[Nutritional and defunctionalized jejunostomy in the chemotherapeutic treatment of gastric non-Hodgkin's lymphoma. Preliminary experience].

The authors propose a jejunostomy on an omega loop distal to the Treitz ligament as a treatment for unresectable gastric lymphomas. The rationale is to provide nutritional support during chemotherapy in a situation when the stomach is defunctionalised. As assessed by clinical, morphological and biological parameters, the association of chemotherapy and jejunostomy may represent a valid therapeutical strategy for patients considered unresectable.

Granulocyte colony stimulating factor (G-CSF) as adjunct therapy in relapsed-resistant high-grade non-Hodgkin's lymphoma.

BACKGROUND: Granulocyte colony stimulating factor (G-CSF) safely stimulates the production of neutrophils in normal people and neutropenic patients. Phase II studies have been completed that explored the role of G-CSF in either abrogating or accelerating recovery from chemotherapy-induced neutropenia. METHODS: Fourteen patients with resistant/relapsed high-grade non-Hodgkin's lymphoma were treated with a dexamethasone, cytarabine and cisplatin (DHAP) regimen, and with G-CSF between chemotherapeutic courses. RESULTS: All patients showed a rise in neutrophil count; none was neutropenic when hospitalized for subsequent DHAP courses. Furthermore, the interval between cycles was decreased in 11 of the 14 patients. The overall response rate was 86%, with 4 patients obtaining complete remission. CONCLUSIONS: Administration of G-CSF was associated with an acceleration of neutrophil recovery, indicating its potential to reduce the risk of infection and to maintain planned chemotherapy doses.

Pure red cell aplasia following peripheral stem cell transplantation: complete response to a short course of high-dose recombinant human erythropoietin.

We studied a patient who developed pure red cell aplasia (PRCA) following peripheral stem cell transplantation for non-Hodgkin lymphoma. Serum erythropoietin was appropriate for the degree of anemia. Corticosteroid treatment was ineffective. Four months after transplantation rHuEpo was administered subcutaneously at a dose of 150 U/Kg per day, five days a week for 8 weeks. Treatment induced an erythropoietic response and corrected anemia. Response was maintained following discontinuation of rHuEpo. This study and previous reports indicate that high doses of rHuEpo given over a short time can resolve PRCA following autologous or allogeneic stem cell transplantation.

Radiation therapy and chemotherapy in the treatment of head and neck extranodal non-Hodgkin's lymphoma in early stage with a high grade of malignancy.

BACKGROUND: Optimal management of patients with localized head and neck extranodal lymphoma remains controversial, both because of the lack of randomized studies and because of the heterogenous grouping of most reported series. MATERIALS AND METHODS: Patients treated at our institution between 1974 and 1993 for extranodal head and neck lymphoma were retrospectively analyzed and classified. The therapy and outcome of 92 patients classified as having an intermediate (42) and high (50) level of malignancy according to the Working formulation and in stage I (39) or II (53) of the Ann Arbor Staging System were considered. Fifty-three patients (57.6%) received chemotherapy alone, and 39 (42.4%) combined radiochemotherapy. RESULTS: The different treatment schedules allowed these patients to achieve global actuarial 5-year overall, event-free, and relapse-free survival rates of 81.2%, 78.1% and 89.3%, respectively. The patients that received combined modality treatment reported actuarial 10-year event-free and relapse-free survival rates of 65.3% and 90.7%, respectively, with a suggestion of decreased treatment-related morbidity compared to patients treated with chemotherapy. CONCLUSIONS: Our results underscore the important treatment role of combined radiochemotherapy for early stage intermediate and high grade lymphomas.
