Predictors of success in a smoking cessation clinic.

We identified factors associated with success among the first 531 patients referred to a Veterans Affairs Medical Center smoking cessation clinic. Both patients and providers completed questionnaires covering smoking history, other health habits, and comorbidity. The 3-month success rate was 23% of all referrals or 42% of clinic attendees. Among those referred, factors associated with their success were being at least 50 years of age, having hyperlipidemia, and not currently drinking alcohol. Among those who attended the clinic at least once, factors associated with success were being at least 50 years of age, being less physically active, and not currently drinking alcohol. Neither patients nor providers could predict who would successfully stop smoking.

Analysis of nickel-titanium versus stainless steel instrumentation by means of direct digital imaging.

This study compared step-back preparations in curved canals using nickel-titanium (Ni-Ti) K-files and stainless steel K-files. Forty canals in mesial roots of mandibular molars were embedded in casting resin and cross-sectioned at three levels: 1 to 2 mm from the apical foramen, middle of the curve, and coronal. Direct digital computer images were recorded before and after instrumentation. Superimposition of the images combined with digital subtraction computer software allowed direct measurement of area instrumented, distance of transportation, and shape analysis. The computer software calculated absolute center of gravity for each image analyzed to get a full 360-degree interpretation of the canal transport. Time of instrumentation was recorded. Results showed Ni-Ti files to cause significantly less transportation and remain more centered at the apical level (p < 0.05). Area removed by Ni-Ti and stainless steel files was not significantly different (p < 0.05). Time of instrumentation was not significantly different for Ni-Ti and stainless steel instruments (p < 0.05). Cross-sectional shape of the instrumented canal was not significantly different (p < 0.05).

Molecular cloning of the olfactory neuronal transcription factor Olf-1 by genetic selection in yeast.

A novel genetic selection in yeast has been used to isolate a complementary DNA for the transcriptional activator, Olf-1, which binds to the regulatory sequences of several olfactory-specific genes. The Olf-1 protein, expressed exclusively in the olfactory receptor neurons and their precursors, contains a new helix-loop-helix motif and functions as an apparent homodimer. Olf-1 may be the first member of a family of related proteins that may direct cellular differentiation in a variety of neuronal tissues.

Genes encoding components of the olfactory signal transduction cascade contain a DNA binding site that may direct neuronal expression.

Genes which mediate odorant signal transduction are expressed at high levels in neurons of the olfactory epithelium. The molecular mechanism governing the restricted expression of these genes likely involves tissue-specific DNA binding proteins which coordinately activate transcription through sequence-specific interactions with olfactory promoter regions. We have identified binding sites for the olfactory neuron-specific transcription factor, Olf-1, in the sequences surrounding the transcriptional initiation site of five olfactory neuron-specific genes. The Olf-1 binding sites described define the consensus sequence YTCCCYRGGGAR. In addition, we have identified a second binding site, the U site, in the olfactory cyclic nucleotide gated channel and type III cyclase promoters, which binds factors present in all tissue examined. These experiments support a model in which expression of Olf-1 in the sensory neurons coordinately activates a set of olfactory neuron-specific genes. Furthermore, expression of a subset of these genes may be modulated by additional binding factors.

Molecular mechanisms of olfactory neuronal gene regulation.

The mammalian olfactory system utilizes a biochemical cascade mediated by specialized proteins to detect odorants with high sensitivity and specificity. The recent identification of olfactory neuron-specific components for each step in the signalling cascade suggests that expression of these proteins is coordinately controlled by cis-acting regulatory sequences and trans-acting transcriptional activators. We have used molecular genetic methods to characterize sequences encoding tissue-specific DNA-binding sites in the genes for components of the odorant transduction pathway and have identified a putative transcription factor, Olf-1, that functions at these sites to regulate gene expression.

Heterotopic gastric mucosa of the esophagus.

We have described three patients with heterotopic gastric mucosa of the proximal esophagus, which has been recognized by pathologists since the early 19th century. In contrast to Barrett's esophagus, which is now known as an acquired metaplastic lesion of the esophagus, this mucosa is believed to be a remnant of incomplete epithelialization of the esophagus during fetal life. It has little clinical significance because of the small size of the mucosal patch and the neutralization of the acid by saliva.

[Carcinoma of the extrahepatic bile duct in young child--a case report].

A child, 2 years old, male suffering from severe jaundice, melena, severe anemia and right upper abdominal mass was admitted into our hospital. Diagnosis of obstructive jaundice was made before operation. On exploration, a tumor mass was found in the pancreatic head, which involved the duodenum and distal common bile duct. Pancreaticoduodenectomy with one stage pancreaticojejunostomy, choledochojejunostomy and gastrojejunostomy was carried out. Postoperative pathology showed adenocarcinoma of the distal common bile duct invading the periampullary tissues of the duodenum and infiltrating, to a lesser extent, into the pancreatic head. Carcinoma arising from the distal common bile duct is rare in infancy and childhood. Radical resection should be attempted if possible. In general, the prognosis is poor in this tumor. No complication after operation occurred on this patient but close follow-up is necessary. Pathology related to this tumor is discussed with a review of literature.

Variceal bleeding from an ileostomy stoma.

Patients with portal hypertension due to cirrhosis commonly have bleeding from varices. In patients with a total colectomy, ileal varices can be a site of bleeding in cases of portal hypertension. Proper diagnosis requires studying the venous phase of the mesenteric arteriogram to show ileal varices, and carefully inspecting the mucocutaneous region of the ileal stoma for venous bleeding sites. Bleeding from ileal varices can be managed by either local control or portal vein decompression.
