as well as intrinsic and extrinsic echocharacteristics. When these criteria were used, the accuracy was about 90%. Although this preliminary study must be confirmed by further prospective studies with a large number of patients, we can conclude that EUS allows accurate determination of tumor depth and node metastasis in patients with esophageal cancer.

A 61-year-old male, who had undergone operation of esophageal carcinoma 11 months earlier, was admitted for left flank pain and high fever. Intravenous pyelography showed a space occupying lesion in the upper half of the left kidney. Computed tomographic scan showed an irregular low density area in the upper half of the left kidney. Angiograms revealed a hypovascular mass with encasement vessels in the same site. Left nephrectomy and paraaortic lymph node biopsy was performed. Pathological examination revealed a metastatic squamous carcinoma from the esophageal carcinoma. Chemotherapy was conducted, but he died 2 months later. Autopsy revealed recurrence in the retroperitoneum, and no metastasis in the right kidney. The literature on secondary renal tumor is reviewed.
Twenty-one papillary thyroid carcinomas (PTCs), grouped into predominantly papillary (14 cases), predominantly follicular (5 cases), and extremely follicular, i.e., follicular variant (2 cases) types, were studied in comparison with three cases each of follicular lesions including follicular carcinoma, follicular adenoma, adenomatous goiter and Graves' disease. Histochemical, immunoperoxidase, and electron microscopic analyses demonstrated no remarkable differences between the predominantly papillary and predominantly follicular PTCs, but the presence of common characteristics distinct from those of the follicular lesions. These two types of PTCs showed less glycogen, more mucoid material, more epidermal keratin, less thyroid hormone with relative predominance of T3 over T4, and more interdigitating reticulum cells (IDCs) than most of the follicular lesions. Ultrastructurally, the tumor cells of these PTCs had markedly irregular nuclei, a vesicular chromatin pattern, and small basally located lysosomes, in contrast with the cells in the follicular lesions which had smooth round nuclei, more heterochromatin, and apical or dispersed lysosomes of various sizes. The follicular variant PTCs showed some mixed features, such as glycogen in the follicular portion and mucoid material in metastatic papillary foci, positive keratin and IDCs but greater amounts of thyroid hormone, and a rather intermediate type of ultrastructure with only mildly irregular but vesicular nuclei and large apical as well as small basal lysosomes. These findings cytologically support the WHO definition of papillary carcinoma that includes tumors with variable mixtures of papillary and follicular patterns. However, separate consideration may be necessary with regard to the follicular variant.

Clinical studies were performed on cefuzonam (L-105, CZON), a new cephem antibiotic, as follows. Cerebrospinal fluid (CSF) and serum concentrations. CSF and serum concentrations of CZON were measured in 1 case of septic arthritis without meningitis. One hour after 50 mg/kg intravenous bolus injection, the CSF and serum concentrations were 0.10 and 18.1 micrograms/ml, respectively, and CSF to serum concentration ratio was 0.55%. Clinical efficacy CZON was administered to 15 patients in doses ranging 54.5 approximately 212.4 mg/kg/day (94.1 mg/kg/day on average) t.i.d. or q.i.d. for 4 approximately 12 days (6.5 days on average). Of those patients, 9 were with pneumonia, one each was with bronchitis, with tonsillitis, with septic arthritis, with septicemia, with purulent meningitis and with urinary tract infection. The overall efficacy rate was 100%, i.e., efficacy was excellent in 12, good in 3. Bacteriological efficacy was excellent, i.e., 8 of 8 strains were eradicated. Side effects were observed in 2 cases, i.e., one case with loose stool and another with eruption. Laboratory abnormalities to the drug were not observed during the treatment. The above results suggested that CZON would be a useful antibiotic for treating pediatric bacterial infections.
Clavulanic acid/ticarcillin (BRL 28500) was administered to 36 patients with severe infections accompanying various hematological diseases. The rate of efficacy was 52% on the whole. Adverse reactions were minimal.
