Value of MRI performed with phased-array coil in the diagnosis and pre-operative classification of perianal and anal fistulas.

PURPOSE: To assess the value of MRI performed with phased-array coil in the diagnosis and preoperative staging of perianal and anal fistulas. MATERIALS AND METHODS: 20 patients (13 with Crohn's disease) with clinical evidence or suspicion of anal fistulas underwent pelvic MRI (1.5 T) performed with phased-array coil. Images were obtained in the axial and coronal planes using TSE T2-weighted high resolution sequences with and without fat suppression, T2-weighted HASTE and T1-weighted FLASH sequences, with and without fat suppression, before and after gadolinium enhancement. The following parameters were considered: presence of a fistula and relation with the sphincters, and presence of abscesses or complications. All patients underwent surgery. The MRI and surgical findings were assessed using the Park's fistula-in-ano classification and the St. James MR imaging classification of perianal fistulas. Surgery was considered the gold standard. RESULTS: MRI documented no evidence of fistula in 2 patients, intersphinteric fistulas in 5 (grade 1 and 2 St. James), trans-sphincteric fistulas in 9 (grade 3 and 4 St. James), translevator in 2 (grade 5) and complex ano-rectum-vaginal fistulas in 2. Concordance with surgery was 90%. CONCLUSIONS: MRI is an accurate technique for the identification and classification of anal and perianal fistulas and their complications. In our experience the phased-array coil offers both high field of view and spatial resolution, enabling the demonstration of perianal pathology.

[Esthesioneuroblastoma. Description of a case investigated with CT and MR].

Esthesioneuroblastoma, also known as olfactory neuroblastoma, is a rare neuroendocrine tumor originating from the olfactory epithelium of the upper nasal cavity and representing < 3% of all tumors of the nasal cavities. This malignant tumor is characterized by a slow growth and local recurrencies and has a more favorable prognosis as compared with other more frequent forms of neuroblastomas originating from the suprarenal medulla and the sympathetic nervous system. Affected patients usually present with a history of progressive nasal obstruction, rhinorrhea, and severe epistaxis--sign of the conspicuous vascularity of this type of tumor. The combined use of CT and MR techniques allows the diagnostic suspicion of esthesioneuroblastoma and is of vital importance in the accurate staging of the disease and in the treatment planning.

[Magnetic resonance of the knee: possibility and current indications].

MRI is a multiparametric, multiplanar, non-invasive largely employed tool for assessing osseous, ligamentous and tendineous injuries, inflammatory and degenerative changes of the knee. Although its wider availability and the lack of ionizing radiations MRI should be used only if clinically useful in patient management, in a appropriate diagnostic iter including plain film and/or ultrasound examination. The aim of our work is to review possibilities, limits and current indications for MRI assessment of diseases of the knee.

[Collateral effects of intraoperative hyperthermic chemotherapy in peritoneal carcinomatosis].

The association between chemotherapy and hypertermia produces a synergic effect. In this study the Authors present their experience, by the analysis of the results. From 1993 to 2000, 17 patients have been treated with surgery associated with hypertermic chemotherapy for peritoneal carcinomatosis. For the management of these patients a constant cooperation among surgeon, cardiologist and anaesthetist is very important.

[Appendiceal lithiasis: case report and review of literature].

Appendiceal calculi are found in a limited percentage of patients. They are in association to an elevated prevalence of necrotic appendicitis and perforation of the appendix. The Authors report a case of acute appendicitis associated to appendiceal lithiasis with perforation of the organ. They emphasize the possibility to perform a preventive appendectomy in case of incidental appendiceal lithiasis. Besides they consider the differential diagnosis of the calcified images located in the lower abdominal quadrants with the aid of the various imaging methods available today, particularity sonography and computerized tomography.

[The colecistectomy in primary sclerosing cholangitis: case report and review of literature].

Primary sclerosing cholangitis (PSC) is an idiopathic chronic inflammatory condition affecting intra- and extra-hepatic biliary system. The aim of this study is to stress the importance of cholecystectomy in patients with PSC. Consideration should be given to performing a cholecystectomy in PSC patients with gallbladder adenomyomas or calculi, even asymptomatic; this happens in approximately 15% of cases.

[Loco-regional fibrinolysis with urokinase in the treatment of deep venous thromboses].

The importance of the fibrinolytic treatment in deep venous thrombosis, to avoid the mortal pulmonary embolism, among other complications, is commented on. The results in 32 patients, presenting deep venous thrombosis and treated with loco-regional Urokinase, are presented. Author carries out some commentaries about surgical and medical treatment.

The in vitro effect of epirubicin on human normal and leukemic hemopoietic cells.

Epirubicin, at concentrations ranging between 10(-7) and 10(-13) M, was assayed in semisolid cultures of human normal hemopoietic cells and in liquid cultures of 5 different human leukemic cell lines. The growth of all normal hemopoietic progenitor cells was inhibited by the higher drug concentrations; at the lowest concentration, only CFU-E and 7th-day CFU-GM were not inhibited. On the other hand, leukemic cells were sensible only to the higher concentration of epirubicin, which, nevertheless, was not fully inhibitory. Leukemic cell differentiation was not promoted by the drug, as evidenced by a panel of monoclonal antibodies, by cytochemistry and by functional tests. These results suggest a marked in vitro myelotoxicity of epirubicin, that does not appear to be compensated by a powerful control of leukemic cell proliferation.

[Incidence of HBeAg in hemodialysis patients: preliminary data (author's transl)].

The incidence of HBeAg in HBsAg positive hemodialysis patients in reported. 21.2% of hemodialysis patients have been resulted positive for serum HBsAg. 63.6% of HBsAg positive patients resulted to have HBeAg in their serum. Anti-HBe antibody was never found. The Authors emphasize the importance of HBeAg in hemodialysis patients to evaluate their infectivity.

De Quervain's disease: diagnostic imaging.

De Quervain's disease constitutes the most frequently occurring stenotic tenosynovitis. The authors have studied which imaging technique allows for the best representation of anatomopathologic modifications. Ultrasound examination appeared to be the most appropriate and advantageous method as it reveals involvement of the sheaths and tendons in the simplest, most complete, and least expensive manner.
