Transvaginal sonography and progesterone challenge for identifying endometrial pathology in postmenopausal women.

OBJECTIVE: To evaluate the usefulness of transvaginal sonographic (TVS) measurement of endometrial thickness combined with a progesterone challenge test (PCT) for identifying endometrial pathology in postmenopausal women. METHODS: Two hundred eighty-four postmenopausal women were examined by TVS: 130 asymptomatic women (group A) and 154 with uterine bleeding (group B). Endometrial thickness > 5 mm was considered pathological. All women with abnormal endometrium from group A underwent PCT. All women from group B underwent D&C. RESULTS: Thirty women with a positive PCT underwent D&C. There was no endometrial cancer. One hundred seven patients from group B had abnormal sonographic and histologic findings: benign (hyperplasia, polyp) or malign (endometrial cancer). There was no cancer in cases with endometrial thickness < or = 6 mm. The sensitivity, specificity and accuracy of TVS for detecting endometrial pathology were 99%, 89% and 96% if the cut-off limit of 5 mm was used. CONCLUSION: TVS combined with PCT is a simple, well-tolerated, safe and reliable method for identifying endometrial pathology in postmenopausal women.

Transvaginal sonography and endometrial thickness in patients with postmenopausal uterine bleeding.

The objective of the present study was to evaluate the usefulness of transvaginal ultrasonography in assessment of the endometrial thickness in patients with postmenopausal uterine bleeding in relation to the histopathological results of the endometrial biopsy. A total of 118 women with postmenopausal uterine bleeding were studied prospectively. Thirty postmenopausal women without vaginal bleeding and without gynaecological complaints served as a control group. In women with histopathological diagnosis of atrophic endometrium the mean endometrial thickness was 3.1 +/- 1.7 mm whereas the corresponding value for patients with endometrial carcinoma was 18.4 +/- 8.2 mm (P < 0.001). No endometrial carcinoma was diagnosed in endometrial thickness < or = 6 mm. The sensitivity was 100% and specificity 64% if a 6-mm cutoff limit was used. We consider that transvaginal ultrasonographic investigation for post-menopausal bleeding may provide useful information for selection of those patients who are under higher risk of endometrial abnormalities. When using vaginal sonography for measuring the endometrial thickness in women with postmenopausal bleeding, it is reasonable not to perform a dilatation and curettage if the endometrial thickness is < or = 6 mm.

[The biochemical changes in overweight pregnant women during the pregnancy].

The degree of weight gain during pregnancy as a risk factor for a mother and her newborn is a disputable problem. Most of the studies have shown, that maternal overweight gain has a bad effect on labor delivery outcome. The present study included 108 pregnant women, subdivided into three groups, according to weight gain during pregnancy: Group I(32) - < 12 kg, Group II(42) - 12 < - < 20 kg. Group III(34) - > 20 kg. The patients were estimated according to their age, parity, pregnancy induced hypertension and mode of delivery. The authors investigated biochemical indicators of protein, lipid and electrolyte balance. The results illustrate the influence of prenatal overweight gain on the pregnancy complication, mode of delivery and some biochemical changes.

[The dynamics and structure of abortions in 1981-1988].

The authors describe the question about dynamics of the structure of abortions and go through the clinical material of the Chair of Obstetrics and Gynecology in the town of Plovdiv for a period of 8 years. The total number of abortions and deliveries are indicated for the same period as the abortions are presented in accordance with the type and refer to the number of nonmarried and married women and their parity. In the inferences the authors make suggestions for liberalization of the legal status, regulating the right of women for abortion, as well as for search of other means mainly social-civic for improvement of the index of birth rate.

[The HLA system in normal and pathological pregnancies].

The authors investigated 205 women, 50 of whom were nonpregnant, 32 with normally progressing pregnancy, 30 parturients, 30 with abortions, 30 with prolonged pregnancy and 35 with pre-eclampsia. There were no statistically significant changes between the percentage of cases with positive lymphocytotoxic test, presence of leucoagglutinins and thrombocytic antibodies among the single groups. There were HLA-antibodies, which were not titrated due to the low titre in 4 of the women with pre-eclampsia.

[Effect of the glucose concentration in the maternal blood on the level of specific beta 1-glycoprotein].

The authors carried out studies on 20 women with normal pregnancy between 38 and 40 weeks gestation. Hypoglycemia, induced by insulin, was provoked in 10 of them, but hyperglycemia--in the other 10 women. Maternal blood samples for glucose and SP1 were taken on 15,30 and 60 min as well as on 2, 6, 12, 24 and 48 h after the onset of the trial. It was established that hypoglycemia, induced by insulin, the mean values of SP1 in the maternal serum on 24th hour were considerably higher, but in the induced hyperglycemia the values of SP1 were higher on the 24th h than on 60 respectively 120 min after the beginning of glucose infusion. The authors found that between serum concentration of SP1 and carbohydrate metabolism of the mother existed interrelationship.

[Uterine hemorrhage as an indication for performing contact hysteroscopic examination].

The authors point out uterine bleedings as the most frequent indication for examination after reviewing a material of 631 hysteroscopic examinations. Hysteroscopic images of glandular hyperplasia of the endometrium are described as well as endometrial polyp, which are the most frequent cause for uterine bleedings. The contact hysteroscopy is considered as a valuable additional method for examination of women with metrorrhagia.
