[Beta-blockers and migraine. Efficacy of time-release propranolol versus placebo].

The efficacy and safety of long-acting propranolol (LA.P) 160 mg once-daily in the prophylactic treatment of migraine were tested against placebo in a multicentric, double-blind, randomized study comparing the two groups in a parallel manner over a treatment period of 12 weeks, and following a 4 week-placebo run-in period. Fifty-five out of the 74 patients who entered the trial included at the end of the run-in period. Forty-one patients completed the study. Out of the 14 patients who withdrew from the study, none discontinued because of side-effects. The statistical analysis was done according to the "intention to treat" principle. LA.P was significantly more efficient than placebo in reducing the frequency of migraine attacks (p = 0.01 by variance analysis). LA.P reduced the average number of monthly crises by 48% on day 84. There was a slight but significant reduction of the systolic blood pressure and heart rate in the erect position. There was no significant difference between LA.P and placebo regarding either the number of complaints or the number of side-effects elicited out of an 17 item questionnaire. None of the observed side effects led to a withdrawal of treatment.

[Estimation of PaO2 using pulsatile oximetry and the HbO2 dissociation curve in the premature infant].

An estimation of arterial PO2 from pulse oximetry using oxyhemoglobin dissociation curve. Forty blood specimens were drawn from umbilical arteries of the neonates having a pulse sensor attached to either foot or hand. The sensor was connected to a Nellcor-N100C in twenty cases and to an Ohmeda Biox-3700 for the remaining. Mean birth weight was 1,513 g, mean gestational age 32.4 weeks, and foetal hemoglobin ranged from 54.8 to 100%. Oxyhemoglobin dissociation curves were drawn for each arterial sample to obtain P50 and estimated PO2. The estimated PaO2 was the PO2 corresponding, on the oxyhemoglobin dissociation curve, to the pulse oximeter saturation reading. Mean difference between blood saturation and pulse oximeter saturation of 1.2% was obtained with Nellcor and of 3.9% with Ohmeda. Arterial and estimated PO2 comparison gave a mean + S.E.M. difference of 4.6 +/- 1.3 torr for all cases. This represents less than 8% error in estimating PaO2 from pulse oximetry. We suggest the following polynomial equation Y = 4250 - 10.4X + 0.069X2 for using pulse oximetry (X) to evaluate PaO2 (Y) in premature infants.

[Macrosaccadic oscillations in a case of paraneoplastic myoclonic encephalopathy (author's transl)].

Abnormal ocular movements were observed in a patient with myoclonic encephalopathy secondary to branchial carcinoma. Clinical, cinematographic, and oculographic studies demonstrated the presence of rapid, large amplitude, conjugated, horizontal movements which occurred in bursts. They had a tapered appearance and were triggered by fixing vision and jerking movements, while they were abolished by rest and closing the eyelids. These features are similar to those seen in macrosaccadic oscillations. Other types of oscillating ocular movements are discussed as well as the physiopathological features of this disorder.

[Electro-oculographic study of a case of abolition of horizontal saccades with viscosity of eye movements in hereditary cerebellar degeneration].

A case of familial cerebellar ataxia is reported during the course of which horizontal ocular movements were slow, and "viscous". The electro-oculographic study showed complete abolition of horizontal jerks, induced by either voluntary, automatic (optokinetic nystagmus), or reflex (vestibular) stimuli. Slow movements, however, were normal, and voluntary vertical jerks were conserved while being slightly slowed towards the top. The physiopathological interpretation of these facts remains a point for discussion. Results from animal experiments and neuropathological studies suggest that the abolition of jerks in such cases is probably due to loss of facilitating influences from the cerebral cortex and central grey nuclei.
