Primary malignant lymphoma of the trachea. Report of a case successfully treated by primary end-to-end anastomosis after circumferential resection of the trachea.

A 67-year-old Japanese man with exertional dyspnea was found by laminagraphy to have tracheal obstruction. The stricture was localized at the cervical and mediastinal trachea and was 5.0 cm long on an x-ray film. Eight tracheal rings were successfully resected and the ends of the trachea were anastomosed. The lumen of the removed trachea was narrowed by intramural and intraluminal polypoid growth originating from the tracheal wall. Histologically, the tumor was diagnosed as a mixed type of malignant lymphoma. The patient had serum hepatitis postoperatively, but now he is living normally without any difficulty in respiration. No signs of recurrence have been seen in the 5 years, 4 months since the operation. From this experience, the rarity and the possibility of curative operation of this tumor are discussed.

[Pharmacologic studies on Ro 12-0068, a new non-steroidal anti-inflammatory drug (author's transl)].

The pharmacological profile of Ro 12-0068 as a new anti-inflammatory agent has been established in 9 different animal models and was compared with the profiles of 10 reference drugs. In adjuvant arthritis, scald-related edema and bradykinin-induced capillary permeability tests, the inhibitory potency of Ro 12-0068 was greater than findings in the case of diclofenac sodium which was the most potent agent available, while Ro 12-0068 exerted a stronger inhibitory effect on granuloma formation than did indomethacin. Anti-inflammatory potency of Ro 12-0068 in other models was equivalent to that of piroxicam. Ro 12-0068 was categorized as a potent anti-inflammatory agent and was grouped with diclofenac sodium, indomethacin and piroxicam, as compared to rather weak agents such as naproxen, carprofen, phenylbutazone, flufenamic, mefenamic, and acetylsalicylic acids. In fasting rats, ulcerogenicity of Ro 12-0068 in the gastric glandular and duodenal portions was weaker than that of indomethacin, appeared to be less than that of piroxicam and was equivalent to that of diclofenac sodium. Induction of fecal occult bleeding in dogs was markedly greater in case of indomethacin and piroxicam than with Ro 12-0068, within few days after oral treatment. Ro 12-0068 and piroxicam had much the same inhibitory effect on prostaglandin synthesis of inflammatory tissues and exudates from gastro-intestinal tissues. Nevertheless, therapeutic indices of Ro 12-0068 in rats were larger than those of piroxicam, in all models, and those of indomethacin in most cases. These results strongly suggest that Ro 12-0068 is a potent anti-inflammatory agent and that the occurrences of related gastro-intestinal disturbances are fewer.

[Two-dimensional echocardiography in preoperative evaluation of mitral stenosis (author's transl)].

Two-dimensional echocardiograms were correlated with the operative findings and surgical procedures in 46 patients with mitral stenosis. In addition, a new projection evaluating subvalvular lesion more precisely was reported. Two-dimensional echocardiographic findings studied were 1) pathology of the two leaflets of the mitral valve, 2) valve area, and 3) subvalvular lesion. The findings of leaflets and valve area were well correlated with the operative findings. But there were six cases with discrepancies in the initial 22 cases about the findings of subvalvular lesion; there were 5 false negative and one false positive case. It was considered that insufficient information about the appositional zone of the mitral valve and chordae tendineae made it difficult to evaluate them correctly. We have designed a new projection for delineating the subvalvular apparatus and employed in the next 24 cases, and the transducer was placed near the apical heart to visualize 4 chambers. This eliminated greatly the false positive cases (only 2 out of 24 cases). There were three cases of mitral valve replacement (MVR) in the initial 22 cases, but we could expect the necessity of MVR in only one case, and we had to perform chordotomy, papillotomy, or debridement as well as commissurotomy in two false positive cases. In the next 24 cases, however, we could predict it in all three cases of MVR before the operation. It was valuable for surgery of mitral stenosis to get enough information about the subvalvular lesion as well as mitral leaflets by two-dimensional echocardiography.

Behavior of vesicular stomatitis virus glycoprotein in mouse LM cells with modified membrane-phospholipids.

LM cells in which the membrane phospholipids had been modified with choline analogues were infected with vesicular stomatitis virus. The choline analogues tested were choline, N,N'-dimethylethanolamine, N-monomethylethanolamine and ethanolamine. These modifications per se did not affect the syntheses of individual viral proteins. The viral glycoprotein was detected in the plasma membranes of all the modified cells by pronase digestion in pulse-chase experiments, but the amount of glycoprotein susceptible to proteolysis varied, decreasing in these modified cells in the following order: N,N'-dimethylethanolamine- greater than choline- greater than N-monomethylethanolamine- greater than ethanolamine-treated cells. After a 4-h chase, glycoprotein was mainly distributed in the plasma membranes of cells modified with N,N'-dimethylethanolamine, whereas it was found in both the microsomes and plasma membranes of cells modified with other analogues. Fairly large amounts of glycoprotein were also found in the soluble fraction of ethanolamine-treated cells, but not in that of choline- or N,N'-dimethylethanolamine-treated cells. More precise experiments on the behaviour of glycoprotein with a short period of chase strongly suggested that migration of glycoprotein from the microsomes to the plasma membranes was fastest in cells modified with N,N'-dimethylethanolamine and slowest in cells modified with ethanolamine. Membrane lipid modifications also resulted in release of different numbers of progeny virions from the cells, release of virions from the cells decreasing in the following order: N,N'-dimethylethanolamine- greater than choline- greather N-monomethylethanolamine- greater than ethanolamine-treated cells. These results indicate that modification of membrane phospholipids influences not only the insertion of glycoprotein into the microsomes and its migration to the plasma membranes, but also the production of progeny virions.

Production of a colony-stimulating factor following differentiation of leukemic myleoblasts to macrophages.

Leukemic cells in the myeloblastic stage from a murine myeloid leukemia cell line (M1) were induced to differentiate to macrophages by lipopolysaccharide (LPS) from Gram-negative bacteria. A granulocyte-macrophage colony-stimulating factor (CSF) was produced only during differentiation. After induction of differentiation, the continued presence of LPS was necessary to stimulate the macrophages to release CSF. In contrast, a macrophage cell line (Mm-1) derived from the M1 line produced CSF without LPS-stimulation, but CSF release was stimulated by the presence of LPS.

Differentiation and production of colony-stimulating factor induced by immunostimulants in a leukemia cell line.

Immunostimulants from various microorganisms were tested on a myeloid leukemia cell line (M1) for the ability to induce production of CSF and to cause differentiation of these cells. Based on their activities, the compounds were divided into two general classes: those inducing extensive cellular differentiation and those devoid of this effect. The stimulants which were active in this regard always produced large quantities of CSF, whereas those devoid of a differentiating effect did not cause CSF production. Even the potent stimulants had no effect on the D subline, in which the differentiation was not inducible.

[Lymphography in the head and neck: observations on lymphatic flow at various points--dog experiments (author's transl)].

Because of technical difficulties, lymphography of the neck is still in its developmental stage. The course of lymph flow in the head and neck has not been definitely elucidated. In order to clarify lymphatic pathways and localise cervical lymph nodes, lymphography was performed in the dog. The results show that, particularly for diseases of the tongue and oral cavity, maximum information is obtained by lymphography of the chin.

Organ culture of adult human skin.

Proliferation and differentiation of the epidermis in organ culture of adult human skin by the sponge matrix method were studied histologically and autoradiographically, and the following results were obtained: 1) On the first day of culture, mitotic figures were already observable in the epidermis. The outgrowth of epidermal cells at the margins of the explants started. On the second day, there was transformation to a zone that will be referred to as the newly formed stratum corneum in the upper epidermis. 2) On the third and fourth days, the increased growth of epidermal cells caused thickening of the epidermis. Simultaneously, Malpighian cells progressively differentiated into a cornified layer. 3) On and after the fifth day, the basospinous cell layer was reduced in thickness in most of explants. On the ninth and tenth days, DNA synthesis in the basal layer was still obvious, although the epidermis showed a thickness of only one or two cells overlaid with a large number of horny layers. 4) In the culture medium supplemented with corticosteroid, the epidermal growth was slightly depressed with lessened formation of stratum corneum in the early stages of culture as compared with the explants cultured in the basic medium. The reduction of the basospinous layer was scarcely notable after the fifth day. Even after 10-11 days, epidermal cells were well preserved and their stratified squamous architecture was less disorganized. It seemed that corticosteroid could prolong the survival of adult human skin in vitro. These findings indicate that this culture technique could be used as a model for organ culture of adult human skin.
